Purpose: This study was conducted to assess needs at each end-of-life care stage and to analyze importance and difficulty of care needs for home care nursing among non-cancer patients. Methods: We used a retrospective design. Total eligible patients were 117 at the ages of 40 and over, who continuously received home care nursing throughout beginning, stable, and near death stages, and finally died at home from January 1, 2006 to December 31, 2006. Descriptive statistics, Cochran's Q test, Friedman's test were used for data analysis. Results: In the area of physical care, the care need for 'assistance for activities in daily life' was significantly highest in the beginning stage. The care need for 'aggravation or adverse changes in physical symptoms' was significantly increased in the near death stage. In the area of psychospiritual care, 'family's psychological burden' was revealed as having the highest rate of care needs in the every stage. Conclusion: Future intervention should consider assessing care needs in end-of-life care for non-cancer patients who are provided with home care nursing.
Purpose: To analyze the home care services provided to the elderly aged 65 and older by a hospital-based home care agencies and to investigate the effects of long-term care insurance for the elderly. Method: The subjects were the home care service recipients aged 65 and older in 172 hospital-based, home care agencies registered in Health Insurance Review & Assessment Service in January, 2007. The data were collected using a questionnaire from March 16 to April 15, 2007. The questionnaire return rate was 43.8%. Result: The hospital-based home care agencies were able to visit 66.5% of the national administrative districts. Of the home care service recipients, over 50% were 65 years old and older. About 43% of the agencies reported that over 50% of their patients would be subject to the long-term care insurance. They expressed concern that home care services would be withdrawn once the insurance system is initiated. Conclusion: This study suggests that hospital-based home care agencies need to manage home care services with long-term care insurance. It also recommends developing guidelines for the use of services and referrals.
Purpose: We analyzed the need for home care services and awareness of U-healthcare in nurses of the Catholic Parish Home Care Center to link the ubiquitous health care system and the home care. Methods: We recruited 46 nurses from a home care center in the catholic parish of the C medical center from April 4th to June 8th, 2007. Results: The highest needs were 'assessment and diagnosis of the problem', 'management of hypertension & diabetes patients', 'counseling of the patient', and 'counseling of major caregiver and family problems'. Therapeutic nursing showed the highest needs in bedsore care. Nurses want hospital medical records available through the ubiquitous health care system. Conclusion: Home care services are supported from the agency, with high needs in assessing and diagnosing the problem, counseling, and managing of hypertension & diabetes patients. Education and public relations efforts on the U-healthcare system should improve system awareness.
Purpose: The purpose of this study was to examine morale and related factors of hospital based home care nurses. Method : The subjects were 159 home care nurses from 94 hospitals and clinics which were operating home care programs around the country. Data were collected for 40days from March 14, to April 24, 2005. The questionnaire consisted of 51 items including 12 general variables and 39 items of nurses' morale. The reliability of the questionnaire by Cronbach's ${\alpha}$, was .88. Result : The level of the morale was found as mean score 2.69 in 4 point scale The high ranks of morale were self actualization($3.05{\pm}0.43$) and job satisfaction($3.03{\pm}0.43$), the factors which showed lower points were evaluation of work ($2.47{\pm}0.53$), welfare($2.42{\pm}0.42$), promotion system ($2.35{\pm}0.45$) and wages($2.23{\pm}0.54$). The level of morale according to the general variables were significantly different in such variables ; home care nursing antecedent(p = .000), motivation for job selection(p= .030), intention to quit the job(P= .000). Variables of intention to quit the job(15.7%) and home care nursing antecedent; 6.7%(p=.001) showed 22.4% of explanatory persuasion effect on level of morale. Conclusion : To improve a morale of home care nurses, the arrangement of nursing department should be consider nurse's aptitude and interest and allow them to have longer period of work in that part. Also wages, promotion system and welfare should be reformed as relevant as their career.
Purpose: This study was performed to identify the nursing needs of home-dwelling breast cancer patients based on counseling contents. Methods: Descriptive research was conducted with content analysis. This study included 185 patients who underwent treatment for breast cancer in a tertiary hospital. The data were collected using personal counseling via telephone or face-to-face between March 2011 and July 2013. A total of 536 counseling contents were used in the analysis. Inductive content analysis was used to analyze the contents related to nursing counselling. Results: According to the results. the most frequently reported nursing needs was symptom management, followed by information needs and supportive intervention needs; symptom management needs included symptoms related to chemotherapy, daily living, surgery, and medication side effects; information needs included treatments, medication, clinical tests, and alternative therapies; and supportive intervention needs were related to emotional and social aspects. Conclusion: Our findings suggest that nursing interventions for home-dwelling breast cancer patients should be designed and provided with a consideration to these relevant nursing needs.
Purpose: This study aimed to identify the training system and current status of home health nurses and to examine ways to retain sufficient number of advanced practice nurses (APNs) in the home health nursing field. Methods: This study analyzed the adequacy of the supply of home health nurse by reviewing the existing research literature and statistical data. Results: Discussions on how to revitalize the home care business have been ongoing since the beginning of 2001. However, despite home health nurses being oversupplied, discussions on the adequacy of supply have always been excluded from the focus of revitalization. The recent expansion of the home care business has resulted in a shortage of workforce, which can be inked not only to the continuous reduction of the designated quota of programs but also to the regional imbalance of educational institutions. The serious imbalance between supply and demand has caused fears that the home care business would drastically reduce. Conclusion: It is necessary to not only increase designated quotas for APNs programs but also integrate those programs of the similar curricula, thus lowering supply shortages in home health nurses.

Even though having many problem to have to check the problems, the preparation of legal and systematic equipment etc., for our goverment will have to prepare all measures for unmovable man to live leisurly in their home by analysing the management of social welfare system of the advanced country or Japan.
Purpose: The purpose of this study was to identify the factors associated with the health-related quality of life of family caregivers. Methods: A cross-sectional study was conducted. This study included 191 primary family caregivers of elders who used home care services (home-visit nursing, home-visit care, daycare) covered by the public long-term care insurance. Data were collected using self-report questionnaires from December 2010 to June 2011. These data were analyzed by using hierarchical multiple regression. Results: The majority of the family caregivers were female (79.6%) and daughters-in-law (28.8%). The mean depression score was $6.33{\pm}6.49$ and the mean health-related quality of life score was $0.69{\pm}0.39$. It was found that the factors affecting the health-related quality of life of family caregivers included depression (${\beta}$=-.406, p<.001), home-visit nursing use (${\beta}$=.296, p<.001), and daycare use (${\beta}$=.178, p=.015), which accounted for 36.6% of their health-related quality of life. Conclusion: Using home-visit nursing and daycare services has a positive effect on the health-related quality of life of family caregivers. To improve health-related quality of life of family caregivers, South Korea needs to fully activate the home-visit nursing and daycare services, and to strengthen family support programs.
Purpose: The world saw a shift into a new society consequent to the coronavirus disease 2019 (COVID-19), which made home quarantine mandatory for a person in close contact with those who tested positive. For children, however, home quarantine was not limited only to themselves but parents, especially mothers were involved and required to quarantine. This qualitative study aims to explore and understand mothers' experience and their related psychosocial issues while caring for their school-aged children in Korea, who had to home quarantine after coming in close contact with COVID-19 positive individuals. Methods: Data were collected from October 2020 to January 2021 via in-depth, semi-structured interviews with nine mothers of children who had to home quarantine. Interviews were conducted face-to-face in an independent space near the participant's home or workplace (n=5) or via online platforms or telephone (n=4). The data were analyzed using thematic analysis through several iterative team meetings. Results: Thematic analysis revealed the following four themes: "Unable to be relieved due to uncertain situations surrounding me," "Blame and hurt toward me, others, and one another," "Pulling myself together for my children in my broken daily life," and "Changes in the meaning of life amid COVID-19." Conclusion: The narratives show that mothers experienced psychosocial difficulties while caring for their children during home quarantine. It is necessary to reduce the social stigma toward individuals in home quarantine and establish policies to ensure work-family compatibility for such mothers.
본 연구는 노인요양시설에서 생활하는 노인들이 경험하는 관계적 공격성이 우울감에 미치는 영향을 분석하고 시설적응이 미치는 매개효과를 검증했다. 이를 위해 서울 경기 지역 기초생활수급자 대상 노인전문요양시설 3곳과 노인요양시설 3곳에서 182부의 설문지를 회수해서 분석의 근거로 삼았다. 그리고 SPSS18.0 프로그램을 이용하여 빈도분석 및 상관관계분석 등을, AMOS 7.0 프로그램을 사용해 구조방정식 모형을 분석했다. 그밖에 Sobel test를 이용해 매개효과의 통계적 유의미성을 검증했다. 조사결과를 분석한 결론은 다음과 같다. 첫째, 노인요양시설에서 생활하는 노인들 사이에 나타나는 관계적 공격성과 우울감 사이에는 통계적 상관관계가 없는 것으로 나타났다. 둘째, 노인요양시설 입소노인 간의 관계적 공격성이 우울감 사이에 시설적응의 매개효과가 있다. 즉, 시설에서 관계적 공격성이 높을수록 노인들의 시설적응은 낮아지고, 시설적응이 낮을수록 노인들의 우울감이 커지는 것으로 나타났다. 이러한 결과를 바탕으로 제언을 하면 다음과 같다. 첫째, 노인요양시설에서 노인들 사이의 긍정적인 유대관계를 형성하기 위해서 새로운 입소노인들이 기존 입소노인들과 함께 공감대를 형성할 수 있는 건강프로그램, 종교프로그램 등을 개발하여 공통된 관심사를 통해 상호관계를 증진시키도록 한다. 둘째, 노인들의 시설적응을 돕기 위한 방안으로써, 개인용품을 시설에 반입하는 허용 범위를 넓히는 방법을 생각할 수 있다. 셋째, 노인들 사이에서 나타나는 괴롭힘 같은 부정적 관계가 발생하는 상황을 중재 혹은 예방 및 경감하기 위한 프로그램을 해당 시설에서 적극적으로 개발할 필요가 있다.
