This study suggests a model for continuing and comprehensive management of hypertension or Type 2 diabetes mellitus (T2DM) in Korea. Moreover, this paper computed the contribution cost of hypertension or T2DM management using the healthcare medical cost, which could have occurred from stroke, myocardial infarction (MI), and end-stage renal disease (ESRD) that were successfully prevented from the effective hypertension or T2DM management. Additionally, these costs were compared with the cost of implementing the hypertension or T2DM management model suggested in this study. This study used the medical fee summary of the health insurance claims submitted to National Health Insurance Corporation by medical facilities for services provided during the period from January 1st 1999 to December 31st 2006. The prevalence rate with treatment referred to cases in which patients submitted their medical claims at least once during the period, along with an accordant diagnosis. The incidence rate with treatment referred to cases in which patients who never submitted claims for the accordant disease during the five years from 1999 to 2003 submitted claims for the accordant disease in 2004 and 2005. The relative risk of the occurrence of stroke, MI and ESRD was 11.0, 13.6, and 30.3, respectively. The attributable risk of hypertension or T2DM for stroke was 0.730, and that for MI and ESRD were 0.773 and 0.888, respectively. Based on these, the contribution cost of hypertension or T2DM is estimated to be 986.3 billion Korean Won(KRW) for stroke patients, 330.5 billion KRW for MI patients, and 561.7 billion KRW for ESRD patients as in 2005. Hence, the total contribution cost of hypertension or T2DM to stroke, MI, and ESRD is 1.878 trillion KRW. The estimate for operational costs included an annual expenditure of 50,000 KRW per each recipient and an annual subsidy of 0.22 million KRW per person for the 1.6 million low.income individuals with hypertension or T2DM to cover their out.of.pocket medical expenses. Under this assumption, it took approximately 0.6 trillion KRW to manage 5 million high.risk patients in the low. and mid.income range, coverings up to 50% of costs. In conclusion, considering the potential benefits of preventing stroke, MI, and ESRD, the costs seems to be reasonable.
This paper was performed for a cost-effectiveness analysis of pharmacologic treatment of hypercholesterolemia. Agents modeled were cholestyramine, gemfibrozil. bezafibrate, lovastatin, pravastatin, simvastatin. Pharmacologic effectiveness was estimated by regression from reported clinical trials. Pharmacologic effects were expressed as the percent change of blood cholesterol level. Cost estimates included patients' travel expenses and time loss as well as resource consumption in the health care sector. Bezafibrate was the most efficient agent for reducing total cholesterol levels, having an cost over 1 year of ₩31.400 per percent reduction in total cholesterol. Simvastatin (10mg/d) was also efficient(₩33,100 per percent reduction). Chole styramine(8g/d) was least efficient at ₩90,200. For low-density lipoprotein cholesterol. simvastatin(10mg/d) was most efficient, at ₩23,200 per percent reduction, followed by lovastatin(20mg/d) at ₩28,000. Gemfibrozil was least efficient at ₩77,800 per percent reduction. For high-density lipoprotein cholesterol. bezafibrate(400mg/d) was most efficient at ₩39,300 per percent increase of high-density lipoprotein cholesterol. Cholestyramine was least efficient at ₩514,700. Analyses combining low-density lipoprotein cholesterol and high-density cholesterol effects suggest that bezafibrate(600mg/d) and simvastatin (10mg/d) were most efficient for reducing cardiovascular risk. The cost-effectiveness analysis results show that both simvastatin and bezafibrate could be efficient treatment. Simvastatin provide more effective treatment at higher cost, whereas bezafibrate is more cost-effective, as it may be less effective, at lower cost. Therefore, clinicians should choose reasonable treatment according to the patient's needs This pharmacoeconimc analysis will provide a guideline for efficient pharmacologic treatment and also be reference data for pricing new drugs.

Large hospitals such as medical centers provide not only medical services, but also carry the responsibilities for emergency refuges, medical researches and education. The function of large hospitals is as important as other infrastructure systems such as highways, bridges, and utilities. When disasters occur, the hospitals have to provide emergency medical services for victims and support the patient's needs of health. In order to keep a smooth operation of hospitals, the maintenance management of hospital buildings should be carefully investigated. However, there are few researches focused on maintenance management issues of hospital buildings. This paper investigated the National Taiwan University Hospital (NTUH) and established a maintenance cost database. The NTUH is the best-known and most high-renowned medical center in Taiwan in which more than 4,000 employees serving approximately 2,000 in-patients and 7,000 out-patients daily. The data were collected from the NTUH which consisted of 16,228 maintenance records in the past ten years. This paper analyzed these data to obtain various characteristics of maintenance records, and revealed the key items of maintenance cost for large hospital buildings, which can provide the facility manager of hospital buildings to execute a proper maintenance policy for hospital buildings.
Exposure factors in the chest radiographic examination vary with hospital. They include low voltage radiation to high voltage hard radiation quality, which are in wide use. In the present report, exposure factors generally employed are reviewed, and the chest radiograhic techniques performed in our hospital are explained. In addition, the attitude of radiological technologists toward patients and so forth are also discussed.
This study describes associated factors of readmission of 213 inpatients from an university hospital in Seoul. This retrospective study reviewed medical records of patients who discharged from a hospital stay for general diseases between 1 August 1995 and 31 October 1995, Cases were 68 discharge patients with an unplanned readmission within 30 days of discharge from an index stay. And the other cases are 145 patients who had more than two discharges and didn't have an unplanned readmission within 30 days. Logistic regression model was analyzed and the results were as follows; 1. duration of readmission, rate of unpayed, room, path, and risk of disease were more likely to be readmitted unexpectedly than the expected readmission patients. 2. early readmission, low risk condition group, and inadquateness of discharge plann for patients had unplanned radmissions rather than planned readmissions. Therefore, discharge planning education to health care provider is required and assessement of discharge planning should be evaluated. Readmissions are usually for related problems that arose during the original hopitaliztion and caused cost problems. Especially the unplanned readmissions are frequently preventable. Ultimately, models for readmissions can serve as a valuable clinical tool for target high-risk patients and older patients and with this kind of tools we can reduce hospital readmissions and maintain high-quality of inpatient care.
Purpose: The biggest problem of wound healing is a possible occurrence of lesion. Especially, in the case of patients who have a skin injury around exposed body parts, if their treatment period drag on for long time, they can suffer from aftereffects and the costs can be passed on to a society. Therefore, in this research, we investigated the need to develop the effective medicine and appliances for the patients by examining which therapy methods are being applying to the skin damage and what is the advantage and limit by evaluating the patient's satisfaction level. Methods: We carried out an online and offline survey targeting medical teams in order to analyze device for wound care. A total of 125 medical teams applied to the research, and investigate the level of customer satisfaction. Results: The moist dressings are the most used method for wound healing. When it comes to the level of customer satisfaction, biological dressing product also has a high satisfaction level. However its high cost tends to limit the use. Conclusion: This research reached a conclusion that it is need to develop a low cost and high efficiency wound care product considering the fact that its high cost and low efficiency induced economic problems. Generally, it is needed to develop a product for skin regeneration based on biological technologies, not a product just for damage cure.
The aim of this study is to evaluate the clinical outcome. Between January 1,2002 to September 30, 2002, we prospectively and retrospectively recruited III hospitalized patients who received Enteral Nutrition (EN group n = 52) and Total Parenteral Nutrition (TPNgroup n = 59) for more than seven days. The factors of clinical outcomes are costs, incidences of infection, lengths of hospital stay, and changes in weight. The characteristics of patients were investigated, which included nutritional status, disease severity CAP ACHE III score) and hypermetabolic severity Chypermetabolic score). Hypermeta-bolic scores were determined by high fever (> $38^{\circ}C$), rapid breathing (> 30 breaths/min) , rapid pulse rate (> 100 beats/min), leukocytosis (WBC > 12000 $mm^{3}$), leukocytopenia (WBC > 3000 $mm^{3}$), status of infection, inflammatory bowel disease, surgery and trauma. There was a positive correlation between hypermetabolic score and length of hospital stay (ICU), medical cost, weight loss, antibiotics adjusted by age while APACHEIII score did not show correlation to clinical outcome. Medical cost was higher by $18.2\%$ in the TPN group than the EN group. In conclusion, there was a strong negative correlation between the clinical outcome (cost, incidence of infection, hospital stay) and hypermetabolic score. Higher metabolic stress caused more malnutrition and complications. For nutritional management of patients with malnutrition, multiple factors, including nutritional assessment, and evaluation of hypermetabolic severity are needed to provide nutritional support for critically ill patients.
장 노년층 인구증가에 따른 건강문제의 다양화, 주민의 건강 요구증가에 부응하기 위해서는 기존의 의료기관 중심의 진료서비스 만으로는 새로운 건강문제를 충분히 해결할 수 없으므로 새로운 보건의료 제공 체계가 도입 되기에 이르렀다. 이미 선진국에서는 오랜 역사를 갖고 있는 가정간호사제도가 우리 나라에서도 입법화되어 1994년부터 병원중심의 가정간호사업이 시범적으로 운영되고 있다. 본 연구는 농촌지역의 중소병원에서 실시하고있는 가정간호사업소에 등록되어 있는 환자의 방문비용을 분석하는 것을 목적으로 실시되었으며, 1996년 5월 - 1997년 4월(1년간)까지 등록된 102명의 진료기록부를 분석하였다. 자료 분석 방법은 수준을 파악하기 위하여 평균과 분포로 분석하였으며 변수간의 차이점 등은 t - test와 ANOVA로 분석하였다. 분석 결과는 다음과 같다. 첫째, 등록환자의 일반적 특성은 남자가 44.1%, 여자가 55.9%로서 여자가 높았으며 연령은 60세 이상이 73.5%로서 노인 환자가 많았다. 둘째, 골다공증이 등록환자의 35.3%로서 제일 높았으며 각종 암이 21.6%, 뇌졸중이 14.7%로 나타나 연령과 관계가 깊은 것으로 나타났다. 질환특성은 복합질환, 즉 한가지 이상의 증상, 혹은 질환을 함께 갖고있는 경우가 73.5%로서 단순질환 26.5% 보다 높았다. 셋째, 남자 환자의 방문당 평균비용이 47,764원으로서 여자 환자의 46,078원 보다 높았다. 연령별로는 연령이 높아질수록 방문당 평균비용이 높은 것으로 나타났다. 성별과 연령별 비용은 통계학적으로 유의한 차이가 있는 것으로 나타났다. 넷째, 질환 특성별 비용은 단순질환이 복합 질환보다 방문당비용이 약간 높았으나 통계학적으로 유의한 차이가 없었으며, 질환별로는 COPD, 각종 암, 당뇨, 골다공증의 순으로 방문비용이 높게 나타났다. 다섯째, 가정간호의 방문비용이 질환으로 병원에서 치료받을 경우 보다 적게 나타나 비용효율이 있는 것으로 나타났다. 본 연구 결과는 제한된 자료를 분석한 것이므로 결과를 일반화시키는데 신중을 기할 필요가 있다. 그러나 가정간호사제도가 향후 정착되기 위해서는 가정간호 대상질환의 선정, 서비스내용 및 질, 비용효율 및 효과에 대한 평가가 계속 실시될 것을 제안한다.

Objective This study aimed to evaluate patient-reported satisfaction following robot-assisted hysterectomy due to benign uterine disease, and to identify the factors associated. Methods We used a questionnaire to evaluate patients' satisfaction with robot-assisted hysterectomy. The questions concerned overall patient-reported satisfaction and specific factors affecting satisfaction, including postoperative pain, return to daily life, the hospital experience, wounds, cost, the doctor-patient relationship, whether expectations were met, and whether detailed information was provided. We also collected data from patient records, such as uterine weight, rate of pelvic adhesion, operation time, rate of transfusion, delayed discharge, and readmission. One hundred patients who underwent robot-assisted hysterectomy participated in the study. Seventy-three fully completed questionnaires were returned. Results The majority of patients (95.9%) were satisfied with robot-assisted hysterectomy. The doctor-patient relationship, whether expectations were met, the hospital experience, wounds, and whether detailed information was provided were statistically significant factors influencing patients' overall satisfaction. Payment of fees and clinical and surgical outcomes did not significantly influence patients' overall satisfaction. Conclusion Our findings show that most patients reported high levels of satisfaction following robot-assisted hysterectomy, regardless of cost or clinical and surgical outcomes. Therefore, if gynecologists consider robot-assisted hysterectomy suitable for patients they need not hesitate based on potential costs; they should feel confident in recommending the procedure to patients.
Endoscopic treatment is a minimally invasive treatment for managing patients with vesicoureteral reflux (VUR). Although several bulking agents have been used for endoscopic treatment, dextranomer/hyaluronic acid is the only bulking agent currently approved by the U.S. Food and Drug Administration for treating VUR. Endoscopic treatment of VUR has gained great popularity owing to several obvious benefits, including short operative time, short hospital stay, minimal invasiveness, high efficacy, low complication rate, and reduced cost. Initially, the success rates of endoscopic treatment have been lower than that of open antireflux surgery. However, because injection techniques have been developed, a recent study showed higher success rates of endoscopic treatment than open surgery in the treatment of patients with intermediate- and high-grade VUR. Despite the controversy surrounding its effectiveness, endoscopic treatment is considered a valuable treatment option and viable alternative to long-term antibiotic prophylaxis.
