Change of Foot Rotation of Gait Analysis Parameters according to Gait Improvement in Post-Stroke Hemiplegic Patients (뇌졸중 이후 편마비 환자들의 보행 분석에 따른 보행 시 발 외전각도의 변화)

  • Objectives: This study investigated the change of foot rotation angle of gait analysis parameters according to gait improvement in post-stroke hemiplegic patients. Methods: We measured the foot rotation angle of eight post-stroke hemiplegic patients at the time of dependent and independent gait. Results: The foot rotation angle of the paralyzed side reduced closer to normal average according to gait improvement, but the non-paralyzed side not significantly. Conclusions: Improvement of foot rotation is an important thing for independent gait of post-stroke hemiplegic patients so this treatment seems worthy of being considered in clinical trials.

Detection of Steps or Gait Assessment of Hemiplegic Patient Based on Accelerometer (가속도계 기반의 편마비 환자 보행 평가를 위한 보 검출)

  • In this paper, an algorithm to detect steps in hemiplegic patients using a 3-axis accelerometer a紅ached on the trunk was proposed. The proposed algorithm consisted of the signal pre-processing, the step detector, the classification of steps and the calculation of stride time. Two FIR band-pass filters were designed and steps were measured by the combination of filtered signals in the vertical and the anteroposterior directions. In addition, the classification of steps and the calculation of stride time were computed by using the detected steps and lateral signals. For the experiment, fourteen hemiplegic patients were participated and the linear accelerations of the trunk and foot switch signals were measured synchronously. To evaluate the system performance, the detected steps and initial contacts by the foot switch were compared. The average error between the steps and initial contacts was 0.024ms and the difference of the average stride time was 0.01s. Finally, all gait events were detected exactly. Results showed that the accelerometry could use for the gait evaluation in clinical rehabilitation therapies.

  • Two hemiplegic cerebral palsy patients were studied to investigate the cortical mechanisms underlying preserved somatosensory capacity, using functional MRI(fMRI). Tactile stimulation was performed by brushing of palm, during fMRI study. By the affected hand stimulation, contralateral primary somatosensory cortex was activated in patient 1 and cortical area anterior to the lesion site was activated in patient 2. We suggest that reorganization of the somatosensory cortex after brain injury can be induced by recruitment of undamaged areas adjacent to lesion site.

Effectiveness of Mental Practice on EMG Activity of the Quadriceps during Sit-to-Stand and Stand-to-Sit Tasks in a Hemiplegic Patient (상상연습이 일어서기와 앉기 과제를 수행하는 동안 편마비 환자의 대퇴사두근 활성도에 미치는 영향)

  • Background : We aimed to determine the effect of a mental practice on the electromyography (EMG) activity of the quadriceps femoris muscle of the affected side in a hemiplegic patient during sit-to-stand and stand-to-sit tasks. Methods : We recruited a male patient who had suffered from right hemiplegia. A single-subject experimental A-B-A design was used. The reliability of visual analysis, which was primarily employed in this study design, was supported by using the "2- standard-deviation band" method. The target motions consisted of two activities: sit-to-stand and stand-to-sit. The EMG measurements of the quadriceps femoris muscles of the affected and unaffected sides were made in 8 sessions in the baseline (A), mental-practice (B), and follow-up (A) phases. During the mental practice phase, the 10-min mental practice was performed, and the mental practice was withdrawn in the follow-up phase in order to identify the carry-over effects of the treatment. Data was calculated as the percentage of the ratio of the quadriceps EMG activity values of the affected side on unaffected side. Result : The percentage EMG-activity ratio showed a greater increase in the mental-practice phase than in the baseline, and it was maintained during the follow-up phase. Most of the data points in the mental-practice and follow-up phases were positioned above the "2-standard-deviation band." Conclusion : These findings suggest that mental practice can be used to improve the physical function of hemiplegic patients.

Design of Calf Link Force Sensor of Walking Assist Robot of Leg Patients (편마비 다리환자를 위한 보행보조로봇의 발목 2축 힘센서 설계)

  • This paper describes the design and manufacture of a ankle two-axis force sensor of a walking assist robot for hemiplegic leg patient. The walking assist robot for the hemiplegic leg patient can safely control the robot by detecting whether the foot wearing the walking assist robot is in contact with the obstacle or not. To do so, a two-axis force sensor should be attached to the robot's ankle. The sensor is used to measure the force of a patient's ankle lower part. The two-axis force sensor is composed of a Fx force sensor, a Fy force sensor and a pulley, and they detect the x and y direction forces, respectively. The two-axis force sensor was designed using by FEM(Finite Element Method), and manufactured using by strain-gages. The characteristics experiment of the two-axis force sensor was carried out respectively. The test results indicated that the interference error of the two-axis force sensor was less than 1.2%, the repeatability error and the non-linearity of the two-axis force sensor was less than 0.04% respectively. Therefore, the fabricated two-axis force sensor can be used to measure the force of ankle lower part in the walking assist robot.

  • Purpose : The purpose of this study was to evaluate the effect of core stability training at deep abdominal muscle for balance control of hemiplegic patient. Method : The subject of this study was a 47-year-old man with right hemiplegia. He was treated five times a week for three weeks with core stability training at deep abdominal muscles. Evaluation tool was used Functional reach test(FRT), timed up and go test(TUG) and one leg standing for stroke patients. Result : The FRT distance increase, TUG time decrease, one leg standing time increase core stability training at deep abdominal muscles for right hemiplegia improved was the ability for maintain balance. Posture and control of trunk stability are changing posture, and so which showed significant improve of total balance control. Conclusion : The result of this study showed that core stability training at deep abdominal muscles is an effective treatment for balance control. Therefore, it could be considered as a treatment method in the rehabilitation of stroke patient with poor postural control and imbalance, although further studies are needed.

Kinematic Analysis of Lower Extremities during Staris and Ramp Walking with Hemiplegic Patients (편마비 환자의 계단과 경사로 보행 동안 하지의 운동학적 분석)

  • Purpose: This study was conducted in order to investigate the kinematic gait parameter of lower extremities with different gait conditions (level walking, stair, ramp) in hemiplegic patients. Methods: Ten hemiplegic patients participated in this study and kinematic data were measured using a 3D motion analysis system (LUKOtronic AS202, Lutz-kovacs-Electronics, Innsbruk, Austria). Statistical analysis was performed using one-way repeated measure of ANOVA in order to determine the difference of lower extremity angle at each gait phase with different gait conditions. Results: Affected degree of ankle joint in the heel strike phase showed significant difference between level walking and climbing stairs, and toe off phase showed significant difference between level walking and climbing stairs, ramps, and climbing stairs. Affected degree of knee joint showed no significant difference in all attempts. Affected degree of hip joint in the toe off phase showed significant difference between level walking, ramps and stairs, and climbing ramps. Swing phase showed significant difference between sides for level walking and stairs, climbing ramps. Affected ankle joint of heel strike and toe off, and affected hip joint of toe off and the maximum angle of swing phase in the angle was increased. Unaffected side of the ankle joint, knee joint, and hip joint showed a significant increase in walking phase. Conclusion: These findings indicate that compared with level walking, different results were obtained for joint angle of lower extremity when climbing stairs and ramps. In hemiplegia patient's climbing ramps, stairs, more movement was observed not only for the non-affected side but also the ankle joint of the affected side and hip joint. According to these findings of hemiplegic patients when climbing stairs or ramps, more joint motion was observed not only on the unaffected side but also on the affected side compared with flat walking.

A Study of the effects of Health Contract on the performance Level for Activity of Daily Living in the Hemiplegic patients (건강계약이 편마비 환자의 일상생활동작 수행정도에 미치는 영향)

  • The purpose of this study was to test the effects of health contract on the performance level for activity of daily living(ADL) in the hemiplegic patients. A quasi - experimental research for health contract, which was approved as an effective method of nursing intervention, was attempted to increase the performance of the hemiplegic patients' ADL. As a purpose sample, 69 hemiplegic patients hospitalized at Oriental medicine hospital of K university were taken and devided into the experimental group and the control group by means of random assignment. After Contracting with the experimental group, they were reinforced everyday for 20 minutes. The experimental group and the control group were observed and interviewed for five times at the in terval of 3~4 days by the trained nurses for this research. The data Collected through above mentioned methods were compute analyzed by t-test and ANOVA according to the purpose of this study. The data collected through above mentioned methods were computer analyzed by t-test and ANOVA according to the purposre of this study. strate increased the performance level for ADL than the Control group” was supported(t=2.96, df=52.76, p= .004). Sub Hypothesis 1,2,4,5 : “The hypothesis that the experimental group with health contract will demonstrate increased the performance level for eating(t=2.29, df=42.70, p= .027), personal hygiene and grooming (t=4.04, df=43.10, p= .000), dressing(t=3.32, df=67, p= .001) and undressing (t=3.47, df=48.44, p= .001) than the control group” was supported. Sub Hypothesis 3.6 : “The hypothesis that the experimental group with health contract will demonstrate increased the performance level for toiletting(t= .19, df=67, p= .849) and mobilization (t= .30, df=67, p= .765) than the control group” was not supported. The conclusion can be that the positive relationship between the nurse and the patient results in the desired performance level for ADL in the hemiplegic patients.

A study on the effects of weight-transfer training upon the gait patterns of hemiplegic patients through visual and auditory feedback (시각 및 청각 되먹임을 통한 하지 체중이동훈련이 편마비환자 보행특성에 미치는 효과에 관한 연구)

  • The objective of this study was to identify the effects of weight-transfer training on the weight bearing distribution and gait patterns of hemiplegic patients through visual and auditory feedback using a limb load monitor. The subjects of this study were 18 hemiplegic patients who had been hospitalized or were visited out-patient department of the Rehabilitation Hospital, College of Medicine, Yonsei University, from January 5, 1995 through March 15, 1995. Pre-and post-training changes in gait patterns were measured using ink foot prints as well as by recording weight bearing distribution using a limb load monitor. The data were analyzed by the repeated measure one-way ANOVA and paired t-test. The finding were as follows: 1. Prior to the training, 18 subjects bore more weight on the sound leg(61.6 %) than on the affected leg(38.4 %). 2. Posterior to the training, the average percentage of weight bearing on the affected leg increased significantly from 38.4 % prior to training; to 46.0 % immediately after training; 45.7 % after a 30 second delay; and 45.3 % after a 60 second 3. The difference in gait patterns between pre- and post- training was statistically significant, with an increase in gait velocity to 3.65 cm/sec post-training; an increase in stride length to 5.37 cm on the affected side; 4.77 cm on the sound side; and a narrowing of the base of support to 1.19 cm. In conclusion, hemiplegic weight-transfer training using visual and auditory feedback with a limb load monitor was found to be enhancing symmetrical standing posture, and simultaneously improve gait patterns.

The Effects The Type of Canes-Handle Affects in Recovering-Balance of Hemiplegic patients (지팡이 손잡이 형태가 편마비 환자의 균형 회복에 미치는 영향)

  • Purpose: We compared T-type and I-type canes on postural balance in 28 hemiplegic patients. Methods: Subjects were allocated randomly into two groups: a T-shape cane group (n=14) and an I-shape cane group (n=14). Before the test, subjects were trained by a physical therapist to walk with a cane for 6 weeks. The Main Outcome Measures were measured as maximal sway velocity, sway path, sway area, and partial weight bearing using a Balance Performance Monitor (BPM) and ambulation velocity using a 'Timed up and go test'. We also measured the maximal ambulation velocity. Results: The distribution of weight bearing on the affected side without the cane was 35% in the I-shape cane group and 36% in the T-shape cane group. After training, weight bearing on the affected side increased by 45% in the I-shape cane group and 40% in the T-shape cane group. With the cane held in the hand, weight bearing on the affected side in the T-shape cane group decreased by 3%. Conclusion: The I-shaped cane increased static standing balance, including hemiplegic side weight bearing. Therefore, I-shape canes can improve the balance of hemiplegic patients.
