Surgical Comparison of Pterional Approach with Interhemispheric Approach for High Positioned Anterior Communicating Artery Aneurysms (고위의 전교통동맥 동맥류에서의 Pterional Approach와 Interhemispheric Approach의 수술적 비교 분석)

  • Objective : The purpose of this study is to evaluate and compare surgical results of pterional(fronto-temporal) approach and interhemispheric approach for the high positioned anterior communicating artery aneurysm with our surgical experience. Methods : During the period between May 1990 and May 2001, 263 anterior communicating aneurysms were treated at the department of neurosurgery of Dong-A university hospital. Among them, 175 patients were operated by same operator. Thirty six out of the 175 cases were high positioned anterior communicating artery aneurysms located more than 15mm above the anterior clinoid process. Results : Pterional approaches were applied in 32 cases and interhemispheric approaches were applied in 4 cases of total 36 cases of the high positioned anterior communicating artery aneurysms. The 32 cases of pterional approach resulted in Good Recovery 20/32(63%), Moderate Disability 6/32(19%), Severe Disability 4/32(12%) and Dead 2/32(6%), and 4 cases of interhemispheric approach resulted in GR 3/4(75%) and MD 1/4(25%). Relatively, pterional approach showed poorer result on high positioned anterior communicating artery aneurysm located more than 19mm above the anterior clinoid process with GR 5/13(39%), MD 3/13(23%), SD 3/13(23%) and Dead 2/13(15%). Conclusion : Interhemispheric approach is preferable to pterional approach for certain cases of high positioned anterior communicating artery aneurysm located more than 19 mm above the anterior clinoid process because it provides adequate orientation to the regional anatomy, less retraction of frontal lobe and preservation of the olfactory tract and gyrus rectus without any surgical complications.

Postoperative Clipping Status after a Pterional versus Interhemispheric Approach for High-Positioned Anterior Communicating Artery Aneurysms

  • Objective : When treating high-positioned anterior communicating artery (ACoA) aneurysms, pterional-transsylvian and interhemispheric approaches are both viable options, yet comparative studies of these two surgical approaches are rare. Accordingly, this retrospective study investigated the surgical results of both approaches. Methods : Twenty-four patients underwent a pterional approach (n=11) or interhemispheric approach (n=13), including a unilateral low anterior interhemispheric approach or bifrontal interhemispheric approach, for high-positioned ACoA aneurysms with an aneurysm dome height >15 mm and aneurysm neck height >10 mm both measured from the level of the anterior clinoid process. The clinical and radiological data were reviewed to investigate the surgical results and risk factors of incomplete clipping. Results : The pterional patient group showed a significantly higher incidence of incomplete clipping than the interhemispheric patient group (p=0.031). Four patients (36.4%) who underwent a pterional approach showed a postclipping aneurysm remnant, whereas all the patients who experienced an interhemispheric approach showed complete clipping. In one case, the aneurysm remnant was obliterated by coiling, while follow-up of the other three cases showed the remnants remained limited to the aneurysm base. A multivariate analysis revealed that a pterional approach for a large aneurysm with a diameter >8 mm presented a statistically significant risk factor for incomplete clipping. Conclusion : For high-positioned ACoA aneurysms with a dome height >15 mm and neck height >10 mm above the level of the anterior clinoid process, a large aneurysm with a diameter >8 mm can be clipped more completely via an interhemispheric approach than via a pterional approach.

POSITION RECOGNITION AND QUALITY EVALUATION OF TOBACCO LEAVES VIA COLOR COMPUTER VISION

  • The position of tobacco leaves is affluence to the quality. To evaluate its quality, sample leaves was collected according to the position of attachment. In Korea, the position was divided into four classes such as high, middle, low and inside positioned leaves. Until now, the grade of standard sample was determined by human expert from korea ginseng and tobacco company. Many research were done by the chemical and spectrum analysis using NIR and computer vision. The grade of tobacco leaves mainly classified into 5 grades according to the attached position and its chemical composition. In high and low positioned leaves shows a low level grade under grade 3. Generally, inside and medium positioned leaf has a high level grade. This is the basic research to develop a real time tobacco leaves grading system combined with portable NIR spectrum analysis system. However, this research just deals with position recognition and grading using the color machine vision. The RGB color information was converted to HSI image format and the sample was all investigated using the bundle of tobacco leaves. Quality grade and position recognition was performed through well known general error back propagation neural network. Finally, the relationship about attached leaf position and its grade was analyzed.

  • The Simple Virtual Environment (SVE) library is the application programming interface fur creating the virtual environment easily. Even though it has a lot of efficient features, applying transformation to absolutely positioned objects is difficult. In this paper, we designed a simple method with which it is possible to rotate the absolutely positioned object freely. To test the method, we designed a simple virtual environment. The environment is designed with people, street and building objects. To reduce time consumption of displaying high-detailed people models, we only used texture-based objects. The person objects are modeled as quadric-shape and textured with front-view images of people. To make more realistic environment, objects always change its orientation themselves following to the user location. The result and the testing environment will be demonstrated.

  • A keratinized gingiva is important to the natural teeth and it is more essential to the health of the peri-implant mucosa of the implants. There are various surgical methods to restore a keratinized gingiva. First, a clinician could utilize apically positioned flap operation. This flap operation technique could be used as a full or partial thickness. If there is little keratinized gingival tissue available for the apically positioned flap operation, free gingival grafting should be used. Its technique sensitivity is relatively high, but using various surgical techniques and disciplines makes it simple and have the good predictability. There have been many considerations for those surgical techniques. Clinicians who treat for periodontitis or operate implant surgeries have to know the considerations and surgical methods.

  • It is important for nurses to try to improve their interpersonal relationship and communication skill. The purpose of this study was to examine the nurse's inerpersonal conflict and expression with physicians, other nurses and patients & their caregivers. A convenient sample of 62 RN-BSN students were recruited. The data collection was done from August, 2001 to September, 2001. Nurses experienced the most conflicts with high-positioned nurses, then doctors, patients and their families, administrators, nurse-aid, nurse colleagues and lower-positioned nurses. There was a significant correlation between nurse's interpersonal conflict with doctors and nurse's age & career, between with high-positioned nurses and region of hospitals & nurse's position. The expression that nurses least-like from physicians was 'Do it as order'. From other nurses was 'manner of speech such like order'. And from patients and their family was 'naming such like young lady or sisters'. The other side, the expression that nurses most like expression from physicians and ohter nurses was 'You are excellent'. From patients and their family was 'You are kindness'. In conclusion, it is necessary to improve nurse's communication skills and interpersonal relationships.

  • A dynamically positioned vessel must be capable of maintaining a specified position and direction by controlling the thruster devices. The motions of a vessel are often assuned to tne sum of low frequency(LF)motions and high frequency(HF)motions. The former is mainly due to wind, current and second order wave forces, while the latter is mainly due to first order wave forces. In order to avoid the high frequency thruser modulation, the control system must include filters to estimate the low frequency motions from the measured motion signals, This paper presents a control system based on Kalman filtering technique and optimal control tyeory. Using the combined kalmam filter, LF motion estimates and HF ones are achieved from the motion measurement of the vessel. The estimated low frequency motions are used as inputs to the dynamic positioning system. The thruster modulation is minimized using the optimal control theory; Linear Quadratic Gaussian(LQG)controller. The performances of the Kalman filter and the dynamic positioned vessel are investigated by computer simulation.

  • It is a relatively common clinical experience to see a impacted maxillary central incisor Most often, the central incisor is impacted labially. The labial impaction has been indicated as the most difficult to manage. The labial impactions have been found to be associated with mucogingival recession, reduced attached gingiva and periodontal pockets. For the successful treatment, clinicians should avoid loss of attached gingiva in surgical exposure. The most common methods of uncovering labially impacted maxillary anterior teeth are gingivectomy, apically positioned flap and closed eruption technique. If gingivectomy will not leave enough attached gingiva, then an apically positioned flap may be the treatment of choice. If the tooth is impacted in the middle of the alveolus or high in the vestibule near the nasal spine, the closed eruption technique may be the treatment of choice. Closed eruption technique was used in one case, apically positioned flap was used in two case. As the result in two cases of impacted maxillary central incisor, apically positioned flap provide the adequate width of attached gingiva. In case of impacted maxillary central incisor, through the clinical and radiologic examination to select correct surgical operation for reduce the complications.

Gravity and Magnetic Surverys for Volcanic Rocks in Yeoncheon Area, kyonggi-do (경기도 인천지역 분포하는 화산암류에 대한 중력 및 자력 탐사 연구)

  • The gravity and magnetic measurements have been obtained from 34 stations with 50m intervals along the survey line positioned between Jangtanri and sindapri for studying subsurface geology and structures of the volcanic rocks in Yeoncheon area. The Bouguer gravity and magnetic anomaly values were evaluated from the reduction of the field observation, and then interpreted by Nettleton's method and maximum-pepth rules, are approximately 160m based on magnetic data and 135m based on gravity data. High Bouguer gravity anomaly zone between 0m in Jangtanri and 900m along the survery line, is caused by thick and high density, older dasalt which is positioned beneath jijangbong tuff breccia, and this result corresponds to the interpretation result based on magnetic anomly. Lower gravity and magnetic anomaly zones ariund 900m are caused by between 1300m and 1550m are caused by high density of Quarternary basalt exposed in the surface, and lower gravity and magnetic anomalies at 200m and 1250m are caused by faults.

TREATMENT OF HEAVY MANDIBULAR BUCCAL FRENUM USING APICALLY POSITIONED FLAP UNDER DEEP SEDATION IN CHILDREN (소아환자의 깊은 진정요법 하에서 근단변위 판막술을 이용한 거대협소대의 치료)

  • The mandibular buccal frenum is defined as a fold of mucous membrane at the posterior labial vestibule and attaches the lips and the cheeks to the alveolar mucosa, gingiva, and underlying periosteum. The buccal frenum becomes a problem when its attachment is too close to the marginal gingiva. It may then pull on healthy gingiva, encourage plaque formation and interfere with tooth brushing. Especially, heavy buccal frenum mucogingivally results in insufficent attached gingiva, inadequate vestibular depth and high frenum attachment and also difficulty in eruption of mandibular premolar. Frenotomy, frenectomy and mucogingival surgery are used in treating heavy buccal frenum. Frenotomy with autogenous free gingival graft has been used popularly because of its stable result. But, it is difficult in younger children because of inadequate donor site, difficulty in making recipient site and behavior management. Frenotomy with apically positioned flap is considered as more efficient way for a very young child with heavy buccal frenum. Additionally, modified deep sedation with $N_2O-O_2$ can be used as an adjunct for the effective treatment outcome. Decrease in muscle pull, adequate width of attached gingiva and increased vestibular depth can be expected from this treatment approach.
