사상처방엑기스제의 한방보험 급여화에 대한 제안 (Insurance Coverage on the Sasang Constitutional Herbal Medicine Extracts)

  • 1. Objectives: Sixty eight kinds of single herbal medicine extracts and fifty six kinds of mixed herbal medicine extracts have been used under a national health insurance since 1987. The number of herbal medicine extracts under an insurance coverage remains unchanged. The demand for covering complex herbal medicine extracts and Sasang Constitutional medicine extracts in a national health insurance increases. This study is to investigate the needs for the insurance coverage. 2. Methods: Advantages and disadvantages between herbal medicine decoction and extracts were explained and Sasang Constitutional medicine extracts according to Sasang constitution were surveyed in production nowadays. 3. Results: Herbal medicine decoction has many advantages of treating patients but herbal medicine extracts have advantages of the rapid adminstration and being easy to carry. From eleven to twenty two products of Sasang Constitutional medicine extracts have been in production in several factories. There are eleven kinds of Soyangin medicine extracts, nine kinds of Taeeumin medicine extracts and five kinds of Soeumin medicine extracts. 4. Conclusions: Complex herbal medicine extracts and Sasang Constitutional medicine extracts have to be in use under a national health insurance as soon as possible.

한방보험제제의 사상의학적 분류 및 운용 (Herbal Extracts Classification and Application in Terms of Sasang Constitution)

  • 1. Objectives Herbal Extracts are used in Korean Oriental Medicine clinics. The sixty eight kinds of single herbal extracts and fifty six kinds of mixed herbal extracts has been using since the first year of execution of Korean medicine national insurance policy in 1987. This paper was performed to make a guideline of using herbal extracts. Single herbal extracts and mixed herbal extracts were discussed and classified according to Sasang constition. 2. Methods The sixty eight single herbal extracts were classified into four categories according to Sasang Constitution and fifty six mixed herbal extracts were classified by six Korean oriental medical doctors. Mixed herbal extracts were considered in terms of drug roles[sovereign, minister, assistant and courier(君臣佐使)] or most included ingredient herb. 3. Results Sasang constitutions which were suitable for fifty six mixed herbal extracts were Soeumin(46.4%), mixed constitution(39.2%) and Soyangin(8.9%), Taeeumin(5.3%). Sasang constitutions which were suitable for sixty eight single herbal extracts were Soeumin(47.1%), Soyangin(32.4%) and Taeeumin(20.6%). 4. Conclusions The rate of Soeumin herbal extracts was the most highest among all constitutions. It is regarded that warming and enforcing herb medicine were most used in all prescriptions.

한의사의 복합과립제 사용 실태 조사 및 복합과립제 건강보험 급여 시행방안에 대한 연구 (A Study on Korean Oriental Medical Doctors' Use of Uninsured Herbal Extracts and How to Promote the Insurance Coverage of Such Herbal Extracts)

  • Objectives: To research Korean oriental medical doctors' use of uninsured herbal extracts and how to bring about the insurance coverage of such herbal extracts. Methods: We surveyed Korean oriental doctors about the following issues from October 17th to November 15th, 2008: (1) Korean oriental medical doctors' knowledge about herbal extract insurance and the circumstances of oriental medicine in Korea, (2) their opinion on the coverage of currently uninsured herbal extracts and the dispensing of herbal extracts from pharmacies rather than from their clinics, (3) their use of herbal extracts, and (4) how to bring about insurance coverage of uninsured herbal extracts. Results: Over 70% of the respondents said that herbal medicine prescriptions have been reduced recently and that the existence of herbal medicine is in danger. In addition, 63.64% respondents agreed with expanding insurance coverage to include currently uninsured herbal extracts in spite of the fact that patients might have to obtain herbal extracts from pharmacies rather than from Korean oriental medical clinics. The average patient number per month of uninsured herbal extracts was 13.64 people, the average dosage was 5.64g, the average cost per day was 3,859 won, and the average prescription period was 2.65 days. Korean oriental medical doctors asked an average of 12,486 won for the medical examination-prescription fee and 3,292 won in fees for prescriptions obtained outside the hospital. If insurance coverage expands to include these herbal extracts, their usage is expected to increase 2.31 times. Conclusions: This study shows Korean oriental medical doctors' use of herbal extracts and their opinions about execution of herbal extracts' insurance. A periodic study such as this one will hopefully aid in establishing polices for uninsured herbal extracts' insurance.

  • Objectives : The discussion of enlargement of Korean Medical insurance has been existed since 1987. But it had less effects. For better discussion, we compared insured herbal extracts and clinical prescriptions. Methods : The database of insured herbal extracts and clinical prescriptions bas set up to compare the ratios of herbal weight and analyze indications of insured herbal extracts and diagnosis and chief complain of clinical prescriptions. Results & Conclusions : The most frequent insured herbal extract is Bojungiki-tang. Analysis about indications of insured herbal extracts and diagnosis and chief complain of clinical prescriptions is that Bojungiki-tang is used frequently for gastrointestinal diseases, pulmonary diseases, and not for diseases. Bojungikgi-tang is also used for muscloskeletal dieases, hemorrhage, and ischemia.

한방병원 입원 허혈성 뇌경색 환자에서 한방 의료보험용 엑스산제와 항혈소판제 및 항응고제의 병용투여 현황 (The current status of the combination therapy of frequently used herbal extracts and anti-platelet drug, anti-coagulant drug in ischemic stroke patients hospitalized in oriental medical hospital)

  • Objective: Nowadays the combined use of herbal extracts and western medicines has been prevalent, but concern about its risk is also increasing. Even though the importance of clinical trials is well recognized, there have been only a few studies on the combined use of herbal extracts and western medicines. This study was aimed to examine which herbal extracts and antiplatelets or anticoagulants are most commonly prescribed together to inpatient ischemicstroke patients and investigate the combined prescription rate. Methods: We investigated the most frequently prescribed herbal extracts from two different sources. First, we chose herbal medicine extracts from 2008 Traditional Korean Medicine Utilization Status. Then, among patients who were admitted to Kyung-Hee Oriental Medical Center diagnosed with ischemic stroke, we found patients who were administered with these herbal medicine extracts and among these patients, we investigated how many were concomitantly administered with antiplatelets or anticoagulants. Second, we chose other herbal medicine extracts that were most often prescribed within Kyung-Hee Oriental Medical Center and found patients who were administered with these herbal medicine extracts, then investigated how many of them were concomitantly administered with specific antiplatelets or anticoagulants. Results: The most commonly prescribed herbal medicine extracts among ischemic stroke patients were Ojeok-san, Bojungikgi-tang, Sochungryong-tang and Samsoeum. About 46 to 69 percent were concomitantly administered with either specific antiplatelets or anticoagulants. Conclusions: The most often prescribed herbal medicine extracts in inpatient ischemic stroke patients are Ojeok-san, Bojungikgi-tang, Sochungryong-tang, and Samsoeum. Among patients who were prescribed with Ojeok-san, Bojungikgi-tang, Sochungryong-tang, Samsoeum, patients who were concomitantly administered with either antiplatelets or anticoagulants were about 46% to 69%.

ICR 마우스를 이용한 삼소음 및 발효삼소음의 급성독성 연구 (Acute toxicity on Samsoeum and fermented Samsoeum in ICR mice)

  • Objectives : This study aims to investigate the acute oral toxicity and safety of Samsoeum (Shensuyin) extracts and fermented Samsoeum extracts. Methods : For that objective, we used ICR mice. ICR mice were administerd orally with dosage of 1250mg/kg, 2500mg/kg and 5000mg/kg of Samsoeum extracts and fermented Samsoeum extracts. Results : We daily examined number of deaths, clinical signs, body weights and gross findings for 2 weeks. 1. The results of acute oral toxicity using ICR mice showed that LD50 of value over 5000 mg/kg. 2. Samsoeum extracts and fermented Samsoeum extracts not affect on bodyweight gross findings of ICR mice. 3. The results of Serum chemistry analysis and Complete Blood Count(CBC) through the autopsy were showed normal range values. Conclusions : Samsoeum extracts and fermented Samsoeum extracts did not show any toxic effects in ICR mice. And oral LD50 value was over 5000mg/kg in ICR mice and it is very safe for ICR mice.

Antimicrobial Activity of Korean Medicinal Plants and Herbal Formulations

  • In this study, we screened twenty four extracts of eight medicinal plants and three extracts of the commercial product for their antimicrobial and antioxidant properties. Ethanol and ethyl acetate extracts were found active where as aqueous extracts were little or no active. Extracts of Sophora flavescens, Salvia miltlorrhiza and Glycyrrhiza uralensis showed strong activity againsttested organisms and could be the potential antimicrobial agent. The increase of antimicrobial and antioxidant activities of formulations might be due to synergic effect. The results also indicated that the activity of bamboo salt and herbal products can be enhanced by making appropriate formulations.

일본인 환자의 사상체질에 따른 한방제제 활용에 관한 연구 (A Study on the Use of Herbal Extracts according to the Sasang Constitution of Patients in Japan)

  • Objectives : This study was performed to find out the correlation between the use of herbal extracts and the Sasang constitution of patients in Japan. Methods : We recruited 315 subjects who had been visiting the Center for Kampo Medicine for the treatment, Keio University in Tokyo, Japan. They answered SSCQ-P(Sasang Constitution Questionaire for Patients) questionnaire, a sasang constitution specialist interviewed them and diagnosed the sasang constitution. And the 80 herbal extracts subject's having taken were classified into four type Sasang constitutional herbal formulas according to "Donguisusebowon", the ingredients in a formula that have different role(sovereign, minister, assistant and courie) and effect and indication of formula. Results : 1. Among the total 315 subjects, the number of Taeyangin, Soyangin, Taeeumin, and Soeumin was 3(1.0%), 85(27.0%), 90(28.6%), and 137(43.5%). 2. Among the 80 herbal extracts subject's having taken, the number of each Soyangin, Taeeumin, and Soeumin herbal formulas was 30, 7 and 43. 3. 1 Soyangin and 2 Soeumin herbal formulas were prescribed in 3 Taeyangin subjects group. 29 Soyangin, 4 Taeeumin, 52 Soeumin herbal formulas were prescribed in 85 Soyangin subjects group. 39 Soyangin, 6 Taeeumin, 45 Soeumin herbal formulas were prescribed in 90 Taeeumin subjects group. 51 Soyangin, 6 Taeeumin, 80 Soeumin herbal formulas were prescribed in 137 Soeumin subjects group. Conclusions : The Soeumin rate in Japan was significantly higher than that in Korea. There was no significant correlation between the use of herbal extracts and the Sasang constitution in Japan.

한국산과 중국산 박하의 항염증 효과에 관한 비교연구 (Comparative Study of the Anti-inflammatory Effects of Menthae Herba from Korea and China)

  • Menthae herba (MH) extracts exhibit anti-inflammatory effects. The purpose of this study was to determine whether the anti-inflammatory effects of MH extracts vary according to the cultivation regions. We performed a comparative analysis of MH extracts by evaluating the production of inflammatory mediators in RAW 264.7 murine macrophage cells and HaCaT human keratinocyte cells. MH extracts obtained from different cultivation regions in Korea and China significantly reduced the production of nitric oxide (NO), prostaglandin $E_2$ ($PGE_2$), and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) in RAW 264.7 cells stimulated with lipopolysaccharide (LPS). No differences in these inhibitory activities were observed between MH extracts. In HaCaT cells stimulated with TNF-${\alpha}$ and interferon-${\gamma}$ (IFN-${\gamma}$), MH extracts did not inhibit the production of macrophage-derived chemokine (MDC/CCL22), but most extracts reduced the production of the regulated on activation normal T-cell expression and secreted (RANTES/CCL5). We used clustering tree analysis of the MH extracts according to the chromatographic pattern and anti-inflammatory potency of MH extracts. We observed differences in the chromatographic pattern of MH extracts but no difference in anti-inflammatory potency. Our findings suggest that MH extracts from different regions do not show any differences in their pharmacological potency in that MH extracts are used as therapeutic agents to treat inflammatory disorders.

경동맥 혈전 유발 동물모델을 이용한 한약재 추출물의 효능 검색 (Screening of Anti-thrombotic Activity of Herbal Extracts in Ferric Chloride-induced Thrombosis Model)

  • Objectives : The aim of this study was to examine anti-thrombotic effect of traditional herbal extracts in a rat model of ferric chloride ($FeCl_3$)-induced carotid arterial thrombosis. Methods : Thirty minutes prior to a 35% $FeCl_3$ application, Sprague Dawley(SD) rats were injected with the 10 types of traditional herbal extracts (100mg/kg, intraperitoneal injection), respectively. The effect of these herbal extracts was examined for time to occlusion(TTO) using the Laser doppler flow meter and measured for thrombus weight (TW) in $FeCl_3$-induced thrombosis model. Results : In the TTO, Salvia miltiorrhiza (Sm, $2.30{\pm}0.28$ min, p<0.001) and Santalum album (Sa, $2.19{\pm}0.19$ min, p<0.001) showed significantly delayed TTO more than twice compared with Saline-treated group. Cnidium officinale (Co), Psoralea corylifolia (Pc), Scutellatia baicalensis (Sba), Panax notoginseng (Pn), Angelica tenuissima (At), Scrophularia buergeriana (Sbu), Rhus verniciflua (Rv) and Picrasma quassioides (Pq), except for Rhus verniciflua (Rv) also meaningfully impeded TTO more than one fold. In addition, Salvia miltiorrhiza, Santalum album, Cnidium officinale, Psoralea corylifolia and Scutellatia baicalensis significantly reduced TW more than 10% compared with Saline-treated group. Especially, Salvia miltiorrhiza and Santalum album showed the most excellent anti-thrombotic effect among the 10 herbal extracts tested on the restoration of altered TTO and TW. Conclusions : These results suggest that Sm and Sa extracts have outstanding anti-thrombotic effect in $FeCl_3$-induced thrombosis model and is potentially useful as herbal medicines for the treatment and prevention of thrombosis.
