Background: Pharmacists communicate with a variety of healthcare experts to prevent medication errors. Situation-Background-Assessment-Recommendation (SBAR) is a tool used for concise and accurate communication. In 2018, we developed the pharmacy-SBAR (P-SBAR) to deliver pharmacists intervention more quickly and effectively through quality improvement activities. Objectives: This study evaluates the efficacy of P-SBAR on pharmacists' intervention activities before and after the implementation of P-SBAR applications. We assessed the impact of P-SBAR on reducing the burden of intervention work, promoting pharmacists' participation, and enhancing the acceptance rate. Methods: This is a retrospective study of the two groups before and after P-SBAR implementation. All pharmacists' intervention records during two periods (2016-2017 and 2019-2020) were extracted from the data warehouse system at Kyunghee University Hospital at Gangdong, Seoul. The outcome was the number of inpatients and pharmacists who participated in the prescription monitoring activity, the number of interventions, and the physicians' acceptance rate. Results: Although the total number of inpatients decreased (364,753 vs. 348,229), the number of pharmacists who participated in intervention activity increased (monthly mean: 15.8 vs. 18.0, p=0.001). The total number of interventions (2,767 vs. 4,389), the frequency of full acceptance (2,018 vs. 3,710), and the monthly acceptance rate increased significantly (73.8% vs. 83.8%, p<0.001). Conclusion: P-SBAR improved accessibility and convenience by digitalizing the intervention activities performed in an offline environment. Improvement in work burden and acceptance rate using P-SBAR is expected to contribute toward reducing medication errors.
게임을 통해 건강을 증진할 수 있는 건강 기능성 게임(SGHs)에 대한 관심이 고조되고 있다. 디지털 치료제(DTx)는 유효성과 안전성을 확보한 치료제이기 때문에 전통적인 의약품 유통방식을 따라야 하지만, SGHs는 웰니스 제품으로 DTx에 비해 보급과 확산이 유연하다. 또한 SGHs는 일시적인 치료 효과가 아닌 지속적인 모니터링과 피드백을 통해 맞춤형 서비스를 제공할 수 있어 효과적이며, SGHs를 인지장애치료나 행동교정 등에 적용할 경우 오작동과 부작용이 경미하다. 본 연구는 SGHs의 지각된 혜택과 위험을 동시에 고려하는 밸런스 프레임워크(Valence Framework)를 기반으로 142명의 학부생을 대상으로 설문을 수집하여 지각된 혜택만이 SGHs 사용 의도에 통계적으로 유의한 정(+)의 영향을 준다는 점을 실증하였다. 이러한 결과를 바탕으로 일반 사용자를 대상으로 한 SGHs 수용에 있어 혜택(유효성)을 중심으로 홍보하고, 데이터 주도형 (data-driven) 접근방법의 중요성과 데이터 주도형 유통 및 분석 플랫폼 전략의 필요하다는 시사점을 제시하였다.
본 연구는 EMG(electromyography) 텍스타일 전극 개발을 목적으로 레이어 수의 디자인 및 원단을 다르게 하여 성능 및 신호 획득 안정성을 평가한다. 레이징 및 프레스 공정을 통하여 텍스타일 전극을 제조하며 Layer-0, Layer-1, Layer-2로 레이어 유무 및 수에 따른 결과를 분석했다. 이에 레이어 유무에 따라서는 근활성 측정에 영향을, 수가 많을수록 높은 성능이 나타남을 확인할 수 있었다. Layer-2 구조로 통일하여 5가지의 원단(네오프렌, 스판덱스 쿠션, 폴리에스테르 100%, 나일론 스판덱스, 광목 캔버스)으로 전극을 제조해 실험해 보았다. 성능적인 면에서, 원단의 중량이 높은 나일론 스판덱스가 높은 성능을 보였으며, 스판쿠션 텍스타일 전극이 근활성도 수득에 높은 안정성을 보였다. 이에 위 연구는 레이어에 따른 성능 연관성과 전극-피부사이의 닿는 면적 간의 관계 등을 고찰하여 슬리브 전체의 의복압을 늘리는 대신 특정 센서 측정 부위에만 높은 압력을 가함으로 차후 연구에서 레이어의 수 및 물성에 따른 전극의 공학적 설계 가능성을 제시한 의의가 있다.

In the complex virtual environment of cyberspace, comprised of digital and communication networks, ensuring the security of information is being recognized as an ongoing challenge. The importance of 'Cyber Situation Awareness (CSA)' is being emphasized in response to this. CSA is understood as a vital capability to identify, understand, and respond to various cyber threats and is positioned at the heart of cyber security strategies from a defensive perspective. Critical industries such as finance, healthcare, manufacturing, telecommunications, transportation, and energy can be subjected to not just economic and societal losses from cyber threats but, in severe cases, national losses. Consequently, the importance of CSA is being accentuated and research activities are being vigorously undertaken. A systematic five-step approach to CSA is introduced against this backdrop, and a deep analysis of recent research trends, techniques, challenges, and future directions since 2019 is provided. The approach encompasses current situation and identification awareness, the impact of attacks and vulnerability assessment, the evolution of situations and tracking of actor behaviors, root cause and forensic analysis, and future scenarios and threat predictions. Through this survey, readers will be deepened in their understanding of the fundamental importance and practical applications of CSA, and their insights into research and applications in this field will be enhanced. This survey is expected to serve as a useful guide and reference for researchers and experts particularly interested in CSA research and applications.
Background: Recognizing the seriousness of the misuse and abuse of medical narcotics, the South Korean government introduced the world's first narcotic management system, the Narcotics Information Management System (NIMS). This study aimed to explore the recent one-year opioid prescribing patterns in South Korea using the NIMS database. Methods: This study analyzed opioid prescription records in South Korea for the year 2022, utilizing the dispensing/administration dataset provided by NIMS. Public data from the Korean Statistical Information Service were also utilized to explore prescription trends over the past four years. The examination covered 16 different opioid analgesics, assessed by the total number of units prescribed based on routes of administration, type of institutions, and patients' sex and age group. Additionally, the disposal rate for each ingredient was computed. Results: In total, 206,941 records of 87,792,968 opioid analgesic units were analyzed. Recently, the overall quantity of prescribed opioid analgesic units has remained relatively stable. The most prescribed ingredient was oral oxycodone, followed by tapentadol and sublingual fentanyl. Tertiary hospitals had the highest number of dispensed units (49.4%), followed by community pharmacies (40.2%). The highest number of prescribed units was attributed to male patients in their 60s. The disposal rates of the oral and transdermal formulations were less than 0.1%. Conclusions: Opioid prescription in South Korea features a high proportion of oral formulations, tertiary hospital administration, pharmacy dispensing, and elderly patients. Sustained education and surveillance of patients and healthcare providers is required.
Purposes: The Common Data Model(CDM) is very important for multi-institutional research. There are various domestic and international CDM construction cases to actively utilize it. In order to construct a CDM, different terms from each institution must be mapped to standard terms. Therefore, we intend to derive the importance and major issues of terminology mapping and propose a solution in CDM construction. Methodology/Approach: This study conducted terminology mapping between Electronic Health Record(EHR) and MOA CDM for constructing Medical Record Observation & Assessment for Drug Safety(MOA) CDM at Dankook University Hospital in 2022. In the process of terminology mapping, a CDM standard terminology process and method were developed and terminology mapping was performed by applying this. The constructions of CDM mapping terms proceeded in the order of diagnosis, drug, measurement, and treatment_procedure. Findings: We developed mapping guideline for CDM construction and used this for mapping. A total of 670,993 EHR data from Dankook University Hospital(January 1, 2013 to December 31, 2021) were mapped. In the case of diagnosis terminology, 19,413 were completely mapped. Drug terminology mapped 92.1% of 2,795. Measurement terminology mapped 94.5% of 7,254 cases. Treatment and procedure were mapped to 2,181 cases, which are the number of mapping targets. Practical Implications: This study found the importance of constructing MOA CDM for drug side effect monitoring and developed terminology mapping guideline. Our results would be useful for all future researchers who are conducting terminology mapping when constructing CDM.
종양학에서 암의 진단, 병기 결정, 재발 판정, 그리고 치료에 대한 반응의 평가에 PET/CT의 유용함은 이미 인정되고 있다. 그런데 크기가 작거나 폐의 기저 부위, 혹은 간의 상부에 위치한 종양일수록 호흡으로 인한 위치 변위와 왜곡의 정도가 크며 PET영상의 SUV에도 영향을 미치게 된다. 따라서 정상 호흡 상태에서 얻어지는 PET 또는 CT영상을 토대로 방사선 치료를 하게 되면 치료해야 할 표적이 치료 범위에서 벗어나거나 정상 조직에 과도한 방사선이 조사될 수 있으므로 치료율이 낮아지거나 방사선에 의한 부작용이 증가할 수 있다. 본 논문의 목적은 호흡에 의한 인공산물을 최소화하고 보다 정확한 SUV 측정을 위해 ACT를 이용한 감쇠 보정 방법을 적용하여 그 유용성을 평가하고자 함이다. 하 흉부에 종양이 있는 13명의 환자를 대상으로 Discovery STE8 PET/CT스캐너를 사용하여 두 가지의 PET/CT영상을 얻었다. HCT를 사용한 감쇠 보정 영상과 ACT를 사용한 감쇠 보정 영상에서 측정한 인공물의 크기와 $SUV_{max}$를 비교 분석하였다. 인공물은 모든 환자의 하 흉부의 백색 음영 영역을 측정하여 평가하였다. $SUV_{max}$는 주요 종양의 $SUV_{max}$를 측정하여 평가하였다. 분석 프로그램은 Advantage Workstation v4.3을 사용하였다. 환자에게 7.4 MBq (0.2 mCi)/kg의 $^{18}F$-FDG를 투여한 1시간 뒤 스캔하였다. 방출 스캔은 3 min/bed로 스캔하였다. HCT 보정 영상과 비교하여 ACT 보정 영상에서 인공산물의 크기가 눈에 띄게 줄어든 것을 관찰할 수 있었다. 하 흉부의 저 보정으로 인한 인공산물 크기는 ACT 보정 영상과 HCT 보정 영상 각각 $1.5{\pm}3.5$ cm과 $13.4{\pm}4.2$ cm를 나타냈다. 주요 병소의 $SUV_{max}$의 변화는 ACT 보정 영상이 HCT 보정 영상보다 눈에 띄게 상승하였다. ACT 보정 영상에서 HCT영상과 비교하여 tumor의 $SUV_{max}$가 평균 $5.3{\pm}3.9%$ 상승하였다. 가장 큰 차이를 보인 종양은 lung의 lower lobe에 있는 종양으로 $SUV_{max}$ 7.7에서 $SUV_{max}$ 8.7로 13% 상승하였다. ACT를 이용한 감쇠 보정 영상은 하 흉부의 인공산물을 눈에 띄게 줄일 수 있어 보다 정확한 병소의 SUV를 측정 및 방사선 치료 범위 설정에 도움이 될 수 있을 것이다. 또한 ACT 기법은 병소가 횡격막 부근에 있는 환자의 경우 폐와 간의 경계를 보다 정확히 구분할 수 있어 판독시 도움이 될 수 있을 것이라고 판단된다. 추가 ACT 촬영에 의한 피폭 선량이 증가하는 점을 고려하여 적용한다면 임상적으로 유용한 효과가 있다고 사료된다.
본 연구의 목적은 배드민턴 동호회 활동을 조사하고 배드민턴 활동에서 발생하는 스포츠 상해를 예방하는 스마트 의류를 디자인 개발하는 것이다. 고령화 사회에서, 스포츠는 생활체육으로 친숙하게 접하고 있으며, 점차 동호회 형식으로 발달되고 있다. 배드민턴 동호회 활동은 많은 스포츠 상해가 있으며, 특히 높은 발생률을 보이는 발목 부상은 스포츠 참여를 방해하는 심각한 문제로 대두되고 있다. 그러므로 본 연구는 발목 부상을 포함한 스포츠 상해를 예방하는 스마트의류의 디자인 프로토타입을 제안하는 것을 목표로 하였다. 첫째, 배드민턴 동호인들의 특성 및 고려사항, 그리고 스포츠 부상을 예방하기 위한 스마트 의류의 구성요소를 도출하였다. 둘째, 배드민턴 동호인들과 엘리트 배드민턴 선수, 전문가의 대상으로 스마트의류 사용에 따른 문제 및 사용자 평가들을 고찰하였다. 셋째, 선행연구와 사용자 평가 항목들을 통하여 스마트의류의 사용 시나리오를 도출하였다. 넷째, 발목 상해를 포함한 스포츠 상해를 방지하는 스마트의류 프로토타입을 사용 시나리오를 기반으로 제안하였다. 본 연구에서 제안된 스마트의류를 통하여, 스포츠 상해 예방에 도움이 필요한 배드민턴 동호인들은 스마트 단말기를 통해서 그들의 상해와 건강 상태를 가상 게임의 아바타로 모니터링이 가능하다. 이러한 가상 게임 시스템은 스포츠의 상해 및 건강에 관련한 정보에 접근을 용이하게 한다. 따라서 이와 같은 스포츠용 스마트의류는 배드민턴 동호인들의 부상을 예방하고 스스로 재검토를 가능하게 한다. 본 연구는 스포츠 상해를 예방하거나 스포츠 활동 또는 생체 신호 모니터링 하는 스마트의류 디자인 시에도 활용될 수 있다.
영화는 간호교육에서 효과적으로 활용되는 학습매체이다. 본 연구는 간호사의 핵심역량의 하나인 간호정보학의 개념을 이해하기 위하여 영화 'The Island'를 활용하고 그 효과를 분석한 것이다. 2012년 2학기 부산광역시 소재 I대학 간호학과에 개설된 "간호와 정보' 과목을 수강한 2학년 학생 중 본 연구의 목적과 의의를 알고 동의한 학생 68명이 연구에 참여하였다. 영화를 활용한 수업은 15주 중 첫 5주에 걸쳐 이루어졌다. 참여 학생들은 매 차시마다 자가 보고식 질문지에 응답하였고, 응답지는 내용분석을 실시하였다. 자료분석 결과, 첨단의료기술을 사용한 질병치료와 건강증진으로서의 '간호정보학의 개념', 정보학의 실제적인 간호실무로의 통합으로서의 '컴퓨터정보기술의 활용', 그리고 높은 보건의료기술뿐만 아니라 '환자안전과 보안'의 주제에 대한 학습목표를 달성하였다. 종합적으로 볼 때 영화 'The Island'는 간호정보학의 개념을 잘 이해할 수 있는 유용한 학습자료임을 알 수 있었다. 간호사를 포함한 보건의료전문가들의 정보학 역량 향상을 위해서도 본 영화가 효과적으로 활용될 수 있을 것으로 기대한다.
현대 사회는 고령층 인구의 확대와 당뇨병 발병률의 증가에 따라 당뇨에 대한 인식이 커지고 있다. 특히 당뇨 환자는 갑작스런 저혈당 쇼크가 발생할 수 있으므로 주기적으로 혈당을 측정하는 것이 중요하다. 하지만 자가 혈당측정기는 휴대의 불편함과 값 관리의 어려움을 동반하고 있다. 이를 해결하기 위해 혈당측정시스템은 센서 형태나 스마트기기와 연동이 되는 형태로 연구되고 있으나 상용화 되지 못하고 있다. 본 논문에서 제안하는 시스템은 이를 해결할 수 있는 혈당측정 기능이 포함된 스마트밴드이다. 사용자는 앱에서 식전/식후와 같은 혈당을 측정해야 하는 일정을 입력하면 밴드에서 알람을 받을 수 있다. 밴드에서는 언제든지 혈당 측정이 가능하며, 측정값은 식전/식후와 같은 값과 날짜와 시간을 포함하여 앱으로 전송되어 관리된다. 이를 통해 사용자의 건강관리, 진단 및 예방에 도움을 준다. 본 시스템은 의료기기인증이 된 2개의 제품과 성능평가를 진행하여 혈당 측정값도 더 정확함을 확인했다.
