보건소 작업치료사의 지역사회 재활 및 보건서비스 제공에 대한 업무 경험에 관한 현상학적 연구 (The Lived Experience of Occupational Therapists in Public Health Centers Regarding Community Rehabilitation and Health Services: Phenomenological Study)

  • 목적 : 본 연구는 보건소에 근무하는 작업치료사들의 지역사회 재활 및 보건서비스 제공에 관한 업무 경험에 대한 이해를 통해 향후 건강한 지역사회를 만들기 위해 보건소 작업치료사들의 역할 및 업무들을 알아보고자 하였다. 연구방법 : Giorgi의 현상학적 연구방법론을 적용하여, 지역사회 내 근무하는 보건소 작업치료사 6명에게 심층 인터뷰를 진행하였다. 사전 동의를 통해 대화 내용은 녹음되었고, 녹음된 자료는 패턴화된 반응이나 주제를 분석하고자 Colaizzi의 주제 분석법(Thematic analysis) 7단계에 따라 분석하였다. 결과 : 분석 결과, 4개의 범주와 16개의 주제, 24개의 하위요인을 도출하였다. 보건소 내 작업치료사가 주로 행하는 업무 영역 및 역할은 대상자 평가, 방문재활치료, 치매관리사업, 지역사회 연계사업으로 나타났다. 그 외 작업치료사가 느끼는 건강한 지역사회를 만들기 위해 향후 보건소에서 필요한 중재는 사례관리 및 프로그램 매니지먼트, 다양한 보상적 접근방법 적용, 건강한 생활습관을 위한 중재로 나타났다. 보건소 근무 시 작업치료사들이 경험하는 어려움으로는 작업치료사의 인력 부족, 고용 불안정으로 인한 사기저하, 실적 위주의 행정 정책으로 인한 업무의 부담감, 전문인력 간 소통의 부족으로 나타나는 업무 효율성 저하가 나타났다. 반면에 보건소에서 근무하는 작업치료사들이 느끼는 보람으로는 치료의 자율성 보장을 가장 큰 장점으로 응답하였다. 보건소 작업치료사의 역량 강화를 위해 필요한 교육으로는 보건행정, 보건소 실습으로 나타났다. 결론 : 본 연구결과는 현재 보건소 및 치매 안심 센터의 필수인력으로 자리 잡은 작업치료사들의 향후 업무 범위 및 역할과 관련된 가이드라인과 지역사회 공공서비스 내 역량 있는 작업치료사를 양성하기 위한 교육의 기초자료로 활용될 수 있을 것이다.

남자 청소년의 식행동 패턴에 따른 간식 섭취, 생활 습관 요인 및 비만과의 연관성 연구 (The association of snack consumption, lifestyle factors, and pediatric obesity with dietary behavior patterns in male adolescents)

  • 본 연구는 만 15~19세 남자 고등학생 902명을 대상으로 식행동, 간식 섭취, 식환경, 생활 습관에 대해 설문 조사를 실시하였고, 비만 판정을 위해 신장과 체중을 측정하여 식행동 패턴에 따른 간식 섭취 및 기타 요인과 비만과의 연관성을 분석하였다. 1) 청소년 식생활 지침의 항목을 이용하여 식행동을 평가한 결과 대상자들은 3가지 패턴으로 분류되었다. 식생활과 생활 습관 모두 건강한 패턴과 식생활과 생활 습관 면에서 건강한 요소와 비건강한 요소가 혼재되어있는 혼합 패턴, 그리고 마지막으로 식생활과 생활 습관이 전반적으로 바람직하지 않은 비건강 패턴이었다. 2) 각 패턴 별로 간식 섭취가 차이를 보였는데, 건강한 패턴은 과일, 우유, 호상요구르트의 섭취 빈도가 높았고, 혼합 패턴은 과일이나 호상요구르트 섭취 빈도가 높은 반면 라면, 아이스크림, 탄산음료, 사탕 섭취 빈도도 같이 높았으며, 비건강 패턴은 과일, 우유, 호상요구르트의 섭취 빈도가 다른 두 패턴에 비해 가장 낮은 반면 라면, 단 간식, 탄산음료 등의 간식 섭취 빈도는 가장 높았다. 3) 식환경을 포함한 생활 습관 요인도 패턴마다 상이했는데, 가정 내 식품 비치율은 각 식품 섭취 빈도와 유사하게 건강한 패턴과 혼합 패턴의 가정 내 과일 비치율이 비건강 패턴에 비해 높았다. 또한 혼합 패턴이 건강기능성 식품이나 식이 보충제 복용 비율이 가장 높았고, 비건강 패턴은 식사 속도, 수면 시간은 짧고 스크린 시간이 하루 2시간 이상인 비율이 가장 높았다. 4) 패턴 별 비만과의 연관성은 건강한 패턴을 기준으로 하였을 때, 혼합 패턴의 교차비는 1.11이었으나 유의하지 않았고, 비건강 패턴의 교차비는 1.88로 유의적이었다. 이상의 결과로부터 청소년의 식행동 패턴에 따라 간식 섭취 뿐 아니라 식습관, 생활 습관 요인이 차이를 보였으며 이러한 요인들이 비만과의 연관성에도 영향을 주었다. 식행동은 식품 섭취나 생활 습관 요인들과 서로 상호작용을 하므로 앞으로 청소년 비만 예방 및 관리를 위한 전략으로 적절한 영양 교육과 함께 식행동과 생활 습관을 함께 개선해 나갈 수 있는 방안을 모색하는 것이 필요하다.

남자 고등학생의 결핵에 대한 지식, 태도 및 예방행위에 관한 연구 (A Study on Knowledge, Attitude and Prevention Behavior of Tuberculosis in Male High School Students)

  • 본 연구는 남자 고등학생의 결핵에 대한 지식, 태도, 예방행위를 파악하고, 예방행위에 영향을 미치는 요인을 파악하여 남자 고등학생의 결핵에 대한 예방행위 향상을 위한 프로그램을 개발하는데 기초 자료를 제공하기 위해 시도되었다. 본 연구 결과 남자 고등학생의 결핵에 대한 지식은 30점 만점에 11.92점, 태도는 60점 만점에 28.32점, 예방행위는 60점 만점에 30.07점으로 점수가 낮았으며 이들의 관계는 모두 정적 상관관계를 나타냈다. 또한 결핵에 대한 예방행위에 영향을 미치는 요인으로는 운동을 매일 하는 경우, 수면시간이 충분한 경우, 주관적 건강상태가 건강한 상태 순으로 나타났다. 이상의 연구 결과를 토대로 남자 고등학생의 결핵에 대한 예방행위를 향상시키기 위해서는 결핵에 대한 태도를 긍정적으로 변화시키는 것뿐만 아니라 건강한 생활습관 형성을 위한 실습 위주의 프로그램 개발이 우선적으로 필요하다고 사료된다. 본 연구의 제한점은 S시와 Y시 고등학교에 다니고 있는 1학년 남학생을 대상으로 편의 표집 하여 연구의 결과를 일반화시키는데 신중을 기할 필요가 있다. 따라서 추후 지역 확대와 학년별, 학교 특성별, 성별 등으로 나누어 비교 연구를 하는 것을 제언하고 이러한 연구 결과를 토대로 고등학생의 결핵에 대한 예방행위를 향상시킬 수 있는 교육 요구도 조사를 바탕으로 프로그램을 개발하여 그 효과를 검증하는 것을 제언한다.

Trends in adherence to dietary recommendations among Korean type 2 diabetes mellitus patients

  • BACKGROUND/OBJECTIVES: The current study examined trends in adherence to dietary recommendations and compared the levels of adherence between diagnosed and undiagnosed subjects with type 2 diabetes mellitus (T2DM) in Korea over the past 14 years. SUBJECTS/METHODS: Data were collected from the 1998-2012 Korea National Health and Nutrition Examination Surveys (KNHANES). Diagnosed diabetes was defined as giving a positive response to questions about awareness of the disease, a physician's diagnosis of diabetes, or medical treatment for diabetes, whereas undiagnosed diabetes was defined as having a fasting glucose level ${\geq}126mg/dl$. Assessment of adherence level was based on 6 components of dietary guidelines, considering meal patterns and intake levels of calories, carbohydrates, vegetable/seaweed, sodium, and alcohol. The participants received 1 point if they met the criteria for each of the 6 components, and the total possible score ranged from 0 to 6 points. Multivariate generalized linear regression was performed, taking into account the complex survey design. RESULTS: Among all diabetic patients aged 30 years or older, the proportion of diagnosed diabetes increased dramatically, from 40.9% in 1998 to 75.9% in 2012 (P for trend < 0.001). The overall adherence levels to dietary recommendations were low and did not significantly differ between diagnosed and undiagnosed subjects with T2DM for all survey years. Several improvements were observed, including increased adherence to maintaining sufficient vegetable/seaweed consumption (increased from 0.12 to 0.16 points) and limiting sodium intake (increased from 0.12-0.13 points to 0.19-0.24 points; P for trend < 0.001), while adherence to maintaining moderate alcohol consumption decreased. CONCLUSIONS: Analysis of data collected by the KNHANES indicates that Korean T2DM patients have poor adherence to dietary recommendations and maintenance of a healthy lifestyle, regardless of disease awareness. This finding suggests that development of practical, evidence-based guidelines is necessary and that provision and expansion of educational programs for T2DM patients is critical after diagnosis.

Structural relations in the effect of convenience food satisfaction and quality of life according to dietary style - Comparative study of singles in metropolitan area of Korea, Japan and China -

  • BACKGROUND/OBJECTIVES: Due to recent changes in social structure, single-person households are on the rise worldwide and therefore the interest in the quality of life of singles is increasing, but the research on the relationships related to their diet-related quality of life is scarce. This research analyzes the structural relations in the effect of convenience food satisfaction and quality of life according to dietary style of Korean, Japanese and Chinese singles. SUBJECTS/METHODS: The targets of this study were Korean, Japanese, and Chinese singles, identified as adults between the ages of 25 and 54, having economic capabilities or working and living alone, either legally or in actuality having no partner. A statistical analysis was conducted using SPSS12.0 for Windows and SEM using AMOS 5.0 statistics package. The reliability of these findings was supported by a Cronbach's alpha coefficient of 0.6 and higher for all the factors. In an attempt to study the level of satisfaction with convenience food in accordance to dietary style and the quality of life of singles, a structural equation model was constructed and analyzed. RESULTS: Of the three countries, regarding the effect of dietary style on singles' quality of life, the convenience-oriented style was found to have a negative effect on Korean singles' overall quality of life, but a positive effect on Japanese singles' overall quality of life, and a negative effect on Chinese singles' positive psychological aspect of quality of life. In addition, although Chinese singles have a high level of interest in health, they have an overall high level of satisfaction regarding fast food and its nutritional value. CONCLUSIONS: The number of singles in Korea, Japan, and China has been consistently increasing in recent years, and there is a need for continuous interest in their healthy dietary lifestyles in terms of convenience, economy, and taste.

한의학 전공 대학생과 식품영양학 전공 대학생의 임상 영양지식과 영양 실천도 비교 (A Comparison of Clinical Nutrition Knowledge and Nutritional Behavior of College Students Majoring in Korean Oriental Medicine vs. College Students Majoring in Food and Nutrition)

  • This study examined two groups of students Korean Oriental Medicine majors and Food and Nutrition majors and compared their knowledge of clinical nutrition as well as their tendency to follow healthy lifestyles. In all, 204 college juniors and seniors majoring in Food & Nutrition, and 324 sophomores majoring in General Korean Oriental Medicine were given a questionnaire. Overall, knowledge of clinical nutrition(out of twenty questions) was significantly higher among the students majoring in food and nutrition (p<0.01). Specifically, these students obtained a higher score in the questions pertaining to the following topics: nutritional support(p<0.05), the relationship between menopause and blood cholesterol(p<0.01), the relationship between weight control and carbohydrates(p<0.01), the relationship between gout and protein(p<0.10), the relationship between hyperlipidemia and carbohydrates(p<0.01), the relationship between trans fatty acids and atherosclerosis(p<0.01), the relationship between blood potassium levels and kidney disease(p<0.01), and the relationship between weight control and diet and water(p<0.05). Conversely, students studying General Korean Oriental Medicine had a better knowledge of the relationship between malnutrition and the mortality rate and health complications(p<0.01). The questions relating to a healthy diet and lifestyle indicated that students majoring in Food and Nutrition had higher incidences of diet irregularity and a higher missing rate (p<0.01); they were more interested in calorie content and nutrients in their diet(p<0.05); and they were more likely to study about nutritional knowledge during their leisure time (p<0.01). The results also indicated, however, Korean Oriental Medicine students consumed proportionally more vegetables(p<0.05); and they were more likely to take the stairs than the elevator(p<0.01).

Dietary intakes and obesity of Malaysian adults

  • BACKGROUND/OBJECTIVES: Over the past few decades, Malaysia has been experiencing an increased prevalence of overweight and obesity that threatens the health of Malaysians. Poor dietary intake is one of the major contributors to the development of obesity and many non-communicable diseases. The dietary intakes of adults in Malaysia were assessed to determine the association between the dietary intake variables and the body mass index (BMI) and waist circumference (WC) measurements. This study examined whether the dietary intake achieved the recommended nutritional guidelines and compared the intakes between both sexes. SUBJECTS/METHODS: The height, weight, and WC of four-hundred-and-ninety adults (n = 490) in Malaysia were measured using standard procedures. The three-day 24-hour dietary recalls were conducted on 422 out of the 490 adults and their dietary intakes were evaluated in detail. The selected dietary intake variables were used to determine the associations with the obesity indicators. RESULTS: Among the participants, 52.8% were overweight or obese. After data analysis, the mean energy intake was 1,550 kcal/day, in which male participants had a significantly higher energy and macronutrients intake than females. Protein consumption and its percentage of energy contribution exceeded the recommended range. The consumption of fruits, vegetables, and milk and milk products were lower than the recommended number of servings for a healthy diet. The male participants consumed significantly more servings of carbohydrate-based foods, meat, and fats, oils, and sweets than females. Among the selected dietary intake variables, only the carbohydrate intake was negatively associated with the BMI (Estimate b = -0.008) and WC measurements (Estimate b = -0.019) after adjusting for covariates. CONCLUSIONS: This study evaluated the dietary intakes of a sample of Malaysian adults and its association with the obesity indicators. The results highlight the need for improvements and modifications of the dietary intake of Malaysians to reduce the overweight and obesity rates.

  • The Committee on Pediatric Obesity of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition newly developed the first Korean Guideline on the Diagnosis and Treatment of Obesity in Children and Adolescents to deliver an evidence-based systematic approach to childhood obesity in South Korea. The following areas were systematically reviewed, especially on the basis of all available references published in South Korea and worldwide, and new guidelines were established in each area with the strength of recommendations based on the levels of evidence: 1) definition and diagnosis of overweight and obesity in children and adolescents; 2) principles of treatment of pediatric obesity; 3) behavioral interventions for children and adolescents with obesity, including diet, exercise, lifestyle, and mental health; 4) pharmacotherapy; and 5) bariatric surgery.

  • The Committee on Pediatric Obesity of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition newly developed the first Korean Guideline on the Diagnosis and Treatment of Obesity in Children and Adolescents to deliver an evidence-based systematic approach to childhood obesity in South Korea. Effect of nutrition education and traditional game-based physical activity interventions, on lipid profile improvement in overweight and obese children in West Java Indonesia

  • BACKGROUND/OBJECTIVES: One ongoing effort to curtail dyslipidemia in school children is through lifestyle intervention. This study analyzes outcomes of the intervention of nutrition education and Javanese traditional game-based physical activity, on lipid profiles of overweight and obese children. SUBJECTS/METHODS: The experimental study consisted of three steps: data collection (October to December 2016), intervention processes (January to March 2017), and final report preparation (April to May 2017). This was a split-plot block study, enrolling a total of 72 subjects. Traditional game intervention (12 meetings) and nutrition education (9 meetings) was carried out within 3 months with material related to obesity. RESULTS: Our results showed no decrease in the levels of total cholesterol in the group receiving nutrition education (rise by 1.56 mg/dL), but when compared to subjects without intervention, total cholesterol levels experienced was increased. The group which did not receive physical activity showed increase in the total cholesterol, whereas the traditional game intervention group showed a decrease of 0.05 mg/dL total cholesterol at the end of the intervention. Nutrition education and physical activity interventions did not influence any reduction in the low-density lipoprotein cholesterol (LDL-C) levels, but rather showed a tendency to increase at the end of the intervention. However, the increase in LDL-c levels in the traditional game group was lower than in other intervention groups. Duncan's test indicated that the effect of nutrition education and physical activity interventions did not differ from the cut-off values of LDL-C and high-density lipoprotein cholesterol. CONCLUSIONS: Total cholesterol and triglyceride levels in the traditional game group tended to decrease at the end of the intervention, but the results did not differ much from other intervention groups. We deduce that nutrition education and traditional game-based physical activity interventions are not capable of improving blood lipid profiles in overweight and obese children within 3 months. It is necessary to increase the time of physical activity intervention to maintain the lipid profile in another study.
