내측 개방 근위 경골 절골술 후 통증 조절에서 관절 주위 다중 약물 국소 주사와 내전근관 차단술의 효과 비교 (Comparison of the Effects of an Adductor Canal Block and Periarticular Multimodal Drug Local Injection on Pain after a Medial Opening High Tibial Osteotomy)

  • 목적: 내측 개방 근위 경골 절골술 후 통증 조절에서 관절 주위 다중 약물 국소 주사와 내전근관 차단술의 효과를 비교하고자 하였다. 대상 및 방법: 2016년 11월부터 2017년 3월까지 개방형 내측 근위 경골 절골술을 시행한 환자 60명을 대상으로 하여 후향적으로 분석하였다. 전 예에서 척추 마취를 시행하였으며, 수술 직전 선제 약물 투여 후 정맥내 자가 통증 조절 장치를 시행하였다. 30명의 환자(I군)는 관절 주위 다중 약물 국소 주사를 맞았고, 다른 30명의 환자(II군)는 내전근관 차단술을 시행 받았다. 두 그룹에 대해 수술 후 통증 수준, 추가적인 tramadol hydrochloride 주사의 빈도, 자가 통증 조절 장치 사용 총량 및 버튼을 누른 횟수 등을 비교하였다. 결과: 수술 후 2주째까지 시각통증점수(visual analogue scale)는 두 군 간에 유의한 차이를 보이지 않았다. 추가 tramadol hydrochloride 주사의 빈도는 두 군 간에 유의한 차이가 없었다. 자가 통증 조절 장치 버튼을 누르는 횟수와 평균 총 fentanyl 소비량에서도 그룹 간에 유의한 차이가 없었다. 결론: 내측 개방적 근위 경골 절골술을 시행한 환자의 급성기 통증 조절에 있어서 관절 주위 다중 약물 주입 및 내전근관 신경 차단술은 비슷한 효과를 가지는 것으로 생각된다.

SKH51의 반응고 상태에서의 가열 및 성형에 의한 부품 제조 (Fabrication of a Part by Heating and Forming in the Semi-solid State of the SKH51 Material)

  • The semi-solid metal forming process has been applied to realize a near-net shape fabrication of a high speed tool steel. A complicatedly shaped part out of SKH51 was successfully manufactured by introducing pertinent materials, tooling and processing conditions. A SKH51 billet with globular grains was heated at temperatures between 1300 and $1350^{\circ}C$ using high frequency induction heater to get semi-solid microstructure before high rate injection of mushy metal into a die cavity for the forming process. It was necessary to control the preheating of dies between 300 and $400^{\circ}C$ to maintain the homogeneous microstructure during the semi-solid metal forming process. Significant defects such as pores, high fraction of liquid fraction and segregation could be removed from the part by using air vents.

흰쥐에서 고용량의 Pilocarpine에 의하여 유발된 간질중첩증의 양상 (EEG Patterns of High dose Pilocarpine-Induced Status Epilepticus in Rats)

  • Background : We studied EEG changes during pilocarpine-induced status epilepticus(SE), a widely used model whose EEG characteristics have not been fully described previously. Methods : Male Sprague-Dawley rats weighing 250-350 grams were used as subjects. SE was induced 5-7 days after placement of chronic epidural electrodes, using 360-380 mg/Kg pilocarpine IP. Rats were observed with continuous EEG recording following pilocarpine injection until end of the SE episode. Results : SE occurred in 10/12 rats studied. SE began with a series of discrete seizures $11.1{\pm}3.93$ minutes after pilocarpine injection. $5.2{\pm}2.71$ seizures occurred over $10.9{\pm}4.62$ minutes, until the EEG converted to a waxing and waning pattern, during which the amplitude and frequency of epileptiform activity increased. After $1.4{\pm}1.82$ minutes, a pattern of continuous high amplitude rapid spiking was established. Continuous spiking continued for $3.4{\pm}0.48$ hours with a very gradual decline in amplitude and frequency, until periodic epileptiform discharges(PEDs) began to occur. The EEG consisted primarily of PEDs for another $7.4{\pm}3.09$ hours, until electrographic generalized seizures began to occur. These continued for $5.8{\pm}4.82$ hours until death. Duration of SE was $17.0{\pm}5.88$ hours. Flat periods were a prominent feature during all EEG patterns in this model. Conclusion : EEG features distinctive in pilocarpine SE(but not unique to it) include flat periods during all patterns and resumption of continuous spiking episodes after the onset of PEDs. The sequence of discrete seizures to waxing and waning to continuous spiking to PEDs was identical to that which has been described in humans and other animal models.

고빈도 120 Hz 전침이 Carrageenan으로 유발된 흰쥐의 Prostaglandin E2와 척수 N-Methyl-D-Aspartate Receptor 발현에 미치는 영향 (The Effects of 120Hz Electroacupuncture on the Prostaglandin E2 and Spinal N-Methyl-D-Aspartate Receptor Expression in the Carrageenan-Injected Rat)

  • Objective : The role of high frequency 120 Hz electroacupuncture(EA) in carrageenan-induced pain was studied by examining the alnalgesic effects, and prostaglandin $E_2(PGE_2)$ levels measurement and spinal N-methyl-D-aspartate(NMDA) receptor expression. Inflammation was induced by an intraplantar injection of 1% carrageenan into the right hind paw. Method : Bilateral EA stimulation with 120 Hz were delivered at those acupoints corresponding to Zusanli and Sanyinjiao in man via the needles for a total of 30 min duration in carrageenan-injected rats. Results : EA stimulation showed significant analgesic effects as measured by analgesy-meter at all time points tested compared with controls. Three hours after carrageenan injection, PGE2 levels were measured by commercial kit. EA significantly inhibited PGE2 production in the right paw. The number of NR1 and NR2A, NMDA receptor, immunoreactive neurons was significantly increased in the superficial dorsal horn(laminae I-II) and nucleus proprius(laminae III-IV) of ipsilateral spinal cord at L4-5. But the number of carrageenan-induced NR1 and NR2A immunoreactive neuron, especially NR1 immunoreaction in the superficial dorsal horn, was reduced by 120 Hz EA stimulation. Conclusions : These results indicate that NMDA receptors may mediate transmission of nociceptive information originating in tissue inflammation of hind paw and high frequency 120 Hz EA stimulation have an alleviating action against local inflammatory pain.

하이브리드 로켓 연소에서의 경계층 진동 변화와 저주파수 연소불안정 (Oscillating Boundary Layer Flow and Low Frequency Instability in Hybrid Rocket Combustion)

  • 하이브리드 로켓 연소의 저주파수 연소불안정은 고체연료의 열적지연(Thermal Lag)과 경계층 유동 변화에 의한 열전달 진동의 공진에 의해 발생한다. 본 연구는 연료 표면 근처의 경계층 유동의 교란이 어떤 물리적 과정에 의해 발생하여 연소불안정으로 발달하는지를 실험적으로 확인하였다. 특히 산화제의 스월 분사는 연소 안정화에 매우 큰 기여를 하므로 스월 강도를 증가시키며 경계층의 변화와 연소불안정의 발생과정을 연구하였다. 경계층 섭동을 확인하기 위하여 연소 유동장을 가시화하였고 이미지에 대한 POD(Proper Orthogonal Decomposition) 분석을 시도하였다. 스월 강도가 증가할수록 500Hz 대역 고주파수 p', q'의 결합이 약해지며 열적지연과 유사한 주파수 특성을 갖는 Rayleigh Index의 섭동 발생도 약해져 경계층 진동의 발생이 점차 감소하는 것을 관찰하였다. 따라서 고주파수 p', q'의 주기적인 결합에 의한 축 방향 경계층 진동이 나타나면 열적지연 주파수와 공진에 의한 연소불안정이 발생함을 확인하였다.

고주파 열플라즈마 토치를 이용한 Ni 금속 입자의 나노화 공정에 대한 전산해석 연구 (Numerical Analysis on RF (Radio-frequency) Thermal Plasma Synthesis of Nano-sized Ni Metal)

  • Numerical analysis on RF (Radio-Frequency) thermal plasma treatment of micro-sized Ni metal was carried out to understand the synthesis mechanism of nano-sized Ni powder by RF thermal plasma. For this purpose, the behaviors of Ni metal particles injected into RF plasma torch were investigated according to their diameters ($1{\sim}100{\mu}m$), RF input power (6 ~ 12 kW) and the flow rates of carrier gases (2 and 5 slpm). From the numerical results, it is predicted firstly that the velocities of carrier gases need to be minimized because the strong injection of carrier gas can cool down the central column of RF thermal plasma significantly, which is used as a main path for RF thermal plasma treatment of micro-sized Ni metal. In addition, the residence time of the injected particles in the high temperature region of RF thermal plasma is found to be also reduced in proportion to the flow rate of the carrier gas In spite of these effects of carrier gas velocities, however, calculation results show that a Ni metal particle even with the diameter of $100{\mu}m$ can be completely evaporated at relatively low power level of 10 kW during its flight of RF thermal plasma torch (< 10 ms) due to the relatively low melting point and high thermal conductivity. Based on these observations, nano-sized Ni metal powders are expected to be produced efficiently by a simple treatment of micro-sized Ni metal using RF thermal plasmas.

Sensorless Control of Non-salient Permanent Magnet Synchronous Motor Drives using Rotor Position Tracking PI Controller

  • This paper presents a new velocity estimation strategy for a non-salient permanent magnet synchronous motor drive without high frequency signal injection or special PWM pattern. This approach is based on the d-axis current regulator output voltage of the drive system, which contains the rotor position error information. The rotor velocity can be estimated through a rotor position tracking PI controller that controls the position error at zero. For zero and low speed operation, the PI gain of the rotor position tracking controller has a variable structure according to the estimated rotor velocity. Then, at zero speed, the rotor position and velocity have sluggish dynamics because the varying gains are very low in this region. In order to boost the bandwidth of the PI controller during zero speed, the loop recovery technique is applied to the control system. The PI tuning formulas are also derived by analyzing this control system by frequency domain specifications such as phase margin and bandwidth assignment.

Assessment of a Phase Doppler Anemometry Technique in Dense Droplet Laden Jet

  • This study represents an assessment of the phase-Doppler technique to the measurements of dense droplet laden jet. High-pressure injection fuel sprays have been investigated to evaluate the use of the Phase-Doppler anemometry (PDA) technique. The critical issue is the stability of the phase-Doppler anemometry technique for dense droplet laden jet such as Diesel fuel spray in order to insure the results from the drop size and velocity measurements are repeatable, consistent, and physically realistic because the validation rate of experimental data is very low due to the thick optical density. The effect of shift frequency is minor, however, the photomultiplier tube (PMT) voltage setting is very sensitive to the data acquisition and noise in dense droplet laden jet. The optimum PMT voltage and shift frequency should be chosen so that the data such as volume flux and drop diameter do not change rapidly.
