전문의약품 대중광고에 대한 의약전문인과 일반인간의 인식 비교 연구 (Comparisons of Perception on Direct-to-Consumer Advertisements of Prescription Drugs between Healthcare Providers and Consumers)

  • Objective: The aim of this study was to investigate the difference of perception on direct-to-consumer advertisement (DTCA) of prescription drugs between healthcare providers and consumers. Methods: The online and offline survey was conducted from May 26th to June 5th, 2013. The questionnaire was composed of 15 items about perception on DTCA of prescription drugs. Results: A total of 215 healthcare providers and 202 consumers responded to the questionnaire. Consumers had an overall positive attitude on permitting DTCA of prescription drugs and carried favorable views about the influence of the DTCA of prescription drugs on providing drug information, promoting communications between healthcare providers and consumers, and improving images of healthcare providers. Healthcare providers displayed negative perception for the needs of permitting the DTCA of prescription drugs compared to consumers. They showed somewhat skeptical perception about the influence of the DTCA of prescription drugs on necessities and efficiencies of delivering drug information, promoting communications between healthcare providers and consumers, and improving images of healthcare providers. Both healthcare providers and consumers were concerned about the increase of drug prices following the increase in advertisement expenses of pharmaceutical products. Conclusion: This study identified the perception differences on direct-to-consumer advertisements of prescription drugs between healthcare providers and consumers. This study could be of much help in the process of review on permitting DTCA of prescription drugs in Korea.

  • In this study, the status of healthcare infrastructure in foreign countries was investigated for a Korean healthcare business planning to expand its business to these countries. Countries selected and surveyed are China, India, Indonesia, and the Middle East. When the surveyors visited the hospitals, the hospital facilities were investigated and medical professionals were interviewed to scrutinize the healthcare conditions in the hospitals. Also studied are healthcare related laws, trend of healthcare policies, hospital operations, medical staffing, and global healthcare service providers. Korea has expanded their overseas healthcare market only to small-sized hospitals and clinics. In order to keep up with global market expansion in the healthcare domain, strategic marketing is required. Especially, the most important key for overseas marketing is to make a synergizing system among hospitals, construction companies, medical equipment providers and IT solution providers. For the next step, the in-depth study will be conducted through real projects in the target countries per type of business.

  • Cloud computing services changed the way the data are managed across the healthcare system that can improve patient care. Currently, most healthcare organizations are using cloud-based applications and related services to deliver better healthcare facilities. But architecting a cloud-based healthcare system needs deep knowledge about the working nature of these services and the requirements of the healthcare environment. The success is based on the usage of appropriate cloud services in the architecture to manage the data flow across the healthcare system.Cloud service providers offer a wide variety of services to ingest, store and process healthcare data securely. The top three public cloud providers- Amazon, Google, and Microsoft offers advanced cloud services for the solution that the healthcare industry is looking for. This article proposes a framework that can effectively utilize cloud services to handle the data flow among the various stages of the healthcare infrastructure. The useful cloud services for ingesting, storing and analyzing the healthcare data for the proposed framework, from the top three cloud providers are listed in this work. Finally, a cloud-based healthcare architecture using Amazon Cloud Services is constructed for reference.

디지털 헬스케어 서비스 제공자의 정보보호의도에 관한 연구 (A Study on the Information Protection Intention of Digital Healthcare Service Providers)

  • 본 연구는 디지털 헬스케어 서비스 제공자의 보호동기를 형성하는 위험평가와 대처평가가 정보보호의도의 주요한 요인인 유도통제의도와 자기방어의도에 미치는 영향을 확인하고자 한다. 연구모형은 보호동기이론을 기반으로 기존 선행연구를 통해 심각성, 취약성, 반응효능감, 자기효능감을 독립변수로 채택하였다. 연구모형 검증을 위해 한국의 디지털 헬스케어 서비스 기업 임직원 222명을 대상으로 설문조사를 실시하였으며, 데이터는 구조방정식을 사용하여 분석하였다. 연구결과에 따르면 (1) 보안위협의 결과에 대한 명확한 인지가 디지털 헬스케어 서비스 제공자의 정보유출 사고에 대한 심각성에 대한 이해를 높여주고, 이를 통해 디지털 헬스케어 서비스 제공자의 오남용을 줄일 수 있으며, (2) 보안시스템에 대한 디지털 헬스케어 서비스 제공자의 신뢰와 만족이 스스로 정보유출에 대응할 수 있다는 자신감을 형성시킬 수 있고, (3) 디지털 헬스케어 서비스 제공자는 정보유출로 인해 돌아오는 결과를 인지하고 있지만, 실제 자신에게 발생할 수 있는 가능성은 적다고 생각하고 있음을 확인하였다. 이 연구결과는 디지털 헬스케어 서비스를 운영하는 벤처기업은 디지털 헬스케어 서비스 제공자의 정보보호의도를 높이기 위해서 디지털 헬스케어 서비스 제공자의 보안수준을 높일 수 있는 지속적인 콘텐츠 제공이 필요하고, 타 벤처기업대비 높은 수준의 보안시스템을 도입하여 디지털 헬스케어 서비스 제공자의 신뢰를 통해 정보보호 동기유발이 중요하다는 점을 시사한다.

Medication Injection Safety Knowledge and Practices among Health Service Providers in Korea

  • Purpose: Outbreaks resulting from medication injections have recently been on the rise in Korea despite various established guidelines. The objective of this study was to assess the degree to which healthcare professionals are aware of safe injection practice guidelines and to account for the adherence to and the deviation from safe injection guidelines formulated by healthcare providers. Methods: In November 2016, a cross-sectional anonymous questionnaire covering general characteristics of injections, patient safety culture, awareness of safe injection practices, and adherence to and barriers to safe injection guidelines was issued to healthcare providers who administer medication injections or manage and supervise these injections (N=550). Multivariate logistic regression analysis via enter method was performed to define the influencing factors of adherence of safe injection practices. Results: On average, respondents adhere to 17 of the 24 guidelines. Multivariate logistic regression found that those who were more likely to adhere to safe injection guidelines either underwent a patient safety training experience within the last year, provided care in a setting characterized by a highly developed patient safety culture, or were employed as physicians or nurses, as opposed to some other type of care provider. Barriers to safe injection guidelines were attributable to; thoughts of waste to discard leftover medicine, provisions that made adherence cumbersome, a weak culture of compliance, and insufficient amounts of injectable medicine, products, and education. Conclusions: The results of this study indicate that controllable factors like training experience of healthcare providers and patient safety culture were positively associated with adherence to safe injection practices. It was suggested that the training of healthcare providers on safe injection practices be a continuous process to promote patient safety. Additionally, there should be an increased focus on developing and implementing policies to improve patient safety culture from a prevention rather than post-management perspective.

Lessons From Healthcare Providers' Attitudes Toward Pay-for-performance: What Should Purchasers Consider in Designing and Implementing a Successful Program?

  • We conducted a systematic review to summarize providers' attitudes toward pay-for-performance (P4P), focusing on their general attitudes, the effects of P4P, their favorable design and implementation methods, and concerns. An electronic search was performed in PubMed and Scopus using selected keywords including P4P. Two reviewers screened target articles using titles and abstract review and then read the full version of the screened articles for the final selections. In addition, one reference of screened articles and one unpublished report were also included. Therefore, 14 articles were included in this study. Healthcare providers' attitudes on P4P were summarized in two ways. First, we gathered their general attitudes and opinions regarding the effects of P4P. Second, we rearranged their opinions regarding desirable P4P design and implementation methods, as well as their concerns. This study showed the possibility that some healthcare providers still have a low level of awareness about P4P and might prefer voluntary participation in P4P. In addition, they felt that adequate quality indicators and additional support for implementation of P4P would be needed. Most healthcare providers also had serious concerns that P4P would induce unintended consequences. In order to conduct successful implementation of P4P, purchaser should make more efforts such as increasing providers' level of awareness about P4P, providing technical and educational support, reducing their burden, developing a cooperative relationship with providers, developing more accurate quality measures, and minimizing the unintended consequences.

Knowledge, Attitude and Behavior of Healthcare Providers towards Breast Cancer in Malaysia: a Systematic Review

  • Breast cancer is the most common cancer among women in Malaysia. Therefore, it is highly important for the public to be educated on breast cancer and to know the steps to detect it early on. Healthcare providers are in the prime position to provide such education to the public due to their high knowledge regarding health and their roles in healthcare. The present systematic review involved studies conducted in recent years to analyze the knowledge, attitudes and behavior of Malaysian healthcare providers regarding breast cancer, in attempts to obtain an overall picture of how well equipped our healthcare providers are to provide optimal breast cancer education, and to see their perceptions and actual involvement in said education. The systematic review was conducted via a primary search of various databases and journal websites, and a secondary search of references used by eligible studies. Criteria for eligibility included being published from the year 2008 till present, being conducted in Malaysia, and being written in the English language. A total of two studies were eligible for this review. Findings show that Malaysian future and current healthcare providers have moderate knowledge on breast cancer, have a positive towards involvement of breast cancer education, but have poor actual involvement.

보건의료인의 이직의도 경로모형 분석 (A Path Analysis of Variables Influencing Turnover Intention Among Healthcare Providers)

  • Purpose: This study was to explore the relationships among emotional labor, social support, job satisfaction, organizational commitment, and turnover intention in healthcare providers. Methods: Participants were healthcare providers (54 male and 328 female) with a mean age of 30.28 years old. Data analysis were done with PASW 18.0 for descriptive statistics and AMOS 5.0 for path analysis. Results: The hypothetical path model showed a good fit to the empirical data (${\chi}^2$=4.178, df=2, p= .124, GFI= .997, NFI= .995, NNFI= .987, RMSEA= .053). Emotional labor, job satisfaction, and organizational commitment had significant direct effects on turnover intention. Social support had a significant direct effect on emotional labor, job satisfaction and organizational commitment, and indirect and total effect on turnover intention. Conclusion: These findings suggest that turnover prevention or education programs for healthcare providers should include maintaining good mood when interact with patients or visitors with various negative moods in healthcare setting. Further strategies may need to resolve emotional labor, enhance job satisfaction and organizational commitment, and provide social support.

Monitoring of Methicillin-resistant Staphylococcus aureus in Nasal Swabs Obtained from Dental Clinic Healthcare Providers and Medical Environment Nurses

  • The aims of this study were to investigate the nosocomial infection route of methicillin-resistant Staphylococcus aureus (MRSA) and explore preventative methods for this pathogen that involve blocking its dispersion. We cultured MRSA from nasal cavity swabs collected between June and July 2008 that we obtained from eight dental healthcare providers, 32 nurses and the sputum specimens of two patients from our hospital. In addition, we used VITEK 2 equipment to measure drug sensitivity, and we further performed biochemical testing and pulse-field gel electrophoresis (PFGE) to isolate MRSA colonies. The incidence of these bacteria on the nasal swabs was 25.0% from dental clinic healthcare providers, 13.6% from the internal medicine ward nurses and 30.0% from intensive care unit nurses. Moreover, MRSA was detectable in sputum specimens of ward patients. The antimicrobial agents resistance and partial PFGE types of MRSA showed a similar pattern. We suggest from these analyses that nasal cavity infection by MRSA could occur by cross contamination between healthcare providers and patients which underscores the importance of stringent MRSA management practices.

  • The effort to measure and improve the quality of healthcare is a common health policy issue worldwide. Korean Value Incentive Programme is one of that effort, but some concerns exist. Compared to pay for performance program in other countries, it measures healthcare quality with relatively narrow performance domain using a small number of clinical indicators. It was designed without involving hospitals and other key stakeholder, and program participation was mandated. Highest and lowest performers get bonus and penalty using relative ranking. As a suggestion for development, the direction for quality management at the national level should be given first. Therefore the philosophy or strategy for quality improvement should be reflected to the program. And various domains and indicators of healthcare quality should be developed with active communication with healthcare providers. The evaluation method is necessary to be changed to provide achievable goal to the healthcare providers and attract quality improvement.
