Why Does Only Contact the Half Area of the High Heel during Ascending Stairs?

  • Purpose : Wearing high-heeled shoes leads to foot and ankle instability, which requires leg muscles to remain in constant contraction. In order to adapt to the instability of the feet and ankles caused by wearing high heels, the muscles of the legs continuously repeat contraction and relaxation. Previous studies of the impact of stair climbing in high-heeled shoes have involved placing the entire stepping area of the shoe on the stair. However, high-heeled shoe wearers sometimes unconsciously contact the stair using only half of the stepping area. Therefore, the objective of this study was to examine differences in leg and ankle muscle activation according to stepping area during stair climbing in high-heeled shoes. Methods : Twenty young women in their early 20s voluntarily agreed to participate in this study. We used surface electromyography to measure gastrocnemius and tibialis anterior activation in the right leg during stair climbing under three conditions: barefoot, using half of high-heeled shoe stepping area, and using the total of high-heeled shoe stepping area. Barefoot, half of high heeled shoe, and total of high-heeled shoe were used to evaluate the effects of different areas of the foot or high heels touching the stairs. Results : Both muscles showed significant activity differences among the three stair climbing conditions. Gastrocnemius activity was significantly different between the high-heeled shoe conditions (p= .032), and tibialis anterior activity was significantly different between barefoot stair climbing and climbing with half of the high-heeled shoe stepping area (p= .021). Conclusion : The stepping area increased as heel size increased, thus increasing excessive gastrocnemius and tibialis anterior activity to control excessive ankle joint movement. We infer that using half of the high-heeled shoe stepping area prevents muscle fatigue by reducing excessive leg and ankle muscle activation.

  • The purpose of this research is to develop functional midsole that can restrain the heightening of plantar pressure in forefoot pressure so as to develop high-heeled shoes that can lessen foot ailments among women. The pumps shoes used for the research were of the same pumps type last. The variant was heel height, 3cm, 6cm, and 9cm, and the shoes were made in both normal midsole and functional midsole. The variant was applied to investigate the changes of foot pressure on forefoot and hindfoot according to heel height.'Heel Cup' was chosen for modification of functional midsolepumps type. to enlarge contact area in hindfoot, and 'Heel Posting Pad' was attached under sustentaculum tali to suppress the weight moving to forefoot pressure. If such functional parts are developed and used, it is possible to lessen the amount of Pmax or Impulse imposed by high-heeled pumps type on forefoot pressure. This can greatly lessen foot ailments, largely caused by high-heeled shoes, among women.

높은 굽 신발 보행 시 전면 접촉인솔이 보행 변수에 미치는 영향 (The Effect of Total Contact Inserts on the Gait Parameters During High-Heeled Shoes Walking)

  • The purpose of this study was to investigate the effect of high heeled shoes with the total contact insert (TCI) on the frontal plane of the joints for the lower extremity during the gait. Ten healthy females voluntarily participated in this study and the height of the high heeled shoes was 7 cm. A three-dimensional motion analysis system (VICON) and force plates were used to analyze the movements of the joints for the lower extremities. The results were as follows: There were no significant differences for the angle value on the event of the gait cycle in the maximum eversion and inversion of the ankle joint, the varus and valgus of the knee joint, and the adduction and abduction of the hip joint (p>.05). But, there was a significant difference or the range of motion in the ankle joint (p<.05). The value of ankle and knee moment with a TCI was less than the value for no TCI. And there were significant differences for the moment value of the maximum inversion and eversion on the ankle joint and for the maximum varus and valgus on the knee joint (p<.05). Therefore, a TCI would be effective in stabilizing the joints of the lower extremities and increasing the balance of a body to reduce the injure from a fall during the gait.

높은 굽 신발이 감각계의 변화와 균형에 미치는 영향 (Effects of the High-Heeled Shoes on the Sensory System and Balance in Women)

  • The purpose of this study were to compare light touch, two-point discrimination, position sense, and static balance ability and to examine the relationship of static balance ability and different senses in those who are wearing various heights of high-heeled shoes. Each 29 women was classified two different heights (below 3 cm, above 7 cm) by experience in wearing shoes. A t-test was used to determine the differences between low- and high-heeled shoe in light touch, two-point discrimination, position sense, and static balance ability. Pearson correlation was used to examine the relationship of static balance ability to these sensation. The results were as follows: 1) No significant differences in average light touch, position sense between two groups were found(p>0.05). But significant differences in average two-point discrimination, static balance ability were found(p<0.05). 2) Static balance ability was not significantly correlated with light touch, position sense, two-point discrimination(p>0.05). The results suggest that long time experience in wearing high-heeled shoe may be caused local sensory change and decreased static balance ability. Even though, static balance ability was not significantly affected but correlated with two-point discrimination, position sense, and light touch in order.

하지 근육의 피로상태 동안 높은 굽 신발에 적용한 전면접촉인솔이 젊은 여성의 보행 특성에 미치는 영향 (The Effect of High-Heeled Shoes With Total Contact Inserts in the Gait Characteristics of Young Female Adults During Lower Extremity Muscle Fatigue)

  • This study investigated gait characteristics, kinematics, and kinetics in the lower extremities between two different shoe conditions (high heeled shoes (7 cm), and high heeled shoes with a total contact insert (TCI)) after lower extremity muscle fatigue. Although TCI shave been applied in high heeled shoes to increase comfort and to decrease foot pressure, no study has attempted to identify the effects of TCI in fatigue conditions. The purpose of this study was to determine the effects of walking in high heeled shoes with TCI after lower extremity muscle fatigue was induced. This study was carried out in a motion analysis laboratory at Hanseo University. A volunteer sample of 14 healthy female subjects participated. All in fatigue conditions, the subjects were divided into two groups. The muscle fatigue was induced by 40 voluntary dorsi- and plantar-flexion exercises and 40 heel-rise exercises of the dominant foot. Surface electromyography was used to confirm the localized muscle fatigue using power spectral analysis of three muscles (tibialis anterior, gastrocnemius medialis and lateralis). The results were as follows: (1) In muscle fatigue conditions, the use of TCI decreased the peak flexion angle of the hip joint significantly in the early stance phase (p<.05) and increased the peak hip flexion moment in the terminal stance phase (p<.05). (2) In muscle fatigue conditions, the application of TCI also increased peak hip power generation in the early stance phase and peak hip power absorption in the terminal stance phase (p<.05). (3) In muscle fatigue conditions, the use of TCI reduced the impact force significantly and increased the secondary peak vertical GRF. These findings suggest that the TCI may provide beneficial effects when muscle fatigue occurs for a high heeled shoe gait. Future research employing the patient population and various types of TCI materials are required to clarify the effects of TCI.

불안정 판을 이용한 훈련이 높은 굽 신발을 신은 여자 대학생의 발목관절 변위와 동적균형능력에 미치는 영향 (The effect of unstable plate on the ankle joint displacement and dynamic balance ability of female college students wearing high-heeled shoes)

  • 본 연구는 불안정 판을 이용한 훈련이 높은 굽 신발을 신은 여자대학생의 발목관절의 변위와 동적균형에 어떤 영향을 미치는지 알아보기 위해 실시하였다. 여자 대학생 18명을 무작위로 실험군과 대조군으로 나누고, 굽 높이가 9cm인 신발을 착용 한 후 3D 동작분석기를 이용하여 5m 보행 시의 발뒤꿈치와 발목의 변위를 검사하고, 동적 균형능력 평가를 실시하였다. 중재 프로그램은 실험군에게 4주 동안 주 3회, 불안정 판을 이용한 균형 훈련을 실시하고, 대조군에게는 중재를 실시하지 않았다. 연구 결과, 중재 후 실험군의 발목관절의 변위는 전반적으로 감소하였고, 동적균형능력은 유의하게 상승하였다. 대조군은 발목관절의 변위, 동적균형능력의 변화가 거의 없었다. 불안정 판을 이용한 훈련이 대상자들의 발목관절을 안정화 시켜주고 동적균형능력을 향상시키는 것으로 나타났다.

불안정 지지면에서 정적 서기 동안 구두 굽 높이의 변화에 따른 자세 조절 전략 (Postural Strategy by the Difference of Shoe Heel Height During Quiet Standing on an Unstable Surface)

  • The purpose of this study was to evaluate the changes in the electromyographic (EMG) activity of the trunk and the lower limb muscles during quiet standing on an unstable surface while wearing low-heeled shoes (3 cm), high-heeled shoes (7 cm) and without footwear (0 cm) in 20 young healthy women. The subjects stood on an unstable surface for 30 seconds. We examined the differences in the EMG data of the erector spinae, rectus abdominis, biceps femoris, rectus femoris, tibialis anterior, and the gastrocnemius medialis muscle. A one-way repeated analysis of variance was used to compare the effects of shoe heel height on the EMG activity with the level of significance set at ${\alpha}=.05$. The EMG activity of the erector spinae and the rectus femoris were significantly increased (p<.05) in the subjects who wore elevated heel height, while the tibialis anterior and the gastrocnemius medialis were significantly decreased (p<.05). However, the rectus abdominis and the biceps femoris exhibited no significant difference among the three conditions. The above results indicate that wearing high-heeled shoes may change the postural strategy. The findings of this study suggest that excessive heel height could contribute to an increased fall risk during quiet standing.

건강한 여성의 하이힐 계단오르기 보행이 안쪽빗넓은근과 가쪽넓은근의 근활성비와 개시시간에 미치는 영향 (Effects of Shoe Heel Height on the Onset of VMO Relative to VL and VMO/VL EMG Ratio during Stair Ascent in Women without Knee Pathology)

  • Purpose: The purpose of this study was to investigate onset timing of vastus medialis oblique(VMO) relative to vastus lateralis(VL) the VMO/VL electromyographic(EMG) ratio according to heel height Methods: A repeated measures design was used. Fifteen healthy female college students with no known knee musculoskeletal dysfunction were recruited this study. They carried out a standardised stair acent activity under 4 conditions; barefoot, and with heel height of 1, 3, 7 cm. Muscle activity was measured by surface EMG (Myosystem 1400A, Noraxon Inc., U.S.A). Data were analysed using $1{\times}4$ repeated measures ANOVA. Results: Onset timing differed with heel height(p<.05). However, the VMO/VL EMG ratio was not significantly difference between conditions. Conclusion: We found that 7 cm heel height delayed in VMO onset compared with 3 cm heel height during stair ascent, but no change in the relative EMG intensity of VMO and VL as measured by th VMO/VL ratio. The findings indicate that high-heeled shoes may have disadvantages in knee stability because of delayed onset of VMO. Due to a lack of knee joint stability, wearing of high heeled shoes should be avoided.

  • Purpose : The purpose of this study is to research the effects of the differences in shoe heel height on balanced performances with a balance master. Method : 40 healthy female adults were divided into 2 groups at random. All the female participants were tested by a balance master system. Differences were analyzed according to high heel shoes and low heel shoes. The ability of static postural balance control was measured by modified CTSIB and unilateral stance test, while that of dynamic postural balance control was measured by LOS (limit of stability) and rhythmic weight shift, among the programs of a balance master. Result : 1. There is no significant correlation between the ability of static and dynamic postural balance control before main test without the shoes. 2. Movement velocity and directional control ability were significant correlation compared result of in high heel shoes and low heel shoes group. Conclusion : In conclusion, the test results of the high heeled shoe group and the low heeled shoe group through a balance master system showed that the former one is inferior to the latter one.

신발 굽 높이와 착용 습관화에 따른 균형 능력 비교 (Comparison of Balance Ability on the Heel Height and the Habituation to the High Heel)

  • The aim of this study was to evaluate effects of the balance ability on the heel height (flat, low heel[3cm], and high heel[7cm]) and high heeled habituation (group A is used to a low heeled shoes (<7cm) and group B is used to a high heeled shoes (${\geq}7cm$). Thirty-five subjects participated in this study. In this study, the static balance with opened eyes and closed eyes and dynamic balance were measured. Also functional reach test was tested. The results found that the static balance ability was the best in a 7cm heeled shoes with open eyes and closed eyes, but there was not significant difference between group A and Group B. Dynamic balance ability with walking velocity and cadence were significant difference between Group A and B. In the walking velocity, a flat was the fastest, followed by 7cm heel and 3cm heel with Group A while 3cm and 7cm, then a flat with Group B. Also Cadence pattern was similar. Functional balance ability was the highest with a flat, but it was not difference significantly between Group A and B. Therefore, this study demonstrated that the habituation to the high heel altered gait velocity and cadence of dynamic balance ability.
