간선종으로 오인된 간에 발생한 자궁외 선근종: 증례 보고 (Extrauterine Adenomyoma of the Liver Mimicking a Hepatic Adenoma: A Case Report)

  • 간에 발생하는 자궁외 선근종은 매우 드문 종양이다. 이제껏 몇 가지의 사례만이 문헌에 보고되었고, 이것들은 조직병리학 소견에 초점을 둔 보고들이었다. 우리는 처음에 간선종으로 오인되었던, 상복부 통증을 주소로 내원한 43세 여성에서의 간에 발생한 자궁외 선근종의 특징적인 영상 소견을 보고하고자 한다. 이것은 전산화단층촬영과 자기공명영상에서 국소적 출혈을 동반한 고형 및 낭성 종괴로 보이며 간우엽 피막하에 위치하였고, 지속적인 약한 조영 증강을 보였다.

  • 기관지 낭종은 기관이나 기관지를 따라 종격동에서 가장 흔하게 나타나는 드문 선천성 기형이며, 특히 뒤쪽 상부 종격동에 생긴다. 기관지 낭종은 다른 위치에서도 발생할 수 있으며 폐, 복강, 그리고 후복강에서 위치가 보고되었다. 저자들은 전천추부 공간에 생긴 후복강 기관지 낭종의 증례를 보고한다. 환자는 수술 후 병리 조직학적 검사에서 최종적으로 진단되었다.

  • 코로나바이러스감염증-19는 전 세계적에서 급속도로 확산되고 있다. 몇몇 논문에서 코로나바이러스감염증-19의 영상의학적 소견들이 보고되었다. 우리는 한국에서 코로나바이러스감염증-19로 확진된 세 명의 환자에 대한 임상적 소견과 흉부 일반촬영 및 흉부 전산화단층촬영에서 나타난 영상의학적 소견에 대하여 보고하고자 한다.

  • 폐에서 발생한 선섬유종은 드물게 발생하는 종양으로 이 양성 병변에 대해 발표된 보고는 매우 적다. 영상의학적으로 대부분의 이 병변은 단일 폐결절로 나타나며, 다발성은 극히 드물다. 저자는 71세 여자 환자에서, 전산화단층촬영 검사상 서로 다른 엽에 두 개의 결절로 보인 폐의 선섬유종에 대한 흔치 않은 증례를 보고하고자 한다. 폐의 선섬유종의 확실한 진단을 위해서는 조직병리학적 검사와 면역조직화학 검사가 필요하다.

Tuberculous Aortitis Complicated with Pseudoaneurysm Formation in the Descending Thoracic Aorta: A Case Report

  • A 51-year-old male with sustained fever was diagnosed with military tuberculosis and tuberculous aortitis complicated with pseudoaneurysm formation at the proximal descending aorta. A follow-up computed tomography evaluation showed an increased size of the pseudoaneurysm in this area, suggestive of a contained rupture. Consequently, the patient underwent emergency excision and replacement of the aorta using a left heart bypass. The patient was discharged without postoperative complications on post-operative day 12. During the one-year follow-up period, the patient was free of any complications or recurrence of tuberculosis. We report a case of pseudoaneurysm of the descending aorta that was successfully surgically repaired.

  • A 27 year-old man, who had a penetrating cardiac injury due to a metal fragment was transferred to our hospital. At admission, his vital signs were stable, and his chest film showed a foreign-body-like finding in the heart silhouette. We evaluated the patient with chest computed tomography and echocardiography for further information. Finally, we removed the metal fragment from the left ventricle by using a cardiopulmonary bypass.

Functional Intracardiac Paraganglioma

  • A 39-year-old man presented to the department of emergency medicine in Seoul National University Hospital complaining of chest pain, heart palpitation, and headache. Upon arrival, a computed tomography scan showed a 7.0 cm×6.2 cm lesion with the typical features of a paraganglioma. The patient was treated with an alpha-blocker and a beta-blocker prior to surgical intervention. We removed the tumor successfully, and histopathologic findings indicated that the tumor was indeed a paraganglioma. Since intracardiac paraganglioma is a rare disease, we present this case together with a literature review.

  • The posterior malleolar fracture is relatively common fracture of the foot and ankle, but several aspects of this are still controversial. If the posterior malleolus is involved in the ankle fracture, the prognosis is usually poor. A computed tomography scan is essential for accurate diagnosis and treatment planning. Although indirect reduction and the anterior to posterior screw fixation technique have the advantages of a small incision with the requirement of relatively simple skills, direct open reduction and fixation from the posterior side provide a more biomechanically stable and accurate reduction. The precise reduction of the posterior malleolar fragment helps to achieve congruency of the tibia and fibula in the incisura and contributes to syndesmotic stability. It is important to determine the indications for surgical treatment by comprehensively evaluating the three-dimensional structure of the posterior malleolar fracture and all related injuries to the ankle.

CT and MRI for Repaired Complex Adult Congenital Heart Diseases

  • An increasing number of adult congenital heart disease (ACHD) patients continue to require life-long diagnostic imaging surveillance using cardiac CT and MRI. These patients typically exhibit a large spectrum of unique anatomical and functional changes resulting from either single- or multi-stage palliation and surgical correction. Radiologists involved in the diagnostic task of monitoring treatment effects and detecting potential complications should be familiar with common cardiac CT and MRI findings observed in patients with repaired complex ACHD. This review article highlights the contemporary role of CT and MRI in three commonly encountered repaired ACHD: repaired tetralogy of Fallot, transposition of the great arteries after arterial switch operation, and functional single ventricle after Fontan operation.

Coronary Artery Calcium Data and Reporting System (CAC-DRS): A Primer

  • The Coronary Artery Calcium Data and Reporting System (CAC-DRS) is a standardized reporting method for calcium scoring on computed tomography. CAC-DRS is applied on a per-patient basis and represents the total calcium score with the number of vessels involved. There are 4 risk categories ranging from CAC-DRS 0 to CAC-DRS 3. CAC-DRS also provides risk prediction and treatment recommendations for each category. The main strengths of CAC-DRS include a detailed and meaningful representation of CAC, improved communication between physicians, risk stratification, appropriate treatment recommendations, and uniform data collection, which provides a framework for education and research. The major limitations of CAC-DRS include a few missing components, an overly simple visual approach without any standard reference, and treatment recommendations lacking a basis in clinical trials. This consistent yet straightforward method has the potential to systemize CAC scoring in both gated and non-gated scans.
