The effect of hip joint exercise using an elastic band on dynamic balance, agility and flexibility in healthy subjects: a randomized controlled trial

  • Objective: The purpose of this study was to examine the effects of hip joint exercise using an elastic band on dynamic balance, agility, and flexibility in healthy subjects. Design: Randomized controlled trial. Methods: Thirty-five subjects (between 19 and 23 years) were randomly allocated to two groups: hip flexion exercise (HFE) group (n=17) and the hip abduction exercise (HAE) group (n=18). The HFE group participated in flexion exercise of the hip joint using an elastic band for 50 minutes a day, three days a week for four weeks, while the HAE group participated in abduction exercises of the hip joint using an elastic band for the same period. Dynamic balance was measured using the timed up and go (TUG) test, agility was measured with the standing long jump, and flexibility was measured using the Schober's test (5 cm, 10 cm). Results: The HFE group showed significant differences in the TUG test, standing long jump, and the Schober's test (10 cm) after training (p<0.05). The HAE group showed significant differences in the TUG test, standing long jump and the Schober's test (5 cm, 10 cm) after training (p<0.05). However, there was no significant difference between the HFE group and the HAE group. Conclusions: Flexion and abduction exercises of the hip joint using and elastic band increased dynamic balance, agility, and flexibility in healthy subjects. Additional research on hip joint exercises using an elastic band for improving dynamic balance, agility and flexibility are necessary.

새로운 체전굴 측정 방법의 개발 (Development of The New Instrument for Measuring of Trunk-Hip Forward Flexion)

  • 이 연구의 목적은 간단하고 실용적이며, 타당성, 신뢰성, 객관성이 높은 체전굴 유연성 측정 도구를 개발하는데 있으며, D여자 대학교의 스포츠학 전공 여대생 44명이 자발적으로 연구의 피검자로 참여하였다. 새 측정 도구의 신뢰도와 객관도 검사는 재검법의 평균 측정값을 사용하여 이원 혼합효과 모델에 의한 급내 상관 계수를 산출하였다. 검사자별 1차와 2차의 급내 상관 계수는 L은 R = .994, Y는 R=.993, 1차와 2차 검사의 L과 Y검사자간 급내 상관 계수는 R = .985, R=.986으로 모두 상관도가 아주 높은 것으로 나타났다. 새로 개발한 체전굴 측정 도구의 타당도 검증을 위하여 Leighton-flexometer-tests의 원리를 이용한 Takei 의 flexometer를 준거 검사 도구로 사용하였으며, 1차 검사와 2차 검사에서의 새로운 체전굴 유연성 측정 도구와 준거 검사 도구 측정값의 Pearson 적률 상관계수는 각각 r = .962와 r = .960로 1, 2차 검사 모두 아주 높은 상관도를 나타냈다. 결론적으로 본 연구에서 새로 개발한 체전굴 검사는 타당도, 신뢰도, 객관도가 매우 높고 실용적인 것으로 평가될 수 있음이 밝혀졌다.

노인의 워커 사용에 따른 보행 시 하지 관절 3차원 동작 분석에 관한 연구 (The Study of 3D Motion Analysis on Lower Limb during Walking with Walker on Older People)

  • Purpose : The purpose of this study was to find out the difference motion of hip, knee and ankle joint during walking according to using walker on older people. Method : Korean older people of 34 subjects was participated in this study. Participants was measured joint motion on hip, knee and ankle joint during both conditions (walking with walker and without walker). The measured data were analyzed using independent t-test to investigate the difference of joint motion on the both condition. The statistical analyses were performed using Predictive Analytics Soft Ware (PASW) for windows(Ver. 19) and p-value less than .05 were considered significant for all cases. Result : The study showed that more joint motion on hip flexion and ankle pronation is increased by using walker. And hip extension, knee external rotation and ankle plantar flexion is decreased by using walker. Conclusion : This study suggest that using walker on older people was change the motion of the lower limb joint during walking. Therefore, It is necessary to develop a new walker that can reduce dependency and ensure stability on older people during walking.

Lower Extremities' Joint Stability during the Elderly Woman's Walking

  • Objective: The aim of this study was to investigate the periodicity of the lower extremity joint flexion/extension angle to compare the local stability between young and elderly women during walking on a treadmill. Method: Eighteen young women (mean $age=21.2{\pm}1.6y$; mean $mass=57.1{\pm}6.1kg$; mean $height=1.61{\pm}0.04m$) and 18 elderly women (mean $age=66.4{\pm}1.2y$; mean $mass=55.4{\pm}8.3kg$; mean $height=1.56{\pm}0.04m$) participated in this study. Approximate entropy (ApEn) was used to determine the periodicity in the lower limb joint angles. Results: The ApEn values of the two groups were statistically greater in the surrogate data test than in the original time series data (p<.05). The periodicity of the hip and ankle flexion/extension angles decreased in the elderly women group compared with the young women group (p<.05). The periodicity of the lower extremity joint flexion/extension angle showed that the ankle joint increased dominatingly in both groups (p<.05); the hip joint decreased compared with the knee joint in the young women group; and the knee joint decreased compared with the hip joint in the elderly women group (p<.05). Conclusion: These results suggest that the lower extremity joint flexion/extension angles of the young and elderly women during walking contained random noises as well as biological signals. In addition, the differences in the periodicity in the lower extremity joint between the young and elderly women may provide some insight in predicting potential falls and be used as a characteristic indicator for determining local stability in elderly women during walking.

하지 수술환자에게 적용한 로봇보조 보행훈련의 단기간 임상적 효과: 예비 연구 (Short-Term Clinical Effects of Robot-Assisted Gait Training Applied to Patients Undergoing Lower Extremity Surgery: A Pilot Study)

  • Purpose: This study aimed to investigate the effect of robot-assisted gait training on the active ranges of motion, gait abilities, and biomechanical characteristics of gait in patients who underwent lower extremity surgery, and to verify the effectiveness and clinical usefulness of robot-assisted gait training. Methods: This study was conducted on 14 subjects who underwent lower extremity surgery. The subjects participated in robot-assisted gait training for 2 weeks. The active ranges of motion of the lower extremities were evaluated, and gait abilities were assessed using 10-m and 2-min walk tests. An STT Systems Inertial Measurement Unit was used to collect data on biomechanical characteristics during gait. Spatiotemporal parameters were used to measure cadence, step length, and velocity, and kinematic parameters were used to measure hip and knee joint movement during gait. Results: Significant improvements in the active ranges of motion of the hip and knee joints (flexion, extension, abduction, and adduction) and in the 10-m and 2-min walk test results were observed after robot-assisted gait training (p < 0.05). In addition, biomechanical characteristics of gait, spatiotemporal factors (cadence, step length, and velocity), and kinematic factors (gait hip flexion-extension, internal rotation-external rotation angle, and knee joint flexion-extension) were also significantly improved (p < 0.05). Conclusion: The results of this study are of clinical importance as they demonstrate that robot-assisted gait training can be used as an effective intervention method for patients who have undergone lower extremity surgery. Furthermore, the findings of this study are clinically meaningful as they expand the scope of robot-assisted gait training, which is currently mainly applied to patients with central nervous system conditions.

  • Objective : The purpose of this study is to evaluate the effects of hip joint exercises and orthotics on RCSP, ankle's range of motion, and core muscle strength of middle school students with pes planus. Method : Out of the original pool of 200 students, 60 students with pes planus (RCSP < -2) were selected for the study. The selected 60 students were then divided into four groups. The first group was a combined orthotics and exercise group (12 students), the second was the orthotics-only group (9 students), the third was the exercise-only group (8 students), and the last was the control group (10 students). Exercise groups worked out twice a week for 60 minutes per session over 8 weeks. The independent variables were corrective hip joint exercises and orthotics. The dependant variables consisted of kinematic and kinetic variables. The kinematic variables were RCSP, and ankle's range of motion (dorsiflexion and plantarflexion). The kinetic variables were muscles forces that consist in core muscle strength, which are hip joint adduction, abduction, and flexion muscles forces. Statistical analysis was performed via SPSS 18.0 with multivariate analysis of covariance (MANCOVA) and a paired t-test was used. Results : The left foot was more responsive to the treatments, both exercise and orthotics, than the right foot. RCSP improved significantly in the left foot for the first and third groups. Only the first group significantly improved hip joint adduction, abduction, and flexion muscles' strengths. As for the ankle's range of motion of the left foot, plantarflexion showed improvement when treated with exercise, orthotics, or both. Conclusion : This study found that exercise is more effective in correcting RCSP and foot orthotics is more effective in reinforcing core muscle strength. Future studies should expand on these results to examine the relationship between the ankle, hip, and pelvis.

심부경부굴곡 운동 시 복근 수축이 표면경부굴곡근의 근활성도, 흉곽 거상, 두개경부굴곡 각도에 미치는 영향 (Effects of Contraction of Abdominal Muscles on Electromyographic Activities of Superficial Cervical Flexors, Rib Cage Elevation and Angle of Craniocervical Flexion During Deep Cervical Flexion Exercise)

  • The purpose of this study was to examine contraction of abdominal muscles on surface electromyographic (EMG) activity of superficial cervical flexors, rib cage elevation and angle of craniocervical flexion during deep cervical flexion exercise in supine position. Fifteen healthy subjects were participated for this study. All subjects performed deer cervical flexion exercise with two methods. The positions of two methods were no volitional contraction of abdominal muscles in hook-lying position with 45 degree hip flexion (method 1) and 90 degrees hip and knee flexion with feet off floor for inducing abdominal muscle contraction (method 2). Surface EMG activities were recorded from five muscles (sternocleidmastoid, anterior scaleneus, recuts abdominis, external oblique, internal oblique). And distance of rib cage elevation and angle of craniocervical flexion were measured using a three dimensional motion analysis system. The EMG activity of each muscle was normalized to the value of reference voluntary contraction (%RVC). The EMG activities, distance of rib cage elevation. and angle of craniocervical were compared using a paired t-test between two methods. The results showed that the EMG activities of sternocleidmastoid and anterior scaleneus during deep cervical flexion exercise in method 2 were significantly decreased compared to method 1 (p<.05). Distance of rib cage elevation and angle of craniocervical flexion were significantly decreased in method 2 (p<.05). The findings of this study indicated that deep cervical flexion exercise with contraction of abdominal muscles could be an effective method to prevent substitute motion for rib cage elevation and contraction of superficial neck flexor muscles.

Effect of Physiotherapeutic Intervention Using TECAR Therapy on Pain Self-Awareness and Hip Joint Function in Hip Impingement Syndrome: A Case Study

  • PURPOSE: The current case study focuses on identifying the effects of the independent application of TECAR therapy and physiotherapeutic intervention using TECAR therapy on pain self-awareness and hip joint function in patients with hip impingement syndrome caused by nonstructural changes. Subjects: The research subject was a 34-year-old woman struggling with acute pain in her left hip, difficulty in actively moving the hip, and a problem in its overall function. METHODS: The subject's pain awareness and hip joint function were measured using a Visual Analog Scale (VAS) and passive range of motion (PROM), respectively. The experimental intervention was carried out in 24 sessions of 16 minutes each, three times a week, for eight weeks. RESULTS: The VAS score decreased to 0 cm on the post-test from 4.3 cm, 6.5 cm, and 7.2 cm in the pre-test at the rest, standing, and gait positions, respectively. The index of PROM measured hip joint flexion, extension, abduction, adduction, internal rotation, external rotation, and passive straight leg raise. The values increased to 122.5°, 24.5°, 78°, 33°, 65°, 42°, and 96.5° in the post-test compared to 88.5°, 15°, 39°, 21.5°, 23°, 22°, and 46.5° in the pre-test, respectively. CONCLUSION: TECAR therapy and physiotherapeutic intervention using TECAR can help reduce pain and enhance the hip joint function in patients with hip impingement syndrome.

Knee Flexion Angles Influence Hip Extensor Activity During Prone Heel Squeeze

  • The purpose of this study was to determine the muscle activities of the erector spinae (ES), gluteus maximus (Gmax), gluteus medius (Gmed), and the hamstring (HAM) and the ratios of Gmax/ES, Gmax/HAM, and Gmed/HAM during the prone heel squeeze (PHS) with different knee flexion angles ($45^{\circ}$, $90^{\circ}$, and $135^{\circ}$). Fifteen young and healthy subjects (8 men, 7 women) were recruited for the study. Surface electromyography signals were collected on ES, Gmax, Gmed, and HAM during PHS. A separate one-way analysis of variance with repeated measures was used to determine the significance of the muscle activities of ES, Gmax, Gmed, and HAM and the ratios of Gmax/ES, Gmax/HAM, and Gmed/HAM with different knee flexion angles during PHS. There was a significant increase in the Gmax activity at the knee flexion of $90^{\circ}$ in comparison with that of the $45^{\circ}$ (p=.016). There were significant increases in the Gmed activity at the knee flexion of $90^{\circ}$ (p=.008) and $135^{\circ}$ (p=.006) in comparison with that of the $45^{\circ}$. There were significant decreases in the HAM activity at the knee flexion of $90^{\circ}$ (p=.009) and $135^{\circ}$ (p=.004) in comparison with that of the $45^{\circ}$. There were significant increases in the Gmax/HAM muscle activity ratio at the knee flexion of $90^{\circ}$ (p=.007) and $135^{\circ}$ (p=.012) in comparison with that of the $45^{\circ}$. There were significant increase in the Gmed/HAM muscle activity ratio at the knee flexion of $135^{\circ}$ in comparison with that of the $45^{\circ}$ (p=.008). The knee flexion of $90^{\circ}$ during PHS can induce decreasing activity of HAM and increasing activity of Gmax, and the knee flexion of $135^{\circ}$ during PHS can induce decreasing activity of HAM and increasing activity of Gmed. Hence, PHS with different knee flexion positions could be considered for the different target muscle.
