The Effect of Medical Service Quality and Hospital's Reputation on Customer Satisfaction, Repurchase Intention, and Negative Word of Mouth as to Disease Severity (질병의 중증도에 따른 의료서비스 품질과 병원의 명성이 고객만족과 재구매의도, 그리고 부정적 구전에 미치는 영향)

  • This model was empirically developed to test the effect of medical service quality and hospital's reputation on customer satisfaction, repurchase intention, and negative word of mouth as to disease severity. The model was tested in the context of the hospital industry. The findings are as follows. First, medical service provider's functional quality and technical service quality have significant effect on customer satisfaction. Second, hospital's reputation has positive(+) effect on customer satisfaction and significant negative effect on negative word of mouth. Third, customer satisfaction with medical service quality has significantly positive effect on customer's repurchase intention and has negative(-) effect on customer's negative word of mouth. Furthermore, customer's negative word of mouth has negative effect on their repurchase intention. Fourth, as to different disease severity, medical service quality and hospital's reputation have different effect on customer satisfaction, repurchase intention, and negative word of mouth. When patients get slightly ill, functional service quality and technical service quality have direct influence on customer satisfaction which has positive influence on repurchase intention and negative influence on negative word of mouth. Finally, negative word of mouth has negative effect on customer's repurchase intention. However, while hospital's reputation doesn't have effect on customer satisfaction, the reputation has significantly negative effect on negative word of mouth. When patients get seriously ill, only functional service quality has positive effect on customer satisfaction which influences on customer's repurchase intention and negatively influences on negative word of mouth. On the contrary, negative word of mouth doesn't influence on customer's repurchase intention as patients want to treat serious diseases in the large general hospitals even though negative word of mouth is known to them.

Impact of an emergency department resident strike during the coronavirus disease 2019 (COVID-19) pandemic in Daegu, South Korea: a retrospective cross-sectional study

  • Background: To prepare for future work stoppages in the medical industry, this study aimed to identify the effects of healthcare worker strikes on the mortality rate of patients visiting the emergency department (ED) at six training hospitals in Daegu, South Korea. Methods: We used a retrospective, cross-sectional, multicenter design to analyze the medical records of patients who visited six training hospitals in Daegu (August 21-September 8, 2020). For comparison, control period 1 was set as the same period in the previous year (August 21-September 8, 2019) and control period 2 was set as July 1-19, 2020. Patient characteristics including age, sex, and time of ED visit were investigated along with mode of arrival, length of ED stay, and in-hospital mortality. The experimental and control groups were compared using t-tests, and Mann-Whitney U-test, chi-square test, and Fisher exact tests, as appropriate. Univariate logistic regression was performed to identify significant factors, followed by multivariate logistic regression analysis. Results: During the study period, 31,357 patients visited the ED, of which 7,749 belonged to the experimental group. Control periods 1 and 2 included 13,100 and 10,243 patients, respectively. No significant in-hospital mortality differences were found between strike periods; however, the results showed statistically significant differences in the length of ED stay. Conclusion: The ED resident strike did not influence the mortality rate of patients who visited the EDs of six training hospitals in Daegu. Furthermore, the number of patients admitted and the length of ED stay decreased during the strike period.

Correlation between shift work and non-alcoholic fatty liver disease among male workers in the steel manufacturing company of Korea: a cross-sectional study

  • Background: Circadian rhythm disturbance caused by shift work has adverse effects on the metabolic homeostasis of the liver. Disruption of the metabolic homeostasis of the liver causes fat accumulation in the liver. The aim of this study was to investigate the correlation between shift work and non-alcoholic fatty liver disease (NAFLD) among male workers in the steel manufacturing industry of Korea. Methods: Based on medical examination data collected in June 2020, 2,511 male subjects from one steel manufacturing company in Korea were selected in total. NAFLD was evaluated using abdominal ultrasound, which was performed by two experienced radiologists. The multinomial logistic regression analysis was performed by adjusting for age, physical activity, smoking history, alcohol consumption, body mass index, waist circumference, blood pressure, blood glucose, lipidemia, liver function test, employment duration, and hepatotoxic materials exposure status. Results: Compared to daytime workers, the odds ratio (OR) of moderate-severe NAFLD in shift workers was 1.449 (95% confidence interval [CI], 1.028-2.043). Compared to daytime workers, the ORs of moderate-severe NAFLD were significantly higher for the group that engaged in total shift work for more than 20 years (OR, 2.285; 95% CI, 1.051-4.970), the group that was not allowed to sleep during night shift work (OR, 1.463; 95% CI, 1.030-2.078), and the group that consumed food during night shift work (OR, 1.580; 95% CI, 1.093-2.284). Conclusions: There was a correlation between shift work and moderate-severe NAFLD in male steel manufacturing workers. There will be a need for more research related to the correlation of shift work with steatohepatitis and cirrhosis in the future.

  • The pre-existing medical treatment was done in person between doctors and patients. EMR (Electronic Medical Record) System computerizing medical history of patients has been proceed and has raised concerns in terms of violation of human right for private information. Which integrates "Identification information" containing patients' personal details as well as "Medical records" such as the medical history of patients and computerizes all the records processed in hospital. Therefore, all medical information should be protected from misuse and abuse since it is very important for every patient. Particularly the right to privacy of medical record for each patient should be surely secured. Medical record means what doctors put down during the medical examination of patients. In this paper, we applies fingerprint identification to EMR system login to raise the quality of personal identification when user access to EMR System. The system implemented in this paper consists of embedded module to carry out fingerprint identification, web server and web site. Existing carries out it in client. And the confidence of hospital service is improved because login is forbidden without fingerprint identification success.

  • By using the U-Healthcare environment, it is possible to receive the health care services anywhere anytime. However, since the user's personal information can be easily exposed in the U-Healthcare environment, it is necessary to strengthen the security system. This thesis proposes the technique which can be used to protect the personal medical records at hospital safely, in order to avoid the exposure of the user's personal information which can occur due to the frequent usage of the electronic chart according to the computerization process of medical records. In the proposed system, the following two strategies are used: i) In order to reduce the amount of the system load, it is necessary to apply the partial encryption process for electronic charts. ii) Regarding the user's authentication process for each patient, the authentication number for each electronic chart, which is in the encrypted form, is transmitted through the patient's mobile device by the National Health Insurance Corporation, when the patient register his or her application at hospital. Regarding the modern health care services, it is important to protect the user's personal information. The proposed technique will be an important method of protecting the user's information.

The Impact of Health Service Quality Factors on Patient's Satisfaction According to Duration of Hospitalization (입원기간에 따른 의료서비스 품질요인이 고객만족에 미치는 영향에 관한 연구)

  • The purpose of this study is to identify which factors of medical service quality were considered important by patients based on survey on eight hospitals located in Seoul. We analyzed the difference in impact of health service quality factors between the two groups with short and long inpatient days. In addition, we analyzed which service factors affected the customer satisfaction more differently across the inpatient groups. We found that patient satisfaction was not different between the two groups with short and long inpatient days. But the two groups showed different priority orders of medical service factors that affect patient satisfaction. In the group with long inpatient days, the order of medical service factors are cleanness, speciality, process. In the group with short stay, important factors are speciality, cleanness and convenience of facilities. These findings imply that hospitals should set up different marketing strategies across the different inpatients groups.

Implementation of hospital information system using the HL7/DICOM gateway (HL7/DICOM 게이트웨이를 활용한 병원정보시스템 구축)

  • With the rapid advance of information technology and the popularity of digital medical, medical information are growing, the process of hospital digitalization are gradually quicken and systems involved are more and more. HIS system mainly deals with patient information, it follows the standard of HL7. While PACS system mainly manages image information, it follows the standard of DICOM. Because HIS and PACS deal with different information and follow different standards, it is difficult to directly communicate. This paper presents a method of establishing HL7/DICOM gateway to realize the information exchange of HIS and PACS. The HL7/DICOM gateway designed in this paper is made up of three modules. First, HL7 messages and triggered events are designed to achieve the function of transaction processing module. According to the designed HL7 messages, four message interface functions are defined to accomplish the function of send/receive messages module. Finally, the algorithm of construct/parse messages is given to complete the function of construct/parse messages module. Result shows that HIS and PACS realize information exchange successfully by using the HL7/DICOM gateway Implemented in this paper.

A Comparative Study on Sanitary Practices and Perception of Employees in Elementary School, Hospital and Industry Food Service in the Inchon Area (인천지역 단체급식소별 조리종사자의 위생실천도 및 위생지식 비교 조사)

  • The purposes of this study were to compare performance of HACCP-based sanitary management and sanitation knowledge of employees in some food services (hospitals, elementary school, industry) in Inchon. Therefore, the survey questionnaire consisted of general background, sanitation performance, sanitation knowledge evaluation. The subjects were 370 employees in hospital, elementary school, industrial food service. The statistical analysis of data was completed using SPSS program. The results were summarized as follows: 99.3% of surveyed employees were female. 95.7% of employees were attended sanitation education and 76.8% of them were educated once a month. Employees in school food service showed higher academic career than the other food service employees. The food service employees' performance level was high(4.48). The performance level of school employees was significantly higher compared to other food services(p<0.001). Average score of sanitation knowledge was 16.36/20. The school food service employees' knowledge 17.03 was also higher compared to other food services. There was no significant correlation between sanitary performance and knowledge for total score. But among items of sanitary performance, a personal hygiene and food sanitary was significantly correlated with sanitation knowledge. The results of survey imply that the suitable contents and methods of education and training must be developed. Also, Financial supports to install necessary sanitary facilities are very important in order to raise effectiveness of education.

A System Approach to the Framework of Medical Tourism Industry (의료관광산업의 구조에 대한 시스템 접근법)

  • Purpose: The purpose of this research is to develop two medical tourism system models which explain medical tourism phenomenon with a systemic approach. Methodology/Approach: This research was conducted using a qualitative data analysis which mainly refer previous references in relation to medical tourism in the areas of tourism and medicine. Leiper's tourism system model was utilized as a conceptual framework. In-depth interviews with experts in the area were attempted in order to pretest the models. Findings: This research suggests a medical tourism system framework and a medical service provision framework. The first model presents medical tourism components and their relationships within a framework presented in a diagram. The second model shows the relationships among medical services required by medical tourists, the service providers, and service human resources along with movements of medical tourists. Practical Implications: The first model presents a spatial composition of medical tourism components and their relationships, whereas the second model shows the linkage among medical services, the service providers, and relevant service human resources along with time sequential steps of medical tourists. These two models are complementary and may be used as useful tools to observe medical tourism phenomenon with a systemic and holistic approach. These two models may enable stake holders avoid unnecessary confusions and conflicts that result in duplication of government policies and a waste of budget and human resources.

Preliminary Analysis on Strategic Planning to Enter Chinese health Care Market: Focusing on SWOT-AHP Analysis (중국 의료시장 진출 전략 기획을 위한 기초 분석 연구: SWOT과 AHP 분석기법을 중심으로)

  • Objectives: The purpose of this study was to identify the intrinsic strengths and weaknesses, as well as opportunities for success and threats of Korean health care industry when planning strategies to enter Chinese health care market. Methods: To suggest directions for planning the strategies, a SWOT(Strengths, Weaknesses, Opportunities, Threats) analysis was used in combination with an AHP(Analytic Hierarchy Process) in this study. A total of twenty SWOT factors and the relative weight of SWOT groups were examined through a survey on the respondents who have work experiences in the area of entering Chinese market. Results: "Geographical proximity between Korea and China" was recognized as a key strength. "Absence of success case in medical service industry" was selected as a critical weakness. "Increase in demand for advanced medical services in China" was identified as an important opportunity. "Difficulty in establishing Korea-China partnership" was considered as a major threat. The respondents prioritized weaknesses, followed by threats, opportunities, and strengths when conducting strategic planning to enter Chinese health care market. Conclusions: Improving local hospital management plan as well as specializing in certain health care services may be necessary.
