Systematic Review on Epstein-Barr Virus (EBV) DNA in Diagnosis of Nasopharyngeal Carcinoma in Asian Populations

  • Objective: To conduct a meta-analysis to investigate the value of EBV DNA in diagnosis of nasopharyngeal cancer (NPC) in Asian populations, and provide important evidence for screening. Methods: Prospective or respective case-control or cohort studies regarding the detection role of EBV DNA for NPC were included in our study. We conducted a comprehensive literature search in PubMed, EMBASE, and the Chinese Biomedical Database (CBM database between January 1980 and March 2012. Results: A total of 18 studies with 1492 NPC cases and 2641 health controls were included. Almost of the included studies were conducted in China, and only one other conducted in Thailand. The overall results demonstrated that the pooled sensitivity, specificity, positive likelihood (+LR) and negative likelihood (-LR) were 0.73 (0.71-0.75), 0.89 (0.88-0.90), 8.84 (5.65-13.84) and 0.19(0.11-0.32), respectively. The overall EBV DNA detection showed the largest area of 0.932 under the summary receiver operator curve (SROC). The accuracy of detection by plasma for NPC (0.86) was higher than in serum (0.81), with largest areas under the SROC of 0.97 and 0.91, respectively. Conclusion: Our results demonstrated the EBV DNA detection in plasma or serum has high sensitivity and specificity in diagnosis of NPC, especially in Chinese populations with a high risk of cancer.

차량 그림자를 이용한 주행 차량 검출 및 차간 거리 측정 (Driving Vehicle Detection and Distance Estimation using Vehicle Shadow)

  • 최근 차량 운전자들의 안전 운행을 보조하기 위해 운전자의 차량과 전방의 차량 간의 거리를 추정하고 안전거리 유무를 알려주기 위한 경보시스템이 개발되고 있다. 본 논문에서도 실제 도로 환경에서 전방의 주행 차량을 검출하여 차간 거리를 측정하고, 충돌 위험 상황을 감지하여 운전자에게 충돌 위험을 알리는 충돌경고시스템을 설계 및 구현하였다. 먼저 전방주시 카메라를 활용하여 촬영한 도로영상으로부터 도로와 차량에 해당하는 관심 영역을 추출하고, 관심 영역에서 전방 차량의 그림자 임계값 분석을 통해 전방 차량 객체를 추출한 후 전방 차량과의 거리를 계산하여 충돌 위험 경고를 알려준다. 주행 차량 검출 및 차간 거리 측정 결과를 기반으로 충돌경고시스템을 설계 및 구현하였으며, 실제 도로상황에 적용하여 실험한 결과 매우 높은 정확도를 나타내어 안전 운전에 대응할 수 있는 것으로 검증되었다.

붕괴지역의 매몰자 위치측위를 위한 모듈 개발 및 검증 (Development and Verification of A Module for Positioning Buried Persons in Collapsed Area)

  • 도심지에 지진, 산사태 등과 같은 재난 발생 시 건물 및 지하 구조물 붕괴로 인해 잔해 내부에 다수의 매몰자가 발생된다. 이때 인명탐지를 위해 주로 음향, 영상 및 전파 등을 활용한 탐지 장비 등이 활용되나 고가이며, 붕괴지 상부로의 직접 투입으로 인한 2차 붕괴위험 및 장비 운용 성능 저하로 인해 신속하고 높은 신뢰성을 갖는 인명탐지 기술이 활용되지 않고 있다. 이러한 문제를 해결하기 위한 매몰자의 휴대 기기에서 송출하는 Wi-Fi 신호 및 기압정보를 제공받아 매몰자의 3차원 위치를 탐색하는 UAV(Unmanned Aerial Vehicle)에 탑재 가능한 무선신호 기반 인명탐지 모듈을 개발하였다. 또한 드론의 비행동안 매몰자 휴대기기 정보를 실시간으로 수집하여 해당 정보를 지상부에 전송하여 신뢰성 있는 매몰자의 3차원 위치정보를 제공하도록 하는 모듈 개발 프레임워크를 제시하였다. 이를 통해 인명탐지 모듈의 개발과 현장 테스트를 통해 적용 타당성을 검증하였다. 이러한 연구결과는 향후 대형 건물 붕괴와 같은 재난 시 매몰자에 대한 매몰 위치의 신속한 탐지 및 구호와 실종자 수색을 위한 핵심기술로 활용될 수 있을 것이다.

UAV 항공 영상에서의 딥러닝 기반 잣송이 검출 (Deep Learning Based Pine Nut Detection in UAV Aerial Video)

  • 잣은 우리나라 대표적인 견과류 임산물이자 수익형 작물이다. 그러나 잣송이는 사람이 직접 나무 위로 올라가 수확하기 때문에 위험성이 높다. 이러한 문제를 해결하기 위해서 로봇 또는 UAV(unmanned aerial vehicle)를 이용한 잣송이 수확이 필요하다. 본 논문에서는 UAV를 이용한 잣송이 수확을 위해 UAV 항공 영상에서 딥러닝(deep learning) 기반의 잣송이 검출 기법을 제안한다. 이를 위해, UAV를 이용하여 실제 잣나무 숲에서 동영상을 촬영했으며, 적은 수의 데이터 보완을 위해 데이터 증강기법을 사용했다. 3D 검출을 위한 데이터로는 Unity3D을 이용하여 가상 잣송이 및 가상환경을 3D 모델링 하였으며 라벨링은 좌표계의 3차원 변환법을 이용해 구축했다. 잣 분포 영역 검출, 잣 객체에 대한 2D 및 3D 검출을 위한 딥러닝 알고리즘은 DeepLabV3, YOLOv4, CenterNet을 각각 이용하였다. 실험 결과, 잣송이 분포 영역 검출률은 82.15%, 2D 검출률은 86.93%, 3D 검출률은 59.45%이었다.

Endoscopic Findings in a Mass Screening Program for Gastric Cancer in a High Risk Region - Guilan Province of Iran

  • Background & Objectives: Gastric cancer is a leading cause of cancer-related deaths in both sexes in Iran. This study was designed to assess upper GI endoscopic findings among people > 50 years targeted in a mass screening program in a hot-point region. Methods: Based on the pilot results in Guilan Cancer Registry study (GCRS), one of the high point regions for GC-Lashtenesha- was selected. The target population was called mainly using two methods: in rural regions, by house-house direct referral and in urban areas using public media. Upper GI endoscopy was performed by trained endoscopists. All participants underwent biopsies for rapid urea test (RUT) from the antrum and also further biopsies from five defined points of stomach for detection of precancerous lesions. In cases of visible gross lesions, more diagnostic biopsies were taken and submitted for histopathologic evaluation. Results: Of 1,394 initial participants, finally 1,382 persons (702 women, 680 men) with a mean age of $61.7{\pm}9.0$ years (range: 50-87 years) underwent upper GI endoscopy. H. pylori infection based on the RUT was positive in 66.6%. Gastric adenocarcinoma and squamous cell carcinoma of esophagus were detected in seven (0.5%) and one (0.07%) persons, respectively. A remarkable proportion of studied participants were found to have esophageal hiatal hernia (38.4%). Asymptomatic gastric masses found in 1.1% (15) of cases which were mostly located in antrum (33.3%), cardia (20.0%) and prepyloric area (20.0%). Gastric and duodenal ulcers were found in 5.9% (82) and 6.9% (96) of the screened population. Conclusion: Upper endoscopy screening is an effective technique for early detection of GC especially in high risk populations. Further studies are required to evaluate cost effectiveness, cost benefit and mortality and morbidity of this method among high and moderate risk population before recommending this method for the GC surveillance program at the national level.

Prevalence and Risk Factors of H. pylori from Dyspeptic Patients in Northwest Ethiopia: A Hospital Based Cross-sectional Study

  • Background: Gastric cancer is the second leading cause of cancer-related deaths worldwide and infection with H. pylori is considered essential for its development. Helicobacter pylori infects more than 50% of the world's population with higher prevalence in developing countries than developed countries. The prevalence of H. pylori varies in different societies and geographical locations. The objectives of this study were to estimate the seroprevalence and determine the risk factors of H. pylori infection in dyspeptic patents in Ethiopia. Materials and Methods: A cross-sectional study involving 209 dyspeptic patients was carried out from February 15 to April 30, 2013. Five to ten ml venous blood was collected from each dyspeptic patient and analyzed for detection of Helicobacter pylori immunoglobulin (IgG). The socio-demographic characteristic, hygienic practices, alcohol consumption, sources of drinking water and types of latrine were also obtained with a pre-tested questionnaire. Results: The overall seroprevalence of Helicobacter pylori was 72.2%. There was statistically significant difference in the prevalence of H. pylori among age groups (p=0.02). Seroprevalence of H. pylori was higher in those patients who used unprotected surface water (76.4%) than those with access to piped tap water (65.9%). There was also statistically significant differences in prevalence of H. pylori with the habit of hand washing before meal (p=0.01) and alcohol consumption (p=0.001). Conclusions: The prevalence of H. pylori was high in the study area and increased with age of dyspeptic patients. Alcohol consumption and the type of drinking water are risk factors that have associations with the prevalence of H. pylori. Molecular epidemiological techniques can show a true picture of H. pylori and improvement in the drinking water quality is recommended.

넙치의 혈청, 간 및 근육 중 Tributyltin 축적 경향 (Accumulation of Tributyltin (TBT) in Blood, Liver and Muscle of Olive Flounder)

  • Accumulation of tributyltin (TBT) in serum, liver and muscle of olive flounder (Paralichtys olivaceus) was investigated in a 60-d static renewal exposure at $0.1{\mu}g$ TBT/L. Tributyltin accumulated rapidly from 83 ng Sn/g to 2,227 ng Sn/g on a wet weight basis in the serum of the olive flounder and to a greater extent than in the other tissues. The accumulated TBT concentrations in tissues were in the order of serum>liver>muscle on wet or dry-weight basis. High concentrations of dibutyltin (DBT: 990 ng Sn/g dry wt) and monobutyltin (MBT: 141 ng Sn/g dry wt), degradation products of TBT were found in liver of olive flounder at the end of exposure. On the other hand, DBT and MBT was below the detection limits in muscle during the exposure, and only low concentration of DBT (56 ng Sn/g) were detectefd in serum. Butyltin compounds were also quantitatively determined in feral fine-spotted flounder (Pleuronichthys cornutus) collected from Gwangyang Bay as one of polluted area and Sori Island as a reference site. All three butyltin compounds were detected from the fine-spotted flounder from Gwangyang Bay up to 3,107 ng Sn/g of total butyltins in liver, while 120 ng Sn/g of total butyltin concentration was found in the liver of fish from Sori Island.

입원 환자 영양검색 지표 개발 (Development of Nutrition Screening Index for Hospitalized Patients)

  • Several studies about hospital malnutrition have been reported that about more than 40% of hospitalized patients are having nutritional risk factors and hospital malnutrition presents a high prevalence. People in a more severe nutritional status ended up with a longer length of hospital stay and higher hospital cost. Nutrition screening tools identify individuals who are malnourished or at risk of becoming malnourished and who may benefit from nutritional support. For the early detection and treatment of malnourished hospital patients , few valid screening instruments fur Koreans exist. Therefore, the aim of this study was to develop a simple, reliable and valid malnutrition screening tool that could be used at hospital admission to identify adult patients at risk of malnutrition using medical electrical record data. Two hundred and one patients of the university affiliated medical center were assessed on nutritional status and classified as well nourished, moderately or severely malnourished by a Patient-Generated subjective global assessment (PG-SGA) being chosen as the 'gold standard' for defining malnutrition. The combination of nutrition screening questions with the highest sensitivity and specificity at prediction PG-SGA was termed the nutrition screening index (NSI). Odd ratio, and binary logistic regression were used to predict the best nutritional status predictors. Based on regression coefficient score, albumin less than 3.5 g/dl, body mass index (BMI) less than $18.5kg/m^2$, total lymphocyte count less than 900 and age over 65 were determined as the best set of NSI. By using best nutritional predictors receiver operating characteristic curve with the area under the curve, sensitivity and 1-specificity were analyzed to determine the best optimal cut-off point to decide normal or abnormal in nutritional status. Therefore simple and beneficial NSI was developed for identifying patients with severe malnutrition. Using NSI, nutritional information of the severe malnutrition patient should be shared with physicians and they should be cared for by clinical dietitians to improve their nutritional status.

주거환경 공기 중 호흡기 바이러스의 검출 (Detection of Airborne Respiratory Viruses in Residential Environments)

  • Objectives: Respiratory virus infections are the most common disease among all ages in all parts of the world and occur through airborne transmission. The purpose of this study was to detect and quantitate human respiratory viruses in residential environments. Methods: Air samples were collected from the residential space of apartments in the Seoul/Gyeonggi-do area. The samples were collected from indoor and outdoor air. Among respiratory viruses, influenza A virus, influenza B virus, parainfluenza virus, metapneumovirus, respiratory syncytial virus, and adenovirus were investigated by multiplex polymerase chain reaction. Among the virus-positive samples, we performed adenovirus quantification by real-time polymerase chain reaction. Results: Virus detection rates were 44.0%, 3.8%, 3.4%, and 17.3% in spring, summer, autumn, and winter, respectively. The virus detection rate was higher in winter and spring than in summer and autumn. Adenovirus was most commonly detected, followed by influenza A virus and parainfluenza virus. Virus distribution was not significantly different between indoor and outdoor environments. Conclusions: Although virus concentrations were not high in residential environments, residents in houses with detected viruses may have an increased risk of exposure to airborne respiratory viruses, especially in winter and spring.

Clinical Validation of a Protein Biomarker Panel for Non-Small Cell Lung Cancer

  • We validated the diagnostic performance of a previously developed blood-based 7-protein biomarker panel, $AptoDetect^{TM}$-Lung (Aptamer Sciences Inc., Pohang, Korea) using modified aptamer-based proteomic technology for lung cancer detection. Non-small cell lung cancer (NSCLC), 200 patients and benign nodule controls, 200 participants were enrolled. In a high-risk population corresponding to ${\geq}55years$ of age and ${\geq}30pack-years$, the diagnostic performance was improved, showing 73.3% sensitivity and 90.5% specificity with an area under the curve of 0.88. $AptoDetect^{TM}$-Lung (Aptamer Sciences Inc.) offers the best validated performance to discriminate NSCLC from benign nodule controls in a high-risk population and could play a complementary role in lung cancer screening.
