We herein report a very rare case of adult right-sided Bochdalek hernia accompanied with hepatic hypoplasia and inguinal hernia. A 29-year-old man was admitted with right-sided pneumothorax. A computed tomography was performed and revealed large right sided Bochdalek hernia with hepatic hypoplasia. Under thoracolaparotomy, the defect was closed with Gore-Tex soft tissue patch. After the operation, left-sided inguinal hernia was found. However, it turned out that it had been present during infancy and spontaneously resolved during adolescence. This is the first report of right-sided Bochdalek hernia with hepatic hypoplasia and inguinal hernia in an adult.

A one year old spayed female Bichon Frise dog presented with gait abnormalities and seizure. Serum biochemical results showed elevated levels of alkaline phosphatase, alanine aminotransferase, and ammonia. Serum bile acid level was also increased to be over 30 μmol/L on preprandial. Urinalysis identified the presence of ammonium urate crystal. Abdominal ultrasonography and CT revealed aberrant, tortuous, and multiple small vessels connected to the caudal vena cava between left kidney and caudal vena cava. Macroscopic specific findings associated with extrahepatic congenital portosystemic shunts (PSS) or other liver diseases were not identified. Liver biopsy was performed. Histopathologic evaluation revealed hepatic lobular hypoplasia with portal arterial duplication and vascular shunts. Based on these finding, this case was diagnosed as multiple acquired PSS secondary to hepatic microvascular dysplasia (HMD) and hepatic encephalopathy. A liver biopsy is recommended to differentiate HMD from other liver diseases and to confirm HMD when a young dog has multiple acquired PSS.

신생아 선천성 대사이상 선별검사에서 갈락토스혈증이 의심되는 경우, 혈관내피종과 같은 간 실질 내 종양을 동반할 수 있으므로 갈락토스혈증 관련 효소검사와 함께 간초음파 검사와 같은 영상학적 진단법, 총담즙산, AFP등의 측정을 병행하여야 원인감별에 도움이 된다. 저자들은 신생아 선천성 대사이상 질환 선별검사에서 갈락토스혈증이 의심되었던 생후 13일된 영아에 대하여 원인을 조사하던 중 복부초음파 검사에서 간 내 혈관내피종을 발견된 1례를 경험하였기에 이에 보고하는 바이다.

Intra- & extra-hepatic duct system의 경화를 야기하는 Biliary atresia는 출생시 1 : 10,000의 발생빈도를 보이며, 5년 이하의 생존율을 나타내는 예후가 좋지 않은 질환이다. 담도폐쇄증의 원인은 아직 명확하지 않으나, 유전적인 요소나 발육성 원인으로 믿고 있다. 임상적 특징으로 두드러진 황달, 간비증대, 가려움증, 지방변, 황색종, 성장지연, 문맥압항진증, 출혈, 골연하증, 복수 그리고 호흡기계 감염 등을 나타낸다. 구강내 소견으로 녹색을 띄는 변색치아, 유치의 확장된 치수강과 치근관, 치아맹출지연, 법랑질 형성부전, 그리고 심한 치아 우식 등이다. 담도폐쇄증은 조기진단과 수술에 의해 사망률이 감소되고, 성장과 발달이 정상으로 돌아오며 생존율도 향상되었으나 여전히 그 원인과 치료에 대한 연구가 계속되어야 할 것이다. 이에 저자는 전북대학교 치과병원에 내원한 2명의 담도폐쇄증환아에 대해 임상, 방사선학적 특징 및 적합한 처치에 대하여 보고하고자 한다.

담도폐쇄증은 간장외 담관의 부분적이거나 완전한 파괴 또는 부재로 인한 담즙흐름의 완전한 폐쇄로 정의된다. 이 질환은 대략 10,000명당 1명 정도 비율로 발생하며 여성에서 약간높은 빈도를 보인다. 원인인자는 명확히 밝혀지지 않았으며 증상은 과빌리루빈혈증과 함께 황달, 점토색변, 지방설사, 어두운 갈색뇨, 간비대 등을 보인다. 최근의 담도폐쇄증의 치료는 간문장문합술 단독 혹은 간이식술을 병행하는 경향이다. 본 증례의 환자에서는 다양한 정도의 치아변색을 보였으며 색상은 노란색에서부터 짙은 초록색까지 다양했다. 법랑질의 저형성증이 모든 치아에 나타났고 구강위생이 좋지 않았으며 다발성 우식증을 가지고 있었다.
