신경병증성 토끼모델에서 고전압맥동전류의 창상치유 효과 (Effects of High Voltage Pulsed Current Stimulation on the Wound Healing in Neuropathic Rabbit Model)

  • The purpose of this study was to determine whether high voltage pulsed current stimulation (HVPCS) would enhance wound healing in neuropathic rabbits. Ten rabbits were assigned to either an experimental or a control group. The wounded part around the peripheral neuropathy of the experimental rabbits was stimulated for two hours twice a day for six days under the following conditions: pulse frequency 80 pps, pulse duration $100{\mu}s$, and stimulation intensity 30~40 V. The results indicated that there was no difference in the wound closure between the experimental and control groups. The two groups showed similar aspects in collagen and reticulum, which were observed by colored Masson's trichome. While the rabbits in the control group had more or less thick fibers, the rabbits in the experimental group had thin and branched-shape fibers. The rabbits in the experimental group showed both strong responses in the shaping of elastic fibers and the increased aspects in fibroblast when compared with the control group.

  • High Voltage Pulsed Current Stimulation(HVPCS) and Microcurrent Electrical Neuromuscular Stimulation(MENS) have been used to promote the healing of decubitus ulcer and surgical wounds. The benefits of HVPCS and MENS are thought to include an inhibitive effect on bacterial growth. The purpose of this study was to compare the inhibitive effect of two different electrical stimulation techniques growth in vitro. Using agarose-based media, the two bacterial species Staphylococcus aureus, Esherichia coli - which are commonly isolated from open wounds were incubated in an incubator for 24 hours following exposure to HVPCS(400 V, 120 pps, $70{\mu}s$) and MENS($100{\mu}A$, 0.3 Hz). We then measured the zone of inhibition around each electrode. Both HVPCS and MENS produced an inhibitive effect on bacterial growth in this vitro study. However MENS was more effective than HVPCS.

미세전류자극과 고전압 맥동직류 통전이 토끼의 비골 골절치유에 미치는 효과 (Effects of Microcurrent and High Voltage Pulsed Galvanic Current Stimulation on Fibular Fracture Healing of the Rabbits)

  • 본 연구는 토끼골절모델에 미세전류자극(직류, 음극), 고전압 맥동직류 음극과 양극을 이용해 골절치유정도를 살펴보았다. 방사선 검사에 의한 육안 계측은 미세전류자극군이 고전압 맥동직류의 음극과 양극 통전군보다 골절치유척도 점수에서 통계학적으로 유의한 차이를 보였으나(p<0.05), 고전압 맥동직류 음극군과 양극군 사이에는 유의한 차이가 없었다(p>0.05). Hematoxylin-Eosin 염색과 Masson's trichrome 염색을 통한 병리조직표본 차이는 미세전류자극군에서 무층골의 증식이 다른 두 실험군보다 더 활발하게 관찰되었으며 연골내골화 과정도 다른 두 실험군에 비해 더 빠른 것으로 관찰되었다. Osteocalcin 면역조직화학 염색은 미세전류자극군이 골모세포, 골세포, 파골세포 및 골기질 내에서 면역양성반응이 다른 두 실험군보다 더 명확히 관찰되었다.

Keratinocyte Proliferation in Aged Rat Skin by High Voltage Pulsed Current Stimulation

  • The purpose of this study was to determine the effect of high voltage pulsed current (HVPC) stimulation on proliferative activities of basal keratinocytes by measured nucleolar organizer region (NOR) expression and thickness of spinous layer in aged rat skin. Fifty-one weeks old twelve male Sprague-Dawley rats (300∼350g) were divided into control and HVPC stimulation groups. Each animal's hair on the back were removed. The HVPC stimulation group received an negative monophasic twin peak pulsed current stimulation with 50 V, while the control group was given the same treatment without electricity. The rats were sacrificed after 3 weeks. The biopsy specimens were fixed in formalin, embedded in paraffin and stained with hematoxyline-eosin and silver nitrate. The thickness of basal to granular layer of the epidennis were measured using a light microscope and computerized image analysis system. The number of argyrophilic nucleolar organizer region (AgNOR) were counted using a light microscope and computerized image analysis system and calculated as the mean number of AgNOR per nucleus in the basal keratinocyte. By using a Student's t-test, an increase in the thickness of basal-spinous layer (P<0.001) of epidermis can be observed in HVPC stimulation rats as compared with the control rats, whereas the thickness of the granular layer is not affected. A Student's t-test showed a significantly higher mean NOR number per nucleus of the basal keratinocyte in the HVPC stimulation rats than control rats (P<0.001). There was significantly positive correlation between the NOR number and the thickness of basal-spinous layer (r=0.80, P<0.05). These results suggest that the HVPC stimulation may increase the thickness of spinous layer in the epidennis due to increased proliferative activities of basal keratinocytes in epidennis in aged rat skin.

전기자극이 개구리 뒷다리 부종형성에 미치는 영향 (Effects of Electrical Stimulation on Edema Formation in Frog Hind Limbs)

  • The purpose of this study was to determine the effects of high voltage pulsed current(HVPC) and low voltage pulsed current(LVPC) on posttraumatic edema formation in frog hind limbs In this study, 16 bullfrogs(Rana Catesbeiana), weighting 189g to 340g were used. Limb 16 anesthetized bullfrogs were systemically injuried by weight drop. One hind limb of each frogs was randomly selected to receive continuous 120 pps HVPC and 100 pps LVPC at $90\%$ of motor threshold(HVPC : 33.3v, LVPC : 0.2-1mA). The opposite hind was remained as a control. A series of six 30-minute treatment(interrupted by 30-minute rest) was begun minutes after injury. The results were as follows. 1. Cathodal HVPC has heed shown to be effective in curbing posttraumatic edema formation in frogs, but anodal HVPC did not effect. 2. Volumes of hind limbs treated with LVPC were not significantly different over time from those of untreated hind limbs. 3. Therefore, waveform(HVPC versus LVPC) seems to influence the efficacy of electrotherapy for edema control. 4. Electrical stimulation were not increased edema formation on frog hind limbs.

고전압맥동전류자극이 흰쥐 창상 치유에 미치는 영향 (Effect of High Voltage Pulsed Current Stimulation on Surgical Wound of Rat)

  • The purpose of this study was to determine the effects of high voltage pulsed current (HVPC) stimulation on wound healing. Thirty-four Sprague-Dawley adult female rats were assigned to experimental and control groups. Each rats were anesthetized with pentotal sodium, and a 10 mm full-thickness incision was made on the back. From 24 hours after surgery, the rats of experimental groups were stimulated with HVPC, $140{\mu}s$, 120 pps, 30-40 V for 30 minutes, daily. The rats were sacrified 2 days, 4 days and 6 days after stimulation, respectively. The skin was removed, and processed for light microscopic examination. The length of incisional wounds were measured by microcaliper, and nucleolar organizer regions were counted under light microscope. For the histological observations the specimens were stained with Hematoxylin and Eosin, Masson Trichrome, Gomori Reticulum and Ag-NOR. There was a significant decrease in the length of incisional wound in experimental group compared with control group at 6 days HVPC stimuation (p<0.05). In experimental group, wound were significant difference (p<0.01) between the duration of post surgery. The mean numbers of nucleolar organizer regions per nucleus were significantly increased in the experimental group at 6 days HVPC stimulation (p<0.05), and were significantly difference (p<0.01) between the duration of post surgery in experimental group. Histological examination of the wound site suggested a more rapid of epithelialization and collagen formation between experimental groups compared with control groups. The result may indicated that the HVPC with $140{\mu}s$, 120 pps, 30-40 V for 30 minutes promoted surgical wound healing in the rat.

고압맥동 평류자극이 가토 상처치유에 미치는 영향 (Effect of High Voltage Pulsed Galvanic Current on Wound Healing in Rabbits)

  • This study was performed to assess the efficacy of high voltage pulsed galvanic current for the healing of wounds in rabbits. Skin wounds were created laterally on the flank of 12 domestic rabbits($3{\times}3cm$). The wounds of each group were treated with an intensity of 170 V at a frequency of 70 pulses per second, which was applied for 30 minutes a day for 10 days. The experimental groups were randomly assigned to either EXP I (n=3), EXP II(n=3), EXP III(n=3) or control(n=3). Each group was stimulated under the following conditions : 1) EXP I (Negative polarity), 2) EXP II (Change in polarity, negative electrode stimulation during the first 3 days and then positive electrode stimulation from 4 to 10 days), 3) EXP III(Positive polarity), 4) control(No stimulation). An active electrode was placed over the wound and a dispersive electrode on the buttock. The rate of wound closure was compared with the original wound size, evaluated by a tracing film in each measurement period. Finally, on the wound in each group, skin tissue was excised for histological evaluation after treatment for 10 days. The results obtained are as follows : 1) It was found that the control group did not show a complete remodeling of epitherial layer and had a chronic inflammatory response. Judging from the irregularity of intercellular space and the loose alignment of connective tissue, these findings show that wound healing was delayed. 2) EXP I showed a significant bactericidal effect, but a moderate response of vasodilation. The rate of wound closure was slower when compared with EXP II, III. 3) EXP II showed a complete remodeling of epitherial layer and a positive repair of connective tissue. Its rate of wound closure was best when compared with the others. 4) EXP III had a slower rate of wound closure than EXP II, but judging from the greater proliferation of collagen fibers and the dense alignment of connective tissue, this positive electrode was very effective in the formation of neo - connective tissue.

고전압맥동전류자극에 의한 생검 건의 세포 이동 및 증식 증진 (Enhance of Migration and Proliferation of Cells from Tendon Biopsies by High Voltage Pulsed Current Stimulation)

  • 본 연구는 고전압맥동전류자극이 생검 건의 세포 이동 및 증식의 증진 여부를 규명하기 위하여 시행하였다. 닭의 심지굴근건을 적출하고 직경 2mm의 생검하여 배양하였다. 생검 건에 100 pps, 50V로 30분간 음극으로 고전압맥동전류자극한 후 생검 건을 피브린로 피복하고 6일간 배양한 후 세포의 이동거리를 측정하고 7일 후 세포증식능을 측정하였다. 대조군과 고전압맥동전류자극군의 세포 이동 거리 및 흡광도를 t-검정한 결과 고전압맥동전류자극군의 세포 이동 거리가 유의하게 증가하였으며 (t=-2.675, p<0.05), 흡광도도 유의하게 증가하였다 (t=-2.136, p<0.05). 이러한 결과는 고전압맥동 전류자극이 생검 건 섬유모세포의 성장과 증식을 유발시키고 있음을 보여주고 있으며, 이러한 결과는 고전압맥동전류자극이 섬유모세포의 세포반응을 활성화시켰음을 시사하고 있어 고전압맥동전류자극의 건손상 치유 기전의 일부를 제시하였다.

경피신경전기자극과 미세전류자극이 정상인의 교감신경 긴장도에 미치는 영향 (The Effects of Transcutaneous Electrical Nerve Stimulation and Microampere Electrical Nerve Stimulation on Sympathetic Tone in Healthy Subjects)

  • The purpose of this study was to determine the effect of two different forms of transcutaneous electrical nerve stimulation(TENS) and one of microcurrent high voltage pulsed galvanic current(HVPC) on sympathetic tone in healthy subjects. Fourty subjects received TENS(20) and PVPC(30) during short time(20min). Left finger tip skin temperatures were measured at four interval for each treatment : 1) before treatment, 2) after 10 minutes treatment, 3)after 20 minutes treatment, and 4) after 10 minutes rest. The results were as follows. 1) TENS treatment group increased skin temperature after treatment 20 minutes, but HVPC treatment increased akin temperature after 10 minutes and recovered normal skin temperature after 10 minutes treatment. It means that short time(20min) electrical stimulation decreased sympathetic activities. 2) Sympathetic activities of TENS stimulation were influenced by age, but HVPC were not. 3) During 10 minutes, both treatment increased sympathetic activities, but HVPC treatment reversed sympathetic activity more rapidly than TENS. 4) The changes of skin temperature means by sex, males in TENS treatment group were higher than females, but HVPC were reverted.

고전압맥동전류자극이 흰쥐 피부 창상치유와 교원질 축적에 미치는 효과 (Acceleration of Wound Healing and Collagen Deposition in Rat Skin by High Voltage Pulsed Current Stimulation)

  • 고전압맥동전류자극이 흰쥐 피부의 창상치유와 섬유모세포 및 교원질 밀도에 미치는 영향을 규명할 목적으로 본 실험을 시행하였다. Sprague-Dawley계 수컷 흰쥐 20마리를 10마리씩 무작위로 고전압맥동전류자극군과 대조군에 배치하고 등에 10 mm의 전층 절개상을 만들었다. 고전압맥동전류자극군은 창상 위에 전극을 부착한 후 맥동빈도 100 pps, 맥동기간 $65{\mu}s$, 강도 50 V로 1일 30분씩 1주 동안 자극하였으며 대조군은 전극만 부착하고 전류를 통전시키지 않았다. 1주 후 창상 길이를 계측한 후 창상부위의 피부를 적출하여 포르말린에 고정한 후 파라핀 포매 절편을 제작하여 Massons trichrome 및 hematoxylin and eosin (H&E) 염색을 하였다. 이 표본을 고배율 현미경하에서 관찰한 다음 현미경에 부착된 CCD 카메라를 통해 개인용 컴퓨터로 보내 영상분석 프로그램을 이용하여 섬유모세포와 교원질 밀도를 계측한 후 비율을 산출하였다. 이를 군간 t-검정한 결과 고전압맥동전류자극군의 창상치유율, 섬유모세포 및 교원질 밀도가 대조군보다 통계학적으로 유의하게 높게 나타났다 (p<0.001). 이러한 결과는 고전압맥동전류자극이 흰쥐의 피부 창상 치유를 촉진시켰음을 보여 주었고 이는 전기자극에 의해 섬유모세포가 활성화되어 교원질 합성이 증가함에 따른 것으로 사료된다.
