우측 협부에 발생한 윤활막육종 증례보고 (A Case Report of Synovial Sarcoma of the Face)

  • Synovial Sarcoma is the fourth most common sarcoma, accounting for 8-10 % of all sarcomas. Synovial sarcoma is highly malignant tumor of mesenchymal origin but rarely occurres in head and neck area. Less than 100 cases of synovial sarcoma occurring in head and neck area have been reported all over the world. Pathologically, there is two type of synovial sarcoma: monophasic variant is composed of only one cell type and "classic" (biphasic) synovial sarcoma has two cellular component, a spindle cell(fibrosarcoma-like) component and a pseudoepithelioma component. Recommended treatment is wide resection with negative margins. The role of chemotherapy and radiation therapy is controversial. We experienced a 42-year-old male patient with slowly enlarging, deep seated mass on right cheek. In the first operation, we suggested that the mass maybe benign tumor. But, initial excisional biopsy specimen of the primary lesion was consistent with synovial sarcoma. The final diagnosis was monophasic synovial sarcoma which was composed of spindle cells. Radical resection was performed two months later because remnant tumor was found on follow up MRI. No further treatment was done. There were no recurrence or metastasis on follow up MRI, chest CT and whole body bone scan after 15 months. This is a report of a rare case of synovial sarcoma of the face with a literature review.

Accuracy and robustness of hysteresis loop analysis in the identification and monitoring of plastic stiffness for highly nonlinear pinching structures

  • Structural health monitoring (SHM) covers a range of damage detection strategies for buildings. In real-time, SHM provides a basis for rapid decision making to optimise the speed and economic efficiency of post-event response. Previous work introduced an SHM method based on identifying structural nonlinear hysteretic parameters and their evolution from structural force-deformation hysteresis loops in real-time. This research extends and generalises this method to investigate the impact of a wide range of flag-shaped or pinching shape nonlinear hysteretic response and its impact on the SHM accuracy. A particular focus is plastic stiffness (Kp), where accurate identification of this parameter enables accurate identification of net and total plastic deformation and plastic energy dissipated, all of which are directly related to damage and infrequently assessed in SHM. A sensitivity study using a realistic seismic case study with known ground truth values investigates the impact of hysteresis loop shape, as well as added noise, on SHM accuracy using a suite of 20 ground motions from the PEER database. Monte Carlo analysis over 22,000 simulations with different hysteresis loops and added noise resulted in absolute percentage identification error (median, (IQR)) in Kp of 1.88% (0.79, 4.94)%. Errors were larger where five events (Earthquakes #1, 6, 9, 14) have very large errors over 100% for resulted Kp as an almost entirely linear response yielded only negligible plastic response, increasing identification error. The sensitivity analysis shows accuracy is reduces to within 3% when plastic drift is induced. This method shows clear potential to provide accurate, real-time metrics of non-linear stiffness and deformation to assist rapid damage assessment and decision making, utilising algorithms significantly simpler than previous non-linear structural model-based parameter identification SHM methods.

Development of Highly Reliable Power and Communication System for Essential Instruments Under Severe Accidents in NPP

  • This article proposes a highly reliable power and communication system that guarantees the protection of essential instruments in a nuclear power plant under a severe accident. Both power and communication lines are established with not only conventional wired channels, but also the proposed wireless channels for emergency reserve. An inductive power transfer system is selected due to its robust power transfer characteristics under high temperature, high pressure, and highly humid environments with a large amount of scattered debris after a severe accident. A thermal insulation box and a glass-fiber reinforced plastic box are proposed to protect the essential instruments, including vulnerable electronic circuits, from extremely high temperatures of up to $627^{\circ}C$ and pressure of up to 5 bar. The proposed wireless power and communication system is experimentally verified by an inductive power transfer system prototype having a dipole coil structure and prototype Zigbee modules over a 7-m distance, where both the thermal insulation box and the glass-fiber reinforced plastic box are fabricated and tested using a high-temperature chamber. Moreover, an experiment on the effects of a high radiation environment on various electronic devices is conducted based on the radiation test having a maximum accumulated dose of 27 Mrad.

성형외과 영역에서 이물질 주사에 대한 고해상초음파 검사의 유용성 (Usefulness of High-Resolution Ultrasonography after Foreign Body Injection on Aesthetic Plastic Surgery)

  • Purpose: The purpose of this study is to demonstrate the usefulness of the high resolutional ultrasonographic features in patients with foreign body. Methods: From September 2007 to August 2009, we retrospectively reviewed high resolutional ultrasonogram using 5~12 MHz linear transducer of 13 patients presenting with inflammation after foreign body injection. They were referred for complications after foreign body injection. Injected foreign bodies were 4 silicone, 4 paraffin, 2 artecoll, and 3 unknown. We treated them with foreign body removal (7), foreign body removal and corrective plastic surgery (4), and conservative treatment with antibiotics and steroid injection (2). Results: High resolutional ultrasonography well demonstrated the existence of foreign body and it's overall size, location within the tissue layer, and vascularity. Comparison between preoperative and postoperative ultrasonographic findings was useful not only to evaluate the prognosis but also to plan the treatment. These ultrasonographic findings aided in precise assessment of the contour and location of the foreign body and led to an accurate surgery. We were able to acquire various information in order to set a detailed plan for the operation which in turn, led to a precise, successful surgery. After the treatment, complication did not occur in 12 patients, except 1 patient. But this patient was also treated after reoperation. Postoperative high resolutional ultrasonography shows almost foreign body removed and inflammation disappeared. Conclusion: Considering the usefulness of highresolution ultrasonography in foreign body injection, highresolution ultrasonography would be necessary for both the patient and the doctor. Preoperative and postoperative high resolutional ultrasonography is highly accurate, safe, inexpensive and easy. It can be a useful modality in foreign body after plastic surgery.

Validation of a Cognitive Task Simulation and Rehearsal Tool for Open Carpal Tunnel Release

  • Background Carpal tunnel release is one of the most common surgical procedures performed by hand surgeons. The authors created a surgical simulation of open carpal tunnel release utilizing a mobile and rehearsal platform app. This study was performed in order to validate the simulator as an effective training platform for carpal tunnel release. Methods The simulator was evaluated using a number of metrics: construct validity (the ability to identify variability in skill levels), face validity (the perceived ability of the simulator to teach the intended material), content validity (that the simulator was an accurate representation of the intended operation), and acceptability validity (willingness of the desired user group to adopt this method of training). Novices and experts were recruited. Each group was tested, and all participants were assigned an objective score, which served as construct validation. A Likert-scale questionnaire was administered to gauge face, content, and acceptability validity. Results Twenty novices and 10 experts were recruited for this study. The objective performance scores from the expert group were significantly higher than those of the novice group, with surgeons scoring a median of 74% and medical students scoring a median of 45%. The questionnaire responses indicated face, content, and acceptability validation. Conclusions This mobile-based surgical simulation platform provides step-by-step instruction for a variety of surgical procedures. The findings of this study help to demonstrate its utility as a learning tool, as we confirmed construct, face, content, and acceptability validity for carpal tunnel release. This easy-to-use educational tool may help bring surgical education to a new- and highly mobile-level.

The Correlation between the Orbital Volume Ratio and Enophthalmos in Unoperated Blowout Fractures

  • Background Enophthalmos may not appear immediately after trauma due to periorbital swelling in a blowout fracture, and preoperative measurements of enophthalmos cannot be used as a reliable guideline. It is important to predict the eventual final extent of enophthalmos in order to determine whether to perform surgery, and there have been several attempts to predict the degree of late enophthalmos using preoperative orbital volume. The purpose of this study is to investigate the correlation between the orbital volume ratio (OVR) with final enophthalmos and the palpebral fissure, and to find the OVR that induced 2 mm of enophthalmos in unilateral unoperated blowout fractures. Methods We retrospectively reviewed the medical records of 38 patients and divided them into 3 groups, determined by the fracture location. The relationships between the OVR and both the degree of enophthalmos and the palpebral fissure ratio (PFR) were assessed and, in particular, the OVR that induced 2 mm of enophthalmos was sought. Results Enophthalmos increased in proportion to the OVR, and there was a highly significant correlation between the increase in the OVR and the degree of enophthalmos (P<0.05). On the other hand, there was no correlation between OVR and PFR (P>0.05). The OVR that induced 2-mm enophthalmos was 112.18%. Conclusions The final degree of enophthalmos can be estimated by the preoperative measurement of OVR. Preoperative measurements of OVR can be used as quantitative values to predict the final degree of enophthalmos in pure blowout fractures.

  • Purpose: Previous studies of the facial artery have shown significant anatomical variability in this region. The vascular anatomy of the region is considered unreliable in predicting the ideal pedicle. Preoperative imaging has been suggested as a means of improving preoperative awareness, with Doppler ultrasound as useful tools. Multi-detector row angiographic computed tomography (angio CT) is a significant improvement, providing noninvasive operator-independent details of the vascular anatomy. This tool was used to perform an $in$ $vivo$ anatomical study of the facial artery, demonstrating the usefulness of facial angio CT in planning the facial reconstruction. Methods: Eleven consecutive patients underwent facial angio CT of the facial vasculature with the anatomical details of the facial artery assessed. Results: Facial angio CT could demonstrate the size and course of the facial vasculature, particularly the facial artery. Conclusion: The vascular anatomy of the facial artery is highly variable, and thus there is a role for preoperative imaging. Facial angio CT can demonstrate cases where there is an aberrant or non-preferred anatomy, or select the method of a facial reconstruction.

'플라스틱'에 대한 초등 예비교사들의 인식 조사 - 플라스틱의 비극성 성질 중심으로 - (Elementary Preservice Teachers' Conceptions about 'Plastics' - Focusing on Non-Polar Property -)

  • The purpose of this study is to investigate the elementary preservice teachers' conceptions about 'plastics' focusing on non-polar property from a National University of Education. For the study, the views about plastics, relative shapes of a water droplet on plastic or glass material, and relative shapes of water surface in the plastic or glass measuring cylinder were surveyed from the preservice teachers. And the responses were analyzed based on the patterns. The results from the study are as follows: First, most preservice teachers were well aware of the plastic products which are used in daily life. Second, the responses concerning the reason why plastics can be used commonly were divided into 2 categories with 14 sub-level groups. However relatively few preservice teachers mentioned regarding 'chemical stability' and 'conductivity', which are associated with the plastics' non-polar property. Third, it was found that 50 participants (30.1%) had 'Scientific conception (Sc)', 38 (22.9%) had 'Partial-scientific conception (Ps)', 66 (39.8%) had 'Misconception (Mc)', and 12 (7.2%) had 'No conception (Nc)' on the subject of the relative shapes of a water droplet. Fourth, the distribution patterns and the ratio of the preservice teachers' conception on the survey question 3 concerning the relative shapes of water surface were quite similar to those of the survey question 2. So it was concluded that overall understanding level of the preservice teachers was pretty low on the subjects of the relative polarities of the plastic, glass, and water as well as their interactions. Fifth, the distribution percentile of 'Sc'/'Ps'/'Mc'/'Nc' was not related with the gender but highly correlated with preservice teachers' academic field and their science subjects taken in high school. Based on the results from the study, some educational guidelines were suggested.

프라스틱 패키징 전과 후 실리콘 칩들의 휨 파괴 운형에 대한 변화 (Variation in Flexural Fracture Behavior of Silicon Chips before and after Plastic Encapsulation)

  • 본 연구는 실리콘 칩의 휨 강도가 칩 이면에 존재하는 웨이퍼 그라인딩 관련 결함(스크래치)의 존재에 크게 영향을 받을 수 있음을 보여준다. 반면, 프라스틱을 이용하여 몰딩된 패키지 상태에서의 휨 강도는 칩 이면과 패키지 몸체와의 접착력이 우수할 경우 칩 이면의 스크래치에 의해 크게 영향을 받지 않음을 보여준다. 본 논문에서 프라스틱 패키징 전후 스크래치 마크에 대한 휨 강도의 차별화된 의존도가 왜 발생하는지에 대한 설명이 수록되어 있다. 본 연구에서는 웨이퍼의 이면연마 과정에서 필연적으로 남게 되는 스크래치 마크를 가진 칩들을 프라스틱 패키징 하여 휨 강도를 평가하였다. 칩의 휨 강도는 스크래치 마크의 깊이에 따라 크게 영향을 받지만, 패키지 상태에서는 그 영향력이 크지 않다는 것을 알 수 있었다. 분석결과 거친 스크래치를 가진 칩들은 프라스틱 패지지 몸체와의 결합력이 양호하여 칩 이면의 스크래치 마크에 집중될 수 있는 응력을 패키지 몸체에 의해 상당부분 흡수할 수 있기 때문인 것으로 평가되었다. 따라서, 얇은 패키지의 개발을 위해서는 칩 이면에 존재하는 스크래치의 제거뿐만 아니라 칩과 패키지 몸체 사이의 접착력 향상을 위한 방안이 함께 고려되어야 한다는 것을 알 수 있다. 또한, EMC 재질 개선에 의해 웨이퍼 그라인딩 공정 단순화를 이루어 제품의 가격 경쟁력을 높일 수 있을 것으로 전망된다.

두피에 발생한 혈관 육종의 치험 2례 (Case Report of Angiosarcoma on Scalp (2 cases))

  • Purpose: Angiosarcoma of the scalp is unusual vascular tumor originating from endothelial cell. Angiosarcoma is an aggressive tumor with high propensity for both local recurrence and distant metastasis. We report 2 cases of angiosarcoma having poor prognosis recently. Methods: Case 1 was a 67 - year - old male patient. He visited the hospital with a $3{\times}5cm$ sized discolored mass in forehead. It began at one month ago from coming to the hospital. Case 2 was a 64 - year - old male patient. He visited for our hospital to remove a $4{\times}5cm$ sized scalp mass. He had a pruritis on scalp from 9 months ago before coming to the hospital. Despite of the conservative treatments, the wound was not healed and advanced necrotic lesion with hemorrhage. Results: Case 1 diagnosed as an angiosarcoma. He underwent a radical operation 5 times. But the tumor expanded multiple area in dura & frontal area of the brain. The patient rejected the treatment any more. And he died one month later. Case 2 diagnosed as an angiosarcoma and metastased to skull in MRI. He got a radical resection including cranium. Three months later, it recurred to ipsilateral Sternocleidomastoideus muscle. He got a additional operation & started radiotherapy. And now he is receiving chemotherapy, but the recurred lesion is expanding. Conclusion: Angiosarcoma is a highly malignant tumor. Especially it arises in vascularized area, it easily metastases. So it is the best to treat angiosarcoma with surgery included wide margins. Despite of the lesion is small, we remember that angiosarcoma is a tumor that consider to metastases.
