Bioequivalence Assesment of Tiropramide in Korean Male Volunteers

  • Two formulations of tiropramide {(${\pm}$)${\alpha}$-(benzoylamino)-4-[2-(diethylamino)-ethoxy]-N,N-dipropyl-benzenepropanamide hydrochloride}, an antispasmodic agent, were orally administered to 16 healthy Korean male volunteers by Latin crossover design with the purpose of evaluating bioeqivalence and phamacokinetics of tiropramide. Tiropramide in human plasma was determined by a gas chromatography/nitrogen phosphorus detector. Detection limit of tiropramide was 5 ng/ml. C$\_$max/ values in test and reference formulations were 93.9 ${\pm}$ 54.3 and 96.4 ${\pm}$ 51.6 ng/ml, respectively. AUC$\_$0\longrightarrowlast/ and AUC$\_$0\longrightarrowinf/ were, respectively, 330.7 ${\pm}$ 193.9 and 349.5 ${\pm}$ 205.3 ng.hr/ml for test formulation, 348.9 ${\pm}$ 207.7 and 380.8 ${\pm}$ 239.0 ng.hr/ml for reference formulation. Terminal half-life was 2.3-2.6 hr. Bioavailability differences for C/aub max/ and AUC$\_$0\longrightarrowlast/ were 2.48% and 5.22%, respectively. Minimum detection differences were less than 20% in both C$\_$max/ AUC. Based on this results, two formulations of tiropramide were considered to be bioequivalent

  • Objectives : Assuming That the Characteristic of Meridian System Has Been Similar to This of Electric Potentials in Human Body and That Measurements of Electric Potential at Well and Sea Points in Branches of the Twelve Meridians Will Be Representative of Measurements of the Twelve Meridians, to Measure the Electric Potentials in Healthy Volunteers(HG), Patients Diagnosed As a Cerebral Infarction(CG), a Arrhythmia(AG), a Other Intervertebral Disc Disorder(IG), and a Joint Disorder(JG), and Then to Find Out the Characteristic of Biophysical Meridian System, Finally to Compare with the Results of the Electric Potential in Those Groups. Methods : We Selected Who HG Were Diagnosed by a Blood Test, Urine Examination and Differentiation of Syndromes by Five Viscera among Volunteers, CG by CT and Wind-Syndrome Caused by Hyperactivity of the Liver-Yang, AG by EKG, CT, and Deficiency or the Heart Blood, IG by X-ray or CT and Pain in Waist, and JG by X-ray and Knee Pain. Their Electric Potential of Well and Sea Points in the Meridians Were Measured by hysiograph. Results and Conclusions : Measurements Were Analyzed by Factor Analysis, Analysis of Variance, and Logistic Regression Model, We Obtained That Most of the Results, In Conclusion, Hold Good for the Classical Meridian Doctrine.

Advanced Method for Determination of Omeprazole in Plasma by HPLC

  • An advance d and sensitive high-performance liquid chromatographic (HPLC) method for determination of omeparzole in human plasma has been developed. After omeprazole was extracted from plasma with diethylether, the organic phase was transferred to another tube and trapped back with 0.1 N NaOH solution. The alkaline aqueous layer was injected into a reversed-phase C8 column. Lansoprazole was used as an internal standard. The mobile phase consisted of 30% of acetonitrile and 70% of 0.2 M $ KH_{2}PO_{4}$, pH 7.0. Recoveries of the analytes and internal standard were >75.48%. The coefficients of variation of intra- and inter-day assay were <5.78 and 4.59% for plasma samples. The detection limit in plasma was 2 ng/ml. The clinical applicability of this assay method was evaluated by determining plasma concentration-time courses of the respective analytes in 24 healthy volunteers after oral administration 40 mg of omeprazole. The present assay is considered to be simple, accurate, economical and suitable for the study of the kinetic disposition of omeprazole in the body.

Pharmacokinetic Characteristics of Oral Clonazepam in Korean (한국인에서의 경구용 클로나제팜의 약물동력학적 특성)

  • In order to determine the bioavailability of c1onazepam, an anxiolytic drug, a simple, rapid and sensitive HPLC analysis was developed in healthy Korean volunteers. The analysis system was validated in specificity, accuracy, precision and linearity. The analysis condition we established was 2.58 min and 5 ng/$m\ell$ in retention time and limit of quantitation of c1onazepam, respectively, using reverse-phase C18 column connected to UV detector. Quantitation was performed at 235 nm wave length with p-hydroxybenzoic acid ethyl ester as internal standard. The method involved a simple extraction. In order to study blood level profiles as a function of time, eight volunteers were enrolled and orally took 6 mg clonazepam once. The blood samples were collected from 0 to 120 h after the drug administration. Mean AUC and Cmax value were 1028.17$\pm$568.165 (ng/$m\ell$$.$hr) and 41.25$\pm$10.82 (ng/$m\ell$), respectively. And mean Tmax and T$_{1}$2/ value were 1.08$\pm$0.42 (hr) and 30.78$\pm$3.26 (hr). From the results we determine the pharmacokinetic characteristics of clonazepam in Korean people using a newly developed and useful HPLC method.

Relationship Between Lower Extremity Extensor Strength and Wall Squat Performance

  • Background: The wall squat exercise has been recommended for strengthening of the lower extremity muscles with maintaining lumbar lordosis. Although squat has been studied to be related to lower extremity extensor strength, the relationship between wall squat and lower extremity extensor strength unclear. Because squat and wall squat are biomechanically different, study on the relationship is needed. Objects: The purpose of this study was to determine the lower extremity extensor strength associated with wall squat performance. Methods: 74 healthy volunteers were recruited to participate in this study. The volunteers were measured hip and knee extensors strength and then performed wall squat exercise for maximum count. Results: We found significant relationships between wall squat performance and hip extensor strength normalized by body weight, knee extensor strength normalized by body weight and the composite value. In a regression analysis, hip extensor strength normalized by body weight explained 29% of the variation in wall squat performance in males and 35% in females. Conclusion: These results demonstrate that hip extensor strength normalized by body weight is critical to wall squat performance in both sexes.

Comparison of Static Balance Abilities on Respiratory Types in Healthy Adults (호흡 유형에 따른 건강한 성인의 정적균형능력 비교)

  • Purpose: To investigate breathing-related changes in the balance ability of healthy adults. Methods: The participants were 36 healthy adults in their 20s and 30s. All participants were trained in three breathing (neutral, thoracic, and abdominal) methods one week before measurements. We used AccuSway to measure each participant's balance ability, using each breathing technique, in two postures (bipedal and unipedal). Results: During the bipedal balance task, abdominal breathing produced significant increases in path length and sway velocity. Abdominal breathing resulted in significant changes in sample entropy at the anteroposterior location compared with neutral breathing (p<.05). In the normalized anteroposterior location, there were significant changes in backward movement during thoracic and abdominal breathing compared with those during neutral breathing (p<.05). During the unipedal balance task, path length and sway velocity increased significantly during voluntary breathing compared with those during neutral breathing (p<.05). There was a significant change in backward movement when abdominal breathing-compared with neutral breathing-was used in the normalized anteroposterior location (p<.05). In the normalized left-right location, there was a significant shift to the right during thoracic breathing compared with that during neutral breathing (p<.05). Conclusion: Compared with neutral breathing, altered voluntary breathing patterns affect balance in healthy adults. Our results indicated that that static balance was more affected by abdominal breathing than by neutral breathing. Future studies should examine variables such as the breathing volume, rhythm, and method.

A study on wiry pulse in hypertensive patients analyzed at 5 levels of applied pressure using 3 dimensional pulse imaging analyzer (3차원 로봇 맥 영상 분석기의 5단계 가압 맥파 분석에 의한 고혈압 환자의 현맥(弦脈) 연구)

  • Objectives: The purpose of this study is to gain the objective indicators for the classification of hypertension by oriental medical pulse diagnosis, through finding out the parameters which can distinguish the pulse of hypertensive patient from that of normal subject, and characterizes the wiry pulse of hypertension. Methods: 30-59 yr, male, 946 healthy volunteers and 35 hypertensive patients were enrolled in this study. All the hypertensive patients were taking medicine to control the blood pressure and the blood pressure of the hypertension group was not statistically different from that of the normal healthy group. Data were acquired using 3 dimensional pulse imaging analyser(DMP-3000, DAEYOMEDI, KOREA) and analysed according to the age bands and the applied pressure levels. Results: 1. RAI/t, w/t and t2/t decreased with the levels of applied pressure and increased with age in the normal healthy group, not in the hypertension group. 2. RAI/t, w/t, t2/t were significantly higher in the hypertension group than the normal healthy group in the 30-age band, and elasticity coefficient was higher in the hypertension group than the normal healthy group in the 40-age band. 3. Researches on the patients without hypotensive agents are needed to figure out whether these parameters are the components of hypertensive wiry pulse. Conclusions: Analysing the radial pulse at 5 applied pressure levels using 3 dimensional pulse imaging analyser may be useful to differentiate the pulses of the hypertensive patients from those of the normal subjects, and characterize the hypertension.

Isoprostane Characteristics in Sick House Syndrome, Atopy and Asthma Patient

  • An imbalance between oxidants and antioxidants, in favor of oxidants leading to oxidative stress, is known to play an important role in the pathogenesis of various diseases. Isoprostanes are structurally stable isomers of the conventional enzymatically derived prostaglandins, which are produced in vivo primarily by a free radical catalyzed peroxidation of polyunsaturated fatty acids. In asthmatics, disease severity can occur from environmental exposure to air pollution. Some surveys suggested that air pollutants, especially diesel-exhaust particulates, could trigger allergic sensitization and development of atopic diseases. Sick house syndrome (SHS) presents healthy damage owing to the indoor environment of a building. The aim of this study was to examine isoprostane as a parameter fur oxidative stress in environments related diseases such as sick house syndrome, atopy and asthma. We measured plasma and urinary levels of isoprostane from health volunteers, sick house syndrome, atopy and asthma patients. Plasma isoprostane concentrations in asthma and sick house syndrome group were significantly higher than in control. Urinary isoprostane levels were significantly higher in volunteers with sick house syndrome and asthma compared with health volunteers. These findings suggest that plasma and urinary isoprostane measurement may have useful clinical implications for investigating sick house syndrome and asthma. The interventions that decrease exposure to environmental reactive oxygen species might be beneficial in these diseases.

  • Objectives : There has been gradually increasing concern about the adverse health effects of electromagnetic radiation originating from cell phones which are widely used in modern life. Cell phone radiation may affect human health by increasing free radicals of human blood cells. This study has been designed to identify DNA damage of blood cells by electromagnetic radiation caused by cell phone use. Methods : This study investigated the health effect of acute exposure to commercially available cell phones on certain parameters such as an indicator of DNA damage for 14 healthy adult volunteers. Each volunteer during the experiment talked over the cell phone with the keypad facing the right side of the face for 4 hours. The single cell gel electrophoresis assay (Comet assay), which is very sensitive in detecting the presence of DNA strand-breaks and alkali-labile damage in individual cells, was used to assess peripheral blood cells (T-cells, B-cells, granulocytes) from volunteers before and after exposure to cell phone radiation. The parameters of Comet assay measured were Olive Tail Moment and Tail DNA %. Results : The Olive Tail Moment of B-cells and granulocytes and Tail DNA % of B-cells and granulocytes were increased by a statistically significant extent after 4-hour use of a cell phone compared with controls. Conclusion : It is concluded that cell phone radiation caused the DNA damage during the 4 hours of experimental condition. Nonetheless, this study suggested that cell phone use may increase DNA damage by electromagnetic radiation and other contributing factors.

The Effect of Tourniquet Inflation on Neural Functions: A Volunteer Study (정상 성인에서 구혈대에 의한 신경기능의 변화)

  • Background: Tourniquet pain has important impacts on anesthesia. Tourniquet pain and accompanying cardiovascular changes are important factors that make patients in distress during anesthesia. As tourniquet pain may be modified by anesthesia, a study on the changes in the neural functions by tourniquet inflation in normal volunteers is important. Methods: Time-dependent changes in tourniquet pain, heart rate, phantom limb sensation, motor function, pain to pressure on upper extremity of 10 healthy and unpremedied volunteers were measured. Each parameter were measured every 5 minutes starting from 10 minutes before inflation to 15 minutes after deflation of tourniquet. Tourniquet was deflated when the subject felt unbearable pain (score 100 with visual analog scale). Results: Subjects manifested time-dependent pain responses to tourniquet inflation, characterized by increase in VAS, systolic and diastolic blood pressure. Mean duration of tourniquet inflation was 36.4 minutes, volunteers experienced motor paralysis at 27.6 minutes and sensory loss at 33.1 minutes. Pain to pressure decreased over time in both arms. The degree of decrease was greater in the arm on which tourniquet was applied than that in the non-applied arm. Phantom limb sensation occurred in 3 subjects. Conclusions: This study demonstrated dynamic changes in the neural functions during tourniquet inflation period. Tourniquet-induced pain and resultant hypertension occurred in all subjects. Appropriate anesthetic management is needed for the surgery using tourniquet.
