표준 치료를 받고 있는 제2형 당뇨병 환자에서 혈당강하제와 평위산 및 향사평위산 병용 투여가 혈당에 미치는 영향 및 안정성 연구 (Assessment of Combined Administration of Hypoglycemic Agents and Herbal Extracts (Pyeongwi-san or HyangsaPyongwi-san) on Blood Glucose Levels in Type 2 Diabetes Mellitus: A Retrospective Study)

  • Abstract: This retrospective study delved into the effects and safety considerations associated with the concomitant usage of hypoglycemic agents and herbal extracts, specifically Pyeongwi-san (PWS) or HyangsaPyongwi-san (HSPWS) in the context of type 2 diabetes mellitus management. Methods: The investigation involved 38 inpatients with type 2 diabetes mellitus who received PWS or HSPWS treatment at Kyung Hee University Korean Medical Hospital from January 2012 to December 2022. By investigating clinical attributes and conducting laboratory assessments, this study aimed to discern the impact of these herbal extracts on blood glucose levels, encompassing fasting blood sugar (FBS) and mean 2-hour postprandial glucose (PP2) levels. Furthermore, the safety profile of the herbal extracts was assessed by comparing liver function indicators, including aspartate aminotransferase (AST), alanine aminotransferase (ALT), and γ-glutamyl transferase (GGT), alongside kidney function markers, such as blood urea nitrogen (BUN) and creatinine (Cr). Results: Following the administration of the herbal extracts, no statistically significant alterations in FBS and mean PP2 levels emerged compared to the baseline levels. Notably, the safety evaluation revealed no significant differences in liver and kidney function parameters following herbal extract administration. Conclusion: The results of this research indicate that using PWS or HSPWS alongside hypoglycemic medications could be a beneficial additional method for addressing digestive symptoms in individuals with type 2 diabetes mellitus. Notably, this combination seems to have no negative interactions with other drugs.

한약 복합추출물 HCE-2가 알코올을 투여한 쥐의 알코올 대사 및 간기능에 미치는 영향 (Protective Effect of the Herbal Combination HCE-2 on Alcohol Metabolism and Alcohol-induced Liver Injury in Ethanol-loaded Rats)

  • This study was performed to investigate the medicinal effects of the herbal combination extracts-2 (HCE-2), consisting of Artemisia capillaris Thunb., Lonicera japonica Thunb., Prunella vulgaris var. lilacina, and Hovenia dulcis Thunb. on the alcohol-induced liver injury in rats. The rats were randomly divided into four groups: normal group (n =6), non-treated control group (n =6), saline-treated group (n =6) and the herbal combination extract (HCE-2)-treated group (n =6). The rats in the alcohol-loaded groups were orally administered with ethanol at a daily dose of 4 g/kg-body weight for 5 weeks. Thirty minutes before the ethanol injection, saline or herbal combination extracts was administered by using a gastrogavage. Blood and liver tissue samples were taken out from the hearts and livers of the rats, respectively, on 15th and 38th days. The activities of serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were measured using an enzyme-linked immunosorbent assay (ELISA). We also investigated the protective effect of the herbal combination extracts by Hematoxylin-Eosin staining on histological sections of rat liver. In this study, the oral administration of the herbal combination extracts significantly reduced the serum levels of AST and ALT, which had been raised by alcohol-induced liver injury. Histological analysis and apparent observation of liver also showed the preventive effect of the herbal combination extracts in a chronic alcohol-induced rat model. Theses results revealed that the herbal combination extracts effectively prevented hepatic damage consequent to the chronic exposure to repetitive administration of ethanol and could be used as a primary resource of a health beverage or herbal medicine, alleviating the alcohol-induced hepatic injury and hangover symptoms.

한약이 골다공증 혹은 골감소증 환자의 골밀도에 미치는 영향: 체계적 문헌고찰 및 메타분석을 위한 프로토콜 (Effects of Herbal Medicines on Bone Mineral Density Score in Osteoporosis or Osteopenia: Study Protocol for a Systematic Review and Meta-Analysis)

  • Objectives The impact of social and economic losses on society due to the occurrence of osteoporosis patients is serious. The purpose of this study is to identify the types and effects of herbal medicine for osteoporosis and osteopenia. Methods We will investigate 8 online databases (MEDLINE/PubMed, Cochrane library, EMbase, China National Knowledge Infrastructure [CNKI], J-stage, National Digital Science Library [NDSL], Research Information Sharing Service [RISS], Koreanstudies Information Service System [KISS]) without language, publication date limitation for the clinical study of herbal medicine for osteoporosis and osteopenia published between the start of the database and 2021. The frequency, duration and pattern of the any type of herbal medicine utilization will be analyzed. Results Randomized controlled trials about herbal medicine or herbal medicine extracts for osteoporosis and osteopenia should be included in the study. Cochrane risk in bias tools will be used to evaluate the methodological quality of the study. A risk ratio or mean difference with a 95% confidence interval will show the effects of herbal medicine or herbal medicine extracts for osteoporosis and osteopenia. Conclusions The results of the systematic review will reflect the current status of herbal medicine treatment for osteoporosis and osteopenia, and it can be expected to verify the validity of the effectiveness, and provide it as a basic data for the use of herbal medicine in clinical utilization.

홍삼.교맥 복합물의 미용식품 응용 가능성에 대한 연구 : 항산화 및 Collagenase 저해 효과 (A study on Application for Beauty Food of Mixture of Korean Red ginseng and Fagopyrum esculentum : Anti-oxidative Effect and Collagenase Inhibitory Activity)

  • Panax ginseng C.A. Meyer, Korean herb medicine, has been widely used in China and Japan for fatigue and enhancement of resistance to many diseases. In this study, we investigated the synergistic effect of Korean red ginseng and Fagopyrum esculentum extracts (RGFE) on dermal bioactive properties. RGFE treatment significantly increased electron donating ability, nitrite scavenging activity and collagenase inhibitory activity compared to red ginseng-treated group. Superoxide dismutase (SOD) activity in RGFE-treated group was similar to that of red ginseng. However, tyrosinase activity as indicator of melanin synthesis was not affected by RGFE or red ginseng. The results indicate that RGFE has anti-oxidative property and inhibitory effect of collagenase, and it may serve as a effective ingredient for beauty food.

에탄올 함량변화에 따른 유백피 추출물의 항산화 활성 (Antioxidant Activities of Ulmi cortex Extracts According to Ethanol Contents)

  • Objectives : This study was performed to find best extraction solvent for application of Ulmi cortex to food or herbal medicine as an antioxidant only using water, ethanol and their mixtures. Methods : The Ulmi cortex extracts were prepared using water and 10%, 20%, 30%, 40%, 50%, 60%, 70%, 80%, 90% and 100% (v/v) ethanol, and were evaluated yields, total polyphenol contents, DPPH and ABTS radical scavenging activities, lipid peroxidation activities, and catechin and epicatechin contents. Results : Among the Ulmi cortex extracts, the yield was highest in water extract (8.9%) and lowest in ethanol extract (3.8%). The yield of 30% ethanol extract (8.5%) also was very high to similar with water extract. The total polyphenol content was highest in the 30% ethanol extract ($253.6{\mu}g/mg$ extract) and lowest in water extract ($109.0{\mu}g/mg$ extract). The DPPH radical scavenging activity was highest in ethanol extract (IC50, $8.53{\mu}g/ml$), ABTS radical scavenging activity was highest in 60% ethanol extract (IC50, $3.08{\mu}g/ml$), and the inhibition of lipid peroxidation was highest in 70% ethanol extract (IC50, $7.96{\mu}g/ml$). As ethanol content of extraction solvent increased from 0% to 30%, the antioxidant activities were remarkably increased whereas from 30% to 100%, the antioxidant activities were increased or decreased a little. Conclusions : The findings of the present study suggest that 30% ethanol is best solvent for extraction of Ulmi cortex, considering yield, polyphenol content, and antioxidant activities with extraction cost.

  • Objectives : This study was carried out to investigate the acute toxicity and safety of Ssanghwa-tang extract fermented with Paecilomyces japonica. Methods : To evaluate the acute toxity and safety, 0(control group), 1250, 2500 and 5000 mg/kg of Ssanghwa-tang and fermented Ssanghwa-tang extracts were orally administered to 35 male and 35 female ICR mice. After single administration, we observed number of death, general toxicity, changes of body weight, and autopsy. Results : Compared with the control group, we could not find any toxic alteration in all treated groups (1250, 2500 and 5000 mg/kg). Conclusions : $LD_{50}$ of Ssanghwa-tang and fermented Ssanghwa-tang extracts might be over 5000 mg/kg and it is very safe to ICR mice.

단삼 추출물이 암세포주에 미치는 세포증식 억제 효과 (Antiproliferative Effect of the Salviae miltiorrhizae Radix Extracts on the Cancer Cell Lines)

  • Objectives : The purpose of this study was to identify antiproliferative effects of Salviae miltiorrhizae Radix(SM) extracts against cancer cell lines. Methods : We used 2 kinds of cancer cell lines such as colon cancer cells(HT-29), human oral epitheloid carcinoma cells(KB). MTT assay was performed to examine the efficacy of SM extracts on the cytostaticity of cancer cells in proportion to time and doses. Apoptosis was evaluated by DNA laddering and DAPI nuclei staining. Results : The MTT absorbances against HT-29 and KB of SM extracts were significantly decresed. DNA ladders could be identified in KB of SM extracts. The morphological change were observed and number of cells were decreased by SM extracts. Conclusions : SM extracts is considered to be effective to induce apoptosis and inhibit cancer cell proliferation.

소청룡탕 및 발효 소청룡탕 추출물의 단회투여 독성에 관한 연구 (Acute Toxicity Study on Socheongryong-Tang and Fermented Socheongryong-Tang extracts in Mice)

  • Objectives : Socheongryong-Tang (小靑龍湯, SCRT) has been widely used to treat respiratory disease. In this study, we investigate the acute toxicity and safety of fermented Socheongryong-Tang extract. Methods : To evaluate the acute toxicity and safety, 0 (control group), 1250, 2500 and 5000 mg/kg of Socheongryong-Tang and fermented Socheongryong-Tang extracts were orally administered to male and female ICR mice. After single administration, we observed survival rates, general toxicity, changes of body weight for the 14 days and autopsy. Results : Compared with the control group, we could not find any toxic alteration in all treated group (1250, 2500 and 5000 mg/kg). Conclusions : $LD_{50}$ of Socheongryong-Tang and fermented Socheongryong-Tang extracts might be over 5000 mg/kg and it is safe to ICR mice.

알레르기 비염 환자의 보험 한약 제제 및 한의 처치 이용 현황 : 건강보험심사평가원 자료 분석 (Analysis of the Use of Insured Herbal Extracts and Korean Medicinal Treatments in Patients with Allergic Rhinitis : Data from Health Insurance Review and Assessment Service)

  • Objectives : The purpose of this study is to analyze the use of insured herbal extracts and Korean medicinal treatments, which are mainly used to treat allergic rhinitis in Korean medicine. Methods : Among all HIRA(Health Insurance Review and Assessment Service) claims data in 2016, we included all statements that included J30(vasomotor and allergic rhinitis) or a subcategory of J30(J30.0, J30.1, J30.2, J30.3, or J30.4) as the main disease, using the Korean Standard Classification of Diseases(KCD-7). This study analyzed the most frequently used insured herbal extracts and Korean medicinal treatments for allergic rhinitis in Korean medicine. We performed a frequency analysis on subgroups based on treatment type(inpatient or outpatient), sex, age, insurance type, and medical institution type. Results : The result shows the 10 most frequently used insured herbal extracts and Korean medicinal treatments for allergic rhinitis. The total number of insured herbal extracts prescriptions was 82,533, and the most commonly prescribed insured herbal extracts was socheongryong-tang(35,131 prescriptions), followed by hyeonggaeyeongyo-tang(18,157 prescriptions), samsoeum(6,257 prescriptions), and galgeun-tang(4,465 prescriptions). The total number of Korean medicinal treatments prescriptions was 1,878,541, of which the most common Korean medicinal treatments was acupuncture(922,977 prescriptions), followed by moxibustion(372,120 prescriptions), cupping(242,094 prescriptions), and segmental acupuncture(161,553 prescriptions). Conclusions : It is expected that the results of this study can be used as a basis for establishing the priorities of evidence-based clinical research topics in the field of Korean medicine and making health care policy decisions to strengthen coverage in the future.

Effects of Root Extracts from Angelica gigas and Angelica acutiloba on Inflammatory Mediators in Mouse Macrophages

  • Root extracts of Angelica gigas and A. acutiloba have been used traditionally for the treatment of gynecological diseases, as well as anemia, blood stasis, and inflammatory pain, as blood tonics in Oriental medicine. In the present study, we investigated the effects of A. gigas and A. acutiloba on inflammatory mediators in mouse macrophages and compared their activities. Many studies suggest that prostaglandin $E_2$ ($PGE_2$) biosynthesis and nitric oxide (NO) production play important roles in the processes of both inflammation and carcinogenesis. Ethanolic extracts from the roots of both species exhibited significant inhibitory effects on $PGE_2$ generation in lipopolysaccharide-stimulated RAW 264.7 cells. In particular, the extract from A. gigas was more effective than that from A. acutiloba. Although neither inhibited NO generation, the extract from A. acutiloba stimulated NO generation. Our results suggest that the roots of A. gigas might possess more anti-inflammatory and/or cancer chemopreventative activity than that of A. acutiloba due to the suppression of cyclooxygenase-2 (COX2)-mediated $PGE_2$ production. In addition, A. acutiloba might exert anti-tumor activity through an increase in macrophage-produced NO.
