Descriptive Study on the Development of Educational Program for Mothers with Children who have had Open Heart Surgery in Congenital Heart Disease (개심술을 받은 심기형 환아모의 교육프로그램 개발을 위한 조사연구)

  • The aims of this study were to identify the mothers educational needs when they have children who have had open heart surgery in congenital heart disease, to identify the children's characteristics with this problem, and also to provide the basic information on the development of the educational program for the mothers. The subjects of the study were 101 mothers of children and their children hospitalized with congenital heart disease in pediatric wards of G. University Hospital in Inchon and S. Hospital in Puch on from June to November 2000. The study used a Likert-type questionnaire with 39 questions which was based on a previous questionnaire developed by Lee Mi-ryun(1989) for adult open heart surgery patients. The reliability of the questionnaire was Cronbach' ${\alpha}$=.9375. The data were collected directly from mothers of the patients in wards. The t-test, ANOVA was adopted for the data analysis. The key results of the study are as follows : 1) The average age of the children was 32.90 months. The majority of the children are male and weighted 3.0-3.5Kg at birth. Most of the children had VSD(29.7%), and 10.9% of children had a family history of heart disease. 2) An average score of 4.62 out of a maximum of 5.00 was recorded for the educational needs of the children's mother. The highest score of 4.69 was for home care after discharge followed by a score of 4.67 for diagnosis and prognosis on congenital heart disease. And pre-post operation care scored 4.51. The mean scores of single-question items of educational need were, in order, 4.81 for immunization after operation, 4.80 for recovery process, and 4.77 for prognosis of the disease. The score of 4.18 for measurement of intake and output was the lowest. 3) The analysis on the children's mothers educational needs by social-demographic characteristics such as sex, age of patient and mother, economical status, educational status, number of child, hospitalization times, and hospitalization cause showed no significant difference statistically, but there was a significant difference between mothers with jobs and with out jobs.

Growth & Development and Fruit Characteristics of Newly Bred and Introduced Grape Cultivars (최근 육성 포도 품종과 해외 도입 품종들의 생육 및 과실 특성 분석)

  • This study was conducted to investigate the growth and fruit characteristics of recently introduced or bred varieties in Korea and to review domestic adaptability. As for the sprouting rate among cultivars, 'Stella' was the highest at 91.6%, followed by 'My Heart' (78.3%), 'IFG-6' (77.3%), and 'Hongju Seedless' (73.2%). As for the flower incidence rate by cultivar, 'Stella' was the highest at 113.5%, and 'IFG-6' had a lower flower incidence at 45.3% compared to other cultivars. The diameter of shoots was less than 11.0 mm in the other three cultivars except for 'Hongju Seedless', and 'Hongju Seedless' was the thickest at 12.5 mm and 'Stella' was the thinnest at 9.6 mm. The berry weight of 'My Heart' was 11.3 g, heavier than other cultivars, followed by 'Stella', 'IFG-6' and 'Hongju Seedless'. Soluble sugar content at harvest time by cultivar was about 19.0°Bx or higher, with 'IFG-6' having the highest level of 20.2°Bx, followed by 'My Heart', 'Stella', and 'Hongju Seedless'. Acidity was the lowest in 'My Heart' at 0.39%, followed by 'IFG-6', 'Stella' and 'Hongju Seedless'. As for the coloring, in the case of 'My Heart', the skin color did not change to red even after the harvest season, therefore the grapes were irregularly colored, so it was judged that the development of cultivation technology to enhance the coloring was necessary.

Thymosin Beta4 Regulates Cardiac Valve Formation Via Endothelial-Mesenchymal Transformation in Zebrafish Embryos

  • Thymosin beta4 (TB4) has multiple functions in cellular response in processes as diverse as embryonic organ development and the pathogeneses of disease, especially those associated with cardiac coronary vessels. However, the specific roles played by TB4 during heart valve development in vertebrates are largely unknown. Here, we identified a novel function of TB4 in endothelial-mesenchymal transformation (EMT) in cardiac valve endocardial cushions in zebrafish. The expressions of thymosin family members in developing zebrafish embryos were determined by whole mount in situ hybridization. Of the thymosin family members only zTB4 was expressed in the developing heart region. Cardiac valve development at 48 h post fertilization was defected in zebrafish TB4 (zTB4) morpholino-injected embryos (morphants). In zTB4 morphants, abnormal linear heart tube development was observed. The expressions of bone morphogenetic protein (BMP) 4, notch1b, and hyaluronic acid synthase (HAS) 2 genes were also markedly reduced in atrio-ventricular canal (AVC). Endocardial cells in the AVC region were stained with anti-Zn5 antibody reactive against Dm-grasp (an EMT marker) to observe EMT in developing cardiac valves in zTB4 morphants. EMT marker expression in valve endothelial cells was confirmed after transfection with TB4 siRNA in the presence of transforming growth factor ${\beta}$ ($TGF{\beta}$) by RT-PCR and immunofluorescent assay. Zn5-positive endocardial AVC cells were not observed in zTB4 morphants, and knockdown of TB4 suppressed TGF-${\beta}$-induced EMT in ovine valve endothelial cells. Taken together, our results demonstrate that TB4 plays a pivotal role in cardiac valve formation by increasing EMT.

Recent advances in transcatheter treatment of congenital heart disease (선천성 심질환에 대한 중재적 치료술의 최근 진전)

  • Over the last several decades there has been a remarkable change in the therapeutic strategy of congenital heart disease. Development of new tools and devices, accumulations of experience, technical refinement have positively affected the outcome of interventional treatment. Many procedures including atrial septostomy, balloon valvuloplasty, balloon dilation of stenotic vessel with or without stent implantation, transcatheter occlusion of abnormal vascular structure, transcatheter closure of patent arterial duct and atrial septal defect, are now performed as routine interventional procedures in many institutes. In diverse conditions, transcatheter techniques also provide complementary and additive role in combination with surgery. Intraoperative stent implantation on stenotic vessels, perventricular device insertion, and hybrid stage 1 palliative procedure for hypoplastic left heart syndrome have been employed in high risk patients for cardiac surgery with encouraging results. Transcatheter closure of ventricular septal defect has been performed safely showing comparable result with surgery. Investigational procedures such as percutaneous valve insertion and valve repair are expected to replace the role of surgery in certain group of patients in the near future. Continuous evolvement in this field will contribute to reduce the risk and suffering from congenital heart disease, while surgery will be still remained as a gold standard for significant portion of congenital heart disease.

β-Adrenergic Receptor and Insulin Resistance in the Heart

  • Insulin resistance is characterized by the reduced ability of insulin to stimulate tissue uptake and disposal of glucose including cardiac muscle. These conditions accelerate the progression of heart failure and increase cardiovascular morbidity and mortality in patients with cardiovascular diseases. It is noteworthy that some conditions of insulin resistance are characterized by up-regulation of the sympathetic nervous system, resulting in enhanced stimulation of ${\beta}$-adrenergic receptor (${\beta}$AR). Overstimulation of ${\beta}$ARs leads to the development of heart failure and is associated with the pathogenesis of insulin resistance in the heart. However, pathological consequences of the cross-talk between the ${\beta}$AR and the insulin sensitivity and the mechanism by which ${\beta}$AR overstimulation promotes insulin resistance remain unclear. This review article examines the hypothesis that ${\beta}$ARs overstimulation leads to induction of insulin resistance in the heart.

Ectopic Intrapulmonary Thyroid: A Case Report

  • An ectopic thyroid is caused by abnormalities in migration of the thyroid during development and rarely occurs in the thoracic cavity or the abdominal cavity. We report the case of a 64-year-old female who had abnormal findings from a thyroid hormone test during follow-up after thyroid cancer surgery. Based on the radioisotope diagnostic test, an ectopic thyroid inside the thoracic cavity was suspected. Through surgical treatment, the patient was diagnosed with ectopic intrapulmonary thyroid. Ectopic intrapulmonary thyroid is reported to be very rare and the case is described along with a literature review.

Double Outlet of Right Ventricle in Criss-Cross Heart -Surgical Experience of One Case (십자형심장에 동반된 양대혈관우심실기시증 -수술치험 1례)

  • Criss-cross heart which is a cardiac malformation caused by abnormal rotation of the ventricles early in embryonic development, is rare but a double outlet of right ventricle in priss-cross heart is very rare. We experienced a case of criss-cross heart which is situs solidus, concordant atrioventricular connection and double outlet of rig t ventricle with remote ventricular septal defect of perimembranous inlet type. A 4-years old female was diagnosed as a double outlet of right ventricle in criss-cross heart after echocardiography, cardiac catheterization and cardiac angiography. The surgical correction was a intraventricular reconstruction of left ventricular outflow with 314 circle of 20 mm Hemashield vascular graft from the ventricular septal defect to the aorta. The patient had a temporary atrioventricular block but was recovered uneventfully, and a postoperative echocardiogram showed no left ventricular outflow obstruction, no intracardiac shunt.

A Study of the influence of stimulus on newborn infant생s response -Specifically in heart rate change- (감각적 자극에 따른 신생아 반응에 관한 연구 -심박동수를 중심으로-)

  • Early extra-uterine development of the newborn infant has the potential for clarifying important questions of nature and care taking environment. In this study, tactile and auditory stimulation is used on .a group of infants to determine the effects of their response. Infants were tested by EKG at 24∼48 hrs after birth ana all they are free from complication. Stimuli presented were-touch and sound (40dB, 70dB and 40∼70dB). The result of the study were as follows: 1) The heart rate was seen to depend on their sleep-wake state. (light sleep ; 125/min, eyes: open & motor activity increase ; 151/min crying ; 185/min). 2) The heart rate affected by tactile & auditory stimulus. The heart rate response was acceleration above the prestimulus level. (tactile, 231min increase, auditory ; 16∼21/min increase), and heart rate change was seen to depend on prestimulus rate. (γ=.73) 3) There was a decrease in heart rate when they were soothe. (touch ; 24.8sec, 21/min decrease, pacifier ; 14.5sec, 26/min decrease) and significant difference in time between touch and pacifier (t= 1.76, p<0.05). This result support the evaluation of newborn infant's sensory abilitics, and has even begin to influence changes in nursing care. But systemic monitoring of newborn infants response would. further define.

  • This research focused on the development of wireless telemetry system that can monitor heart rates of multiple rehabilitation patients in real time without constraint. The whole system consists of the multiple patient's side devices (PSDs) and one central monitoring system (CMS). The PSD consists of a microphone, amplifier, filter, microcontroller, and RF (Radio Frequency) modem. In addition, the PSD was designed to be wearable and low power consumption. The CMS consists of an RF modem and general PC and it was designed to monitor heart rates from multiple patients simultaneously. The system warns an alarm signal when a patient's heart rate exceeds the pre-set range for each patient. This system can be useful to monitor the heart rate of exercising rehabilitation patients and control the patients condition and the exercising level.

Non-contact Heart Rate Monitoring using IR-UWB Radar and Lomb-Scargle Periodogram (IR-UWB 레이더와 Lomb-Scargle Periodogram을 이용한 비접촉 심박 탐지)

  • IR-UWB radar has been regarded as the most promising technology for non-contact respiration and heartbeat monitoring because of its ability of detecting slight motion even in submillimeter range. Measuring heart rate is most challenging since the chest movement by heartbeat is quite subtle and easily interfered with by a random body motion or background noise. Additionally, periodic sampling can be limited by the performance of computer that handles the radar signals. In this paper, we deploy Lomb-Scargle periodogram method that estimates heart rate even with irregularly sampled data and uneven signal amplitude. Lomb-Scargle periodogram is known as a method for finding periodicity in irregularly-sampled and noisy data set. We also implement a motion detection scheme in order to make the heart rate estimation pause when a random motion is detected. Our scheme is implemented using Novelda's X4M03 radar development kit and its corresponding drivers and Python packages. Experimental results show that the estimation with Lomb-Scargle periodogram yield more accurate heart rate than the method of measuring peak-to-peak distance.
