Purpose: This study was aimed to evaluate the performance of a community-based home care services model. Method: The subjects were 138 patients who have been enrolled during a 6 month's period, nursing records and nursing service bills. The data was collected by self report and chart review. The mean cost per visit was compared with those of hospitalization & clinic visit in the data from National Health Insurance Corporation. Result: A significant number of patients were bed ridden(63.8%) and unconscious (27.5%), and most of the patients had complex chronic diseases. Except nursing assessment, bed sore care was the most frequent nursing treatment(25.1%) in home care services. The mean cost per visit of home care services was 34,665 won, which was lower than those of hospitalization & clinic visit for medical aid. The patients were highly satisfied with the services by visiting nurse specialists. Conclusion: Community-based home care services provided cost-effective and satisfied services. Community-based home care services needs systematic supports to expand it's domain for promoting community health.

The purpose of this study was to analyze the work of visiting health care providers in public health centers. A descriptive analysis of self-records for work data from 875 visiting health care providers working in 242 public health centers in South Korea was done. The results of the study are as follows: 1. The number of households for each visiting health care provider ranged from under 100 households to over 500 households. 2. Low performance was found for several items on the work list for visiting health care providers. 3. There were significant differences in the work performance of visiting health care providers between nurses and nursing assistants. 4. There were significant differences in the work performance of visiting health care providers according to region. In conclusion. work performance of visiting health care providers was low and differed according to type of occupation and region. This study suggest the need for further studies which analyse the quality of visiting health care providers and services, and the visiting health care system.
Purpose: This study aimed to investigate possible ways to expand the services of home-visit nursing through a review of the progress, achievements, and obstacles of home-visit nursing; a pilot project of an integrated home-service; the application of the Omaha System; as well as a case analysis of providing home-visit nursing services. Method: An integrated review was conducted using various source materials, including laws, previous studies, and a case analysis. Results: In case analysis of providing visiting nursing service, rehabilitation nursing, end-of-life nursing, and dementia care showed high nursing needs. It was necessary that the various home visit nursing services in the intervention area of the Omaha System, administrative services, case management, and center operation activities were all included in the payment systems of long-term care insurance. Conclusion: In the future, home visit nursing services of long-term care insurance should be reborn in the form of a center for integrated case management in the community, which would set long-term goals until the time of a client's death and encompass the realm of human rights for health, quality of daily life, and a dignity of life.
Purpose: This study was done to report nursing case for ADL improvement of elders who have CVA(Cerebrovascular Accident) sequelae. Methods: The client had registered in the C visiting nursing center after being decided a long-term care Grade 2. Data were collected through consultation logs for recipients, Activities of Daily Living (ADL) records, fall risk assessment (Huhn) sheets, decubitus ulcer risk assessment (Braden Scale) sheets, cognition assessment (K-MMSE) sheets, long-term care benefit provision records, and interviews with visiting nurse. Data were collected and analyzed according to the Omaha System problem classification. The intervention scheme and the problem rating scale for performance were applied to present the case for home-visit nursing. Results: The client registered in August, 2018, was provided home-visit nursing care once a week as of September 2020. ADL, cognitive levels and decubitus ulcer risks were found to have improved. Conclusion: This case report presents the value of classifying nursing problems and checking nursing intervention provided to patients with problems of ADL. The presentation of home-visit nursing cases applying a standardized nursing problem classification scheme for clients with various problems showed that a high quality level of care is guaranteed and evidence-based nursing can be provided by visiting nurses.
This study is conducted to promote the activation of volunteer activities on the basic of the voluntaarry participation of local by investigating the utillzation of volunteers and analyzing this realted factors in the visiting health services of health centers. Subjects in this study were the whole 245 health centers. Data were collected from April, 12, 1999 to May, 31, 1999, and data for analyses were ones of 41 respondents, which answer " they utilize volunteers in visting health services". The summary of resulth was as follows : 1. In case that the type of health centers is one of county, in case that the number of staff in health center is over 80, in case that model business in not performed, in case that the degree of local financial independence is over 50%, and in case that the location of health centers in not good, there had more of volunteers in visiting health services. 2. 51.2% of analysing health centers answered that the utilization of volunteers has been done since 1998. 56.1% answered that they utillzed volunteers "over 4 hours" per person in a week. The average number of volunteers who was engaged at those visiting health services was 43.3. 3. Most of volinteers were housewives(73.2%). As for the action duration of volunteers. 68.8% answered " under 6 months". 4. As to the tasks of volunter activities, 75.6% were "home services" and 63.4% were "movable bath services". As for the tasks they intend to utilize, 90.2% answered " home services", 73.2% answered "movable health services". 5. Asked abount the purpose in utilizing volunteers, 75.6% answered "to induce the participation og local people". 65.9% answered "to provide various kinds of services". 66.7% provided some kind of education and training for volunteers. 6. Concerming evaluation of performance by volunteers, 90.2% answered " satisfactory". With reagards to the reason for that, 52.9% answered " volunteers can provide kinds of services". and 50.0% answered " volunteers can help local people to care their health". Purpose: This study investigated the occupational status and job stress of visiting nurses during the COVID-19 pandemic. Methods: Participants were 151 visiting nurses working in the public health services in Seoul and Gyeonggi-do. The study was conducted using a structured online questionnaire. Results: The average age of the visiting nurses was 49.5 years. A occupational period was 38.4% of the participants had worked for a period of 6-10 years and 34.5% of them had worked for a period of 11 years or more. In terms of employment type, 74.8% had permanent contracts and 13.9% had fixed terms. 93.4% of nurses were working on COVID-19-related work; and 57.6% of them were working more than 50% of their work. Each visiting nurse provided health services for a total of 436.4 cases on average and the per day count was 14.3 cases. The service was provided through home visits or phone consultations. The results of the job stress were relationship conflict (60.92±20.72), job demand (59.05±14.10), job autonomy (57.83±14.63), job instability (45.25±27.37), organizational system (44.97±17.21), workplace culture (42.71±18.00), and inadequate compensation (35.29±18.14). There was a significant difference in job stress according to the proportion of COVID-19 work was 'workplace culture' (p=.023), Job autonomy (p=.053) and Inappropriate compensation(p=.054). Conclusion: In order to provide a stable working environment for visiting nurses and effective public visiting health services during unexpected circumstances, such as the COVID-19 pandemic, it is necessary to promote progressive policies and systemic improvements.

The purpose of this study is to promote the efficient operation of the Home visit nursing care services by identifying the perception and experience of visiting nurses at the community center. To this end, a methodological triangle study method was applied to visiting nurses. As a result of analysis the delivery system of the home visit nursing care services, the perception and participation experience of nurses, it was found that it is necessary to supplement the program standards such as the number of visiting target, health assesment, and information system for the efficiency of the visit. It is suggested that policy alternatives should be developed by sharing the major issues raised in this study.

Purpose: The purpose of this study was to develop child's health assessment tools and tailored home visiting nursing service model in a community. Methods: Based on the literature review and several types of workshops participated with the child health nursing professors and visiting nurses in public health centers from May to December 2009, the standards of child health assessment tools, service model and education materials for visiting nurses were developed. Results: Some record forms were newly developed, including neonatal assessment, breast feeding, mother-infant interaction, oral care, vaccination and safety, and appropriate developmental screening tests in the community were selected. For systematic health care management in the community, problem list, problem criteria, health care plan, outcome criteria were also developed. Conclusion: On the demand of growing need for health promotion and early intervention for children and their association with parenting and socioeconomic status, assessment and control measures are indispensable to the promotion of child health for vulnerable population. Children's health and developmental problems, and safe circumstances can be assessed using this assessment tools, and can be used for tailored home visiting nursing care for children.
The purpose of this study was to 1) identify the current management status of the Visiting Health Care Services (VHCS) and 2) to analyze the workload of the staff in the VHCS located in the Public Health Centers (PHCs) in the urban and rural areas. Method: A descriptive research design and a prospective, time and motion research design were used. A total of 18 PHCs in Gangwon Province participated in this study. A questionnaire and semi-structured observational sheet were utilized. A total of 650 self report records of the work load from the VHCS personnel were collected for a 10 day period at each of the 18 PHCs. A descriptive analysis was then done. Results: The major results were as follows. 1) The VHCS staff (nurses and nurse aids) was being assigned additional work such as maternal health care, chronic disease care, mental health care and health promotion on top of their VHCS duties. 2) The average number of home visits per client during the past year was 5.8. More specifically, the clients in the severe dependent group received an average of 27.1 visits, those clients in the moderate dependent group received 14.0 visits those clients in the slightly dependent group received 5.0 visits and those clients in the self-care group received 1.6 visits. 3) The time required for the work duties of the VHCS staff totaled 488 minutes per day. The percentage of time for home visits was only 17.4%, and this didn't include travel time. Conclusion: The main problems of VHCS were identified as a lack of personnel and a lack of time for the home visits. Strategies that are directed at the construction of a better infrastructure for VHCS are needed.
Purpose: The purpose of this study was to follow-up the frailty of the old who received home health care by Registered Nurse in Public Health Center over 8 years. Methods: We used the second wave data which was a comprehensive longitudinal data set, Public Health Information System of a public health center located in Seoul from 2010 to 2018. For statistical analysis, a mixed model of repeated measures by R program was used. Results: Frailty (range: 0~31) was getting worse significantly from 5.38 on registration to 6.54 on 4th year, 7.40 on 7th year, 7.69 on 8th year with adjustment for age, sex, economic status, the number of family, and the number diseases. The coefficient of parameters with frailty change was serviced year (β=0.29, p<.001), age (70~79 to 60~69; β=0.98, p=.018) and sex (female to male; β=2.55, p<.001). Conclusion: This study showed that the home visiting health service needs to take attention to aged 70s and over, female. The home health care of public health center need to be extended more practical and effective services in terms of 'community care'and 'ageing in place'.
