Objective : The purpose of this study is to find out the effects of a Home-based cognitive occupational therapy program conducted for the dementia patients in the initial stages living in Taebaek region in Gangwon-do. Methods : This study carried out Home-based cognitive occupational therapy intervention by 30 Occupational Therapy Dept. students and volunteers targeting 30 dementia patients in the initiative stages for eight sessions, once a week. To identify the change of cognitive function of the participants before and after the program, MMSE-K was used for measuring. Results : As a result of applying Home-based cognitive occupational therapy to the dementia patients in the initial stages, overall cognitive function improvement was demonstrated. Especially, statistically significant improvement was exhibited in orientation, memory retrieval, the concentration of attention, and language ability. Conclusion : The program for cognitive function improvement is considered to be usefully applied to intervention in the dementia patients. The development of various Home-based occupational therapy intervention programs is required in order to adopt the Home-based occupational therapy service in the future.

This study investigated the situations and satisfaction of home-based occupational therapy service for children under eighteen residing in Suncheon and Gwangyang. Arithmetic statistics were conducted by dividing the general facts of home-based occupational therapy service, the kinds of service used in home-based therapy, the satisfaction of home-based occupational therapy service, facts helpful for children, and facts helpful for parents or home. As a result, the most common source of home-based occupational therapy information was the association for children with disabilities and its satisfaction was high. This study will provide basic materials to build up systematic, stable, and continuous rehabilitation therapy support projects by the government.

Objective : The purpose of this study was to verify the effectiveness of home-visit occupational therapy in stroke patients. Methods : Two patients with stroke who applied for home-based occupational therapy services at a health center in Seoul were enrolled. The home-visit occupational therapy program evaluates the subject's daily living, task performance, cognitive, and emotional functions, sets occupational therapy goals and plans interventions based on a client-centered approach. Occupational therapy programs consisted of 12 sessions based on the client's major problems. Results : COPM scores improved in both cases, there was an improvement in COPM scores, and in Case 2 there were improvements in MBI and K-MMSE scores. There was also an improvement in KGDS scores in Case 1. Conclusion : Home-visit occupational therapy was found to be effective in improving daily activities, cognition, and mental function of stroke patients. During home and community integration, continuous and continuous rehabilitation services need to be activated from institutional rehabilitation to community-based rehabilitation. Active home-visit occupational therapy is needed to promote physical, cognitive, mental and social access in stroke patients discharged from hospitals.

Objective : This study investigates correlations between job-stress and job-relation feture in occupational therapists in nursing home. Method : For this study, self-reported questionnaires were distributed by online and offline survey to 237 occupational therapist from May to June in 2012. Fifty two of the questionnaires were returned, acceptable 46 survey responses were analyzed, expect 6 unclearly answered. Result : The results correlations between the 6 job-stress factors(confliction of professional role, lack of professional knowledge and skills, problem of personal relationship, disagreement with predecessor(manager, chief, etc), limitation of medical system, inappropriate salary) and 4 job-relation feature(period of job, annual salary, work out of therapy, job stability). Conclusion : This study demonstrates correlations between job-stress factors and job-relation feature in occupational therapist working in nursing home. Job-stress average 3.52points of 5points contributes more affecting the stress. Further research is necessary to more sample occupational therapist in nursing home.

The proportion of the aged people in Korea has increased rapidly by more than 7% in 2000. A survey says that the changing rate from the aging society to the aged is the highest among the OECD countries. The prevalence rate of the aged population is on the increase. Chronic diseases such as arthritis and strokes limits their body's functions and activities of daily living. It is necessary that functional training. ADL training and continuous treatment be provided by Physical or Occupational Therapists. Discontinuance of treatment can cause the aged people to be in a bed state. The aged people need a variety of home care services at their convenience. Occupational and Physical therapist should play a major role in providing the rehabilitation-base home care services. Further they are expected to contribute to the extension of health care services such as home care and nursing home programs.

Objective : To explore the effectiveness of sensory integration program of home-based outcomes in child with sensory modulation disorder. Methods : This study used the simple case study. The subjects were three children diagnosed as developmentally delayed who 14 month and 26 month males and a 15 month female. After initial evaluation, parents were educated on sensory diet and Wilbarger protocol method for 30 minutes twice for home-based treatment and asked to make out daily treatment planning. To measure improvement of children, we used for the Denver Developmental Screening Test II(DDST II), Wee Functional Independence Measure(WeeFIM), hand function, functional ability, oral motor function, Sensory Profile(Dunn, 1999). Results : After the home-based program, the scored of the Denver Developmental Screening Test II(DDST II), Wee Functional Independence Measure(WeeFIM), hand function, functional ability, oral motor function, Sensory Profile(Dunn, 1999) were higher rather than initial evaluation after the home-based program. Conclusion : The home-based program is effective for children with sensory modulation disorder but parents had to be educated by therapist specific evaluation and treatment in sensory integration.

This paper proposes that people with disability can be successfully supported by smart homes only when their needs of the technological interventions are understood and integrated in traditional universal design. Understanding the patient's home environment is an integral part of treatment and discharge planning. This paper suggests on architectural barriers commonly found in the home, ways to eliminate them and a general overview of methods for assessment and intervention. It is expected that the electronics, information and communication technology can be an alternative of traditional universal designs for motor dysfucntion, sensory dysfunction and cognitive dysfunction.

Background: This article reviewed the current status of home-based physical therapy infrastructure in long term care insurance and then solved the problem. Method: We used two forms of data that were acquired from ⅰ) the Ministry of Health & Welfare and the Family and National Health Insurance Corporation, ⅱ) a home-visiting health care program, and ⅲ) evaluation data from the Korea Health Industry Development Institute. The home-based physical therapy program was then analyzed. Results: The role and concept of home-based physical therapy was not clearly established. There were few home-based physical therapy programs in the community. The manpower of home-based physical therapists in the home-visiting health care program was very low. The role between home-visiting nurses (caregivers) and home-based physical therapists was mixed. Research and promotion regarding home-based physical therapy was poor. Conclusion: To establish a system of legal, long-term care insurance, we must increase the manpower of home-based physical therapists and the amount of research pertaining to the demand for home-based physical therapy.

Objectives: The aim of this study was to examine the effects of home-based cognitive physico-occupational therapy(HBCPOT) on cognitive function, depression, and quality of life in dementia patients, using a cognitive impairment model. Methods: The data was analysed for Mini-Mental State Examination-Korean version (MMSE-KC) for assessing cognitive function, Quality of life-Alzheimer's Disease (QOL-AD) and Geriatric Depression Scale (GDS) in 31 dementia patients who received home-based cognitive physico-occupational therapy for one hour once a week for 12 weeks by a trained occupational therapist at the Dementia Prevention and Management Center of Y-city during 20111-2013. Results: Among these 31 patients, 18(58.1%) were female, and 25(80.6%) had Alzheimer's Disease. After HBCPOT, the mean scores of MMSE-KC and QOL-AD were significantly improved, but GDS score was not significantly decreased. Conclusions: It was suggested that HBCPOT was effective in improving cognitive function and quality of life.

Objective : he aim of this study is to describe on the definition of the welfare state and scope of the social welfare and to analyze the development of the social welfare policy for the disabled in South Korea. Methods : We described on the definition of the welfare state and scope of the social welfare and analyzed the social welfare policy and welfare policy for the disabled among Korean Government through literature review. Finally, We also analyzed the welfare policy for the disabled in the Roh Moo-hyun Administration. Results : Welfare policy or welfare policy for the disabled was developed as economic growth from 'selective welfare' to 'universal welfare'. Scope of the social welfare was expanded from 'minimum guarantee' of the Lee seong-man administration to 'participatory welfare' of the Roh moo-hyun administration and that was similar to the welfare policy of administration for the disabled. Conclusion : We suggest that occupational therapist based community should find unmet health need and provide home-based rehabilitation services to the disabled and the elderly.
