To develop effective training methods for strengthening a weakened quadriceps femoris muscle in hemiplegic patients, we examined the effects of maximal isometric contraction of the nonparalyzed knee joint on the electromyographic activities of the paralytic muscle. An electromyogram (EMG) was used to record the electromyographic activities of the paralytic quadriceps femoris muscle in 27 hemiplegic patients. The maximal isometric contraction was measured for each subject to normalize the electromyographic activities. The maximal isometric extension and flexion exercises were randomly conducted when the knee joint angles of the nonparalyzed knees were $0^{\circ}$, $45^{\circ}$, and $90^{\circ}$. The patients were encouraged to maintain maximal isometric contractions in both knee joints during each measurement, and three measurements were taken. A one-minute rest interval was given between each measurement to minimize the effects of muscle fatigue. An average from the three values was taken as being the root mean square of the EMG and was recorded as being the maximal isometric contraction. The electromyographic activity obtained for each measurement was expressed as a percentage of the reference voluntary contraction, which was determined using the values obtained during the maximal isometric contraction. The results of this study are summarized as follows: First, when the knee joint angle of the nonparalyzed knee was $0^{\circ}$, the electromyographic activities of the paralytic medial aspect of rectus femoris were related to measurement by a maximal isometric flexion exercise than by an extension exercise (p<.05). Second, when the knee joint angle of the nonparalyzed knee was $90^{\circ}$, the electromyographic activities of the paralytic lateral aspect of rectus femoris were related to measurement by a maximal isometric flexion exercise than by an extension exercise (p<.05). The results show that myoelectrical activities of paralytic quardriceps were not related to measurement angles and exercise directions of the nonparalized knee joint. Studies on various indirect intervention to improve muscular strength of patients with nervous system disorders of the weakened muscle should be constantly conducted.
In motor learning, the relative frequency of external feedback is the proportion of external feedback presentations divided by the total number of practice trials. In earlier studies, increasing the percentage of body weight loading on the affected leg of hemiplegic patients, external feedback was continuously produced as the patient attempted to perform a movement. This feedback was produced to enhance the learning effect. However, recent studies in nondisabled populations have suggested that compared with 100% relative frequency conditions, practice with lower relative frequencies is more effective. My study compared the effect of 100% relative frequency conditions with 67% relative frequency conditions to determine what effect they exerted on motor learning for increasing the percentage of body weight loading on the affected lower limbs of patients with hemiplegia. Twenty-four hemiplegic patients were randomly assigned to one of two experimental groups. Each group practiced weight transfer motor learning on a machine. During practice, visual feedback was offered to all subjects. The experiment was carried out with full visual feedback for patients in group one but only 67% visual feedback for patients in group two. The percentage of loading on the affected leg was recorded four times: before learning (baseline value), immediately after learning, 30 minutes after learning, 24 hours after learning. The results were as follows: 1. In the 100% visual feedback group, the percentage of loading on the affected leg increased significantly in all three testing modes over the baseline value. 2. In the 67% visual feedback group, the percentage of loading on the affected leg increased significantly in all three measurements. 3. Immediately after learning, the learning effect was not significantly different between the two groups, but was significantly greater after both the 30 minutes delay and the 24 hours period. These results suggest that the 33% reduction in the provision of visual feedback may enhance the learning effect of increasing the percentage of body weight loading on the affected leg in patients with hemiplegia.
Hemiplegic migraine (HM) is a rare subtype of migraine with aura and is accompanied by a fully reversible motor aura. HM can occur in two forms: familial or sporadic. Currently, three genes are related to familial HM. Typically, HM occurs in the first or second decade of life and involves gradually progressing aura symptoms in succession, accompanied by headaches. The aura includes visual, sensory, motor, aphasic and often basilar-type symptoms. Motor aura (weakness) is related to the regions where the sensory aura is involved, and it usually starts at the hand before spreading to the arm and face. Aphasia is a common form of speech aura, but does not typically present as a difficulty in understanding. In this case report, the sensory-motor aura started at the right face and then gradually progressed to the right leg without any symptoms in the ipsilateral upper extremity. To the best of my knowledge, there has been no previous case report for the presentation of a hemiplegic migraine, as in this case report. As there is a possibility of misdiagnosis of Bell's palsy at the early stage of this case, this case report suggests that a physician should consider the rare possibility of stroke or HM when a patient presents with unilateral facial palsy.
Purpose: To investigate the effects of initiation side on gait symmetry in the chronic stroke patients. Methods: Twenty one patients with independent gait after stroke were divided into the paretic-leg gait initiation group (PLI) and the nonparetic-leg gait initiation group (NPLI). The symmetry ratio (SR) was calculated from of the spatiotemoral and kinematic parameter which measured by 3D motion analysis. Results: In the spatiotemporal variables, SR-step length and SR-velocity was significantly different between groups (p<0.05). In the kinematic variables, SR-TOAA and SR-SwPAA of the hip joint was significantly different between groups (p<0.05). Conclusion: We suggest that the initiating leg may influence on the gait symmetry of stroke patient These results will be a helpful reference in hemiplegic gait training or intervention.
The purpose of this study was to identify the characteristics oi gait patterns and to find the effects of the gait function on the P.N.F. pelvic pattern. The subjects of this study were 40 hemiplegic patients who either hospitalized in or out patients at department physical therapy, Tae-Bong hospital and Masan college health education center. The study on these patient were done from July 20.1999 through May 10,2000. Method of gait pattern assessment used ink foot print record. The results were as follows: 1. There were significiant difference in cadence and stride length(p<0.05). 2. There were significiant difference in gait velocity (p<0.05).

응시위치 추적이란 현재 모니터 상에 사용자가 응시하고 있는 지점을 카메라로부터 얻은 영상정보로부터 파악해 내는 것을 말한다. 응시위치 추적을 이용하여 현재 사용자가 모니터 상에 바라보고 있는 응시위치점을 시각적으로 표시해 줌으로써 한쪽 면을 잘 인지하지 못하는 장애인들에 대한 진단 및 재활치료를 도울 수 있는 시스템을 개발하였다. 이와 같은 이와같은 시스템은 사용자가 관심있어할 만한 상황을 컴퓨터 시뮬레이션을 통해 가상적으로 만들고 그 안에 있는 물체들을 사용자가 고개움직임을 이용하여 움직일 수 있게 함으로써 사용자로 하여금 강한 동기력을 유발할 수 있으며 또한 편측 시각 무시에 대한 보다 정량적 인 진단을 가능하게 하며 기존의 작업치료사의 도움을 받아야 했던 것에서 탈피할 수 있으므로 인력 소모를 줄일 수 있는 등의 잇점이 있다.

It is important to train lower limb muscle strength using a tilting table to recover the lower extremity function of hemiplegia patients. It is known that the foot deformity and poor posture of hemiplegia patients can reduce the effectiveness of lower limb rehabilitation training. In this study, we developed a sensor system that can measure the foot pressure distribution of the patients for the load control of the lower extremity during lower limb rehabilitation training and it can be substituted for conventional high-cost technologies.
Objectives: The study investigated the effect of Korean medicine treatment on a hemiplegic patient with conscious and cognitive disorders due to hemorrhagic transformation after interventional reperfusion therapy of anterior circulation infarction. Case presentation: The patient was treated with acupuncture, moxibustion, cupping, and herbal medicine in combination with Western medicine and physical therapy. The effects on clinical symptoms were evaluated using the Manual Muscle Test (MMT), Glasgow Coma Scale (GCS), Korean Mini-Mental State Examination (K-MMSE), Korean Nursing Delirium Screening Scale (Korean Nu-DESC), and Modified Bathel Index (MBI). After the treatment, the MMT grade increased from Gr.0-1 to Gr.0-3, the GCS score increased from 10 to 15, the K-MMSE score increased from 8 to 15, the Korean Nu-DESC score decreased from 3 to 1, night delirium disappeared, and the MBI score increased from 13 to 26. Conclusions: Complex Korean medicine treatments were effective for improving the clinical symptoms of hemorrhagic transformation after interventional reperfusion therapy for anterior circulation infarction in a patient with hemiplegia and conscious and cognitive disorders. However, further studies are needed.

Objective: This study examined the effectiveness of Pyung-Hyung acupuncture and herbal medicine for a hemiplegic patient diagnosed with intracerebral hemorrhage. Methods: The patient was treated with Pyung-Hyung acupuncture and herbal medicine for one month. Intracerebral hemorrhage symptoms were evaluated using the Korean version of the Modified Barthel Index (K-MBI), the Manual Muscle Test (MMT), and brain CT images. Results: Following Pyung-Hyung acupuncture and herbal medicine, K-MBI, MMT, and brain CT image results improved. Conclusion: This case showed that Pyung-Hyung acupuncture and herbal medicine effectively treated intracerebral hemorrhage with hemiplegia.

Purpose:The purpose of this study was to investigate the effect of $\alpha$-wave music and art appreciation on hand function in stroke with hemiplegia. Methods:A total of 32 stroke with hemiplegia participated in this study experimental group(16 subjects) received $\alpha$-wave music and art appreciation with general neurologic therapy. Control group(16 subjects) received general neurologic therapy. All subjects were assessed for hand function(manual dexterity, power grip, pinch grip, two point discrimination(parm, finger), tactile sense(parm, finger) using a purdue pegboard, dynamometer, pinch gauge, two-point anethesiometer and semmes-weinstein monofilament wire. The data were analyzed using paired and independent t-test. Results:The results were as follows : 1. In the experimental group, manual dexterity were significantly increased between pre and post intervention(p<.05). 2. In the experimental group, tactile sesne in finger were sifnificantly increased between pre and post intervention(p<.05). Conclusion:The results of this study shows that $\alpha$-wave music and art appreciation affect the hand function of hemiplegic side with regard to manual dexterity and tactile sense.
