Health care has two different facets. One is commodity and another is a right of human being. Health care as a commodity is utilized by demand approach in market. Demand is determined by economic factors such as price and income. From the last third of the 19th century until the early 1920s, priority of sickness insurance was replacing the income that workers lost as a result of illness and injury. By the 1920s, the capacity of applied biological and medical science was remarkably developed. Development of medical science stimulated the cost of medical care, and the burden of increased medical care cost required new role of medical care security system. In 1942, Beveridge report was published in United Kingdom, and health care was considered as a right of human being. In 1948, United Nations declared heath care as a right in the Universal Declaration of Human Right. In most countries introduced new medical care security policy based on health care as a right. The viewing health care as a commodity must be shifted toward need based care as a right. Need were understood to rest on demographic, epidemiological, scientific, and medical knowledge factors. Bring needed care to the population could best be achieved institutionally by a hierarchy of provider organizations, guided by planning bodies, which would provide comprehensive benefits. In Korea, health care in social health insurance (SHI) is considered as a commodity not a right. However, health policies under SHI must be need approach based on health care as a right. Mismatch between health policies and ideology of SHI made big troubles. It is important to realize ideology of SHI for good health policies.
The purposes of this study were to examine the relationships among health promoting lifestyle, stress. depression, and general characteristics and to reveal these variables affecting depression in college female students. The data were collected between November 29th and December 4th. 1999 by using self-administered questionnaire. Data analysis were conducted by using Peason's correlation. t-test, ANOVA. stepwise multiple regression. The results were as follows : 1. Health promoting lifestyle was significantly different by religion, smoking. school record, perceived health status. satisfaction of school life, and department. 2. The mean score of stress was 2.28. the level of stress showed significant differences according to economic status, perceived health status, and satisfaction of school life. 3. The mean score of depression was 12.05. the level of depression showed significant differences according to religion, smoking, school record, economic status. perceived health status, satisfaction. of school life, and department. 4. The mean score of health promoting lifestyle was 2.43. both interpersonal relationship(M=2.96) and self-actualization(M=2.79) of the subscales of health promoting lifestyle revealed high scores, whereas the scores of health responsibility(M=2.07), and exercise (M=1.64) were below. 5. Negative correlations were observed between health promoting lifestyle and stress(r=-.21), between heath promoting lifestyle and depression(r=-38). Positive correlations were observed between stress and depression(r= .60). 6. Negative correlations were observed between self-actualization(r=-.29). nutrition Cr=-.17), and stress management(r=-.17) domains of health promoting lifestyle and stress. 7. Negative correlations were observed between self-actualization(r=-.47), nutrition (r=-.31), stress management(r=-.25), interpersonal relationship(r=-.23), health responsibility(r=-.13), and exercise(r=-.l1) domains of health promoting lifestyle and depression. 8. Depression was significantly predicted by stress$(36.2\%)$, health promoting lifestyle $(7.0\%)$. these variables explained $43.2\%$ of variance of depression.
This study aims to analyze economically active females' factors influencing problematic drinking, by their jobs. It examined the socio-demographic and health-related characteristics and problematic drinking status of economically active females over 19, by using data from the 6th Korea National Health and Nutrition Examination Survey. They were analyzed by using IBM SPSS 25.0 and the complex sampling design. The findings show that the problematic drinking rates of economically active females were varied by their jobs: 19.1% for office job; 24.8% for manufacturing job; 12.3% for service job. The factors influencing problematic drinking were also differed by jobs: subjective heath status for office job; marital status and smoking for service job; marital status for manufacturing job. They suggest that convergent drinking management programs customized to jobs should be developed by considering the characteristics of females' physical and emotional traits and their jobs, to reduce their problematic drinking rates.
Background: This study aimed to properly manage diseases such as blood sugar control so that patients with diabetes can benefit from both medication and health activities. Also, these health practices are greatly influenced by self-rated health, a subjective assessment of health status. Because self-rated health does not necessarily match the objective health status, it is important to identify which factors affect self-rated health. Methods: For the study, the data was gathered from the 4th-6th National Health Nutrition Survey (2007-2015). Out of the total 73,353 participants in the survey, 2,303 patients with uncontrolled blood sugar with an HbA1c level of more than 7% were selected for the final study. Dependent variables fell into two categories depending on how the participant reported whether he or she was in good health or not. Independent variables included socio-demographics, health behavioral, and health status factors. This study performed logistic regression analysis. Results: Out of 2,303 participants, 18.1% reported that their heath was 'good,' despite the fact that their blood sugar level was not controlled. After running a logistic regression model, the odds ratio of groups that perceive subjective health awareness as good was higher in the groups of people as below: in the people over 60 years old; in the people who graduated from a junior college or higher than those who had a level of education of primary school completion or less; in the people living in Chungnam than those living in Seoul; and in the group with hypertriglyceridemia. Conclusion: The study identified factors associated with those failed to perceive the blood sugar level as a severe health problem despite of the fact that blood sugar was not controlled. To improve public health, diabetes management policies need to be addressed to population groups with these problems above.
Objectives : To measure DALE (Disability-Adjusted Life Expectancy) in Korea to find out how long Koreans live in a state of full heath. Methods : DALE was calculated using the life table of 1999 and the disability prevalence from the National Health Interview Survey (NHIS), which was conducted with a sample of 13,523 households in 1998. The disability prevalence was measured using the annual prevalence of the long-term limitation of activities, which were divided into classes 1, 2, 3, 4, 5 and 6 according to the severity of the limitation. The disability weights were measured for each 6 class by conducting a survey of 16 healthcare professionals. The severity-adjusted disability prevalence was calculated by multiplying the disability prevalence of each class by the disability weights respectively. Healthy life years lost due to disability was calculated by multiplying the life expectancy by the severity-adjusted disability prevalence. Finally DALE was measured as the life expectancy minus healthy life years lost due to disability. Results : DALE for 1999, which refers to the expectation of equivalent years of good health, were 72.5, 69.5 and 75.3 years, for total, for males and for females, respectively. The percentages for DALE out of the life expectancy were 95.8, 96.6 and 94.4% for total, for males and for females, respectively. Conclusions : DALE is a newly developed indicator, which could effectively show the healthy life expectancy of populations. A greater notice and use of DALE would be expected as life expectancies increase and the quality of life changes in Korea.
Purpose: The Serious Civil Accidents consist of the public use facilities, the public transports, and the material and its products. According to the Serious Civil Accidents of the Serious Accidents Punishment Act, it must be constructed the safety and health management framework and execution system. Objectives: This study investigated the relevance of dietary behavior and oral management for oral heath in adults to extend the recognition of the importance of food intake in oral health and to provide basic information for oral health management. Methods: A survey and clinical assessment were conducted for 119 adults over 20 years of age. The chi-square test and logistic regression analysis were carried out using SPSS Statistics 20.0. Results: Gingivitis was influenced less when ham and sausage in the sub-area of meat, fish, egg, and bean were consumed once a week rather than the cases when consumed rarely (3.47 times) or when consumed two times or more a week (7.87 times) (p<0.05). Periodontitis was influenced more in the groups of 30s and 40s age than in that of over 50 years of age (14.97 times, 7.73 times) and in the group without drinking experience than in group with that (4.17 times) (p<0.05). Conclusions: The results verified the close relationship between food intake frequency and oral health in patients with periodontitis and gingivitis. Thus, it would be necessary to study the relationship between dietary behavior and oral health continuously.
Background: As the prevalence of hypertension is increasing in Korea, the government is seeking policy actions to manage patients with hypertension more efficiently. In this paper, we aimed to identify factors associated with the use of medical care at hospitals among outpatients with hypertension. Methods: We analyzed a total of 15,040 cases of 3,877 outpatients with hypertension obtained from the Korea Medical Panel database from 2010 to 2016. The dependent variable was whether a patient with hypertension visited a hospital or not; and independent variables were the patient's various socio-demographic, health-related, and heath-status characteristics. We conducted a generalized linear mixed model analysis with logit link for all the cases and then conducted it stratified by gender. Results: As a result of a multivariable analysis, women were less likely than to visit at a hospital (odds ratio [OR], 0.44; 95% confidence interval [CI], 0.32-0.61) and people aged 65 years and older than those aged less than 65 years (OR, 0.71; 95% CI, 0.57-0.89). Residents in Busan, Ulsan, and Gyeongnam were more likely than those in than Seoul, Gyeonggi, Incheon, and Jeju to visit a hospital (OR, 1.40; 95% CI, 1.05-1.86). The likelihood of visiting a hospital was high in people belonging to a group of: the highest level of annual household income (OR, 1.73; 95% CI, 1.30-2.29); Medical care aid recipients (OR, 1.94; 95% CI, 1.34-2.83); people having three or more complex chronic diseases (OR, 1.59; 95% CI, 1.19-2.11); people having diabetes (OR, 1.81; 95% CI, 1.41-2.32); or people having ischemic heart disease or cerebrovascular disease (OR, 6.80; 95% CI, 5.28-8.76). Also, we found that factors associated with the use of medical care at hospitals among outpatients with hypertension differed between genders. Conclusion: A variety of factors seem to be associated with the use of medical care at hospitals among outpatients with hypertension. Future research needs to find a way to help patients with hypertension visit an appropriate medical institution between clinics and hospitals.
