종합병원 의료인의 교육훈련 인식이 의료인 상호간 커뮤니케이션에 미치는 영향 (The Effects of Supplementary Education Awareness on Interpersonal Communication for Health Care Providers)

  • 본 연구는 종합병원 의료인의 교육훈련 인식이 의료인 상호간 커뮤니케이션에 미치는 영향을 파악하기 위한 목적으로 수행된 연구이다. 자료조사는 광주광역시, 전라남도 소재 29개 종합병원에서 근무하는 의료인 433명을 대상으로 2018년 6월 8일부터 2018년 6월 25일까지 자료를 수집하였다. 자료 분석은 t-검정과 분산분석, 사후검정, 상관관계 및 단계적 회귀분석을 실시하였다. 연구결과 인구사회학적 특성 및 건강관련 특성과 의료인 상호간 커뮤니케이션 차이를 검정한 결과 연령, 학력, 병상규모, 직업만족도, 소재지, 건강상태, 건강관리 교육경험, 우울경험에서, 교육훈련 인식은 연령, 병상규모, 직종, 급여수준, 의료기관 종류, 직업만족도, 소재지, 건강상태, 건강관리 교육경험, 만성질환 여부에서 통계적으로 유의한 차이가 있었다. 의료인 상호간의 커뮤니케이션에 미치는 영향 요인으로 학력, 건강관련 교육경험에서 긍정적인 영향을 미쳤고, 연령과 병상규모(801병상 이상), 직업만족도에서는 연령이 높을수록, 병상규모가 801병상 이상인 경우, 직업에 만족하지 못할수록 부정적인 영향을 미쳤으며, 교육훈련 하부요인인 지원환경, 학습전이, 성과에서 영향을 미쳤다. 이러한 연구를 바탕으로 교육훈련 계획수립 시 성과와 지원환경, 학습전이를 높일 수 있는 교육훈련 프로그램 구축이 필요하며, 의학 및 간호학생들의 교육 과정에 커뮤니케이션의 정규 교육과정을 도입. 의료인 상호간 커뮤니케이션을 높여 환자에게 안전한 의료서비스를 제공해야 할 것이다.

Optimal Service Contract Policies for Outsourcing Maintenance Service of Assets to the Service Providers

  • There is a growing trend for asset intensive industries to outsource maintenance services of their complex assets since outsourcing through service contract reduces upfront investments in infrastructure, expertise and specialised maintenance facilities. Estimation of costs for such contracts is complex and it is important to the user and the service providers for economic variability. The service provider's profit is influenced by many factors such as the terms of the contract, reliability of asset, and the servicing strategies, costs of resources needed to carryout maintenance. There is a need to develop mathematical models for understanding future costs to build it into the contract price. Three policies for service contracts are proposed in this paper considering the concepts of outsourcing maintenance service of assets to the service providers. Conceptual models are developed for estimating servicing costs of outsourcing through service contracts by considering time dependent failure mode.

척추질환자의 의료기관 정보탐색행태와 탐색성과 (Information Searching Behavior for Medical Institutions of Spine Patients and Searching Outcomes)

  • Background: Information asymmetry between physicians and patients is one of the most unique characteristics of health care. But as consumerism spreads in health care sector, health care consumers are searching comparative information about quality and cost of providers from many information sources. Providing comparative information to health care consumers not only makes consumers choose hospital rationally, but also invigorates the health care market by providers' competition. However there are few studies regarding information searching behavior of health care consumers, then this study is carried out. Methods: The purpose of this study is to understand the information searching behavior of health care consumers based on their characteristics and the types of medical institutions. For this purpose, 313 spinal patients' data of 11 medical institutions (university hospital, spine specialized hospital, clinic) located in Seoul were collected by self-administered surveys. Results: The results of this study are as follows: 1) t-test/analysis of variance analysis showed that according to various characteristics of health care consumers and the types of medical institutions, the level of information searching of each source and the amount of information searching and searching outcomes are statistically different. 2) Regression analysis showed that influence on searching outcomes are statistically different according to the level of information searching of each source and searching content and the amount of information searching has positive effects on searching outcomes. Conclusion: The significance of this study is to provide empirical basis for establishment of health care policy reflecting information needs and preference of health care consumers.

소비자와 제공자가 자각하는 간호서비스와 측정도구에 관한 연구 (A Study of the Nursing Service Perceived by Consumers and Providers, and the Tool that Measures Nursing Service)

  • Purpose: This study was performed to measure the nursing service perceived value by consumer and providers, and to investigate the relationship among nursing service, general satisfaction and hospital revisiting intent, and to examinate the tools that measures nursing service's reliability, construct validity and usefulness. Method: The questionnaire was developed and distributed to 300 patients and 210 nurses at three general hospitals. For data analysis, SPSS/PC program was used. Result: The nursing service perceived value by providers is higher than that by consumers. There are distinctive difference in the expectation, and importance and performance values of the nursing service perceived. In examination of the relationship among nursing service, general satisfaction and hospital revisiting intent, nursing service has a deep relationship with general satisfaction, but doesn't have relationship with hospital revisiting intent. The tool that measures nursing service has a good reliability and construct validity. In analysis on the usefulness between the tools, SERVPERF tool is more useful than SERVQUAL tool. conclusion: The nursing service perceived value by consumer is different that by providers, and the tool that measures nursing service has a good reliability and construct validity. The SERVPERF tool is more useful than SERVQUAL tool.

Monitoring of Methicillin-resistant Staphylococcus aureus in Nasal Swabs Obtained from Dental Clinic Healthcare Providers and Medical Environment Nurses

  • The aims of this study were to investigate the nosocomial infection route of methicillin-resistant Staphylococcus aureus (MRSA) and explore preventative methods for this pathogen that involve blocking its dispersion. We cultured MRSA from nasal cavity swabs collected between June and July 2008 that we obtained from eight dental healthcare providers, 32 nurses and the sputum specimens of two patients from our hospital. In addition, we used VITEK 2 equipment to measure drug sensitivity, and we further performed biochemical testing and pulse-field gel electrophoresis (PFGE) to isolate MRSA colonies. The incidence of these bacteria on the nasal swabs was 25.0% from dental clinic healthcare providers, 13.6% from the internal medicine ward nurses and 30.0% from intensive care unit nurses. Moreover, MRSA was detectable in sputum specimens of ward patients. The antimicrobial agents resistance and partial PFGE types of MRSA showed a similar pattern. We suggest from these analyses that nasal cavity infection by MRSA could occur by cross contamination between healthcare providers and patients which underscores the importance of stringent MRSA management practices.

중환자실 환자 가족의 경험: 의료인들과의 상호작용 (Experiences of Families in the Intensive Care Unit: Interactions with Health Care Providers)

  • Purpose: The purpose of the study was to provide deep understanding of the reported experiences of families with their loved one in the intensive care unit (ICU), focusing on interactions with healthcare providers. Methods: The data were collected by individual interviews of eleven participants. The transcribed data were analyzed using qualitative content analysis to identify major themes and sub-themes that represented the experiences of families. Results: Five themes and 13 sub-themes emerged. "Captive of patients' delayed death: Fear and anxiety" describes psychological problems arising when the family member became critically ill enough to warrant being admitted to the ICU. "Families as the weak: Suppression and resistance" describes interpersonal difficulties arising due to lack of information and trust with healthcare providers. "Deprivation of authority and duty as families: Helplessness" illustrate situational barriers in attempting to protect and support family member. "Re-establishment of trust relationship with healthcare providers: Gratitude and appreciation" describes how they satisfied with themselves by regaining trust relationship. Lastly, "Acceptance of reality through direct care participation: Relief and peace" illustrates peace of mind by gaining sense of reality through active direct care participation. Conclusion: This study demonstrates the positive and negative experiences of families with ICU patients. The results will be useful in developing family-centered nursing interventions.

Myths and truths about pediatric psychogenic nonepileptic seizures

  • Psychogenic nonepileptic seizures (PNES) is a neuropsychiatric condition that causes a transient alteration of consciousness and loss of self-control. PNES, which occur in vulnerable individuals who often have experienced trauma and are precipitated by overwhelming circumstances, are a body's expression of a distressed mind, a cry for help. PNES are misunderstood, mistreated, under-recognized, and underdiagnosed. The mind-body dichotomy, an artificial divide between physical and mental health and brain disorders into neurology and psychiatry, contributes to undue delays in the diagnosis and treatment of PNES. One of the major barriers in the effective diagnosis and treatment of PNES is the dissonance caused by different illness perceptions between patients and providers. While patients are bewildered by their experiences of disabling attacks beyond their control or comprehension, providers consider PNES trivial because they are not epileptic seizures and are caused by psychological stress. The belief that patients with PNES are feigning or controlling their symptoms leads to negative attitudes of healthcare providers, which in turn lead to a failure to provide the support and respect that patients with PNES so desperately need and deserve. A biopsychosocial perspective and better understanding of the neurobiology of PNES may help bridge this great divide between brain and behavior and improve our interaction with patients, thereby improving prognosis. Knowledge of dysregulated stress hormones, autonomic nervous system dysfunction, and altered brain connectivity in PNES will better prepare providers to communicate with patients how intangible emotional stressors could cause tangible involuntary movements and altered awareness.

Analysis of YouTube Content on Oral Disease Information about the Elderly

  • Background: The elderly have, a higher disease morbidity than other age groups due to a decrease in resistance to the disease and have complex diseases, so care should be taken. Accordingly, it is considered important to provide information for improving the health of the elderly. Health information plays an important role in individual health promotion and education, so the degree of exposure to information about oral health of the elderly is expected to have a significant impact on understanding and acquiring information on oral content videos on the importance, prevention, and management of oral health of the elderly in the future. Methods: This study analyzed video content related to oral diseases of the elderly in a total of 150 videos uploaded on YouTube from January 1, 2012 to May 13, 2021, using a total of three books of dental hygiene for the elderly. Results: Forty-nine broadcasters accounted for the most of this information. Among the information providers, there were two dental hygienists. They accounted for 1.3% of all the information providers. The highest number of dental hygienists who broadcasted information was 42 in 2019. The average number of views was 37,303 periodontal diseases, the highest. Among the videos, dry mouth was the most common with 34 oral diseases. Conclusion: The number of images for each disease varies, so it seems that information should be provided in various ways. Dental hygienists should widely improve oral health knowledge by providing various dental hygiene management images for each oral disease to improve the oral health of the general public. In addition, based on the information of the Health Insurance Review and Assessment Service, the development and provision of content should be actively carried out so that people can obtain the information they desire.

의료인 기본심폐소생술 교육이 보건계열 대학생의 지식과 태도에 미치는 효과 (The Effects of Basic Life Support Course for Healthcare Providers on the Knowledge and Attitude of Health Science College Students)

  • 본 연구는 의료인 기본심폐소생술 교육이 보건계열 대학생의 지식과 태도에 미치는 효과를 검증하기 위해 시도된 비동등성 대조군 전후 유사 실험연구이다. 연구 대상은 D광역시에 소재한 D대학에 재학 중인 보건계열 학생 140명으로 하였고, 자료수집기간은 2010년 8월 3일부터 8월 29일까지였다. 연구 도구는 심폐소생술 지식과 태도를 사용하였고, 자료분석은 SPSS/PC 12.0 win을 사용하여 실수와 백분율, 평균과 표준편차, ${\chi}^2$ test, Fisher's exact test, independent t-test, paired t-test, ANCOVA를 이용하였다. 결론은 의료인 기본심폐소생술 교육을 받은 실험군은 대조군보다 지식점수가 유의하게 높았다(F=47.22, p<.001). 의료인 기본심폐소생술 교육을 받은 실험군은 대조군보다 태도점수가 유의하게 높았다(F=40.20, p<.001). 이상의 결과를 볼 때, 의료인 기본심폐소생술 교육은 보건계열 대학생의 지식과 태도에 긍정적인 효과가 있음을 확인할 수 있었다. 따라서 보건계열 학과 교과과정에 심폐소생술 관련 교과목 개설을 검토해 볼 것을 제언하며, 보건계열 대학생을 위한 체계적이고 표준화된 교육 프로그램 개발이 필요할 것으로 사료된다.

약사의 전문성강화를 위한 국가별 평생교육제도 비교연구 (A Comparative Study on the Continuing Professional Development for the Pharmacists in Korea)

  • The purpose of this study is to provide the comparative study results for the purpose of the developing continuing professional educational systems for Korean pharmacists. The professional continuing educational systems of the UK, USA, Japan and Korea were analyzed. General Pharmaceutical Council in UK controls the pharmacy professions for the protection of the health of the citizens in UK and certifies the schools of pharmacy and the continuing education providers. The USA and Japan have the several accreditation bodies for the pharmacy education and the continuing pharmaceutical education. However, the quality assurance systems in Korea for the continuing education and specialty programs are not implemented, yet. The renewal system of the pharmacist license should be introduced like the health care providers like doctors to improve professionalism. Finally, it is recommended that all of these things should be done by a single administrative authority such as Korean Accreditation Council for Pharmacy Education.
