Human health is affected by not only physical conditions but also mental and social well-being. Changes of human emotions show up as gestures, facial expressions and sweating. Human emotions are affected by such automatic nerve system functions as blood pressure, blood circulation speed, heart beats, pupillary reflex, fluid transfusion, muscular contraction and digestive organs, all of which influence the holistic diseases. The Oriental Medicine sees from a perspective of unity of divinity and men that human life activities are united in terms of their physical and mental functions. From such a perspective, human Five Organs are linked with Five Mental(五神) and Seven Emotions(七情), while they are affected by each other, influencing the life activities both directly and indirectly. Based on Confucianism, Sa-Sang Theory argues that human emotions can be categorized into four energy states and therefore, that human diseases and physiological conditions there of may be determined differently depending on the Four Energies(四氣). There seems to be some common points between Sa-Sang Theory and the conventional Oriental Medicine in that human emotions affect individuals' health conditions, so there seems to be much room for mutual complementation.
The treatment of craniofacial anomalies has been challenging as a result of technological shortcomings that could not provide a consistent protocol to perfectly restore patient-specific anatomy. In the past, wax-up and impression-based maneuvers were implemented to achieve this clinical end. However, with the advent of computer-aided design and computer-aided manufacturing (CAD/CAM) technology, a rapid and cost-effective workflow in prosthetic rehabilitation has taken the place of the outdated procedures. Because the use of implants is so profound in different facets of restorative dentistry, their placement for craniofacial prosthesis retention has also been widely popular and advantageous in a variety of clinical settings. This review aims to effectively describe the well-rounded and interdisciplinary practice of craniofacial prosthesis fabrication and retention by outlining fabrication, osseointegrated implant placement for prosthesis retention, a myriad of clinical examples in the craniofacial complex, and a glimpse of the future of bioengineering principles to restore bioactivity and physiology to the previously defected tissue.
Purpose: The purpose of this study was to evaluate the validity and reliability of the Korean version of the Ethical Sensitivity Questionnaire for Nursing Students (KESQ-NS). Methods: The participants were 138 nursing students who have experience in clinical practice. The Korean version of ESQ-NS (KESQ-NS) was examined using content validity, construct validity, criterion-related validity, and testing of internal consistency reliability. Data were collected from November to December of 2019 through an online-survey. Results: The KESQ-NS that was composed of 13 items was divided into three dimensions: Critical understanding of the patient, patient holistic care, patient privacy, and confidentiality. The instrument explained 67.9% of the total variance for ethical sensitivity. Cronbach's α was .88. Conclusion: The KESQ-NS showed good validity and reliability. This instrument can be used to evaluate ethical sensitivity in nursing students in Korea.
In this paper, a multi-objective wireless sensor network configuration optimization method is proposed. The proposed method aims to determine the optimal information and lifespan wireless sensor network for structural health monitoring of large-scale infrastructures. In particular, cluster-based wireless sensor networks with multi-type of sensors are considered. To optimize the lifetime of the wireless sensor network, a cluster-based network optimization algorithm that optimizes the arrangement of cluster heads and base station is developed. On the other hand, based on the Bayesian inference, the uncertainty of the estimated parameters can be quantified. The coefficient of variance of the estimated parameters can be obtained, which is utilized as a holistic measure to evaluate the estimation accuracy of sensor configurations with multi-type of sensors. The proposed method provides the optimal wireless sensor network configuration that satisfies the required estimation accuracy with the longest lifetime. The proposed method is illustrated by designing the optimal wireless sensor network configuration of a cable-stayed bridge and a space truss.
The role of the neonatal nurse specialist has been well established over the past decade and now reform in 21st century. Neonatal nurse specialists responsibilities in caring for critically and long-term chronically ill infants and their families are very important. Neonatal nurse specialists have a two fold responsibility in caring for these infants. First, through acquiring advanced practice education in complex neonatal care and diagnostic skills, neonatal nurse specialists meet the physiologic needs of the infant. Second, neonatal nurse specialists provide a more holistic approach to their care through evaluating the family in treatment plans and involving the family in discharge planning for the infant. In some institutions, neonatal nurse specialists are directly involved in institutional and/or home follow-up care and case management also. It is the neonatal nurse specialists responsibility to function collaboratively with the multidisciplinary team in managing critically or chronically ill infants from admission to discharge. The role of the neonatal nurse specialist case manager can be described as one that focuses on individualized care of the infant, while providing continuity of care to both the infant and family. The neonatal nurse specialist's role will vary depending on the neonatal intensive care unit(NICU). Therefore, the multidisciplinary collaborative approach to long-term management of infants in the NICU is extremely important to provide successful transition to home or to long-term rehabilitative care facilities because care for the chronically ill infant is complex and multifaceted. I suggest the role of neonatal nurse specialist in 21st century are as follows. 1. Diagnostic/patient assessment 2. Management of patient health/illness 3. Consulting role The advanced practice nursing model of care delivered by neonatal nurse specialist's in the NICU incorporates medical and nursing role functions and emphasizes holism, caring, and a health perspective for critically and chronically ill neonates and their families.
Purpose: This study aimed to investigate the factors affecting cardiovascular disease in middle-aged male workers. Methods: The participants were 137 male office workers over 40 years old and under 60 years old working in small- and medium-sized workplaces from three southern provinces of Korea. Data were collected through self-reported questionnaires and health screening reports from the 2014 National Health Examination. The influencing factors included general characteristics of participants, anger expression style, and occupational stress. Data were analyzed using t-test, ANOVA, and regression analysis with SPSS 22.0. Results: The mean score of risk of cardiovascular disease was 6.73±4.69 and there were significant differences in exercise (t=2.13, p=.035) working time (t=-2.15, p=.034). Logistic regression analysis showed that, when adjusted for exercise and working time, the probability of becoming under a cardiovascular disease risk was 21% higher for those who anger-in (OR=1.21, 95% CI=1.02~1.44, p=.027) and 12% higher for those perceived occupational stress (OR=1.12, 95% CI=1.01~1.24, p=.030). Conclusion: The results suggest developing the strategies for middle-aged male workers to encourage exercise and to decrease occupational stress, as well as an appropriate anger expression style to improve holistic aspect of health considering their demographic characteristics.
Systems thinking, a linking domain of health systems science (HSS), is an approach that investigates specific problems from a holistic perspective. It supports improving patients' health, fulfilling their health needs, and anticipating issues that threaten patient safety within the healthcare system. It also helps solve problems through critical thinking and ref lection. This study aimed to develop an curriculum on systems thinking, explore the effectiveness of the course, and investigate the applicability of HSS education at individual universities. In this study, the ADDIE (Analysis, Design, Development, Implementation, and Evaluation) model was utilized to design, develop, implement, and evaluate an elective course on systems thinking. In the design process, learning outcomes and goals were developed, and educational content, teaching-learning methods, and student evaluation methods were linked. In the development process, class materials and evaluation materials were prepared. In the implementation process, the course was implemented, and the evaluation process analyzed the results of learning performance and curriculum assessments. The evaluation found the following results. First, the students in the study realized the importance of systems thinking and experienced the need for systems thinking through non-medical and medical situations. Second, the students were very satisfied with the learning activities in the course (mean=4.84), and the results of the self-competence evaluation, conducted before and after the course, also showed a significant improvement. This study confirmed the effectiveness of the elective course, and its results can serve as a reference for developing an HSS curriculum.
This study addresses the need to adopt teaching-learning approaches in physical therapy education that develop links between theory and clinical practice in a meaningful way. Problem-based learning (PBL) is presented as a useful way to educate physical therapy for the future. The essential characteristics of problem-based learning include: curricular organization around problems rather than disciplines; an integrated curriculum rather than one separated into clinical and theoretical components; and an inherent emphasis on cognitive skills as well as on knowledge. PBL as implemented in the health sciences, is an educational method in which the focus of learning is a small-group tutorial in which students work through health care scenarios. The goals of the health care scenarios are to provide a context for learning, to activate prior knowledge, to motivate students, and to stimulate discussion. Learning is student-centered rather than faculty-centered, and self-directed learning is emphasized. Whereas the former focuses on critical thinking and clinical judgement, the latter's emphasis is on clinical competency. The physical therapist (PT) program at Cheju Halla college is a partial integrated problem-based curriculum. The history and process of PBL in general and in the PT program are reviewed. Long-term advocates of PBL stress that it is the only known method for preparing future professionals to be able to adapt to change, learning how to reason critically, enabling a holistic approach to health.
Objectives: The objective of this review was to summarize the primary role of three representative endocrine axes in aquatic vertebrates and discuss the effects on endocrine systems and their interactions in teleost fish after exposure to environmental contaminants. Methods: We summarized individual traits and mechanisms for hormonal and transcriptional interactions between the hypothalamic-pituitary-gonad (HPG), hypothalamic-pituitary-thyroid (HPT), and hypothalamic-pituitary-adrenal (HPA) axes in fish. We also provided a brief discussion on the effects of nonylphenol-induced toxicity on endocrine systems and their interactions in fish as a demonstration of holistic explanation. Results: Currently-available data showed that thyroid dysfunction is associated with reproductive toxicity due to changes in steroidogenic gene expressions and sex hormone levels as well as gonad glands in fish. As an example, we demonstrated that exposure to nonylphenol could induce estrogenicity in male fish by decreasing thyroid hormones, which contributes to increased aromatase expression. Although the mechanisms are complicated and involved in multiple ways, a number of studies have shown that sex steroids influence the HPT axis or the HPA axis in fish, indicating bi-directional crosstalk. Critically missing is information on the primary target or toxicity mechanisms of environmental contaminants among the three endocrine axes, so further studies are needed to explore those possibilities. Conclusions: This review highlights the interactions between the HPG, HPT, and HPA axes in fish in order to better understand how these endocrine systems could interact with each other in situations of exposure to endocrine disrupting chemicals.

Despite proven effectiveness and accuracy in laboratories, the existing damage assessment based on guided ultrasonic waves (GUWs) or acoustic emission (AE) confronts challenges when extended to real-world structural health monitoring (SHM) for railway tracks. Central to the concerns are the extremely complex signal appearance due to highly dispersive and multimodal wave features, restriction on transducer installations, and severe contaminations of ambient noise. It remains a critical yet unsolved problem along with recent attempts to implement SHM in bourgeoning high-speed railway (HSR). By leveraging authors' continued endeavours, an SHM framework, based on actively generated diffuse ultrasonic waves (DUWs) and a benchmark-free condition contrast algorithm, has been developed and deployed via an all-in-one SHM system. Miniaturized lead zirconate titanate (PZT) wafers are utilized to generate and acquire DUWs in long-range railway tracks. Fatigue cracks in the tracks show unique contact behaviours under different conditions of external loads and further disturb DUW propagation. By contrast DUW propagation traits, fatigue cracks in railway tracks can be characterised quantitatively and the holistic health status of the tracks can be evaluated in a real-time manner. Compared with GUW- or AE-based methods, the DUW-driven inspection philosophy exhibits immunity to ambient noise and measurement uncertainty, less dependence on baseline signals, use of significantly reduced number of transducers, and high robustness in atrocious engineering conditions. Conformance tests are performed on HSR tracks, in which the evolution of fatigue damage is monitored continuously and quantitatively, demonstrating effectiveness, adaptability, reliability and robustness of DUW-driven SHM towards HSR applications.
