An Experience of Management of Homeless Neurosurgical Patients

  • Objective : Homeless patients probably have epidemiologic features that are different from those of general population. However, there have been no published articles about clinical characteristics of neurosurgical homeless patients. The authors tried to assess the clinical characteristics and treatment outcome of homeless neurosurgical patients. Methods : We retrospectively reviewed the medical records and radiological films of 76 homeless patients and 72 non-homeless patients following head trauma who were admitted to our neurosurgical department between June 2001 and June 2005. We compared two groups of the patients with the demographics, the clinical and laboratory characteristics. Also, 3D-day mortality was determined according to Glasgow Coma Scale (GCS) score. Results : Age of homeless patients was younger than that of non-homeless patients. Homeless patients had previous craniotomy evidences in skull x-rays more frequently (10.5% vs. 1.4%). Acute subdural hematoma was the most common type of head injury in the two groups. Moderate and severe head injury, based on GCS score on admission was more frequent in homeless patients (64% vs. 39%). Fifty percent of homeless patients underwent operation for traumatic head injury. However, 3D-day mortality according to GCS score was not significantly higher in homeless patients. Conclusion : Most homeless neurological patients were relatively young men. Also, moderate or severe brain injuries were observed more frequently. However, mortality rate of homeless patients in neurosurgical field is not significantly higher in the present study.

행려환자의 응급의료기관 체류시간 분석을 통한 의료서비스 개선방안에 대한 연구 (The Study on Improving Medical Care Service by Analyzing the Time While the Homeless Patients Length of Stay Emergency Medical Institution)

  • 본 연구는 응급의료기관을 내원 한 행려환자들의 내원, 진료, 발생특성에 따라 체류시간을 살펴본 후 영향을 미치는 관련요인을 파악하여 향후 의료서비스 개선방안을 위한 기초자료를 제공하고자 하는데 그 의의가 있다. 조사대상은 2012년 1월 1일부터 동년 12월 31일까지 1년 동안 충남소재의 응급의료기관을 내원한 행려환자 691명으로 하였으며, 분석방법은 빈도분석, ANOVA, 상관관계분석 후 독립변수를 더 미화하여 다중회귀분석을 실시하였다. 결론을 보면 첫째, 의료기관은 행려환자에 대한 부정적인 인식에서 벗어날 필요가 있으며, 질 좋은 의료서비스 제공 및 응급진료지원체계를 갖추고 있어야 한다. 둘째, 행려환자의 대부분이 남자이고, 40,50대가 절반이상을 차지하고 있어, 이들이 사회적으로 아직까지는 생산성이 높은 연령층이라고 할 수 있으며, 향후 의료적지원 뿐만 아니라 사회복귀프로그램이 포함된 정부 정책의 관리구축 방안이 필요하다.

The nature of the homeless tuberculosis screening and directed to the distance homeless specific location are likely to evaluate the actual incidence low and aggressive or management needs, the rationale is allowed insufficient reality is. Through this research, future, for tuberculosis high risk tuberculosis patient, such as homeless to expand the tuberculosis screening of infectious tuberculosis patients in private medical institutions, and one-stop service that chest X-ray examination and sputum examination is carried out at the same time introduced immediately to prevent the inspection and examination, cure, and should establish a foundation that can be up to post administration.

A Retrospective Analysis of 303 Cases of Facial Bone Fracture: Socioeconomic Status and Injury Characteristics

  • Background: The incidence and etiology of facial bone fracture differ widely according to time and geographic setting. Because of this, prevention and management of facial bone fracture requires ongoing research. This study examines the relationship between socioeconomic status and the incidence of facial bone fractures in patients who had been admitted for facial bone fractures. Methods: A retrospective study was performed for all patients admitted for facial bone fracture at the National Medical Center (Seoul, Korea) from 2010 to 2014. We sought correlations amongst age, gender, fracture type, injury mechanism, alcohol consumption, and type of medical insurance. Results: Out of the 303 patients meeting inclusion criteria, 214 (70.6%) patients were enrolled in National Health Insurance (NHI), 46 (15.2%) patients had Medical Aid, and 43 (14.2%) patients were homeless. The main causes of facial bone fractures were accidental trauma (51.4%), physical altercation (23.1%), and traffic accident (14.2%). On Pearson's chi-square test, alcohol consumption was correlated significantly with accidental trauma (p<0.05). And, the ratio of alcohol consumption leading to facial bone fractures differed significantly in the homeless group compared to the NHI group and the Medical Aid group (p<0.05). Conclusion: We found a significant inverse correlation between economic status and the incidence of facial bone fractures caused by alcohol consumption. Our findings indicate that more elaborate guidelines and prevention programs are needed for socioeconomically marginalized populations.

  • It hasbeen individual and social problems that mattoids, alcohols and old people alienated from their families have wandered about the streets for rapid urbanzation, industrialization and nuclear families. In our country, measures for proper guidance for the tramps under the supports of government in 1982 have been carried out. But it has been difficult that we protected without Welfare Acts for them. According to those the study has aimed to provide for the fundamental data to advance public utilities as needs of an administrative organ with focus on contents of nursing records charts, furthermore general traits of subjects perceived details related to diseases. Medical record service has been a 'D' municipal hospital. All the subjects were a total of 4,855 persons, 2.436 in 1992, 2.419 in 1994. 1. Social Traits They mainly have attended a hospital, especially Thursday and Friday from a week and winter from the seasons. They equally have attended a hospital in the beginning, middle and end of the month. They equally have attended a hospital in the months. Government and public offices such as police stations and district offices have trusted them to a hospital. After they left a hospital. their residence has been their home confirmed in the ratio of $59.2\%$. 2. Controling Characteristics of a Medical Institution The proportion of males to females was 7 to 1. In the ages, the adults at the first of adulthood from 25 years to 44 years were $52.6\%$ mostly entered a hospital. In the observations the percentage of patients should be entered total was $8.8\%$. In general. the proportion of changes by fee was about $4\%$. Then the number of patients were on an average from 30. 76 to 32, 55 persons in 1992. It was from 25. 63 to 26. 87 in 1994. But the numbers were 5 to 6. It showed that the number was decreased. 3. Medical Traits The number of days by measured and the number of days which have been entered to a hospital have not made differences. The kinds of diseases were 1.11 in 1992. but were 1. 16 in 1994. In terms of 17 kinds of international diseases divided. the diseases recorded probably have been accidents. poisoning and violence( XVII). digestive trouble(IX). endoctrin disoders. metabolism and dystrophy(III) in orders, then they have been measured $51.0\%,\; 12.7\%,\; 6.8\%$. All patients who have been entered to a hospital severly have been drinking. As the seriousness of drinking and the kinds of diseases have indicated positively correlated. It have indicated that the more serious drinking have been. the more the number of diseases have been. As we have known according to abover results. the Acts for Homeless People related to special medical problems been have revised. furthemore they need to support the basic medical adds in order to maintaim their lives.

정보사회의 도서관봉사 (Library Services in Information Society)

  • As information technologies are applied to the libraries in information society, the library services have been changing its feature. Therefore, the purpose of this paper is to explore and to establish a paradigm of the library services in information society. It is hypothesized that the application of information technologies leads to the change of the library services in information society. To prove the hypothesis, the data were collected from various research results carried out in the developed countries and by observing various libraries where the information technologies are extensively applied. The findings are as follows: 1. As information technologies are applied to the library, many new library services emerge for the society. 2. As the electronic data replace the paper data, the collection of a library becomes the collection of the libraries in the world. Therefore, the accessibility to the information network is more important than to own information in the library. Librarians select various electronic data according to the library policy which distinguishes their own library collection from others. The policy also solves the various problems related to weeding and preserving library collection. And the use of CD-ROM selection tools enable library users to select their own data. Now, the censorship becomes the concems of the library users, not the library. 3. The catalogs are reorganized for the electronic data for the international use. The most important information in the catalog is the location of the data and the multi access to the data are necessary. 4. As the information technologies are applied in book selection, cataloguing, information retreival and circulation, the library users are enable to service themselves in the library. And most of the routine works related to the information service are taken over by the library staffs. Professional librarians engage in user education, information marketing and fund raising. 5. Public libraries in information society serves those who have no access to the information. They help the illiterates. patients in the hospital, prisoners, and homeless in the city. Therefore, the information technologies enhance the role of librarians in professional work in the library as well as in the information society.
