This study examined the nurse's emotional exhaustion and influencing factors, and the relationship between emotional burnout and hospital nursing workload using multilevel logistic regression analysis. The study subjects were 3,083 nurses of 65 hospitals, who participated in the training conducted by the Healthcare Industry Trade Union in 2010 and responded to the questionnaire. First, 71.6% of nurses experienced emotional exhaustion, and the average score of emotional exhaustion was 33.53. Second, the 'non - nursing work experience' showed a significant effect on emotional exhaustion. Third, the 'sufficient nursing staff' variable increased the emotional exhaustion. Fourth, the nursing grades were correlated significantly with emotional exhaustion compared to those with more than four beds per nurse. Fifth, in the case of working in the internal ward, the shorter the clinical career, the higher the emotional exhaustion. In other words, higher emotional exhaustion was associated with more nursing work experience, more perceived insufficient nursing staff, more nurses per bed, the department of internal medicine, and a shorter clinical career.
This study grasped recognition on a system of long-term care insurance for the elderly targeting dental professionals who are working at dental hospitals and clinics where are located in Daejeon Metropolitan City. It developed and utilized materials available for educating the care staff in a system of long-term care insurance for the elderly and the dental professionals who participate in the oral hygiene service. It carried out the effective duty performance for the elderly in a situation of long-term care protection. Thus, the purpose of this study was to contribute to the early settlement in a system of long-term care insurance for the elderly. The following conclusions were obtained as a result of having carried out self-administered questionnaire research targeting 238 people from August 1 to August 30 in 2008. 1. In the general characteristics of the research subject, the present working place was indicated to be 22.7% for dental hospital, 71.8% for dental clinic, and 5.5% for others. As for the main duty field, the medical treatment & cooperative duty was indicated to be the highest with 61.8%, and was statistically significant(p=0.000). 2. The necessity for a system of long-term care insurance for the elderly was indicated to be 77.7% for 'necessary' and 1.7% for 'unnecessary,' and was statistically significant(p=0.016). 3. In the item of dividing the service of long-term care insurance for the elderly, the dental hygienists showed higher recognition than non-dental hygienists, and indicated significant difference(p=0.010). 4. As for recognition on a system of long-term care insurance for the elderly in dental professionals who responded as saying of 'knowing name and contents' about a system of long-term care insurance for the elderly, the recognition level was high in recognition of subjects' age(p=0.000), division in services(p=0.012), contents in at-home care service(p=0.000), execution in oral-hygiene service(p=0.004), procedure of using the long-term care insurance for the elderly(p=0.016), item of judging grade of long-term care insurance for the elderly(p=0.013), medical charge by service according to judging grade of long-term care insurance for the elderly(p=0.015), burden of cost for a system of long-term care insurance for the elderly(p=0.011), qualification of care staff(p=0.002), and contents of oral-hygiene service(p=0.027), and showed significant difference. 5. The service of long-term care insurance for the elderly and the oral-hygiene service indicated the statistically significant correlation. Accordingly, all of dental professionals need to make a desperate effort to improve dental professionals' knowledge on a system of long-term care insurance for the elderly enough to be required a system of long-term care insurance for the elderly. The more systematic and standardized professional education and materials are thought to be needed to be developed aiming at the success in oral-hygiene service within a system of long-term care insurance for the elderly, by strengthening professionalism in dentists and dental hygienists.
This study was attempted to use as basic data for developing a nursing intervention program suitable for terminal cancer patients by analyzing the unmet needs of advanced cancer patients receiving palliative chemotherapy and identifying the influencing factors. The subject of the study consisted of 71 patients with advanced cancer who were admitted to a university hospital and received palliative chemotherapy, and the research tool used CNAT (Comprehensive Needs Assessment tool in Cancer) developed by Shin et al., and data analysis was an independent sample. It was analyzed by t-test, ANOVA and multiple regression analysis. As a result of the study, the medical staff's area was the highest at 2.34 ± 0.63 out of 4 points, followed by psychological problems (2.09 ± 0.53) and information education (1.69 ± 0.45). Factors influencing unmet needs include educational background (β=.207, p=.047), thoughts on death (β=-.269, p=.018), diagnosis (β=-.283, p=. 013). Based on the results of this study, it is necessary to consider the needs of the medical staff in advanced cancer patients, provide psychological support, and consider ways to meet the needs according to academic background and diagnosis.
Purpose: The purpose of this study is to examine the level of understanding of hospice palliative care (HPC) and shared decision making (SDM) among middle-aged adults. Methods: Data were collected from 90 middle-aged adults living in D city using a self-reported questionnaire. The SPSS program was used to analyze the data. Results: Among the participants, 76.7% were aware of the HPC while 82.2% of the participants were not aware of SDM. Among all, 85.6% responded positively for needs of HPC, and 77.8% of participants for SDM. Participants with Christian faith (Protestants and Catholics), high income level and present illness were better aware of HPC than others. The most needed services were nursing care and treatment along with systematic counseling and explanation provided by the medical staff. There was also a significant relationship between the HPC factors, gender, income, current health status and current illness status. Participants with high income level were well aware of SDM. Stronger SDM needs were observed among participants with professional jobs or current illness. Conclusion: For continued discussion on HPC and SDM, it is necessary to implement and promote various education programs for medical staff and the public.
Purpose Yttrium-90 (Y-90) is high-energy beta emitters ($E{\beta}$, max = 2.28 MeV) with the mean penetration depth of 2.5 mm in tissue. Radioactive microspheres containing Y-90 is widely used for the transarterial radioembolization of hepatocellular carcinoma. However, bremsstrahlung radiation from Y-90 can cause the external radiation exposure to medical staff who handle the Y-90 microspheres. In this study, shielding device for Y-90 microspheres was developed to minimize the external radiation exposure. Materials and Methods Y-90 microsphere shielding device was made from 6 mm thicknesses of tungsten including the lead glass window. Radiation shielding ability of Y-90 microsphere shielding device was evaluated using 4 GBq of $SIR-Spheres^{(R)}$ Y-90 microspheres. The bremsstrahlung radiation was measured using radiation survey meter. Results The mean radiation dose of Y-90 microspheres in acrylic shield was $261.7{\pm}2.3{\mu}Sv/h$ (n=5) at 10 cm away from the shield. With the additional tungsten shielding device, it was $23.7{\pm}1.3{\mu}Sv/h$ (n=5). Thus, the bremsstrahlung radiation dose was decreased by 90.9%. At 50 cm away from the shield, bremsstrahlung radiation was reduced by 89.2% after using tungsten shielding device. Conclusion During the preparation and radioembolization of Y-90 microsphere, medical staff are exposed to external radiation. In this study, we demonstrated that the use of tungsten shielding device devices significantly reduced the amount of bremsstrahlung radiation. Y-90 microsphere tungsten shielding device can be highly effective in reducing the bremsstrahlung radiation.
Background: Breast cancer is the leading cause of cancer deaths among women worldwide. High breast cancer mortality has been attributed to lack of public awareness of the disease. Little is known about the level of knowledge of breast cancer in Central African Republic. This study aimed to investigate the knowledge of health professionals on breast cancer. Materials and Methods: This cross-sectional study was conducted among 158 health professionals (27 medical; 131 paramedical) in 17 hospitals in Bangui using a self-administered questionnaire. Descriptive statistical analysis, Person's ${\chi}^2$ test and ANOVA were applied to examine associations between variables with p < 0.05 being considered significant. Results: Data analyzed using SPSS version 20 indicates that average knowledge about breast cancer perception of the entire population was 47.6%, diagnosis method 45.5%, treatment 34.3% and risk factors 23.8%. Most respondents (65.8%) agreed that breast cancer is important in Central African Republic and that family history is a risk factor (44.3%). Clinical assessments and mammography were considered most suitable diagnostic methods, and surgery as the best treatment. The knowledge level was significantly higher among medical than paramedical staff with regard to risk factors, diagnosis and treatment. However the trainee group had very high significant differences of knowledge compared with all other groups. Conclusions: There is a very urgent need to update the various training programs for these professionals, with recommendations of retraining. Health authorities must create suitable structures for the overall management of cancer observed as a serious public health problem.

Purpose: This study was done to clarify effects of bispectral index monitoring sedative administration, compared to MOAA/S (Modified Observer's Assessment of Alertness and Sedation), on conscious sedation, physiological stability and recovery time for patients undergoing endoscopic submucosal dissection. Methods: Participants In this study were patients who underwent endoscopic submucosal dissection because of early gastric cancer. Participants were assigned randomly to an experimental group receiving sedatives based on bispectral index monitoring or to a control group with the MOAA/S instrument. Movements, belching, memory, pain, discomfort, physiological stability (MBP, PR, $SpO_2$), and recovery time were measured during the treatment and recovery. Data were analyzed using Spearman partial correlation coefficient analysis, Mixed model and Wilcoxon rank sum test to determine differences in the parameters. Results: There were no statistically significant differences between the two groups for conscious sedation(movement, belching, memory, pain, or discomfort), physiological stability and recovery time. Conclusion: The results of this study indicate that no differences were found between the two types of monitoring. Thus, use of a bispectral index monitor in clinical practice enabling medical staff to readily assess the conscious sedation of for these patients is expected to be increasingly used as an objective assessment tool for conscious sedation for patient safety.

Purpose: The purpose of this study was to identify characteristics of serum ferrum, TIBC and ferritin's circadian rhythm in normal adults and to prepare a standard to determine the examination material extraction time. Method: Nine women and ten men made up the convenience sample for this study they were from the staff of D university hospital and students in D medical School located in K city who met the qualifications for inclusion in the sample. The value of serum ferrum, TIBC and circadian rhythm were calculated as follows : First. each variable's amplitude. the acrophase and average were measured for a 24 hour cycle using the cosinor method, and then each person's rhythm was analyzed. Results: There were significant serum iron circadian rhythm for both men and women (p<.05). For the men, mesor was $105.91{\mu}g/dl$. amplitude was $29.52{\mu}g/dl$, and the acrophase was 9.76 hour. For the women, mesor was $108.17{\mu}g/dl$, amplitude was $28.09{\mu}g/dl$, and the acrophase was 11.42 hour The rhythm change of TIBC was only significant for the women (p<.05), mesor was 383.39mg/dl, amplitude was 60.29mg/dl. and the acrophase was 14.93hour. As for the circadian rhythm of the ferritin, there are no diurnal variation in either sex, men were between 134.0ng/ml and 137.4ng/ml, and women, between 29.1ng/ml and 30.1ng/ml. Conclusion: To help diagnose the boundary line between normal or deficiency in iron, measurement should be carried out at a fixed time in the morning and evening, or a more proper time would be in the afternoon at the time when the width of amplitude is the least.
Purpose: This retrospective study was done to evaluate the status of nosocomial urinary tract infections and to determine the risk factors and transmission route of causal IRPA through molecular epidemiology. Method: Two hundred ninety-nine of 423 patients admitted to the internal medicine and surgery ICU at a university hospital incity B had a positiveurine culture. Twelve of the 299 patients who had a urinary tract infection had IRPA strains. The data was collected from November 1, 2004 to January 31, 2005. The following results were obtained after the data was analyzed using percentile and UPGMA. Result: The rate of nosocomial urinary tract infections in the ICU was 10.8%. Therewere 16.8 cases of infection based on the period of hospitalization. There were 16.9 cases of infection based on the use of a foley catheter. The rate of nosocomial urinary tract infection in the ICU and urinary tract infections related to IRPA were higher in patients with the following characteristics: men, old age, admission through the emergency room, longer than seven days admission, severity of admitting causes, disturbance of consciousness, hydration less than 300cc in 24hours, a long course of antibiotics, a long period of foley catheterization and perineal care. Most of the microorganisms that caused the urinary tract infection were gram negative bacilli, among which P. aeruginosa was found in 70 patients (18.5%) and IRPA in 12 (4.0%). Among the 12 IRPA strains that were tested with PFGE, eight showed a dice coefficient higher than 80%, suggesting a genetic relationship. They were related with the period of hospitalization in the same ICU. These patients all received direct care for a urinary tract infection. Conclusion: Through these results, IRPA can be consideredas a contributing factors to urinary tract infections thus, active preventative measures are needed by the medical staff.

The purpose of this study was to identify the relationship between the nursing organizational characteristics and job satisfaction. Subjects were 362 nurses at a general hospital in Seoul. Data were collected from November 1, 1999 to November 31, 1999 with the structured questionnaires and analyzed by $SPSS-PC^+$ 8.0 for descriptive statistics, ANOVA, Pearson correlation, and stepwise multiple regression. The results were as follows; 1. The average score of the nursing organizational characteristics was 3.7. The score of head nurse's leadership was highest and supervisor's was lowest. The average score of the job satisfaction was 3.1 and was similar to the results of other studies. 2. Being regarded to the nursing organizational characteristics and job satisfaction by general demographic variables, The head nurse's leadership, the supervisor's leadership and interaction, and the overall nursing organizational characteristics showed significant differences by year of career, position status, and service-department(p<.1, p<.05). The job satisfaction showed significant difference by position status(p<.01). 3. There were correlations between the nursing organizational characteristics and job satisfaction(r=0.358-0.815). 4. The most important variable in the job satisfaction was 'organizational climate' and it was explained 32.0% out of the total variance of job satisfaction and 'all of nursing organizational characteristic factors' explained 47.5%. According to the results of this study, the suggestions were as follow: 1. To improve nurses' job satisfaction, the strategy for the development of the cooperative nursing organizational climate and for the strengthening of 'the nursing manager's leadership and interaction' in the nursing department are necessary. It should to be considered the differences in the nursing organizational characteristics and job satisfaction by nursing manager and staff nurses. 2. Further study needed to analysis the organizational climate for the management of the nursing organization. Also it is necessary for further investigation regarding to the relationship between the nursing organizational characteristics and job satisfaction of pre and post-organizational change.
