범발성 해면상 S-결장 및 직장 혈관종 -1 예 보고- (Diffuse Cavernous Hemangioma of the Rectosigmoid Colon - a Case Report -)

  • Diffuse cavernous hemangioma of the rectosigmoid is a rare lesion usually presented in children and young adult, often with life threatening hemorrhage. The clinical diagnosis is difficult and often delayed because of lack of awareness of the clinical features. We report a case of diffuse cavernous hemangioma of the rectosigmoid in a 12 year old boy who had undergone suture ligation under the impression of hemorrhoid at his age 3. Sphincter saving coloanal pull through procedure were applied as the surgical treatment. Sclerotherapy was needed with recurrent rectal bleeding 6 months after the pull through operation. The patient is well at 12 months follow-up.

Multiple Cavernous Hemangiomas of the Posterior Mediastinum, Lung, and Liver: A Case Report

  • A 71-year-old male patient visited Yeungnam University Hospital with abnormal chest computed tomography (CT) findings. Chest CT revealed multiple lung nodules and a posterior mediastinal tumor, the diagnosis of which was confirmed surgically. Magnetic resonance imaging (MRI) of the abdomen showed multiple small nodules, which were diagnosed as cavernous hemangioma in the liver based on the pathology results of the mediastinal and lung masses in combination with MRI findings. Cavernous hemangiomas are benign tumors that can occur throughout the body, mainly in the skin and subcutaneous tissue. The liver is the most common internal organ containing hemangiomas, whereas they are very rarely found in the lungs or mediastinum.

  • 해면상 혈관종은 종격동에 발생한 혈관종중 가장 많은 형태로 주로 전종격동에 발생하고 그 빈도가 매우 드물다. 환자는 13세된 여아로 우연히 발견된 종격동 종괴로 수술적 절제를 하여 해면상 혈관종으로 판명되었다. 술후 추적한 x-ray 검사상 재발의 소견이 없으며 수술적치료시 너무 광범위한 무리한 박리와 절제는 필요하지 않을 것으로 보인다.

  • Extracerebral cavernous hemangiomas are rare vascular tumors that are very difficult to remove because of severe intraoperative bleeding. We report a case of 57-year-old male with extracerebral cavernous hemangioma with review of 126 cases in the literature. Patient presented with blurred vision, diplopia, numbness on the left side of his face. Magnetic resonance imaging revealed a well defined mass of $3{\times}4{\times}3cm$ size with heterogenous iso-or hypointensity on T1-weighted image showing strong homogenous contrast enhancement and marked hyperintensity on T2-weighted image. Digital subtraction angiography[DSA] revealed a faint tumor blush by feeders from the left internal carotid artery[ICA] and left external carotid artery[ECA] in the delayed phase. Even with profuse intratumoral bleeding, near total removal was achieved. In addition to preoperative neurologic deficits such as ophthalmoplegia, facial numbness in the V1-2 dermatomes, ptosis appeared postoperatively.

  • The etiology of laryngeal hemangioma is unclear, and it is classified into infant and adult types. The former is capillary hemangioma and relatively common, the latter is cavernous type and very rare. The adult laryngeal hemangioma mainly occurs in supraglottis and glottis. A 75-year-old man came to our clinic with a voice change that started four months ago. The laryngoscopic finding showed that the surface of oval-shaped mass is covered with turbid exudates. We performed the laryngeal microsurgery with CO2 laser. The mass was pathologically proven as cavernous hemangioma. We report a very rare and didactic case with review of relevant literature.

  • Although hemangiomas are common vascular tumors that can occur anywhere in the body, they seldom involve the gastrointestinal tract. Hemangiomas of the gastrointestinal tract in infants and children are rare benign vascular tumors that most commonly present with gastrointestinal bleeding. We describe here the case of 2-year-old boy with intestinal bleeding caused by a large jejunal cavernous hemangioma, which was treated by laparoscopy-assisted resection of the affected portion of the jejunum.

  • Radiation is a common treatment modality for central nervous system neoplasms. However, secondary tumor development must be considered in the differential diagnosis in patients with new or recurring symptoms after treatment with conventional radiotherapy. A 28-year-old woman developed a cavernous hemangioma about 9 years after brain irradiation for astrocytoma. Clinical and histopathological details are presented, and previous reports of radiation-induced intracranial cavernous hemangioma are reviewed.

노인에서 발생한 타액선의 해면상 혈관종 2예 (Two Cases of Cavernous Hemangioma in the Salivary Gland in Old Ages)

  • Non-lymphoid mesenchymal neoplasms of salivary gland origin are rare, accounting for 1.4% of major salivary gland tumors. Of this group 30% were hemangiomas. Hemangiomas are the most common benign tumors in children, but are rare in old ages. 90% of hemangiomas arise in the first three decades of life. Hemangiomas are classified as capillary(including juvenile type), cavernous, or mixed in type. Salivary gland hemangioma is more common in the parotid(90%) and extremely rare in submandibular gland. We present two adult patients, 69 and 60 years of age, who were referred for palpable mass in the parotid area and submandibular area, respectively. After surgical removal of the lesion, histopathologic examination showed characteristic feature of cavernous hemangioma. The rarity of such a lesion in these locations in old ages have prompted this case report.

Intractable Hiccup as the Presenting Symptom of Cavernous Hemangioma in the Medulla Oblongata : A Case Report and Literature Review

  • A case of intractable hiccup developed by cavernous hemangioma in the medulla oblongata is reported. There have been only five previously reported cases of medullary cavernoma that triggered intractable hiccup. The patient was a 28-year-old man who was presented with intractable hiccup for 15 days. It developed suddenly, then aggravated progressively and did not respond to any types of medication. On magnetic resonance images, a well-demarcated and non-enhancing mass with hemorrhagic changes was noted in the left medulla oblongata. Intraoperative findings showed that the lesion was fully embedded within the brain stem and pathology confirmed the diagnosis of cavernous hemangioma. The hiccup resolved completely after the operation. Based on the presumption that the medullary cavernoma may trigger intractable hiccup by displacing or compression the hiccup arc of the dorsolateral medulla, surgical excision can eliminate the symptoms, even in the case totally buried in brainstem.

Role of fractionated radiotherapy in patients with hemangioma of the cavernous sinus

  • Purpose: We performed this retrospective study to investigate the outcomes of patients with hemangioma of the cavernous sinus after fractionated radiotherapy. Materials and Methods: We analyzed 10 patients with hemangioma of the cavernous sinus who were treated with conventional radiotherapy between January 2000 and December 2016. The median patient age was 54 years (range, 31-65 years), and 8 patients (80.0%) were female. The mean hemangioma volume was $34.1cm^3$ (range, $6.8-83.2cm^3$), and fractionated radiation was administered to a total dose of 50-54 Gy with a daily dose of 2 Gy. Results: The median follow-up period was 6.8 years (range, 2.2-8.8 years). At last follow-up, the volume of the tumor had decreased in all patients. The average tumor volume reduction rate from the initial volume was 72.9% (range, 18.9-95.3%). All 10 of the cranial neuropathies observed before radiation therapy had improved, with complete symptomatic remission in 9 cases (90%) and partial remission in 1 case (10%). No new acute neurologic impairments were reported after radiotherapy. One probable compressive optic neuropathy was observed at 1 year after radiotherapy. Conclusion: Fractionated radiotherapy achieves both symptomatic and radiologic improvements. It is a well-tolerated treatment modality for hemangiomas of the cavernous sinus.
