Transient osteoporosis of the hip with a femoral neck fracture during follow-up: a case report

  • We report a case of transient osteoporosis of the hip with a femoral neck fracture found during follow-up. A 53-year-old man presented with left hip pain without trauma. The pain did not improve after 2 weeks and he was brought to our hospital by ambulance. Magnetic resonance imaging (MRI) of the left hip joint showed diffuse edema in the bone marrow, which was identified by low signal intensity on T1-weighted images, high signal intensity on T2-weighted images, and increased signal intensity on short tau inversion recovery. This edema extended from the femoral head and neck to the intertrochanteric area. He was diagnosed with transient osteoporosis of the left hip. Rest gradually improved his pain; however, 3 weeks later, his left hip pain worsened without trauma. X-ray, computed tomography, and MRI results of the hip joint demonstrated a left femoral neck fracture, and osteosynthesis was performed. Differential diagnoses included avascular necrosis of the femoral head, infection, complex regional pain syndrome, rheumatoid arthritis, leukemia, and other cancers. Transient osteoporosis of the hip generally has a good prognosis with spontaneous remission within a few months to 1 year. However, a sufficient length of follow-up from condition onset to full recovery is necessary to avoid all probable complications such as fractures.

  • This study was developed the metallic plate for fixation in the femur fracture for the orthopedic region and rigid fixation with plates has a firm place in fracture treatment. Most plates can be used for rigid as well as biological and dynamical fracture fixation. The device's designation and sizing has a specific with bending structural stiffness and strength, known meaning that is reliable regardless of the plate by the short type and long type. Short plate have a wrapping of femur and long plate have to preserve a pole of femur. The bending strength of the curved metallic long plate has to evaluate a 11,000N and The bending strength of the curved metallic short plate has to evaluate a 6,525N. The tensile stress through to press a plate is $1573N/m^2\;and\;1539N/m^2$. The device can be used to support Revision case of Hip Implant and to use a case of Hip screw compression of Hip Neck Fracture.

Physical Activity and Hip Fracture in Elderly People : A Cohort Study in Korea (노인 인구에서 육체적 활동과 고관절 골절 발생간의 관련성 : 코호트 연구)

  • Objective : To evaluate the relationship between physical activity and the risk of hip fracture in the elderly Korean people. Methods : The study population was a Physical Activity Subcohort (n=8,908) extracted from the Korean Elderly Pharmacoepidemiological Cohort (KEPEC). Physical activity information was obtained from a mailed questionnaire surveys. The outcome data was collected from claims data gathered between Jan. 1993 and Dec. 1998. A hospital survey relating to potential cases was conducted to confirm the final diagnoses. The abstracted data was reviewed by a medical doctor before the final diagnoses were confirmed. A mailing questionnaire survey was performed to obtain information on potentially confounding variables, including alcohol intake, smoking habits, weight, height and postmenopausal duration. There were 79 confirmed cases hospitalized due to hip fractures between Jan. 1993 and Dec. 1998. Relative risk of physical activity scores on the hip fracture, and their 95% confidence intervals, were estimated by a Cox's proportional hazard model using SAS for Windows ver.6.12. Results : Compared to the reference group, the adjusted relative risk of hip fracture associated with the most physical active category: after controlling for age, weight and alcohol intake in the males, and for weight, alcohol intake and postmenopausal duration in the females, were 1.04 (95% CI=0.35-3.06) and 0.f4 (95% CI=0.26-0.77), respectively. Conclusions : Physical activity may protect elderly women from hip fracture.

  • Directional effect of applied pressure during sintering on the microstructure and fracture toughness of the heat-treated silicon nitride ceramics has been investigated. The specimens with a composition of 92Si3N4-8Y2O3(in wt%) were sintered at 172$0^{\circ}C$ by a hot press (HP ) and a hot isostatic press (HIP) and heat-treated for grain growth at 1800~20$0^{\circ}C$. The fracture toughness of the HP samples increased with the grain size while the fracture toughness of the HIP treated samples remained the same even though the grain growth occurred. This discrepancy was explained by a bimodal grain size distribution and large aspect ratio of the HPed samples and a monomodal grain size distributjion and samll aspect ratio of the HIP treated samples.

  • Purpose: To investigate the results of treatment of reverse oblique trochanteric fractures with compression hip screw. Methods: We reviewed the results of 12 cases of reverse oblique trochanteric fracture treated with compression hip screw from January 2000 to December 2006 which could be followed up for more than 1 year. The mean follow up period was 26 months (15~40). The mean age was 48 years old. Injury mechanism was composed of 6 cases of traffic accident and 6 cases of fall down. 8 persons were man. We investigated the union time, degree of neck-shaft angle change, amount of sliding of compression hip screw, complications, functional and clinical results. Results: 10 cases were united and the mean union time were 5 months (3~8). The mean neck-shaft angle change was 3.5 degrees (0~12). The amount of sliding of compression hip screw was 8.9 mm (2~24). There were six coxa vara, six leg due to coxa vara shortening, two nonunion, and one superficial infection. Unsatisfactory results of Jensen's social function score and Parker and Palmer's mobility score were studied. Conclusion: The results of treatment of reverse oblique trochanteric fractures with compression hip screw were relatively unsatisfied.

  • Traumatic fracture-dislocations of the hip frequently result from high-energy injury, and hip dislocations are commonly associated with severe concomitant injuries. Sciatic nerve injury often accompanies traumatic fracture-dislocation of the hip, but neurologic examination at the time of injury is difficult in severely traumatized patients with decreased consciousness. We present such a case of multiple traumas with traumatic hip dislocation and sciatic nerve injury after reduction, and we found that magnetic resonance image (MRI) played an important role in developing a management plan.

  • MOR test and concentric ring test were performed to evaluate the failure probability of sin/hip Si3N4 under uniaxial and biaxial stress state, respectively. Their failure probabilities were analized with KARA program based on Weibull PIA model and Batdorf model with 5 criteria, and they were compared with experiments. PIA model is in best accordance with experiments in higher fracture strength regions, especially for Pf 0.3. But in lower fracture strength region, none of the models predicts the failure probabilities appropriately.

  • Purpose:This study performed to evaluate the effect of hip fracture among persons over 50 years old, in Jeju-do, Korea, on mortality, activity of daily living(ADL). Methods:Three hundred eighteen patients over to 50 years of age who lived in Jeju-do and sustained a hip fracture from January 1st 2003 to December 31th 2004 were recruited. The information collected from the hospital records for each patient included age, sex, date of onset, and patients investigated activity grade, ADL, outdoor walking aid on 3month, 6month and 12month. Results:The incidence was 12.5/10,000 for total patients(female 17.0/10,000, male 6.3/10,000) in population over 50 years old. Patients who had lower activity grade and ADL level, showed higher mortality, and patients using outdoor walking aid showed higher mortality. Conclusion:The activity grade, ADL level, Using of outdoor walking aid affect mortality, so more programs about ADL must be developed for the life span and quality of life.

Factors Influencing on Pressure Ulcer Incidence among Older Patients with Hip Fracture in a Hospital (고관절 골절로 입원한 노인 환자의 욕창 발생 위험요인)

  • Purpose: This study aimed to identify the incidence and risks for pressure ulcer among older patients with hip fracture. Methods: The subject were 215 older patients suffering from hip fracture who were admitted for surgical operation from January 1, 2012 to April 30, 2016 in a university-affiliated hospital. The incidence of pressure ulcer was collected retrospectively through medical record review and the risk factors were analyzed using Cox's proportional hazard model. Results: Out of the total, 32 patients (14.9%) developed pressure ulcer with the average occurrence period being 4.72 (${\pm}3.81$) days. Stage II pressure ulcer was the most common at 72.0%. Risk factors included ambulation status before injury (p= .039), spinal anesthesia (p= .029), and stay at intensive care unit after operation (p= .009). Conclusion: Despite pressure ulcer prevention efforts, the incidence remained relatively high. Considering the identified risk factors, more efforts is needed for early detection and prevention of pressure ulcers in such patients.

Incidence of Deep Vein Thrombosis before Hip Fracture Surgery Diagnosed by Color Doppler Sonography Surveillance (고관절 주위 골절환자에서 술 전 색 도플러 초음파를 이용하여 진단한 심부 정맥 혈전의 빈도)

  • Purpose: To evaluate the incidence of deep vein thrombosis (DVT) before hip fracture by duplex color Doppler ultrasonography. Materials and Methods: From June 2013 to May 2014, 27 patients who had agreed to perform color Doppler ultrasonography before hip fracture surgery were evaluated for the incidence of DVT. Patients who had history of DVT were excluded. Five patients were men and 19 patients were women. The mean age was 74.3 years old (41-87). There were 15 cases of femoral neck fracture, 11 cases of intertrochanteric fractures and one case of acetabular fracture. Surgical intervention underwent within 48 hours from admission and duplex color Doppler ultrasonography was carried out at the day of admission. Results: DVT occurred in six cases (22.2%). Four cases (14.8%) occurred in proximal deep vein and two cases (7.4%) occurred in distal deep vein. The mean period of immobilization was longer in patients who had DVT. But there was no significant difference. The mean age was 79 years old (75-87) in patients who had DVT and 72 years old (65-86) in patients who had not. There was significant difference (p=0.038). Conclusion: The incidence of DVT which was diagnosed by duplex color Doppler ultrasonography before hip fracture surgery was relatively high (22.2%). So it is necessary to undergo duplex color Doppler ultrasonography more aggressively to rule out DVT before hip fracture surgery.
