Background: Korean Hospital Standardization Program(KHSP), as a hospital accreditation program initiated and promoted by Korean Hospital Association since 1981, has played a key role in the hospital quality improvement in structural aspect particularly. Recently, however, KHSP has been critisized by hospitals and health personnels in that it is unpractical to be utilized as an initiative to improve hospital quality. In particular, the section of quality assurance in KHSP, strongly influenced by old version of Hospital Accreditation of Joint Commission in U.S.A., has been required to be fundamentally revised. For design of new criteria in quality assurance section, a survey for evaluation of existing program and collection of opinions for ideal one was conducted. Methods: For a month in 1994, structured questionnaires were administered by mailing to 470 hospital staffs from 200 hospitals, participated in the survey for the 1994 KHSP in each hospital. The total number of respondents was 116. Results: Less than half of the respondents(34.5%) value positively on the impact of current KHSP in general on the quality improvement of their hospitals. Moreover, most responses indicated that KHSP should be reorganized towards more practical and applicable one. Current KHSP criteria for quality activities in hospitals were regarded as a unpractical one which should be basically renewed. For new criteria and standards, most respondents emphasized the importance of applicability of those in real situations. Conclusion: For the KHSP to be effective, new evaluation criteria for quality activities should be more practical and fully accommodated to hospital situations in reality.
To evaluate the effects of critical pathway on the length of hospital stay, the cost and quality of care provided to gastrectomy patients in a tertiary care academic medical center. The basic research design is a retrospective case-control comparative study. 470 patients of gastrectomy were included in the study; 180 before pathway development (control), 163 right after pathway implementation (path group I) and 127 one year after pathway implementation (path groupII). 476 patients of modified radical mastectomy were also analyzed to examine whether the reduction of the length of hospital stay is from the hospital-wide trend or due to the critical pathway. Death after operation, ICU stay, unplanned re-operation, readmission after discharge, the length of hospital stay and cost were analyzed. 2-test, one-way ANOVA, Bonferroni and Turkey's test were used for statistical analysis. (1) There were no significant differences in patient clinical conditions and no sign of deterioration of quality from critical pathway. (2) The length of hospital stay was 13.0 days in control group, 12.2 days in path group I and 10.0 days (p<0.01) in path groupII. (3) The total costs during the hospital stay were reduced. However the cost per day was significantly increased from reduction of hospital stay (358,488won in control, 366,017won in path group I and 413,220won (p<0.01) in path groupII). Critical pathway reduced the length of hospital stay, total hospital costs and resource utilization without harming quality of patient care.
Sa-am acupuncture originated in the Chosun Dynasty and is a distinct feature of Korean medicine. It has been used to treat various diseases and conditions in clinical practice however, there is insufficient scientific evidence to support the use of Sa-am acupuncture. We aimed to comprehensively review the clinical studies of Sa-am acupuncture retrieved from national and international databases (MEDLINE, EMBASE, the China National Knowledge Infrastructure, and 3 Korean databases). There were 52 articles reviewed including 29 case studies, 19 randomized controlled trials (RCTs), and 4 uncontrolled trials. Neurological disorders were the most frequently studied, and kidney tonification, and directional supplementation and draining were the most frequently used methods. Overall, the outcomes were generally positive however, there were many additional treatments together with Sa-am acupuncture reported in the case reports, and the quality of evidence was low in the RCTs. Future studies should report the detailed method of practicing Sa-am acupuncture treatment and focus on the specific effect of Sa-am acupuncture with rigorous design to scientifically support the clinical use of Sa-am acupuncture.
Purpose: This study was conducted to design services for improving the quality of care of hospitalized children with acute diseases. Methods: The service design process had four phases: discovery, definition, development, and delivery. The participants were 23 mothers of hospitalized children with an acute disease, and seven nurses and three doctors working at a pediatric hospital. Data were collected through self-report questionnaires, in-depth interviews, and observations. The data were analyzed using content analysis and descriptive statistics. Results: The participants reported needs for explanations about the treatment, skillful nursing, and environmental improvements. The concept of the services was familiarity and enjoyment, aimed at solving the problems of unfamiliarity and boredom. A six-guideline was presented for improving the quality of care of hospitalized children with acute diseases: improvement of awareness, development of educational materials, improvement of skills, environmental improvements, play activities, and evaluations of user satisfaction. Conclusion: These findings indicate that nursing services should deliver familiarity and enjoyment to hospitalized children and their families. The findings of this study emphasize that the service design methodology can be used to improve the quality of care of hospitalized children with acute diseases.

Purpose: The planning and design of hospital generally requires the participation and consultation of skilled experts since it has more complex space program than any other buildings. Therefore, the BIM systems for the planning of hospital have been tried continuously. The purpose of this study is to identify the precondition for space Program validation of healthcare architecture based on BIM, which is recently receiving wide attention. Method: For this study, United States, Australia and Finland's guidelines were analyzed among the description space program validation system in 14 overseas BIM Guidelines. And the propose precondition that can be applied to healthcare architecture from among these description of space program validation items, target, process etc for General building. Result: 1) spatial program validation is the following four evaluation phase. Step 1: Standard setting phase Step 2: BIM model accuracy assessment phase Step 3: space validation phase Step 4: Performance evaluation phase 2) The standards for the building elements at Standards Setting stage is considered to the standards for the architectural elements of General building. 3) Healthcare Architecture Area calculation method is considered to be reasonable that borrowing the area calculation standard of general architecture according to the UIA of international standards. However, Be proposed of measuring method that reflect the efficiency of the design process step-by-step area calculation method. The performance assessment indicators of reflect the Hospital uniqueness have to developed. And the research needs to be carried out continuously according to the purpose for healthcare architecture of feature-oriented. Implications: In this paper like to understanding that precondition of space program validation considering the BIM. As a result, understanding to condition about step of the evaluation, the evaluation standards. Is expected to keep the focus on the development of performance indicators that reflect the uniqueness of the hospital for the efficient evaluation of the Hospital building.
In South Korea, hospital clothes for medical staff, which require high performance and functionality, are still regarded just as uniforms. Few researches have been made into the underwear for operating room staff, which is intended to protect medical staff against possible risks happening during an operation and allow them to do their job in an easy and comfortable manner. In order to present a new design of underwear for operating room staff at general hospitals in South Korea, an investigation was made into medical staff's preference for overseas brand samples, and it was found that they preferred to wear such clothes as are blue-colored, of a V-neck line, of a box style and has good absorptiveness and breathability. After developing the primary prototype and the conventional underwear for operating room staff at Hospital A, their appearances were evaluated, and it was found that the primary prototype was more favorably evaluated. After developing the secondary prototype, their preference for the colors and shapes was surveyed, and it was found that they preferred to wear No.6 blue colors and T/C materials. In conclusion, it is considered that a systematic research need be conducted into the designs of various hospital clothes, whose roles and functions are subdivided, including the underwear for operating room staff developed in this research.

Purpose : The purpose of this study was to examine the nurses' needs for the spatial composition of geriatric hospital. Methods : The survey method was used for collecting the data. The subjects were 110 nurses who worked at 6 geriatric hospitals in Busan. They answered the questionnaire by the self-administered method. The questionnaire consisted of items which asked the spatial composition of patient room, bathroom, dining space, resting space, care-givers' room, and garden. Results : 1) The patient room using a bed on ondol floor would be useful. Also, it would be desirable that most patient rooms consisted of 4 or 6 persons per patient room and the others were 1 or 2 persons per room. 2) The bathroom for only patients and the shower room for their families or care-givers are needed in the geriatric hospital. 3) The dining room for patients in each floor would be useful to old patients. In addition to that, the cafeteria for their families or care-givers are needed. 4) The resting space for patients and their families, for the staff, and for care-givers are needed. 5) The personal locker and refrigerator are necessary fixtures in care-givers' room. Also, table, sofa, sink, and shower booth are also needed. 6) On the base of activities of care-givers, the space for preserving, washing, and heating foods are needed. Also, the storage space for personal items and the fitting space are needed. Implications : The results of this study would be the fundamental data for space planning of the geriatric hospital.

This study is aimed at investigating and comparing the differences in the uniform of nurses according to regions and hospital size in Korea. This study takes two large regions as its survey area; one is Seoul, Incheon and Gyeonggi Province, and the other is Daejon and Chungcheong Province. The hospital size is divided into general hospitals and neighborhood hospitals. The survey was conducted among the nurses working in 12 randomly-selected hospitals and its statistical results are analysed firm 818 respondents'questionnaires. The results are divided into 3 categories as follows; 1) Results of analysis of survey on the uniform of nurses : Nurses usually wear two-piece suits and more than half of the hospitals choose a white-color pattern. And the nurse's uniform with color varying according to their position and ward is considered to be desirable; 2) Analysis on regional differences : In the case of Daejon and Chungcheong Province, there is a relatively high ratio of respondents who answered questions with 'high percentage of young nurses', 'position differentiation by means of different design and fabric according to ward'. In case of Seoul, Incheon and Gyeonggi Province, questions with 'use of a patterned textile in nurses'uniform', 'ready-made size system'are higher; 3) Analysis of the differences in accordance with hospital size : In the case of general hospitals, there is relatively a high ratio of respondents who answered questions with 'use of different design and fabric according to a nurse's position', 'use of patterns in nurses'uniform'. In the case of neighborhood hospitals, there is a high ratio of respondents with'use of classical one-piece or two-piece type wear', 'ready-made type size system', 'obligatory use off nurse's cap'.

Medical Gateway(MG), that integrates medical devices with its unique data format in various hospital information systems, provides effectiveness in the cost of the management unlike existing systems that uses PC as a gateway to hospital information systems because MG enables to interface medical devices with itself. However, in spite of using MG for integrating medical devices, we need additional programs or systems to interwork with EMR system of each hospital in accordance with the configuration of DB. In this paper, we design and implement the format of medical information and server system for interworking MG with EMR system. The proposed system makes easy exchanging medical data between MG and EMR system. Thus, the proposed system can contribute to a national project to export abroad advanced hospital setup converged with IT.

Suicide, the fourth leading cause of death in Korea, is a serious national problem. The Korea National Suicide Survey (KNSS) is the result of the first legislation to address this issue, "Article 11 of THE ACT FOR THE PREVENTION OF SUICIDE AND THE CREATION OF A CULTURE OF RESPECT FOR LIFE" (the "Act for the Prevention of Suicide"). To overcome the limitations of previous studies, the KNSS was designed by collaborators from a variety of fields : psychology, epidemiology, social welfare, and psychiatry. The KNSS was composed of four substudies that addressed the multifaceted process involved in suicide-related behavior over time, exploring general attitudes toward suicide and suicidal ideation, suicide planning, suicide attempts, and completed suicides. Study 1 examined the risk factors for suicide based on data regarding completed suicides ; Study 2 adopted the approach of a psychological autopsy ; Study 3 focused on suicide attempters ; and Study 4 explored attitudes toward suicide in the general population. The KNSS was designed to provide a comprehensive understanding of suicide from a longitudinal, multifaceted perspective to serve as a basis for policies aimed at suicide prevention.
