임상간호사의 병원폭력에 대한 태도 측정도구 개발 (Development and Validation of the Hospital Violence Attitude Scale-18 (HVAS-18) for Clinical Nurses)

  • Purpose: The purpose of this study was to develop and validate a Hospital Violence Attitude Scale-18 (HVAS-18) for clinical nurses. Methods: The HVAS-18 was developed and validated in 3 steps: Item generation through literature reviews and in-depth interviews, pilot study, and the validity and reliability tests using a test-retest technique. Forty-one items were initially extracted by 8 experts, and 18 items were finally developed by item and factor analysis. The final HVAS-18 was evaluated by 326 clinical nurses from seven general hospitals in three cities. The collected data were analyzed using factor analysis, Pearson correlation coefficient, and Cronbach's ${\alpha}$. Results: Five discrete factors emerged, which explained 64.0% of the total variance. Each five factor was labeled: Factor 1 (6 items) 'awareness'explained 18.2%; Factor 2 (4 items) 'response' explained 12.9%; Factor 3 (4 items) 'reaction' explained 12.9%; Factor 4 (2 items) 'result-nursing' explained 10.2%; and Factor 5 (2 items) 'result-violence offender' explained 9.6%. The internal consistency, Cronbach's ${\alpha}$, was .87, and reliability of the sub-scales ranged from .72 to .83. Conclusion: The results of this study indicate that HVAS-18 can be an useful, reliable, and valid instrument for measuring hospital violence attitude of clinical nurses.

수술실 간호사의 언어폭력경험, 사회적 지지, 대처양상이 업무 스트레스에 미치는 영향 (The Effects of Verbal Violence Experience, Social Support, and Coping Patterns on Job Stress among Operating Room Nurses)

  • Purpose: The purpose of this study was to understand the effects of verbal violence experience, social support, and coping patterns on job stress among operating room(OR) nurses. Methods: The participants were 214 operating room nurses in general hospitals. Data were collected from April 1st to May 1st in 2013 and were analyzed using a multiple regression. Results: The most influencing factor on job stress for OR nurses was verbal violence experience from doctor(${\beta}=.35$), and verbal violence experience from nurse (${\beta}=.27$) followed. Social support from co-workers was a significant factor in decreasing job stress (${\beta}=-.22$). Those three factors explained 43.0% (F=54.76, p<.001) of OR nurses' job stress. Conclusion: The findings of this study revealed that doctors' and nurses' verbal violence increased job stress of OR nurses and social support from co-workers was found decreasing OR nurses' job stress. The results of the study provides an necessity for developing an effective program for minimizing job stress of OR nurses caused by work place verbal violence.

응급실에서의 폭력 환자 예측을 위한 한국어판 행동신호 사정도구의 유용성 검증 (Usefulness of Korean Version of Behavioral Cue Checklist for Predicting of Patient Violence in Emergency Departments)

  • Purpose: This study aimed to evaluate the usefulness of a behavioral cue checklist (BCC) containing 17 items developed by Wilkes et al. (2010) for identifying potentially violent patients in emergency departments. Methods: This was a prospective observational study to evaluate the usefulness of the Korean version of a BCC (K-BCC) as an assessment tool for predicting patient violence in emergency departments, and was conducted over 4 weeks in a regional emergency medical center located in B City. A total of 1,324 patients were finally analyzed. Results: Logistic regression analysis was performed to investigate whether each item of the K-BCC predicts violence, and a parsimonious set of 8 statistically significant items was selected for the tool. Receiver operating characteristic analysis of the BCC showed that the area under the curve was .97 (95% confidence interval: .94~1.0). The sensitivity, specificity, positive predictive value, and negative predictive value at the cut-off score of 2 were 75.6%, 98.9%, 68.2%, and 99.2%, respectively. Conclusion: The K-BCC was found to be useful in predicting patient violence toward emergency department staff. This tool is simple, and fast to use and can play a significantly role identifying potentially violent patients. Owing to this advance identification, this tool can be helpful in preventing the potential for violence from manifesting as violent behaviors.

회복력 중이론을 기반으로 한 폭력대처 프로그램이 응급실 간호사의 회복력, 폭력대처, 간호역량과 소진에 미치는 효과 (The Effects of Violence Coping Program Based on Middle-Range Theory of Resilience on Emergency Room Nurses' Resilience, Violence Coping, Nursing Competency and Burnout)

  • Purpose: The purpose of this study was to examine the effects of a violence coping program (VCP) based on Polk's middle-range theory of resilience on nursing competency, resilience, burnout, and the ability to cope with violence in nurses working in emergency rooms. Methods: A quasi-experimental study, with a nonequivalent control group and a pretest-posttest design, was conducted. Participants were 36 nurses who worked in emergency rooms and had experienced violence; 18 nurses from D hospital and 18 nurses from C hospital were assigned to the experimental and control groups, respectively. The experimental group received the VCP twice per week for 8 weeks. Results: Levels of resilience, F=59.41, p<.001, active coping behavior, ${\chi}^2=33.09$, p<.001, and nursing competency, F=59.41 p<.001, increased significantly and levels of passive coping behavior, ${\chi}^2=22.92$, p<.001, and burnout, F=52.74, p<.001, decreased significantly in the experimental group. Conclusion: The results suggest that the VCP could be an effective strategy for reducing burnout and improving resilience, active coping behavior, and nursing competency. Therefore, it would be a useful intervention for improving the quality of nursing care provided in emergency rooms.

Effects of School Bullying Prevention Camp on the Adolescent Perpetrators of School Violence

  • Objectives: The purpose of this study was to evaluate the effects of a school bullying prevention camp for adolescent perpetrators of school violence. Methods: A total of 95 adolescents (boys=73, girls=22) were included in this study. The changes after the program were evaluated using several self-rating scales for the assessment of the degree of depression, loneliness, impulsivity, self-esteem, and interpersonal reactivity compared with the baseline scales. Results: The school bullying prevention camp significantly reduced the degree of depression severity (z=-5.874, p<0.001) and impulsivity scale (z=-3.861, p<0.001), while increasing self-esteem (z=-2.091, p=0.037). The degree of depression severity (z=-4.635, p<0.001), loneliness (z=-2.168, p=0.030), and impulsivity (z=-2.987, p=0.003) significantly reduced, whereas self-esteem significantly increased (z=-2.221, p=0.026) for male students who had no history of child abuse. However, there were no significant changes after the program for the degree of depression, loneliness, impulsivity, self-esteem, and interpersonal reactivity among female students with a history of child abuse. Conclusion: The results of our study suggest that the development of diverse standardized guidance programs is necessary for teenagers with different characteristics and backgrounds. Such programs are expected to be more effective in preventing school violence.

The relationship between workplace violence, depression, burnout, subjective health status, job and life satisfaction of physical therapists in South Korea

  • Objective: To investigate the physical therapists' actual experience of workplace violence and examine its relationship with depression, burnout, subjective health status, and job and life satisfaction. Design: Cross-sectional study. Methods: The level of workplace violence was assessed using the Korean Workplace Violence Scale composed of 24 questions in the following 4 subscales of workplace violence. Depression was assessed using the Korean version of the Patient Health Questionnaire-9. Burnout was assessed using the Korean version of the Maslach Burnout Inventory-Human Services Survey, which is a 5-item scale. Subjective health status was measured on a 5-point scale. Job and life satisfaction was measured based on the assessment tool used in the World Values Survey. Results: There was a significant correlation between the three subscales of workplace violence and health indicators, including depression, subjective health status, job and life satisfaction (p<0.05). The organizational protective system subscale showed a significant negative correlation with depression and a positive correlation with subjective health status as well as job and life satisfaction (p<0.05). The area of 'psychological and sexual violence from customers' showed a significant moderate correlation with depression and job satisfaction (p<0.05). Also, there was a negative correlation that was significant between depression and subjective health status, job satisfaction, and life satisfaction (p<0.05). Conclusions: This study suggested that future studies should continue to investigate and reveal the causal relationships between workplace violence and physical therapists' health indicators.

수술실 간호사의 언어폭력경험, 언어폭력충격, 및 소진간의 상관성에 대한 융합연구 (Convergence study on Relationship among Verbal violence experience, Verbal violence impact and Burnout in Operating Room Nurses)

  • 본 연구목적은 수술실 간호사의 언어폭력경험, 언어폭력충격 및 소진정도를 파악하고 그 상관관계를 분석하기 위함이다. 수술실 간호사 202명을 대상으로 구조화된 설문지를 이용하여 자료수집하였으며 분석은 기술통계, t-test, ANOVA, Scheffe test, Pearson상관계수 및 다중회귀분석으로 하였다. 연구결과에서 수술실 간호사들의 언어폭력경험수준은 평균 1.96점, 언어폭력충격수준은 평균 1.67점, 소진정도는 평균 3.08점으로 나타났다. 언어폭력경험수준은 언어폭력충격수준 및 소진정도와 유의한 정적 상관관계가 있었으며(r=.39, p<.001; r=.41, p<.001) 언어폭력충격수준도 소진정도와 유의한 정적 상관관계를 나타내었다(r=.29, p<.001). 소진정도에 영향을 미치는 요인은 언어폭력경험, 언어폭력충격수준 및 직위(일반간호사)였으며 이들 요인은 수술실 간호사의 소진정도를 30% 설명하였다(F=9.15, p<.001). 이상의 결과를 통해 수술실 간호사의 언어폭력경험은 개인적, 사회적 측면에서 안정감과 생산성을 위협하며, 의료현장에서의 언어폭력예방을 위한 중재방안의 모색이 필요함을 확인하였다.

정신과병동 간호사의 소진 영향 요인에 관한 연구 -폭력경험, 폭력대처, 사회적지지 (A Study on the Factors Influencing Burnout of Psychiatric Ward Nurses -Violence Experience, Violence Coping, Social Support-)

  • 본 연구는 폭력경험으로 인한 정신과병동 간호사의 소진을 감소시키고 문제 중심의 표준화된 폭력대처 중재방안을 수립하기 위해 폭력경험, 폭력대처, 사회적지지가 소진에 미치는 영향요인을 파악하기 위한 서술적 조사연구이다. 연구 대상자는 정신과 병원에서 근무하는 임상경력 1년 이상인 병동 간호사 204명이었다. 자료수집기간은 2018년 7월 1일부터 8월 31일까지 구조화된 설문지로 시행하였다. 수집된 자료는 SPSS program을 이용하여 평균, 표준편차, t-test, ANOVA, Pearson's Correlation coefficient, Multiple Regression으로 분석하였다. 연구결과는 다음과 같았다. 정신과병동 간호사는 언어적 폭력 92.2%, 신체적 위협 92.2%, 신체적 폭력 75.5%를 경험하고 있었다. 대상자의 폭력경험과 폭력대처(r=0.15. p<.027), 폭력경험과 소진(r=0.16, p<.017)은 정적 상관관계, 폭력대처와 사회적지지(r=0.30, p<.001)는 정적 상관관계, 사회적지지와 소진(r=-0.28, p<.001)은 부적 상관관계를 나타냈다. 또한 소진에 영향을 미치는 요인으로 간호직 만족도, 사회적지지, 신체적 폭력경험이 나타났다. 소진을 설명하는 회귀모형은 약 33.3%의 설명력을 나타내었다. 그러므로 정신과병동 간호사의 폭력경험을 낮출 수 있는 예방교육과 사회적지지 강화를 위한 체계마련 및 간호직 만족도를 향상시키는 환경을 조성한다면 소진을 감소시켜 양질의 간호를 제공할 수 있으리라 생각된다.

임상간호사의 폭력경험과 이직의도 관계에서 직무만족의 매개효과 (Mediation Effect of Job satisfaction in the Relationship between Nurses' violence experience and turnover intention)

  • 본 연구는 임상간호사의 폭력경험이 직무만족과 이직의도에 미치는 영향과 폭력경험과 이직의도 관계에서 직무만족의 매개효과를 파악하기 위해 시도되었다. U광역시 3개의 종합병원에 근무 중인 간호사 213명을 연구대상자로 하였으며, 자료수집은 2019년 2월부터 4월까지 하였다. 수집된 자료는 SPSS/WIN 25.0을이용하여 분석하였다. 폭력경험은 직무만족과 유의한 역상관관계를 나타내었고, 폭력경험은 이직의도와 정상관관계이었으며, 직무만족과 이직의도는 역상관관계를 나타내었다. 폭력경험이 직무만족을 설명하는 정도는 16.5%, 폭력경험이 이직의도를 설명하는 정도는 6.1%였다. 폭력경험과 이직의도 간의 관계에서 직무만족이 부분 매개를 하는 것으로 나타났다. 따라서 임상간호사의 직무만족도를 향상시키고 이직의도를 감소하기 위하여 폭력경험을 감소시켜야 할 필요성이 있다. 간호사의 폭력경험과 이직의도를 감소시키고 직무만족을 높이기 위해 병원에서 제도적 장치를 마련에 유용한 기초자료가 되리라 본다.

환자 및 보호자로부터 구급대원이 경험한 병원 전 응급의료현장 폭력실태 (From Patients and Caregivers 119 Rescue party's Experiences for Violence in the Pre-Hospital Emergency Medical Field)

  • 본 연구는 119 구급대원들이 경험하는 폭력실태와 그에 따른 반응을 조사하기 위해 2009년 1월 1일부터 3월 2일까지 전라북도 지역의 구급대원 226명을 대상으로 설문조사한 결과이다. 첫째, 병원 전 응급의료 현장의 구급대원 모두가 주 1회 이상의 폭력을 경험하고 있으며, 62%의 구급대원이 물리적 폭력에 노출되어 있다. 이러한 폭력발생의 환자측 요인으로는 약물 또는 음주가 56.2%를 차치하였으며, 구급대측 요인으로는 응급의료인력 부족이 36.3%였다. 둘째, 구급대원의 42%가 5년 이내에 폭력예방과 대처방법에 대한 교육 경험이 없으며, 77.4%의 구급대원이 향후 폭력예방과 대처 방법에 대한 교육을 원하였다. 셋째, 폭력반응 점수는 감정적 반응 3.20점, 사회적 반응 2.22점, 신체적 반응 2.17점으로 조사되었고, 전체 폭력 반응의 평균은 2.53점으로 조사되었다. 넷째, 일반적 특성에 따른 폭력 반응 점수는 신체적 반응(t=-2.08, p=.038)과 감정적 반응(t=-7.13, p=.000), 전체반응(t=-4.764, p=.000)에서 통계적으로 유의한 차이를 나타냈으며, 연령에 따른 감정적 반응(t=4.257, p=.006)과 전체(t=2.932, p=.034)에서 통계적으로 유의한 차이를 나타냈다. 또한, 현재의 보유 자격별 점수는 사회적 반응(t=3.661, p=.000), 전체(t=8.130, p=.000)에서 유의한 차이를 나타냈고, 소방서별 폭력 반응 점수는 신체적 반응(t=9.987, p=.000), 감정적 반응(t=2.433, p=.012), 사회적 반응(t=6.738, p=.000), 전체(t=5.943, p=.000)에서 유의한 차이를 보였다.
