The purpose of this study was to analyze of hospital administrator based on the DACUM(Developing A Curriculum) method. The contents of this study were to extract the duties, tasks consisting of job of hospital administrator and to investigate levels of importance, difficulty, frequency and entry level on each task, and to make out a job model of hospital administrator. A DACUM committee was composed to analyze job of hospital administrator and the committee members were total 9, a facilitator, 7 hospital administrator and a recorder. The major findings of this study were as the followings; first, duties in job of hospital administrator were total 13, which were organization of hospital administration affairs, health insurance review & assessment, general affairs, personnel management, hospital planning & management, medical quality improvement, hospital financial affairs, logistics management, facilities management, computerized system management, education & study supports, medical staffs' perceptions of service quality both directly and indirectly through their perceptions of patient orientation. These findings implied that HPWS would be a way of survival in drastically changing hospital environments.
Objectives: The purpose of this study was to identify the health promotion activities of the elderly Korean aged 65 or older and to examine the related factors associated with the health promotion activities. Methods: Data were obtained from 2008 Social Statistics Survey of Korea National Statistical Office of 6,207 people aged 65 or older. We measured the socio-demographic characteristics, physical health status, social health status, and health promotion activities. Statistical analyses were employed through the $X^2$-test and Odds ratio using Logit Model. Results: In our study, health promotion activity practice rates were varied among the socio-demographic characteristics, physical and social health status. Our findings also support that better socio-demographic and physical health status explain the higher practice rates of health promotion activities. In addition, the higher social health status was associated with better practice rates of health promotion activities. Conclusion: We found that the health promotion activities of the elderly could be encouraged by better socio-demographic status and physical and social health status. To better accomplish the health promotion for the elderly in our community, policy-makers should need careful political deliberation for executing health promotion services considering the distinctions of programme and target groups.
Hypotheses In health services research are becoming increasingly more complex and specific. As a result, health services research studies often include multiple independent, intervening, and dependent variables in a single hypothesis. Nevertheless, the statistical models adopted by health services researchers have failed to keep pace with the increasing complexity and specificity of hypotheses and research designs. This article introduces a statistical model well suited for complex and specific hypotheses tests in health services research studies. The covariance structure modeling(CSM) methodology is especially applied to regional relevance indices(RIs) to assess the impact of health resources and healthcare utilization. Data on secondary statistics and health insurance claims were collected by each catchment area. The model for RI was justified by direct and indirect effects of three latent variables measured by seven observed variables, using ten structural equations. The resulting structural model revealed significant direct effects of the structure of health resources but indirect effects of the quantity on RIs, and explained 82% of correlation matrix of measurement variables. Two variables, the number of beds and the portion of specialists among medical doctors, became to have significant effects on RIs by being analyzed using the CSM methodology, while they were insignificant in the regression model. Recommendations for the CSM methodology on health service research data are provided.
Background: This study was conducted to investigate factors related to unmet medical needs of medical care in adult diabetes patients and to suggest factors related to unmet medical in Korea. Methods: This study used data from the Korea National Health and Nutrition Examination Survey (KNHNES) 2014-2017. The subjects of the study were conducted on patients with unmet medical needs experience among the patients and analyzed using the IBM SPSS ver. 25.0 program (IBM Corp., Armonk, NY, USA). Results: Overall, 10.9% of patients had unmet medical needs. Being female, less educated, and lower medical aid were related to unmet medical needs. And subjects with poor subjective health and higher stress level were more likely to report unmet medical needs. Conclusion: Although comprehensive health insurance coverage, 10.9% of people with diabetes experienced unmet healthcare needs. The results of this study suggest that socioeconomic factors such as low education and medical aid were associated with unmet medical needs.

Purpose: The purpose of this study was to examine the factors affecting of health screening among persons with disabilities. Method: The study examines the factors affecting of health screening using over 20 years of age who participated in the 2017 National survey of the disabled data. Subjects were 6,332 person with disabilities over 20 years of age. Data were analyzed using descriptive statistics, X2-test, multiple logistic regression with the SPSS win 21.0 program. Results: Health screening among persons with disabilities is differentiated with education level, marital status, Subjective House economic status, Health insurance, Economic activity, Type of disability, Grade of disability, Stress recognition, Depressive symptom, Suicidal thinking, Suicidal attempt, and grade of disability. The significant predictors of the health screening were age, Subjective House economic status, Economic activity, and Grade of disability. Conclusion: We should consider age, disability level, and economic level when developing a screening program for people with disabilities. Especially, the development and promotion of the health screening program for persons with disabilities and related education are required.
Background: There are regional variations in the incidence of coronavirus disease 2019 (COVID-19), which means that some regions are more exposed to the risk of COVID-19 than others. Therefore, this study aims to investigate regional variations in the incidence of COVID-19 in Korea and identify risk factors associated with the incidence of COVID-19 using community-level data. Methods: This study was conducted at the districts (si·gun·gu) level in Korea. Data of COVID-19 incidence by districts were collected from the official website of each province. Data was also obtained from the Korean Statistical Information Service and the Community Health Survey; socio-demographic factor, transmission pathway, healthcare resource, and factor in response to COVID-19. Community risk factors that drive the incidence of COVID-19 were selected using a least absolute shrinkage and selection operator regression. Results: As of June 2021, the incidence of COVID-19 differed by more than 80 times between districts. Among the candidate factors, sex ratio, population aged 20-29, local financial independence, population density, diabetes prevalence, and failure to comply with the quarantine rules were significantly associated with COVID-19 incidence. Conclusion: This study suggests setting COVID-19 quarantine policy and allocating resources, considering the community risk factors. Protecting vulnerable groups should be a high priority for these policies.

The aim of this study was to identify the association between candidiasis and early childhood caries and to investigate whether the experience of candidiasis or oral candidiasis before age 1 can be considered as a risk factor for early childhood caries. The database used in this study was provided by Health Insurance Review and Assessment Service. Medical records of children born from January 2010 to December 2012 were obtained, and those without dental records were excluded. Subjects were divided into several groups based on the experience of candidiasis or oral candidiasis before age 6: candidiasis group and non-candidiasis group; oral candidiasis group and non-oral candidiasis group. Another categorization was done according to the experience of candidiasis or oral candidiasis before age 1. The incidence rate of early childhood caries in each group were compared. The prevalence of dental caries in children who have been diagnosed with candidiasis or oral candidiasis before age 6 was significantly different from those who have not experienced candidiasis. Similarly, children who have suffered from candidiasis or oral candidiasis before age 1 had significantly different incidence of caries from the children without candidiasis experience.

Objectives : The objective of this study is to estimate the economic costs of cancer on society. Methods : We estimated the economic burden of people with cancer in South Korea. To perform the analysis, we reviewed the records of people who were cancer patients and those who were newly diagnosed with cancer. The data was compiled from the National Health Insurance Corporation, which included the insurance claims database, a list of cancer patients, a database that records the cancer rates, the Korea Central Cancer Registry Center s cancer patient registry database and the Korea National Statistical Office s causes of death database. We classified the costs as related to cancer into direct costs and indirect costs, and we estimated each cost. Direct costs included both medical and non-medical care expenses and the indirect costs consisted of morbidity, mortality and the caregiver's time costs. Results : The total economic costs of cancer in South Korea stood at 14.1 trillion won in 2005. The largest amount of the cost 7.4 trillion won, was the mortality costs. Following this were the morbidity costs (3.2 trillion won), the medical care costs (2.2 trillion won), the non-medical care costs (1.1 trillion won) and the costs related to the caregiver's time (100 billion won). As a result, the economic cost of cancer to South Korea is estimated to be between 11.6 trillion won to 14.1 trillion won for the year 2005. Conclusions : We need to reduce the cancer burden through encouraging people to undergo early screening for cancer and curing it in the early stage of cancer, as well as implementing policies to actively prevent cancer.
Globally, efforts are being made to develop and strengthen a palliative care policy to support a comprehensive healthcare system. Korea has implemented a hospice and palliative care (HPC) policy as part of a cancer policy under the 10 year plan to conquer cancer and a comprehensive measure for national cancer management. A legal ground for the HPC policy was laid by the Cancer Control Act passed in 2003. Currently in the process is legislation of a law on the decision for life-sustaining treatment for HPC and terminally-ill patients. The relevant law has expanded the policy-affected disease group from terminal cancer to cancer, human immunodeficiency virus/acquired immune deficiency syndrome, chronic obstructive pulmonary disease and chronic liver disease/liver cirrhosis. Since 2015, the National Health Insurance (NHI) scheme reimburses for HPC with a combination of the daily fixed sum and the fee for service systems. By the provision type, the HPC is classified into hospitalization, consultation, and home-based treatment. Also in place is the system that designates, evaluates and supports facilities specializing in HPC, and such facilities are funded by the NHI fund and government subsidy. Also needed along with the legal system are consensus reached by people affected by the policy and more realistic fee levels for HPC. The public and private domains should also cooperate to set HPC standards, train professional caregivers, control quality and establish an evaluation system. A stable funding system should be prepared by utilizing the long-term care insurance fund and hospice care fund.

The aim of the study was to examine levels of satisfaction with health care in North Korea and to identify factors associated with it using a convenience sample of North Korean refugees in China. Data from the 2004 Survey of Health Seeking Behavior of North Korean Households conducted by the Center for Refugee and Disaster Response, Johns Hopkins Bloomberg School of Public Health were used. The study subjects were 273 North Korean refugees whose length of stay in China was less than 3 months. Factor analysis was used to extract factor dimensions from the 12 satisfaction items. Bivariate (t test and ANOVA) and multiple regression analyses were used in examining factors associated with satisfaction with health care use in North Korea Overall, satisfaction level was low ($2.36{\pm}0.36$, score range: 1-5). Of the three-factor dimensions, physician skills scored the highest $(2.93{\pm}0.36)$, followed by drug availability $(2.51{\pm}0.07)$ and general cleanliness $(1.66{\pm}0.55)$. In the multiple regression analysis, having a usual source of care was significantly associated with patient satisfaction. Respondents who identified primary care (section) doctors as their usual source of care tended to be less satisfied than those with the city or county hospital as their usual source of care. County residents tended to report a lower degree of satisfaction with general cleanliness than city residents. Among socioeconomic characteristics, the number of household assets positively predicted satisfaction with drug availability. North Korean residents appear to be dissatisfied with their medical care. It may reflect some inadequacies in the North's universal health care system to meet the healthcare needs of its people.
