S-PBL의 연계순서에 따른 간호대학생의 학습성과 비교: 무작위 교차설계 (A Comparative Study on Learning Outcomes according to the Integration Sequences of S-PBL in Nursing Students: Randomized Crossover Design)

  • Purpose: This study aimed to compare the effects of simulation integrated with problem based learning (S-PBL) according to the sequences of problem-based learning (PBL) and high fidelity simulation training (HFS) on knowledge, clinical performance, clinical judgment, self-confidence, and satisfaction in fourth-grade nursing students. Methods: In this randomized crossover design study, four S-PBLs on medical-surgical nursing were applied alternatively to two randomly-assigned groups of 26 senior nursing students for 8 weeks. The collected data were analyzed using an independent t-test. Results: The method of administering PBL prior to HFS led to significantly higher scores on knowledge (t=2.28, p=.025) as compared to the method of administering HFS prior to PBL. However, the latter method led to significantly higher scores on clinical performance (t=-6.49, p<.001) and clinical judgment (t=-4.71, p<.001) as compared to the method of administering PBL prior to HFS. There were no differences in the effect of the two methods on self-confidence (t=1.53, p=.128) and satisfaction (t=1.28, p=.202). Conclusion: The integration sequences of S-PBL was associated with different learning outcomes. Therefore, when implementing S-PBL, it is necessary to consider the educational goal to executes an appropriate sequence of integration.

간호대학생의 고성능 인체 환자 모형 시뮬레이션 교육 평가를 위한 한국판 시뮬레이션 만족도 경험 도구의 타당도와 신뢰도 연구 (The Validity and Reliability of a Korean Version of the Satisfaction with Simulation Experience Scale for Evaluating Satisfaction with High-Fidelity Simulation Education for Nursing Students)

  • 본 연구는 간호대학생을 대상으로 한 고성능 인체 환자 모형(high-fidelity simulator) 시뮬레이션 교육 평가를 위해 Satisfaction with Simulation Experience (SSE) scale의 타당도 및 신뢰도를 검증하는데 목적이 있다. 연구 대상자는 2개 지역, 2개 대학교 4학년 간호대학생 174명이다. 수집된 자료는 SPSS/WIN 22.0을 이용하여 분석하였으며, 구성타당도 검정(요인분석, 집단비교법), 신뢰도 검정(내적 일관성 신뢰도)을 시행하였다. 요인분석 결과, 17개 문항, 3개의 요인이 나타났으며, 71.581%의 설명력을 보였다. 집단비교법 결과, 시뮬레이션 교육 만족도는 학교생활 만족도, 학교 성적에 따라 유의한 차이를 보였다. 또한 전체 문항의 내적 일관성신뢰도는 .945였으며, 하위영역별로 'Debrief(디브리핑)'은 .929, 'Clinical learning and reflection(임상 학습과 반영)'은 .908, 'Clinical reasoning(임상 추론)'은 .860이었다. 간호대학생의 고성능 인체 환자 모형 시뮬레이션 만족도는 평균 3.92점이었다. 본 연구 결과는 추후 고성능 인체 환자 모형 시뮬레이션 교육을 받은 간호대학생의 만족도 평가에 활용될 것으로 사료되며, 간호 시뮬레이션 교육 개발 및 적용 발전에 기초가 될 것이다.

Simulation Training for Inactive Nurses with 360 VR content

  • This study evaluated the effect of simulation training on cardiac arrest in hospitals for inactive nurses with 360 VR content, and attempted to prepare basic data for simulation training for inactiv nurses in the future. The design of this study is an experiment study before and after a single group. The study period was from October 13, 2020 to December 17, 2020. The subjects of the study were a total of 21 nurses who participated in the education program for inactive nurses. For simulation training for inactive nurses, Microsoft Powerpoint, hybrid simulation, high-fidelity simulation, and 360 VR content were applied for theories education and practical education. As a result of the study, the satisfaction level of the curriculum for the cardiac arrest situation in the hospital for inactive nurses was 4.78±0.36 points out of 5 points. Understanding of education was 4.71±0.46 points out of 5 points. Usefulness of education was 4.80±0.40 points out of 5 points. Confidence in airway maintenance before and after training, BLS review, manual defibrillator, emergency medication administration, airway maintenance, emergency situation simulation, and debriefing were all significant. According to the results of this study, simulation training of the situation of cardiac arrest in the hospitals for inactive nurses was effective. In future studies, it will be necessary to develope and verify specific teaching and learning methods by applying various cases of cardiac arrest situations in consideration of the type of hospitals.

간호학생을 위한 방문간호 비위관 관리교육 시뮬레이션 모듈 평가와 적용 효과: NLN Jeffries 시뮬레이션 이론 적용 (Evaluation and Application Effect of a Home Nasogastric Tube Feeding Simulation Module for Nursing Students: An Application of the NLN Jeffries Simulation Theory)

  • Purpose: The purpose of this study was to develop a simulation module for teaching home health care and evaluate the applicability of the program to nursing students' practical training. Methods: The simulation module was developed based on the National League for Nursing Jeffries Simulation Theory. The theme of the developed scenario was teaching nasogastric tube feeding to the caregiver of patient with Parkinson disease. Participants were 61 nursing students who had learned tube feeding, and participated in the questionnaire survey after the simulation training. Results: The evaluation of simulation design showed the highest score on feedback/guided reflection, and was highly evaluated in the order of objectives/information, problem solving and fidelity. The educational practice of the simulation was highly evaluated in the order of active learning, high expectation and diversity of learning. The nursing students showed high satisfaction and self-confidence after the simulation education. Conclusion: We suggest that the developed simulation module can be applied to practical training for home health care. In the future, the change of self-efficacy, clinical judgment and performance ability of the students after the simulation education should be identified. Also, various simulation modules related to the community health nursing competencies should be continuously developed and verified.

  • Microsurgery is a precise surgical skill that requires an extensive training period and the supervision of expert instructors. The classical training schemes in microsurgery have started with multiday experimental courses on the rat model. These courses have offered a low threat supervised high fidelity laboratory setting in which students can steadily and rapidly progress. This simulated environment allows students to make and recognise mistakes in microsurgery techniques and thus shifts any related risks of the early training period from the operating room to the lab. To achieve a high level of skill acquisition before beginning clinical practice, students are trained on a comprehensive set of exercises the rat model can uniquely provide, with progressive complexity as competency improves. This paper presents the utility of the classical rat model in three of the earliest microsurgery training centres and the new prospects that this versatile and expansive training model offers.

고충실도 시뮬레이터를 활용한 모성간호 시뮬레이션 교육의 효과 (Effects of Maternity Nursing Simulation using High-fidelity Patient Simulator for Undergraduate Nursing Students)

  • 본 연구는 간호대학생에게 고충실도 시뮬레이터를 활용한 모성간호 시뮬레이션 교육을 적용하고, 그 효과를 평가하기 위한 단일군 전후설계 유사실험연구이다. 간호학과 4학년에 재학 중인 학생들에게 고충실도 환자 시뮬레이터를 활용하여 임부, 산부, 산욕부 간호에 대한 시뮬레이션 교육을 실시하였고, 교육기간은 2014년 11월 10일부터 3주간이었으며, 차시별 3시간씩 총 9시간에 걸쳐 진행되었다. 교육 전후 설문을 통해 자료수집을 실시하였고, 분석에 사용된 대상자 수는 83명이었다. 수집된 자료는 IBM SPSS Statistics 20.0 프로그램을 이용하여 실수와 백분율, 평균과 표준편차, paired t-test, independent t-test, Pearson's correlation coefficients를 분석하였다. 시뮬레이션 교육 후 시뮬레이션 효과성은 총점 26점 만점에 17.4이었으며, 시뮬레이션 교육 전과 후에 의사소통능력(t=4.58, p=<.001)과 모성간호 수행에 대한 자신감(t=9.70, p=<.001)은 유의하게 향상되었으나, 문제해결능력에는 차이가 없었다. 임상실습에 만족하는 경우에 시뮬레이션 교육 후 시뮬레이션 효과성(t=2.09, p=.040), 문제해결능력(t=3.13, p=.002), 의사소통능력(t=3.54, p=.001), 모성간호 수행에 대한 자신감(t=2.64, p=.010)이 유의하게 높은 것으로 나타났으며, 시뮬레이션 효과성은 문제해결능력(r=.494, p<.001), 의사소통능력(r=.361, p<.001) 및 모성간호에 대한 자신감(r=.497, p<.001)과 유의한 양의 상관관계가 있었다. 따라서 현 임상실습의 한계에 대한 대안이자, 학생들의 임상 간호에서 필요한 역량을 강화하기 위한 전략으로써 고충실도 시뮬레이터를 활용한 모성간호 시뮬레이션 교육이 적용될 수 있으며, 이 때 학습 과정 동안 학습자의 학습경험이 학습성과로 이어질 수 있도록 하기 위한 체계적인 시뮬레이션 교수학습 전략이 필요할 것이다.

시뮬레이션을 활용한 급성 흉통환자간호 실습교육의 효과 (Effects of High-fidelity Simulation-based Education on Nursing Care for Patients with Acute Chest Pain)

  • 본 연구는 간호학생의 급성 심근경색증 간호에 대한 시뮬레이션 기반 교육을 적용하고 비판적 사고, 문제해결력 및 학업성취도의 효과를 규명한 단일군 전후 원시실험설계이다. 대상자는 총 137명이었으며 중재기간은 2011년 9월부터 10월까지 총 6주간 시뮬레이션 기반 교육을 시행하고 중재 전후를 평가하였다. 자료분석은 SPSS Win17.0을 사용하였고, 평균과 표준편차, Paired t-test, 피어슨 상관계수를 이용하였다. 연구 결과 시뮬레이션 기반 교육프로그램은 비판적 사고, 문제해결력 및 학업성취도를 향상시키는 것으로 나타났다. 비판적사고가 증대될수록 문제해결력은 향상되는 것으로 나타났다. 반면 비판적사고 능력과 문제해결력은 학업성취도와 유의한 차이가 없었다. 시뮬레이션 기반 교육프로그램은 간호학생의 실무능력을 향상시키기 위해 유용한 학습방법임을 알 수 있었고, 임상현장에서 흔히 접할 수 있는 다양한 사례의 시나리오를 개발하여 실습교육에 적극 활용할 필요가 있다. 이를 위해서는 더 다양하고 적절한 실습교육방법에 대한 개발과 운영이 필요할 것으로 보인다.

환자 안전 역량을 위한 수혈 시뮬레이션에서 간호학부생의 오류 발생과 복구 수준 (Nursing Students' Error and Recovery in Transfusion Simulation for Safety Competency)

  • Purpose: The purpose of this study was to identify the types of errors that occurred and were recovered in a simulated transfusion scenario by nursing students. Methods: Twenty-eight teams of a total of 89 nursing students participated in a transfusion simulation using a high fidelity simulator. Data were collected by observing rule based errors and built in errors recovered according to the framework of Eindhoven model. Reflective journaling was used to identify perceived safety-threatening errors and commitment to improvement. Data were analyzed using descriptive statistics. Results: All teams committed the rule based errors in the scenario. The most common errors occurred in the coordination category related to communication with physician. Most of students perceived the transfusion reaction as a safety-threatening error. Conclusion: The findings indicate that students lack patient safety competence. The simulation training to decrease errors and improve safe practice provides nursing students with an effective strategy to develop patient safety competence.

SiRENE: A new generation of engineering simulator for real-time simulators at EDF

  • For Safety Assisted Engineering works, real-time simulators have emerged as a mandatory tool among all the key actors involved in the nuclear industry (utilities, designers and safety authorities). EDF, Electricité de France, as the leading worldwide nuclear power plant operator, has a crucial need for efficient and updated simulation tools for training, operating and safety analysis support. This paper will present the work performed at EDF/DT to develop a new generation of engineering simulator to fulfil these tasks. The project is called SiRENE, which is the acronym of Re-hosted Engineering Simulator in French. The project has been economically challenging. Therefore, to benefit from existing tools and experience, the SiRENE project combines: - A part of the process issued from the operating fleet training full-scope simulator. - An improvement of the simulator prediction reliability with the integration of High-Fidelity models, used in Safety Analysis. These High-Fidelity models address Nuclear Steam Supply System code, with CATHARE thermal-hydraulics system code and neutronics, with COCCINELLE code. - And taking advantage of the last generation and improvements of instructor station. The intensive and challenging uses of the new SiRENE engineering simulator are also discussed. The SiRENE simulator has to address different topics such as verification and validation of operating procedures, identification of safety paths, tests of I&C developments or modifications, tests on hydraulics system components (pump, valve etc.), support studies for Probabilistic Safety Analysis (PSA). etc. It also emerges that SiRENE simulator is a valuable tool for self-training of the newcomers in EDF nuclear engineering centers. As a modifiable tool and thanks to a skillful team managing the SiRENE project, specific and adapted modifications can be taken into account very quickly, in order to provide the best answers for our users' specific issues. Finally, the SiRENE simulator, and the associated configurations, has been distributed among the different engineering centers at EDF (DT in Lyon, DIPDE in Marseille and CNEPE in Tours). This distribution highlights a strong synergy and complementarity of the different engineering institutes at EDF, working together for a safer and a more profitable operating fleet.

복강경수술 훈련용 담낭 절제술 시뮬레이션 개발 (Development of Cholecystectomy Simulation for Laparoscopic Surgery Training)

  • Laparoscopic surgery is a surgical procedure which uses long laparoscopic instruments through tiny holes in abdomen while watching images from a laparoscopic camera through umbilicus. Laparoscopic surgeries have many advantages rather than open surgeries, however it is hard to learn the surgical skills for laparoscopic surgery. Recently, some virtual simulation systems for laparoscopic surgery are developed to train novice surgeons or resident surgeons. In this study, we introduce the techniques that we developed for laparoscopic surgical training simulator for cholecystectomy (gallbladder removal), which is one of the most frequently performed by laparoscopic surgery. The techniques for cholecystectomy simulation include modeling of human organs (liver, gallbladder, bile ducts, etc.), real-time deformable body calculation, realistic 3D visualization of surgical scene, high-fidelity haptic rendering and haptic device technology, and so on. We propose each simulation technique for the laparoscopic cholecystectomy procedures such as identifying cystic duct and cystic artery to clamp and cut, dissecting connective tissues between the gallbladder and liver. In this paper, we describe the techniques and discuss about the results of the proposed cholecystectomy simulation for laparoscopic surgical training.
