부신의 종괴로 처음 발현한 전이성 간세포암: 증례보고 (Adrenal Mass as Initial Presentation of Metastatic Hepatocellular Carcinoma: A Case Report)

  • 간세포암은 부신을 포함하여 여러 장기에 전이를 일으킬 수 있다. 그러나, 간의 병변 없이 부신으로 전이된 간세포암의 예는 매우 드물다. 저자들은 부신의 종괴로 처음 발현한 전이성 간세포암의 전산화 단층촬영 (CT), 자기공명영상 소견을 보고하고자 한다. 간세포암의 전이로 인한 부신 종괴는 MRI에서 간세포암과 유사한 신호강도와 조영증강 양상을 보이고 있었다.

Tumor lysis syndrome following sorafenib treatment in hepatocellular carcinoma

  • Sorafenib is indicated for the treatment of advanced hepatocellular carcinoma (HCC), but although rare, tumor lysis syndrome (TLS) can be fatal in HCC patients with a large tumor burden. The authors describe the case of a 55-year-old hepatitis B carrier who visited our clinic with progressive dyspnea for 3 weeks. Chest and abdominal computed tomography revealed a huge HCC in the left lobe of the liver with invasion of the inferior vena cava, right atrium, and pulmonary arteries. After 8 days of sorafenib administration, TLS was diagnosed based on the characteristic findings of hyperuricemia, hyperkalemia, and acute kidney injury with massive tumor necrosis by follow-up imaging. Despite discontinuation of sorafenib and supportive care, the patient's clinical course rapidly deteriorated. The authors describe a rare but fatal complication that occurred soon after sorafenib initiation for HCC. Careful follow-up is required after commencing sorafenib therapy for the early diagnosis and management of TLS.

  • 예후인자의 범주화는 질병의 진단, 치료법 결정 및 임상시험의 참여기준 설정 등에 매우 유용하다. 예후인자의 범주화 방법으로는 생물학적 이론에 따라 경계값을 정하는 방법, 자료의 그래프를 이용하는 방법, 모든 가능한 경계 값들을 적용해보는 최소 유의확률 방법 등이 있다. 또한 최소 유의확률 방법의 다중검정문제를 보완한 몇 가지 방법들이 있다. 본 연구에서는 우리 나라에서 높은 발생률을 보이는 간세포암종의 절제술을 받은 환자들에 있어서 간 절제술 후 재발위험이 높은 군을 구별하는 한 근거로 종양의 크기를 범주화하기 위한 경계값을 결정하고자 하였다.

인체 간암세포에서 Apigenin에 의한 TRAIL 유도 Apoptosis의 증진 효과 (Enhancement of TRAIL-Induced Apoptosis in Human Hepatocellular Carcinoma Cells by Apigenin)

  • Tumor necrosis factor-related apoptosis inducing ligand (TRAIL) is one of the promising anti-cancer agent because of its ability to selectively induce apoptosis in tumor cell lines but not in normal cells. However, TRAIL resistance has been reported in some cancer cells including hepatocarcinoma cells. Therefore, studies of agents that sensitize TRAIL-resistant cancer cells could be a effective therapeutic approach in cancer management. In our study, we examined the effect of combination of TRAIL with apigenin in human hepatocellular carcinoma cells. As a result, the combined use of TRAIL and apigenin significantly enhanced the cytotoxicity in PLC-PRF5 cells. Flow cytometry analysis after annexin V-FITC/PI dual staining showed that this increase of cell cytotoxicity was related to enhanced apoptosis in combined treatment of TRAIL with apigenin. Furthermore, synergistic induction of apoptosis was also confirmed by observation of morphological changes and annexin V-FITC/PI fluorescence. Our findings suggests that apigenin has the potential to improve the efficiency of TRAIL-based therapies in human hepatocellular carcinoma cells. Further study is needed to reveal the molecular mechanisms of this combined therapy.

개의 종괴형 간세포암종에 대한 진단 영상 소견에 대한 고찰 (Diagnostic Imaging of Massive Hepatocellular Carcinoma in 5 Dogs)

  • Hepatocellular carcinoma (HCC) is a primary hepatic neoplasms in dogs. Three types of HCC such as massive, nodular and diffuse form were reported. Massive HCC is most common and has relatively better prognosis than other forms because this type of HCC can be removed surgically and has low frequency of relapse or metastasis. Diagnostic image can provide useful information for shape and internal structure of the hepatic mass as well as the location and adhesion or invasiveness of the mass to establish surgical plan to remove the mass safely. In this study, we investigated diagnostic features of massive HCC in 5 dogs. Radiography showed soft tissue mass in cranial abdomen in 3 dogs. On ultrasonography, all dogs had a solitary hepatic mass with mixed echo pattern with anechoic cysts, which represented necrosis and hemorrhage. The radiographic and ultrasonographic findings of affected hepatic lobe were compared with the macroscopic findings through laparotomy. Computed tomography was performed to check metastasis and figure out the origin of the mass in two dogs. Ultrasonography is useful and relatively sensitive examination for diagnosis and planning for surgery in canine massive HCC.

  • Cryptogenic organizing pneumonia (COP) is an inflammatory lung disease involving the distal bronchioles, respiratory bronchioles, bronchiolar ducts, and alveolae. The etiology is usually unknown; however, there are several known causes and associated systemic diseases. Corticosteroid therapy is the best treatment option and the prognosis of COP is good, with recovery in up to 80% of patients. We described a patient with in-operable hepatocellular carcinoma (HCC) undergoing chemoembolization with doxorubicin in a drug-eluting bead (DEB). COP developed in the patient after chemoembolization but resolved spontaneously in several months.

  • Pulmonary lipiodol embolism is a rare but very fatal complication of transcatheter arterial chemoembolization (TACE), Here we present the case of an unusual complication of TACE in a 67-year-old man who presented with dyspnea, hemoptysis, and a history of a third session of TACE for hepatocellular carcinoma (HCC) that had been performed 3 days prior to presenting. On the basis of chest X-ray and computed tomography (CT) scan findings, we diagnosed pulmonary lipiodol embolism. He was conservatively treated with oxygen and haemostatic agents. The patient recovered quickly without any significant sequela and was discharged.

Effect of respiratory syncytial virus on the growth of hepatocellular carcinoma cell-lines

  • In several reports, the respiratory syncytial virus (RSV) was identified as an oncolytic virus in cancer cells (e.g., lung and prostate cancer). However, the effects of RSV in hepatocellular carcinoma (HCC) cells have not yet been investigated. Here, we observed the inhibitory effects of RSV infection in HCC cell-lines. Cell growth was significantly decreased by RSV infection in BNL-HCC, Hep3B, Huh-7 and SNU-739 cells. After RSV infection, plaque formation and syncytial formation were observed in affected Hep3B and Huh-7 cells. RSV protein-expression was also detected in Hep3B and Huh-7 cells; however, only Huh-7 cells showed apoptosis after RSV infection. Furthermore, inhibition of cell migration by RSV infection was observed in BNL-HCC, Hep3B, Huh-7 and SNU-739 cells. Therefore, further investigation is required to clarify the molecular mechanism of RSV-mediated inhibition of HCC cell growth, and to develop potential RSV oncolytic viro-therapeutics.

  • A crude extract of Smphytum officinale L. (comfrey) was for its ability to inhibit he growth of hepatocellular carcinoma cells and expression of the insulin-like growth factor I (IGF-II) gene. The DNA synthesis of hepatocellular carcinoma cell lines, Hep G2, Hep 3B, and PLC/PRF/5 was inhibited by a crude extract of Smphytum officinale in both a time- and a dose-dependent manners. This plant extract also inhibited expression of the IGF-II gene. Since IGF-II exerts a mitogenic effect on Hep G2 cells, these results suggest that the growth inhibition by Symphytum officinale extract is, in part, mediated through the inhibition of IGF-II gene expression.

Effects of rosmarinic acid on immunoregulatory activity and hepatocellular carcinoma cell apoptosis in H22 tumor-bearing mice

  • Rosmarinic acid (RA) is a natural polyphenolic compound that exists in many medicinal species of Boraginaceae and Lamiaceae. The previous studies have revealed that RA had therapeutic effects on hepatocellular carcinoma (HCC) in the H22-xenograft models by inhibiting the inflammatory cytokines and $NF-{\kappa}B$ p65 pathway in the tumor microenvironment. However, its molecular mechanisms of immunoregulation and pro-apoptotic effect in HCC have not been fully explored. In the present study, RA at 75, 150, and 300 mg/kg was given to H22 tumor-bearing mice via gavage once a day for 10 days. The results showed that RA can effectively inhibit the tumor growth through regulating the ratio of $CD4^+/CD8^+$ and the secretion of interleukin (IL)-2 and interferon-${\gamma}$, inhibiting the expressions of IL-6, IL-10 and signal transducer and activator of transcription 3, thereby up-regulating Bax and Caspase-3 and down-regulating Bcl-2. The underlying mechanisms involved regulation of immune response and induction of HCC cell apoptosis. These results may provide a more comprehensive perspective to clarify the anti-tumor mechanism of RA in HCC.
