The Hematologic Study on Cerebral Infarction (뇌경색환자의 혈액학적 소견 연구)

  • 1. Purpose Stroke is the most frequent cause of mortality and morbidity rate in Korea with coronary heart disease and cancer. The ischemic type of stroke is increasing tendency. The purpose of this studies is to examine the hematologic difference between cb infarction patients and Korean normal adults. 2. Method The study group consisted of neurologically confirmd 50 cb-infarction patients as the case group and 278 patients as control group that they had no significant result by CT through early examination of stroke in Cheonan oriental hospital of Daejeon university. We tested and compared RBC, hemoglobin, hematocrit, WBC, PT, PTT, total protein, albumin, total bilirubin, direct bilirubin, GOT GPT, $\gamma$-GTP, total cholesterol, triglyceride, HDL-C, glucose, BUN, creatinine, Na, K in the both group. 3. Result In the sample group(cb infarction group), there were many patients with low RBC count but no significant and with low hemoglobin, hematocrit(p<0.05). Thereas, increased WBC count and delayed PTT were showed respectively 20%, 36% in patient group, 7.9%, 4.7% in normal group(p<0.05). Comparing the LFT between cb infarction patients and normal adults, we founded significant cases with low HDL-C level, high glucose level, low potassium level in patient group(p<0.05). But the percentage of high total cholesterol level and triglyceride level were 18%, 29%(norma1 group), 20%, 28%(cb infarction group) respectively, there were no significant difference. 4. Conclusion In this study, we demonstrated that low HDL-C level and high glucose level is one of important risk factor of cb infarction. Also prospective studies are needed to evaluate many risk factor(hemoglobin, hematocrit, WBC, cholesterol, triglyceride, K, etc) of cb infarction.

Clinical Observation and Prognostic Analysis of Pemetrexed plus Platinum as First-line Treatment in Patients with Advanced Non-small Cell Lung Cancer

  • Objective: To determine clinical efficacy, safety and prognostic factors of pemetrexed plus platinum as first-line treatment in patients with advanced non-small cell lung cancer (NSCLC). Materials and Methods: Clinical characteristics, short-term efficacy, survival and adverse reactions of 47 advanced non-squamous NSCLC patients who had received pemetrexed plus platinum as first-line treatment in Shanghai Pulmonary Hospital from January 2009 to June 2011 were retrospectively analyzed. The Chi-squared test was applied to statistically analyze the overall response rate (ORR), disease control rate (DCR) and toxicity reactions in both groups, while survival data wereanalyzed by Kaplan-Meier and logrank methods, and the COX proportional hazards model was adopted for a series of multi-factor analyses. Results: Only two patients were lost to follow-up. The ORR, DCR, medium progression-free survival time (PFS) and medium overall survival (OS) were 31.9%, 74.5%, 5 months and 15.2 months, while 1- and 2-year survival rates were 63.8% (30/47) and 19.2% (9/47), respectively. Single-factor analysis showed that tumor pathological patterns and efficacy were in association with medium PFS (P<0.05), whereas tumor pathological patterns, smoking history and efficacy were closely connected with medium OS (P<0.05). Multi-factor analyses demonstrated that pathological patterns and efficacy were independent factors influencing OS (P<0.05). The rate of toxicity reactions in degree III/IV was low, including hematologic toxicity marked by decline in white blood cell count and decrease in the platelet count (PLT), and non-hematologic toxicity manifested by gastrointestinal reactions, such as nausea and vomiting. Conclusions: Pemetrexed plus platinum as first-line treatment has excellent efficacy and slight adverse reactions with favorable drug-tolerance in patients with advanced non-squamous NSCLC.

Analysis of Risk and Benefit of Open Lung Biopsy in Severe Immunocompromised Patients with Pulmonary Complications (폐합병증을 동반한 심한 면역저하 환자에서 폐생검술의 유효성 및 위험성에 대한 분석)

  • Background: Pulmonary complications in immunocompromised patients are often fatal. Empirical treatment is usually applied based on the clinical and radiological findings because of the risk of the aggressive diagnostic procedures such as open lung biopsy. However, recent advancements in operative procedures and perioperative management has decreased the procedure-related risks. We have prospectively analyzed the risks and benefits of the early application of open lung biopsy in such patients. Material and Method: Forty-two consecutive immunocompromised patients with critical pulmonary complications were included from June, 1996 to December, 1999. The definition of the immunocompromised is as those with chemotherapy and/or other modality for hematologic disorders, with usage of immunosuppressive drug after transplantation, with usage of steroid for more than 1 month, and with primary immunodeficiency disorders. The indication of open lung biopsy was those with no significant improvement after a week of aggressive application of empirical treatment or with rapidly aggressive process. The underlying disease included hematologic disorder(31 patients), post-transplantation(3 patients), chemotherapy for solid tumor(2 patients), and others(6 patients). Operations were done through thoracotomy(conventional or mini-) or VATS.

Micronucleus Expression and Acute Leukemia Prognosis

  • The micronucleus frequency (MNF) in peripheral blood lymphocytes (PBL) is a biomarker of chromosomal damage and genome instability in human populations.The relationship of micronucleus frequency with prognosis of patients with acute leukemia is not clear. We therefore investigated MNF in mitogen-activated peripheral blood lymphocytes from patients with hematologic diseases and solid tumours. Patients included 50 with acute leukemia, 49 diagnosed with myelodysplastic syndrome (MDS), 54 with benign blood diseases, and 45 with solid tumours, examined with 50 healthy controls. The mean MNF was significantly higher in cases of hematologic diseases and solid tumor patients than in healthy controls (P<0.001). There was no evident difference between MNF in the acute leukemia ($7.15{\pm}2.18$) and solid tumor groups ($7.11{\pm}1.47$), but both were higher than in the MDS group ($5.12{\pm}1.29$) and benign blood diseases group ($3.08{\pm}1.08$). Taking 7.15‰, the average MNF of the acute leukemia group as standard, and dividing 50 cases of acute leukemia patients into high MNF group ($MNF{\geq}7.15$‰) and low MNF group (MNF<7.15‰). The overall response (complete remission + partial remission) rates of the low MNF group were significantly higher than in the high MNF group (P=0.001). The high MNF group further showed lower overall survival rates than the low MNF group. MNF expression and progression-free survival seemed to have a opposite relationship, with a correlation coefficient of -0.702. These data indicate that MNF in peripheral blood lymphocytes is important for evaluation of prognosis of acute leukemia patients, and it can reflect progression of disease to a certain degree.

The Hematologic Study as risk factors on premature examination of stroke (PES) (중풍위험인자로서의 혈액학적 소견 연구 - 중풍조기검진 326명에 대한 분석 -)

Recent recurrence rate of stroke is high, but little attention had been given to the features of its' cause. The purpose of this studies is to examine the hematologic states in Korean normal adults through premature-examination of stroke(PES). 2.Method The study group consisted of 326 cases that they had no significant results by CT through premature examination of stroke(PES) in Cheonan oriental hospital of Daejeon university. We tested RBC, hemoglobin, hematocrit, WBC, PT, PTT, PLT, total protein, albumin, total bilirubin, direct bilirubin, GOT, GPT, $\gamma$-GTP, total cholesterol, triglyceride, HDL-C, glucose, BUN, creatinine, Na, K and evaluated by cross sectional study. 3.Result In the PES, there were many patients with low RBC(11%), shorten PT(26.7%), delayed PTT(19.3%), high TP, GPT, ${\gamma}$ -GTP(respective1y 16.9%, 14.4%, 13.5%), high cholesterol level and triglyceride level(respectively 18.7%, 28.2%). We didn't found significant cases in hemoglobin, hematocrit, WBC, PLT, albumin, total bilirubin, direct bilirubin, GOT, HDL-C, glucose, BUN, creatinine, Na, K. 4.Conclusion In this study, we demonstrated that short PT, high triglyceride level are special views in PES. Also prospective studies are needed continuously to search the preventing methods of the CVA.

Leukemia Stem Cells in Blood Cells; Focused on Acute Myeloid Leukemia

  • It is known that acute myeloid leukemia (AML) is a heterogeneous blood cancer, which is enormously propagated by self-renewing leukemia stem cells (LSCs). The persistence of LSCs after chemotherapy can contribute to minimal residual disease and relapse by LSCs can be evoked promptly. Elucidating special molecules and cellular activity of LSCs is an extremely important to eliminate AML. Despite an increasing understanding of the origin of LSCs by incessant study, AML still remains a notorious disease with high mortality. An exact identification of the LSCs that sustain the proliferation of neoplastic clone is a fundamental issue in AML treatment. CD34+CD38- conventional phenotype is overall regarded as LSCs, but it has a limitation that is still hard to demarcate exactly due to similarity with normal hematopoietic stem cells (HSCs). Not all primary blasts and progenitors have equal function, thus a bona fide marker for identifying LSCs from HSCs is needed in hematologic malignancy, especially in AML. These findings have direct important implications in both in mechanistic study of LSCs as well as in the strategies of more effective therapies. In this review, I briefly summarized current advances in LSCs biology, focusing on membrane markers and a functional behavior of LSCs in AML treatment with monoclonal antibodies. Ultimately, it may be helpful in overviewing the status of LSC research, while expecting the clinic benefits of target therapy by specific inhibition.

  • Thoracic extramedullary hematopoiesis (EMH) is a rare disease entity that is usually associated with hematologic disorders, such as myelodysplastic or hemolytic disease. Because thoracic EMH is usually encountered as a mass during radiologic examinations, it should be differentiated from posterior mediastinal neurogenic tumors. Here, the authors report a case of EMH associated with hereditary spherocytosis. The patient underwent a complete excision by thoracoscopic surgery to differentiate it from other mediastinal tumors.

Assessing Hematological Change Associated with Cardiovascular Disease Risk among Korean Taxi Drivers Using Data from the Second (2012-2014) Korean National Environmental Health Survey: A Propensity Score Matching Approach (제2기(2012-2014) 국민환경보건 기초조사 자료를 활용한 국내 남성 택시 기사의 심혈관계 위험도 관련 혈액학적 변화에 대한 연구: 성향점수 매칭을 활용하여)

  • Objectives: Taxi drivers are exposed to various hazards, such as long periods of sedentary work and traffic-related air pollutants. However, studies on the health effects among taxi drivers in South Korea are insufficient. Methods: To assess subclinical hematologic change related to cardiovascular disease among male taxi drivers, we analyzed data from the second Korean National Environmental Health Survey. Fifty-nine taxi drivers and 1,912 controls were included in the analysis. Propensity score matching was performed to adjust for age, body mass index, and urinary cotinine. A total of 295 subjects were matched with 59 taxi drivers. Leukocyte count, platelet count, hematocrit, triglyceride, total cholesterol, HDL cholesterol land total IgE of the taxi drivers were compared with the control groups. Results: Taxi drivers showed significantly elevated blood leukocytes and platelets. Serum total IgE was significantly reduced in taxi drivers. However, blood leukocytes, platelets, and serum total IgE were not significantly correlated with work period among taxi drivers. Conclusions: Regarding the change of the blood leukocyte count, platelet count, and serum total IgE, taxi driving has the possibility to be associated with peripheral inflammation, humoral immunity and cardiovascular risk.

  • In the past few decades, biological drugs and small molecule inhibitors targeting inflammatory cytokines, immune cells, and intracellular kinases have become the standard-of-care to treat autoimmune diseases. Inhibition of TNF, IL-6, IL-17, and IL-23 has revolutionized the treatment of autoimmune diseases, such as rheumatoid arthritis, ankylosing spondylitis, and psoriasis. B cell depletion therapy using anti-CD20 mAbs has shown promising results in patients with neuroinflammatory diseases, and inhibition of B cell survival factors is approved for treatment of systemic lupus erythematosus. Targeting co-stimulatory molecules expressed on Ag-presenting cells and T cells is also expected to have therapeutic potential in autoimmune diseases by modulating T cell function. Recently, small molecule kinase inhibitors targeting the JAK family, which is responsible for signal transduction from multiple receptors, have garnered great interest in the field of autoimmune and hematologic diseases. However, there are still unmet medical needs in terms of therapeutic efficacy and safety profiles. Emerging therapies aim to induce immune tolerance without compromising immune function, using advanced molecular engineering techniques.

Review of pediatric cerebrovascular accident in terms of insurance medicine (소아뇌졸중의 보험의학적 고찰)

  • Moyamoya disease (MMD) is a progressive occlusive disease of the cerebral vasculature with particular involvement of the circle of Willis and the arteries that feed it. MMD is one of cerebrovacular accident,which is treated with sugical maeuver in pediatic neurosurgery. Moyamoya (ie, Japanese for "puff of smoke") characterizes the appearance on angiography of abnormal vascular collateral networks that develop adjacent to the stenotic vessels. The steno-occlusive areas are usually bilateral, but unilateral involvement does not exclude the diagnosis. The exact etiology of moyamoya disease is unknown. Some genetic predisposition is apparent because it is familial 10% of the time. The disease may be hereditary and multifactorial. It may occur by itself in a previously healthy individual. However, many disease states have been reported in association with moyamoya disease, including the following: 1) Immunological - Graves disease/thyrotoxicosis 2) Infections - Leptospirosis and tuberculosis 3) Hematologic disorders - Aplastic anemia, Fanconi anemia, sickle cell anemia, and lupus 4) Congenital syndromes - Apert syndrome, Down syndrome, Marfan syndrome, tuberous sclerosis, Turner syndrome, von Recklinghausen disease, and Hirschsprung disease 5) Vascular diseases - Atherosclerotic disease, coarctation of the aorta and fibromuscular dysplasia, 6)cranial trauma, radiation injury, parasellar tumors, and hypertension etc. These associations may not necessarily be causative but do warrant consideration due to impact on treatment.(Mainly neurosurgical operation.) The incidence of moyamoya disease is highest in Japan. The prevalence of MMD is 1 person per 100,000 population. The prevalence and incidence of moyamoya disease in Japan has been reported to be 3.16 cases and 0.35 case per 100,000 people, respectively. With regard to sex, the female-to-male ratio is 1.4:1. A bimodal peak of incidence is noted, with symptoms occurring either in the first decade(5-10yr) or in the third and fourth decades (30-40yr)of life. Mortality rates of moyamoya disease are approximately 10% in adults and 4.3% in children. Death is usually from hemorrhage. In aspect of life insurance, MR is 1700%, EDR is 16 per 1000 persons. Children and adults with moyamoya disease (MMD) may have different clinical presentations. The symptoms and clinical course vary widely from asymptomatic to transient events to severe neurologic deficits. Adults experience hemorrhage more commonly; cerebral ischemic events are more common in children. Children may have hemiparesis, monoparesis, sensory impairment, involuntary movements, headaches, dizziness, or seizures. Mental retardation or persistent neurologic deficits may be present. Adults may have symptoms and signs similar to those in children, but intraventricular, subarachnoid, or intracerebral hemorrhage of sudden onset is more common in adults. Recently increasing diagnosis of MMD with MRI, followed by surgical operation is noted. MMD needs to be considered as the "CI" state now in life insurance fields.
