  • Background : Utilization review has been adopted as a vehicle for cost and utilization control of health care services. Its role was further stressed and expanded through the establishment of Health Insurance Review Agency in 2001. This article is to introduce concept, activities, and effect of utilization review based on the experiences of U.S. and to suggest important characteristics for ideal utilization review activities at the national level in Korea. Method : Twenty-five articles related with utilization review were reviewed after being selected through web site search through Med Line and Richis. Result : Utilization review was introduced mainly for health care expenditure control either by insurer, provider or the third parties under the pressure of increasing health care cost. It's activities can be categorized to prospective, concurrent and retrospective review according to the time of service provision. Based on most of studies, utilization review has been effective in controling rising health care cost and utilization. However it's effectiveness assumes a reimbursement structure of managed care like capitation payment. More worse, it is still unknown it's effectiveness on quality of care. Conclusion : Utilization review should be employed to increase the cost effectiveness of medical care by optimizing quality and patient's outcomes while also attempting to reduce the use of resources. So, it should consider outcomes before expenditures, check for both under and over-use, and construct an structure in which consumption is reduced equitably. Aggressive adoption of utilization review in Korean health care setting with fee-for-service reimbursement structure might not be a cost-effective approach before adoption of prospective payment system such as D.R.G. and capitation.

  • In a wireless sensor network (WSN) setting, this paper presents a distributed decision-making framework and illustrates its application in an online structural health monitoring (SHM) system. The objective is to recover a damage severity vector, which identifies, localizes, and quantifies damages in a structure, via distributive and collaborative decision-making among wireless sensors. Observing the fact that damages are generally scarce in a structure, this paper develops a nonlinear 0-norm minimization formulation to recover the sparse damage severity vector, then relaxes it to a linear and distributively tractable one. An optimal algorithm based on the alternating direction method of multipliers (ADMM) and a heuristic distributed linear programming (DLP) algorithm are proposed to estimate the damage severity vector distributively. By limiting sensors to exchange information among neighboring sensors, the distributed decision-making algorithms reduce communication costs, thus alleviate the channel interference and prolong the network lifetime. Simulation results in monitoring a steel frame structure prove the effectiveness of the proposed algorithms.

For the study, the relevant articles of 4 daily newspapers published from 1st of January in 1960 to 28th of February in 2010 were collected and analyzed. The results of the study were as follows: First, the articles about the health characteristics and the elements of Han-Ok have rapidly grown since the year of 2000. Especially, the residents' interviews, case reports and actual living condition of Han-Ok recently tend to increase. Second, the health characteristics of Han-Ok in the recent articles were focused more on the mental and social health than on the physical health. The physical health had the characteristics of the block of hazard environments, the offer of good quality environments, and the enhancements of residents' health. The mental health consisted of the psychological comfort, the sensory satisfaction and the emotional growth. There have been improvements of family relationships and the interactions with neighbors in the characteristics of social health since 2000. Third, the health elements of Han-Ok in terms of architecture and interior design were the structure, space, material, doors & windows, furniture & props, nature, and villages. The material elements were more stressed before 2000, but the spaces and the nature elements were highly focused after 2000 and the structure elements were steadily high regardless of years. Fourth, of the health elements, the structure and the material had the high effects on the physical health characteristics, on the other hand the spaces and the nature influenced more on the mental health. On-dol, Ma-dang, Dae-cheong, Ma-ru, soil, wood, Changhoji, natural environments, and doors & windows were the detail health elements which had high effects on the health characteristics of Han-Ok. For the future study, the experience based in-depth studies of the residents living in Han-Ok are suggested to grasp the actual effects of Han-Ok on the health. The professional consultations are also needed for the extracts of the health characteristics of Han-Ok and the functional elements which can be applied to the modern buildings.

  • The purpose of this study was to test the revised Health Promotion Model of Pender and to determine the factors to promote health behavior for adolescents' smoking behavior. The subjects of the study was 783 boys of 4 high school students. among 39. schools locating in Daejeon metropolitan city. The data was collected from July 1st to 15th. 1997 by school health nurse The research tool were HPLP of Walker. Pender. General self-efficacy scale of Sherer. control scale was measured by subconcept of hardiness scale of Pollock. and perceived barrier. perceived benefit. activity-related-affect tool were made by researcher via literature review The data were analyzed by SAS program using frequency. t-test. ANOVA. Schefee test. regression. The results were as follows 1. The mean of total health promoting behavior was $2.27\pm.35$. Among sub domain of health promoting behavior, the highest score was interpersonal support$(2.72\pm.60)$. and the lowest was health responsibility $(1.58\pm.44)$. 2. There were statistically significant difference in total health promoting behavior according to religion. parenting style. school performance. girl friend. father's smoking of individual characteristics. 3. The socioeconomic status. smoking, parent pattern. family structure of individual characteristics and experience domain associated with perceived benefit. perceived barrier. activity-related affect. interpersonal influence of behavior-specific cognition and affect domain. The perceived barrier. self-efficacy. girl friend and father's smoking of interpersonal influence. and control explained $25.8\%$ of variance of health promoting behavior. From above results school health nurse has to emphasize on health responsibility for health promotion of adolescent. But they couldn't intervene for parent pattern. socioeconomic status. family structure of individual characteristics and experience domain. it could be possible for school health nurse to promote health of adolescents through improving perceived barrier. also develop program to increase self-efficacy and through parent health class for fathers. Above results point to the importance of including parents in smoking prevention effort targeting adolescents. Because increasing control also promotes health of adolescents. it should be studied further about the specific measure. To verify the variables for increasing the fitness of health promoting model. it needs further replication of the research.

  • The purpose of this study was to provide the necessary fundamental data in composing a systematic text content of the public health by analyzing each text, domain, and teaching contents suggested in the textbooks as well as teachers' guides of the 7th elementary school education curriculum, while the study subjects were as follows. 1. The health education content suggested in the 7th physical education curriculum were analyzed and examined. 2. The health teaching content of each textbook in the 7th elementary school curriculum was to be analyzed and examined. In order to resolve the above research issues, the physical, spiritual, and social domain along with the (1) Proper living habit, (2) Health and nutrition, (3) Sex education, (4) Prevention of the sense-organic diseases, (5) Cleanliness of food, (6) Oral hygiene, (7) Individual health and public health, (8) Safety in living, (9) Abuse and usage of medication, educational content suggested in the 7tand (10) Environment pollution focused around the health of the elementary school education curriculum was analyzed and its outcome was as below First, compared with the 6th elementary school education curriculum, the health content suggested in the 7th elementary school education curriculum was decreased. Second, although each grade's teaching content of the health domain in the physical education was considered in its structure following after the according systems, they were preponderant in partial subjects such as the safety in living, nutrition, proper living habit, sport, and health in sport. oo. Third, the health education content was organized in 4 units such as the physical growth and development, prevention of diseases, safe living, and leisure living(leisure, spiritual health, and etc.) for the 3rd and 4th grade. Then, as for 5th and 6th grade, it was organized in 3 units such as the understanding the human body, prevention of disease, and leisure and safe living. Fourth, in the physical educational health domain, a strong point was constructed within the physical, spiritual, and social areas of the elementary school physical education. Fifth, the number of the public health education contents directly related with the health education was 43 as with 25 indirect contents. Sixth, each grade's domain unit structure of the public health content was heavy upon the physical and social area throughout every grade while in opposite, the spiritual domain' s unit structure was weak. In according to each grade, the physical domain was stressed in 4, 5, and 6 grades while the social domain was stressed in 1, 5, and 6 grades.
