Two adolescent patients with coexistent Graves' disease and Moyamoya disease in Korea

  • Moyamoya disease is a cerebrovascular condition that results in the narrowing of the vessels of the circle of Willis and collateral vessel formation at the base of the brain. Although relationships between Graves' disease and cerebrovascular accidents in Moyamoya disease are obscure, the coexistence of the two diseases is noteworthy. Moyamoya disease has been rarely reported in adolescent patients with thyrotoxicosis. Recently, we encountered two adolescent Korean patients with Moyamoya disease associated with Graves' disease who presented with episodic right-sided hemiparesis and syncope. These two girls who had Graves' disease had no history of other diseases or head trauma. A thyroid function test revealed a euthyroid state and a high thyroid-stimulating hormone (TSH) receptor antibody titer at that time. The patients were diagnosed with Moyamoya disease based on brain magnetic resonance angiography and cerebral four-vessel angiography. The patients underwent cranial revascularization by encephalo-duroarterio-synangiosis as soon as a diagnosis was made, which resulted in successful symptom resolution. They fared well and had no additional neurological symptoms as of their last follow-up visits. Here, we report these two cases of confirmed Moyamoya disease complicated by Graves' disease with a review of the literature, and discuss the possible association between the two diseases. To our knowledge, this is the first report in South Korea on Moyamoya disease associated with Graves' disease in adolescents with a euthyroid.

The Approach of Robot-assisted Gait Therapy for Locomotor Recovery of Chronic Stroke Patients: a Case Report

  • In this case report, we investigated the effects of robot-assisted gait therapy in a chronic stroke patient using motor assessment and gait analysis. A patient who suffered from the right hemiparesis following the left corona radiata and basal ganglia infarction received 30 minutes of robot-assisted gait therapy, 3 times a week for 4 weeks. Outcome was measured using Motoricity index(MI), Fugl-Meyer assessment(FMA), modified motor assessment scale(MMAS), isometric torque, body tissue composition, 10-meter gait speed and gait analysis. After robot-assisted gait therapy, the patient showed improvement in motor functions measured by MI, FMA, MMAS, isometric torque, skeletal muscle mass, 10-meter gait speed. In gait analysis, cadence, single support time, double support time, step length, walking speed improvement in after robot-assisted gait therapy. The results of this study showed that robot-assisted gait therapy is considered to facilitate locomotor recovery of the chronic hemiparetic stroke patient.

  • To evaluate the relations between strokes and psychological characteristics, the authors surveyed Minnesota Multiphasic Personal Inventory(MMPI) in CVA Patients. The results are as follows.1. In the cerebral infarction patients group, the measure of F, Hy, Pd, Pt and Sc has appeared significantly high. 2. In the cerebral hemorrhage patients group, the measure of F and Ma has appeared significantly high. 3. In dysphasia-hemiparesis patients group, the measure of D has appeared high.4. In the Soyangin patients group, the measure of Pd and Ma has appeared high. 5. In the Soeumin patients group, the measure of D, Pt and Si has appeared high.

강직성 편마비 환자에서의 운동장애는 강직 때문인가? 근육약화 때문인가? (Movement Dysfunction in Spastic Hemiparesis: A Problem of Spasticity or Muscular Weakness?)

  • In most of the medical literature that discusses the common problem of movement in patients with cerebral lesions. This critical problem is ascribed to a mechanism involving uninhibited neural activity. The goals of neurological physical therapy are focus on reduce of muscle hypertonicity, facilitates muscle activities, and improve of performance in living environment. A variety of studies suggest that spasticity is a distinct problem and separate from the muscle weakness. It has become increasingly recognized that the major functional deficits following brain damage are largely due to negative features such as muscle weakness and loss of performance rather than spasticity. Adequate recruitment of prime mover, not release was able to carry out the movement tasks well. The strengthening exercise of spastic limbs on changes in muscle properties and performance skill, the repeated motor practice has been identified as crucial for motor recovery. This article support the concept that strengthening is an appropriate intervention to improve the quality of physical function in patients with central nervous system lesions. Further studies and therapeutic approaches should be efforts at improving motor neuron recruitment in agonist rather than reducing activity in antagonists while retraining muscle strengthening.

신경가동기법, 정적신장기법, 유지-이완기법이 뇌졸중 환자의 슬괵근 유연성에 미치는 효과 비교 (Comparison of Nerve Mobilization, Static Stretching, and Hold-Relax on Hamstring Flexibility in Stroke Patients)

  • The purpose of this study was to examine the acute effects of nerve mobilization, static stretching, and hold-relax on the flexibility of hamstring muscles and their surface electromyographic (EMG) responses to passive stretches in poststroke hemiparesis. This study was a randomized cross-over trial. Fourteen subjects received three treatment sessions in random order with each consisting of ten repetitions. The treatment sessions included nerve mobilization, static stretching, and hold-relax. The treatment sessions were held at least 24 hours apart to minimize any carryover effect. The outcome was measured by the distance between the greater trochanter and lateral malleolus and hamstring EMG activity during passive knee extension stretching. Repeated-measures analysis of variance showed significant changes in hamstring flexibility and EMG activity in main effect of time pre, post and followup (p<.05). However, no significant differences occurred among the three stretching techniques. No technique was consistently found to be superior. The three stretching techniques in this study make it difficult to determine the most effective technique. Therefore, clinicians use nerve mobilization of effective stretching techniques with other stretching techniques.

수중물리치료를 통한 뇌졸중환자의 치료적 중재효과 (The Effect of Therapeutic Intervention by Aquatic Physical Therapy on Stroke Patient)

  • Purpose : The purpose of this study is to compare the rehabilitation effects of hydrotherapy and Bobath therapy. Methods : The study was performed with patients of hemiparesis caused by cerebral stroke. The participants were divided into two groups based on random sampling method. One group received hydrotherapy while the other received Bobath therapy. Each rehabilitation program lasted 40 minutes a day, 5 days a week, for 6 weeks. Pertinent indicators-Berg's balance scale (BBS), gait parameter, and static balance analysis-were recorded before andafter the programs, as well as every 2 weeks during the rehabilitation programs Results : There was showed a significantly increase of BBS score. Static open and close showed statistically significant in interaction by time and groups. There was significant differences of gait velocity. Conclusion : These findings in this study that the hydrotherapy was effective therapy in improving balance and gait velocity.

모야모야병 환아(患兒) 1예(例)에 대한 증례(症例) (A Case of Moyamoya Disease)

  • Moyamoya is a chronic cerebrovascular disease chracterized by progressive stenosis or occlusion of the terminal parts of both internal carotid arteries with telangiectatic vascular network of collateral circuration at the base of the brain and leptomeningeal arteries. The etiology and pathophysiology of this disease are still unknown. The clinical course in those whose first symptoms occur in childhood is different from those in whom symptoms develop in adult life. The term moyamoya disease should be resserved for those cases in which the chracteristic angiographic pattern is idiopathic; moayamoya syndrome is used when the underlying condition is known. we have experienced a case of moyamoya syndrome in a 5-year-2-month-old boy who presented right-sided hemiparesis. A cerebral angiogram revealed occlusion of abnomal collateral network. Moyamoya disease is applicable to stroke of an infant from oriental medicine point of view, and The symptoms is similar to adult stroke, we have treated adult stroke patint with herb medicine and acupuncutre and physical treatment. The acute stage of stroke is applied to the external treatment(標治), and The recovery stage is applied to the basic treament(本治).

중심성 뇌교 수초용해증 환자에 관한 한양방 복합치료 1례 (A Case Report of a Central Pontine Myelinolysis Patient Treated with Korean and Western Medicine)

  • Objectives : The purpose of this study is to report the clinical effect of Korean and Western medical treatment on a hemiplegic inpatient with central pontine myelinolysis(CPM). Methods : A patient who was diagnosed with central pontine myelinolysis(CPM) was treated with acupuncture, herbal medicine, Korean physical therapy, along with receiving continuous Western medication and rehabilitation treatment. The patient was evaluated through Manual Muscle Testing(MMT), measurement of grip strength, dysarthria, and dysuresia. The Korean and Western medical treatment was conducted for 4 weeks. Results : After the 4 weeks of Korean and Western medical treatment, MMT grade improved from 2 to 4 and grip strength improved from 6 to 13. Dysarthria and dysuresia also improved. Conclusion : According to these results, this report suggests that Korean and Western medical treatment could be effective in the treatment of central pontine myelinolysis(CPM) patients.

종양내 출혈을 동반한 뇌수막종 - 증례보고 - (Meningioma with Intratumoral Hemorrhage - A Case Report -)

  • Meningioma associated with intratumoral hemorrhage is rarely reported. We present two patients with intratumoral hemorrhage. One 70-year-old man was admitted to our department with a decreased level of consciousness and left hemiparesis. CT scan and MRI scan revealed huge tumor with intratumoral hemorrhage in the frontal lobe. After surgical removal of the tumor, histopathological diagnosis was a meningotheliomatous meningioma. The other seemed patients was 56-year-old woman with headache and vomiting. She showed no specific neurological deficit. CT and MRI scan revealed large size tumor with intratumoral hemorrhage that looks like meningioma. The patient died suddenly before surgery. We reviewed the relevant literature and discussed the possible mechanism of hemorrhage in the meningioma.
