We proceeded the judgement of Sasang constitution of 79 children(the age 101 between 15) who have visited Dongguk Bundang Oriental Medical Hospital during 3 months from August 2002 to October 2002 by Questionnaire of Sasang Constitution Classification and conducted questionnaire survey about the type of disease and symptom occurred frequently. The conclusion is following. 1. The distribution of Sasang constitution: Soeum group(少陰人) 32, Taeeuml group(太陰人) 29, Soyang group(少陽人) 18. 2. The distribution of systemic disease occurred frequently : in digestive system, Soeum (46.9%) & Soyang(44.4%) group more than Taeeum group(34.5%) and in genitourinary system and others, Soeum(28.1%) more than Taeeum group(17.2%). 3. The distribution of symptom occurred frequently : in rhinitis and sinusitis, Taeeum(60.0%) & Soeum(60.0%) more than Soyang(45.5%), in abdominal pain, Soeum(58.8%) & Soyang(62.5%) more than Taeeum(36.4%), and in diarrhea, Taeeum(45.5%) more than Soeum(123.5%) & Soyang group(25.0%). Headache & vexation(心煩) are almost found in Soeum group with abdominal pain and excessive sweating is only found in Taeeum group. 4. The efficacy of herbal medication combined with Cervix Cornu Pranum (鹿茸加味劑) : after the medication, change of condition have showed in Taeeum group at the most high rate(31.0%) and showed in Soeum(15.6%) and Soyang(11.1%) group. 5. The distribution of allergic disease: allergic rhinitis and atopic dermatitis took up the majority of all allergic diseases. In morbidity of allergic rhinitis, Taeeum(62.1%) & Soyang(55.6%) more than Soeum(37.5%). In morbidity of other allergic diseases and degree of severity, it has no difference among three Sasang constitution groups.
Objectives: This study was conducted to investigate practical experiences of telemedicine of community health practitioners(CHPs). Methods: Qualitative data were collected by in-depth interviews from 10 CHPs who have experiences in managing telemedicine system. All interviews were recorded and transcribed according to qualitative conventional content analysis processes. Results: As a result, 32 themes were deduced and 11 theme clusters and 3 categories were formed and each coding categories were derived directly from the text data. 11 theme clusters derived from the 32 meaningful themes were as follows: Human resources, equipments and systems, computer program (Input resources), human resource management, patient registration and management, medication, laboratory test (Progress), benefits in telemedicine system managing, difficulties in telemedicine system managing, complains in telemedicine system managing, client responses to telemedicine system (Outcome evaluation). 3 categories derived 11 theme clusters were 'input', 'progress', and 'outcome evaluation'. Conclusions: This study has contributed to the understanding of operation of telemedicine by CHPs in community health posts. For more systematic and comprehensive management, further study should be conducted to reflect experience and positions of public health center physicians, collaborative hospital physician and patients.

Objectives : The goal of this study was to examine the association between zinc and lead level and symptoms of attention-deficit hyperactivity disorder (ADHD) among Korean children. Methods : A total of 89 clinic-referred children participated in the study (ADHD group=45, control group=44). The participants were 5-15 years old, and were mainly from urban areas of Seoul, Korea. ADHD was diagnosed using the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version. We excluded children with a comorbid psychiatric disorder, medical illness requiring medication, or a prior history of taking ADHD medication. In order to evaluate the severity of ADHD symptoms, parents' Korean ADHD Rating Scale (K-ARS) was used. The ADHD diagnostic system (ADS) was used for evaluation of the severity of inattention and impulsivity. All participants completed the intelligence test and hair mineral analysis. Multiple regression analysis was used to examine the effect of hair zinc and lead levels on the K-ARS and ADS. We measured the predictive ability of the zinc and lead levels using logistic regression analysis. Results : The lead level explained the score for omission errors, commission errors, and response time SD in visual ADS in the ADHD group (adjusted $R^2$=.243, p<.01, adjusted $R^2$=.362, p<.01, and adjusted $R^2$=.275, p<.01), the score for omission errors of auditory ADS in ADHD group (adjusted $R^2$=.407, p<.01) and the entire group (adjusted $R^2$=.292, p<.01). Zinc was significantly explanatory for the K-ARS scores for the entire group (adjusted $R^2$=.248, p<.001) and the ADHD group (adjusted $R^2$=.247, p<.05). Conclusion : These findings suggest a possible role of zinc and lead in ADHD. Lead concentration in hair samples affected the ADS scores, and this was more prominent in children with ADHD. Children with ADHD had a lower zinc concentration in their hair, and the zinc concentration in hair showed negative correlation with the K-ARS score.
Due to the development of communications technology, it is now possible to be offered online from remote places. This kind of communications technology can be applied to the medical field. The medical treatment appointments in hospitals can be its typical example. But still, in most of hospitals, patient or guardian have to physically visit or call to the hospital to set up an appointment for the medical treatment. In addition, they have to wait in line in order to pay after receiving the medical treatment. The patient or guardian, after paying, receive a paper prescription and they go to a nearby pharmacy to take the medicines. They must wait in line again there in order to receive the medicine from the pharmacy. In this paper, we would like to suggest a smart medical treatment system in order to solve the problems discussed above. With this proposed system, the user will be able to make an appointment, make payments and receive medication quickly and easily without spending extra time. Also, there will be no need for paper prescriptions with this system. We discuss about the security of medical information for this proposed smart medical treatment system proposed.

Objectives : The purpose of this study was to investigate whether the cognitive functions would be correlated with psychotic symptoms and whether antipsychotic treatments would affect the cognitive functions after 8 weeks. Methods : The thirty-five schizophrenic patients were conducted in this study. The psychopathology was measured using PANSS. The memory function, executive function, and sustained attention were measured using Memory Assessment Scale(MAS), Wisconsin Card Sorting Test(WCST), and Vigilance(VIG) and Cognitrone(COG) in Vienna Test System. After 8 weeks of antipsychotic treatment, we retested the cognitive tests. Results : 1) The cognitive tests after the 8 week's treatment showed significant improvements in memory and executive function in the schizophrenic patients. On the other side, sustained attention did not show improvement. 2) The change of PANSS were correlated with perseverative response, perseverative error and total correct in WCST at baseline. WCST scores at baseline were correlated with negative symptoms, but not positive ones. Conclusion : These study suggests that 1) the impaired sustained attention could be a vulnerability marker in schizophrenia, 2) memory & executive function deficit could be reversible after treatment, and 3) medication might have a benefit in improving the cognitive functions in schizophrenia. Furthermore, the data supports that the better premorbid executive function was, the more favorable was the treatment response in schizophrenic patients. Finally, this study indicates that executive function might be an index of treatment improvement.
Junggi-haham (中氣下陷) is a term referring to lethargic symptoms such as digestive system disorder originating from energy deficiency. The authors applied therapy including Bojungikgi-tang(or Seungyangikgi-tang) and Bi-jeonggyuk method to two patients, one having ptosis and the other stress urinary incontinence, respectively, as Junggi-haham. Bojungikgi-tang is an herbal medication preparation and Bi-jeonggyuk is an acupuncture therapy method, and both are used to treat Junggi-haham symptoms. The combined treatment showed positive outcome, and the result suggests that Bojungikgi-tang and Bi-jeonggyuk are effective treatments for patients diagnosed as Junggi-haham.

Background and Objectives Breast reconstruction has the advantage of reducing the loss of the body image of patients after mastectomy surgery, and also improving the quality of the social and sexual life of breast cancer patients. However, in surprising and unfortunate number of patients, acute postoperative pain persists beyond the normal course of postsurgical recovery. We set out to investigate the effect of local dynamical micro-massage (LDM) treatment for achieving pain relief and reducing other postoperative complications. Materials and Methods We performed a retrospective analysis on 58 patients who underwent LDM treatment for postoperative pain management at Soonchunhyang University Bucheon Hospital between February 2017 and June 2019. Those patients who complained of persistent postoperative pain, which was uncontrollable with medication, were treated with LDM. The degree of pain and discomfort with contracture were recorded using numerical rating scale (NRS) scoring system with numbers from 0 to 10 ('none' to 'worst'). Results The median NRS score of pain was reduced by 62.3% from the start to the end of LDM treatment (p<0.001). Further, the NRS score of discomfort with contracture was reduced 66.0% (p<0.001). There was no complication related to the LDM treatment. Conclusion Dual-frequency ultrasound LDM can be an effective therapeutic option for persisting pain after breast reconstruction surgery. It was also effective in improvement of discomfort with contracture and erythema of the surgical wound.
A 25-years-old woman with mandibular prognathism underwent a mandibular setback by way of mandibular sagittal split ramus osteotomy (MSSRO). After 2 days of operation, she developed difficulty of closing her right eye. The blink reflex test and motor nerve conduction study of the right orbicularis oris muscle were revealed right facial neuropathy of unknown origin and House-Brackmann facial nerve grading system (HBFNGS) grade V. For treatment, we initially prescribed oral prednisolone and nimodipine including physical therapy. The samples consisted of 11 facial nerve palsy patients caused by MSSRO and were analysed about onset of facial nerve palsy, postoperative HBFNGS, final HBFNGS, treatment method and recovery time. At 10 weeks of treatment of nimodipine, she had completely regained normal function (HBFNGS grade I) of the right facial nerve. The clinical results lead to assume a fast recovery of facial nerve function by the nimodipine medication, whereas average time of recovery is 16.32 weeks in references. Despite of the limited one patient treated, the result was very promising with respect to a faster recovery of the facial nerve function. Considering the use of nimodipine treatment for peripheral facial nerve palsy following a surgical approach with an anatomically preserved nerve can be recommended.
Objectives : The diagnosis of depression is based on a highly variable set of symptoms. Therefore, depression should not be viewed as a single disease, but a heterogenous syndrome comprised of different pathophysiologies. There are several subtypes of depression which were already incorporated in DSM-IV. This article provides a systematic review of pharmacological treatments of two recognized subtypes of depression-dysthymic disorder and atypical depression. Methods : Systematic search of relevant literatures on dysthymic disorder and atypical depression was performed by proposed search strategy of the Clinical Research Center for Depression of Korean Health 21 R&D Project. All identified literatures were carefully reviewed and classified according to SIGN grading system and summarized in a narrative manner. Results : For the treatment of dysthymic disorder and atypical depression, selective serotonin reuptake inhibitors( SSRIs) and moclobemide have more evidence than the other antidepressants. SSRIs and moclobemide showed superior tolerability than tricyclic antidepressants. Conclusions : The authors proposed treatment recommendations for dysthymic disorder and atypical depression by the methods of evidence-based medicine(EBM). However, guideline developing methods of EBM also have several inevitable limitations. Therefore, in the absence of clear and significant differences in efficacy, the choice of medication must be individualized for a particular patient based on psychiatrist's own clinical decision.

Objectives : The purpose of this study is to investigate the effect of chuna treatment on cervical sprain caused by traffic accident in early stage. Methods : This study carried out on 20 patients who have received hospital treatment in Daejeon Univ. cheonan Oriental Hospital. Non-chuna group got acupunture-therapy, herbal medication, physical therapy and Chuna group got chuna treatment besides. We measured Heart Rate Variability(HRV) and Visual Analog Scale(VAS) on 2nd, 4th day. Results : After being treated by our methods, Chuna Group showed the inclination to balance the sympathetic and parasympathetic nerve. In chuna group, an autonomic nerve activity showed the inclination to increase. But there were no significant difference between both groups. Chuna group's VAS were significantly decreased(p=0.043). Conclusions : The results suggest that Chuna treatment help traffic accident patients in early stage to reduce pain. Refer to autonomic nerve system, chuna treatment seem to do positive effect but Further long tenn study in a large scale is needed.
