Piriformis Syndrome is characterized by pain of the low back, groin, perineum, buttock, hip, posterior thigh, leg and foot. Symptoms are aggravated by sitting, prolonged combination of hip flexion, adduction, the medial rotation, or by activity. In addition, patient may complain of painful swelling of the limb and sexual dysfunction-dyspareunia in female, and impotence in male. It currently appears that three specific conditions may contribute to the piriformis syndrome: (a) myofascial pain; (b) nerve and vascular entrapment; (c) dysfunction of the sacroiliac joint. The important keys of diagnosis are history and physical examination. There is no known objective diagnostic method. We described the clinical features of four cases of piriformis syndrome and reviewed foreign literature.
Prosthetic replacement is one of the most common methods of reconstruction after resection of malignant tumor around the knee. Gait analysis provides a relative objective data about the gait function of patients with prosthesis. The purpose of this study was to compare the gait pattern of the patients who underwent limb salvage surgery with prosthesis for distal femur and that of patients with prosthesis for proximal tibia. This study included ten patients (4 males, 6 females, mean age 22.7 years, range 14-36) who underwent a wide resection and Kotz hinged modular reconstruction prosthesis replacement and six normal adult(Control). The site of bone tumor was the distal femur (Group 1) in six patients and proximal tibia (Group 2) in 4 patients. The follow-up period ranged from 15 to 82 months (mean : 33 months). The evaluation consisted of clinical assessment, radiographic assessment, gait analysis using VICON 370 Motion Analysis System. The gait analysis included the linear parameters such as, walking velocity, cadence, step length, stride length, stance time, swing time, single support and double support time and the three-dimensional kinematics (joint rotation angle, velocity of joint rotation) of ankle, knee, hip and pelvis in sagittal, coronal and transverse plane. For the kinetic evaluation, the moment of force (unit: Nm/kg) and power (unit: Watt/kg) of ankle, knee and hip joint in sagittal, coronal and transverse plane. In the linear parameters, cadence, velocity, step time and single support were decreased in both group 1 and group 2 compared with control. Double support decreased in group 2 compared with control significantly(p<.05). In contrast to our hypothesis, there was no significant difference between group 1 and group 2. 이 연구는 여성노인의 태권에어로빅스 12주 훈련 후 앞굽이 몸통지르기 동작시 하지관절의 생체역학적 변화를 구명하는 것이다. 대상자는 여성노인 10명이 참여하였으며 카메라(MCU-240) 7대와 지면반력기(Kist1er-9286AA) 2대를 이용하여 데이터를 수집하였다. 유의수준 .10에서 운동전 후 차이는 다음과 같다. 첫째, 최소 관절각의 변화는 발목의 저측/배측굴곡(왼쪽, $p=0.001^*$), 외번/내번(양쪽, $p=0.009^*$, $p=0.04^*$)과 무릎의 외전/내전(왼쪽, $p=0.04^*$) 및 엉덩이의 내측/외측 회전(양쪽, $p=0.07^*$, $p=0.02^*$)에서 통계적으로 유의하게 나타났다. 둘째, 최대 관절모멘트 변화는 발목관절의 외번/내번 모멘트(양쪽, $p=0.05^*$, $p=0.05^*$), 무릎관절의 외전/내전 모멘트(왼쪽, $p=0.08^*$) 및 엉덩이관절의 내측/외측 회전 모멘트(오른쪽, $p=0.09^*$)가 통계적으로 유의하게 나타났다. 셋째, 최대 관절파워의 변화는 엉덩이관절의 굴곡/신전(양쪽, $p=0.05^*$, $p=0.01^*$)과 내전/외전(양쪽, $p=0.02^*$, $p=0.00^*$) 및 무릎의 내전/외전(왼쪽, $p=0.00^*$) 파워가 통계적으로 유의한 차이를 보였다. 결론적으로 태권에어로빅스 몸통지르기동작이 여성노인들의 하지 관절에 부분적인 생체변화를 일으켰다.
This paper reports on the development of a new foot for a quadrupedal jointed-leg type walking robot. The foot has 2 toes, one at the front and the other at the rear side, for stable landing on uneven ground by point contact. The toes can move up and down independantly, guided by double-wishbone shaped parallel links which enable the lower leg to rotate with respect to a remote center on the ground surface. The motion of each toe is damped by a hydropneumatic shock absorber integrated in the foot in order to absorb the dynamic landing shock. Furthermore, the new foot can reduce the maximum hip joint drive torque by shortening the moment arm length between the hip joint and the landing force vector on the ground. Intensive experiments were carried out in this study by using a one-leg walking model to investigate the soft landing performance of the foot which could be hardly offered by conventional robot feet such as a flat plate with a gimbal type ankle joint. And it was confirmed that the hip joint torque of the leg walking on the flat surface could be reduced remarkably by using the new foot.
PURPOSE: This study examined whether a task-oriented training program is an effective intervention to improve the body function, activity, and participation of children with cerebral palsy (CP). METHODS: Ten children with CP (7-13 years old) performed a task-oriented training program for eight weeks (three sessions per week, 30 minutes each). The taskoriented training program consisted of eight activities. The subjects' body function was assessed using a handheld dynamometer, goniometer, Modified Ashworth Scale (MAS), Balance Performance Monitor (BPM), and the Bruininks-Oseretsky Test of Motor Proficiency (BOTMP). The subjects' activity and participation were assessed using the Gross Motor Function Measure (GMFM) and Timed Up and Go (TUG) test. RESULTS: Task-oriented training provided significant improvements in the subjects' body function. The subjects improved the bilateral isometric muscle strength of the hip flexors, extensors and abductors, knee flexors and extensors, and ankle dorsi- and plantar flexors (p<.05). Bilateral passive hip flexion, abduction, and external rotation, knee flexion, and ankle dorsi- and plantar flexion were also increased (p<.05). In addition, the MAS score of the hip adductors decreased (p<.05) and the BOTMP score increased after training (p<.05). The subjects' activity and participation also improved significantly after training, increasing the GMFM score (p<.05) and decreasing the TUG score (p<.05). On the other hand, the BPM score did not change after training. CONCLUSION: This study suggests that a task-oriented training program can be an effective intervention to improve the body function, activity, and participation for children with CP.
The purpose of this study was to compare kinematic characteristics on the limbs at 3 different walking speed during the energy and the normal walking. Eight subjects performed energy walking and normal walking at the slow speed(65 beats/min), the normal speed(115beats/min), the fast speed(160 beats/min). The 3-d angle was calculated by vector projected with least squares solution with three-dimensional cinematography(Motion Analysis corporation). The range of motion was calculated on the trunk, shoulder, elbow, hip, knee joint. The results showed that stride length was no difference of the two walking pattern. The duration of support phase was also no difference of the two walking pattern. The range of motion of shoulder joint significantly increased in the sagittal and frontal planes, and the range of motion of elbow joint significantly increased as the energy walking. The range of motion of hip joint had no significant difference in the any planes in changing of walking speed. But the most remarkable difference of the two walking patterns revealed at the trunk. The range of flexion/extension angle had significant increasing $2.36^{\circ}$ at normal speed, and the range of the right/left flexion angle had significant increasing below $4^{\circ}$ at the 3 walking speed, and The range of rotation angle had significant increasing $7.35^{\circ}$, $9.22^{\circ}$, respectively at the normal and slow speed. But there was no significant difference of range of motion at the hip and knee joints between energy walking and normal walking.
The purpose of this study was to investigate the biomechanical of K-Pop dance movement. The study was conducted on 15 male and 15 female subjects in 20-30 age groups. And they choose 150 K-Pop dance choreographies in the top 10 ranking of the main charts. We analyzed the RoM, joint moment and impulse force of the highlight movements. First, During the K-Pop dance motion, the usage of knee joints are more than the hip joints and the ankle joints, and female dancers has a larger range of motion than the male dancers. Second, male dancer uses more than female dancers when they compared the load of male dancers and female dancers. In particular, flexion and extension of the hip joints are mostly used in this study. Third, the impulse force of male dancers was greater than of female dancers, but it was statistically insignificant, this is equal to the impulse on walking. In conclusion, Female dancers use more range of motion than male dancers, but male dancer choreography requires greater torque, which can strain joints. Most choreographic exercises involve movements such as hip joint, knee joint, flexion of ankle joint, extension, rotation, and jumping.
최근 영유아 성장기에 발생하는 고관절 이형성증(Developmental Dysplasia of Hip, DDH)의 숫자가 늘어나고 있다. DDH는 영유아 성장을 방해하고 다른 부작용도 많이 발생시키기 때문에 최대한 조기에 발견하여 치료해야 한다. 최근 들어 Convolutional Neural Networks (CNN) 및 개선된 Resnet50을 활용한 머신러닝 기법이 초음파 영상 분석에 많이 활용되고 있다. 연구 결과를 보면 컴퓨터 보조 이미지 분석이 의료현장에서 객관성과 생산성을 크게 향상시키고 있다. 본 연구의 결과는 정형외과에서의 난제인 초음파 영상을 통한 DDH 컴퓨터 보조 진단 알고리즘에도 충분히 활용될 수 있다는 것을 보여주고 있다. 본 논문에서는 CNN을 활용하여 DDH를 자동으로 측정하고 진단할 수 있는 컴퓨터 보조 진단 알고리즘을 제안하였다. DDH 측정을 위해 유아 고관절의 정상/비정상 판독을 위해 Acetabulum-Femoral head의 angle을 자동으로 계산하였으며 기존 영상을 딥 러닝하여 진단을 자동으로 하는 알고리즘을 설계하였다. 실험 결과 의사와 비교하여 진단의 속도와 정확도가 향상된다는 것을 확인하였다.
Background: This study was to investigate the effects of active stretching and stability exercise on piriformis muscle thickness and low back pain of male patients with chronic low back pain. Design: Randomized Controlled Trial. Methods: 45 male patients participated in this study. All subjects were randomly assigned. Subjects divided into 15 who underwent contract relaxation (CR) of the proprioceptive neuromuscular facilitation (PNF) to their hip joints, 15 who underwent a combination of isotonic (CI) of the PNF, and the rest 15 who underwent the two techniques (CR+CI). Real-time diagnostic ultrasound was used to measure thickness of the piriformis muscles. The visual analogue scale (VAS) was used to measure degrees of low back pain. A two-way repeated measures ANOVA was used to compare the average values. The SPSS 24.0 was used as the statistical program, and the significance level was set at .05. Results: The CR and the CR+CI groups had more decreased thickness of piriformis muscle when compared to the CI group. The CR+CI group had more decreased pain when compared to the CR and the CI groups. Conclusion: These results suggest that combination of stretching and stability exercise was effective on male patients with chronic low back pain showing limited hip medial rotation.
This study was attempted to Kinematical characteristics of the Endo $360^{\circ}$El-grip Swing on the horizontal bar. To do this, we selected and analyzed the performance of two athletes who did Endo $360^{\circ}$El-grip Swing in the horizontal bar competition of male artistic gymnastic in the 22nd Universiad Games 2003 Daegu. We drew the conclusions from the kinematical factors that were came out through analyzing three-dimensional cinematography of the athletes' movements, by using two video cameras. In point of analyzing the actual competition situation, it is expected that gymnastics and coaches have the effective informations, and the following conclusion had resulted. 1. When performing Endo $360^{\circ}$El-Grip, the average for entire required time was $1.93{\pm}0.06sec$. The average for descent phase time was $0.24{\pm}0.02sec$, ascent phase time was 0.22${\pm}0.07sec$, connecting phase time was $0.87{\pm}0.07sec$, and El-Grip phase time was $0.61{\pm}0.02sec$. The descent phase need short period of time but however to have a stable performance, ensuring ascent and connecting phase time are needed. El-Grip phase need short period of time to have a stable re-grasp. 2. To have a convenient preparation for El-Grip in descent and ascent phase, lowering CM, and ease up in sway and plunge from the High Bar would make descent and ascent even more faster and would have increase effect in trunk rotation. 3. In descent and ascent phase, if shoulder angle and arm slope is dwindling then it would effect rotation angle so might risk it from hitting a Bar when putting legs in and out. 4. In connecting phase, it requires some time to show stable performance when El-Grip phase is continued by using hip angle which would make trunk rotation angle bigger and make descent and ascent time slower. 5. In El-Grip phase, when doing motions like hand standing. using hip angle more than maximum would make CM even faster and it is stable position while performing.
