병원의 특성에 따른 시장지향성 분석과 성과에 있어서 환경의 조절 역할 (The Degree of Market Orientation by Type and Size of Hospital, and Moderating Effect of Environments on Performance)

  • This study was to examine the degree of market orientated effort by hospital type and size,. and analyzed the moderating effect of environments between market orientation and it's performance. First, there was no significant differences by hospital type, such public hospital, university hospital, and private hospital. However, the orders of market orientation implementation was university hospital. private hospital, and public hospital This finding suggest that university hospital and public hospital, should be market-oriented, or customer-oriented more now, relatively. Second, there were no significant differences in marketing as management policy by hospital types, i.e. public, university or private sector hospitals. However, as to the intensity of the implementation of the market-oriented approach, private hospitals came first, followed by the university hospitals, then the public ones. This finding suggests that university and public hospitals should be more market or customer oriented to compete in the market. Third, only the competitive environment does active moderating roll on hospital performance. The competitive environmental factor forces the organization to be more market oriented. This means the more you are positively situated in the competitive environment, the more you are looking for a market oriented approach. You become more aware of the value of customers and you become more focused on the customer satisfaction and thus become more patient oriented in every facet of decision making. And market oriented hospitals recognize the importance of utilization of high tech medical treatment skills and equipment in patient care. Fourth, according the analysis, market oriented effort gives more influence on hospital performance than the environmental factors. This means that the market oriented effort should become a culture of the hospital which seeks to out distance themselves from its competitors.

환자중심서비스를 위한 온톨로지 기반의 u-Healthcare 시스템 (Ontology-based u-Healthcare System for Patient-centric Service)

  • U-Healthcare는 홈 네트워크, 휴대용 장치 등에 기반한 정보통신기술과 의료시스템이 서로 융합되어 개인의 생체정보 등을 실시간으로 모니터링하고, 자동으로 병원 및 의사와 연결되어 시공간의 제약을 줄임으로써 언제 어디서나 건강을 관리하고 질병을 예방하는 새로운 형태의 의료서비스이다. 본 논문에서는 진료 중심에서 예방 중심으로 변화되어가고 있는 최근의 U-Healthcare 시스템의 기술 발전 추세에 맞추어 조기 대응이 가능한 Healthcare 정보시스템 구축을 위한 요구분석 사항들에 대해 정리하고, 이를 기반으로 u-Healthcare의 실현을 위한 기존의 단위 시스템인 PACS, OCS, EMR, 응급의료시스템을 통합한 환자중심의 클라이언트 시스템을 설계한다. 특히, 온톨로지는 특정분야의 정보 모델에 이용되어 그 분야에서 공통의 어휘를 제공하고, 그 용어의 의미와 용어간의 관계를 다양한 수준의 형식성을 가지고 제공한다. 본 논문에서는 이러한 온톨로지 및 무질서한 데이터에 대한 관계를 정의하고, 보다 체계적으로 데이터를 군집화하는 클러스터링의 개념을 포함한 환자중심의 서비스를 위한 온톨로지 기반의 시스템을 제안한다.

간호사만족이 병원성과에 미치는 영향 -환자만족과 재방문의향, 타인추천의향 중심으로- (The Effects of Nurses' Satisfaction on Hospital Performance -Focused on the Patient Satisfaction and Revisit Intention, Recommendation Intention-)

  • 본 연구는 이론적 고찰을 기초로 하여 대학병원의 간호사만족도, 간호사에 대한 환자만족도, 병원에 대한 환자만족도, 환자의 병원재방문의향 및 타인추천의향 간의 가설적 구조모형을 구축하고, 모형의 적합성과 경로의 유의성을 확인하였다. 연구대상은 5개 대학병원의 간호사 2,079명과 해당 병원의 의료서비스를 받은 6,776명의 환자이다. 연구결과 가설적 모형은 전체적인 적합지수를 고려하였을 때 적합한 모형으로 판단되었으며, 간호사의 만족도가 환자의 간호사에 대한 만족도에 미치는 영향, 환자의 간호사에 대한 만족도가 환자의 병원에 대한 만족도에 미치는 영향, 환자의 병원에 대한 만족도가 병원재이용의향과 타인추천의향 미치는 영향 등의 4개 경로가 유의한 것으로 나타났다. 본 연구결과는 병원의 경영자와 관리자들이 간호사 만족도를 병원성과의 주요 요소로 포함하여 고객만족전략을 실천할 수 있는 기초자료로 활용될 수 있을 것으로 기대된다.

전업주부들의 두피모발 관리에 대한 인식 및 태도 (Perceptions and Attitudes of Housewives on Scalp and Hair Management)

  • In this study, "housewives" are defined as married women who take care of housework mostly and do not work outside of home. Changing family types and higher education level of housewives enable them to be more interested in heath, beauty, and appearance. Increased interest and more frequent visits to hair salons cause scalp damage as well as hair damage, which results in hair loss. Moreover, some women suffers from scalp and hair damage due to childbirth, but many of them are not aware of it. Therefore, this study tries to find out the following; housewives' perceptions on hair and scalp management, and how such perceptions are formed. In addition, this study will present the direction for beauty marketing taking into account the characteristics of housewives and provide data necessary for them to keep their hair and scalp healthy. For these purposes, a survey was carried out after modifying the questionnaire through a presurvey. The survey tried to find 1) perceptions on hair and scalp management according to demographic characteristics, and attitude toward hair and scalp management according to demographic characteristics. Data were analyzed using frequency analysis, cross tabulation, t-test, factor analysis, and Analysis of Variance. The results were as follows, most recognize the importance of hair and scalp management but do not receive management because of the high cost, most of them receive information about hair and scalp management from people around them, and the higher the education level and health status, the more active the attitude toward hair and scalp management. These differences were all statistically significant.

Zigbee와 GPS를 이용한 해녀 사고예방 스마트 모니터링 시스템 개발 (Development of Accident-prevention Smart Monitoring System for Woman Diver using Zigbee Module and GPS Sensor)

  • 본 논문에서는 Zigbee와 GPS센서를 이용한 해녀 사고예방 스마트 모니터링 시스템을 제안하였다. 이 시스템은 해녀들이 바다속에서 조업중에 있을 때, 물질하는 위치, 체온, 잠수의 깊이, 잠수 시간 등의 정보를 수집하여 안전사고 발생시 곧바로 대처할 수 있다. 본 연구에서는 해녀의 상태 및 물질 활동 정보 측정을 위한 스마트테왁과 스마트물안경을 개발하였다. GPS 수신이 가능한 스마트테왁은 스마트물안경과 Zigbee통신으로 측정된 데이터를 IHSS(IoT based Haenyeo Safety service Software)의 서버로 전송한다. 또한, IHSS는 반응형 웹으로 해녀의 위치 정보와 상태 정보를 스마트폰으로 서비스할 수 있다. 결론적으로 이 시스템은 해녀의 건강관리 및 안전 측면에서 매우 유용할 것이며, 유네스코 세계 무형문화유산 등재 기반 등 제주해녀의 세계화 마케팅에도 크게 기여할 것이다.

헬스케어 ICT 서비스의 글로벌 컨버전스 (Global Convergence for Healthcare ICT Services)

  • 해외 의료서비스 시장의 메가 트렌드는 글로벌 융합, 인접 분야와의 융합, ICT기술의 적극적 도입, 공공과 민간의 파트너십(Public Private Partnership)을 통한 혁신 4가지로 요약될 수 있다. 의료서비스는 더 이상 local industry가 아니며, Global Convergence 되어가고 있다. 선진국의 경우 소득 수준의 증가, 새로운 의학기술의 발전, 전문화된 의료서비스의 증가, 인구 고령화 등 의료 수요가 증가하면서 외국 의료진의 이주 증가하고 있으며, 지리적 근접성이나 비용에 상관없이 최고 의료기술에 대한 선택 증가하고 있다. 고품질이면서 상대적으로 가격이 낮은 외국 의료서비스에 대한 수요 증가하며 특히 미국 JCI등 국제적 병원인증 기준의 확산되고 있다. 병원 수출은 관련 기술을 더욱 더 효율적으로 융합해서 수출 할 수 있는 'ICT 융합병원' 수출 산업화의 길을 열었다고 평가되고 있다. 현재 국내의 병원은 이미 포화상태에 이르렀고, 국내병원들의 세계화가 필요한 시점임. 따라서 국가별로 전략을 달리하고, 기술 이전뿐만 아니라 건물마련, 의료장비 구매, 현지 의료인력(의사 및 간호사) 선발 및 교육훈련, 홍보마케팅 등 토탈 수출도 가능하다. 이에 공공성 유지와 해외 진출을 위해 현행 의료법이 전향적으로 개정될 필요가 있으며, 해외 의료서비스의 국내법 적용에 보다 신축성 있는 법적용과 더 나아가 적극적 정책지원이 필요하다.

간호학사 편입학제도의 교과과정 비교분석 (Comparative analysis of RN-BSN Program in Korea and U. S. A.)

  • In response of the increasing demand for professional degree in nursing, some university in Korea offers RN-BSN program for R. N. from diploma in nursing. However, RN-BSN program in Korea is in formative period. Therefore, the purpose of this survey study is for the comparative analysis of RN-BSN curriculum in Korea and U.S.A. In this study, subjects consisted of 18 department of nursing in university and 5 RN-BSN programs in Korea and 18 department of nursing in university and 12 RN-BSN programs in U.S.A. For earn the degree of Bachelor of Science in Nursing, the student earns 134 of mean credits in U.S.A., whereas 150.3 of mean credits in Korea. The mean credit for clinical pratice is 30.1 in U.S.A., whereas 23.9 in Korea. Students are assigned to individually planned clinical experiences under the direction of a preceptor in U.S.A. In RN-BSN program, total mean credits through lecture and clinical practice for earn the degree of BSN is 35.5(lecture : 27.7, practice ; 7.8)in U.S.A., whereas,48.1 (lecture;42.1, practice;6.0) in Korea. RN-BSN program can be taken on a full-or-part time basis in U.S.A., whereas didn't in Korea. Especially, emphasis is place on the advanced nursing practicum that focus on the role of the professional nurse in providing health care to individuals, families, and groups in community setting in U.S.A. 27.7 of mean credits was earned through lecture in U.S.A., whereas 42.1 of mean credits in Korea. It means that RN-BSN program in Korea is the lesser development in teaching method and appraisal method than in U.S.A. Students of RN-BSN program in U.S.A. can earns credit through CLEP, NLN achievement test, portfolio review session etc as well as lecture. Therefore, the authors suggests some recommendations for the development of curriculum of RN-BSN program in Korea based on comparative analysis of RN-BSN curricula in U.S.A. and Korea. 1. The curriculum of RN-BSN Program in nursing was required to do some alterations. Nursing care, today, is complex and ever changing. According to change of public need, RN-BSN curriculum intensified primary care program in community setting, geriatric nursing, marketing skill, computer language. 2. The various and new methods of earning credit should be developed. That is, the students will earn credits through the transfer of previous nursing college credits, accredited examination of university, advanced placement examination, portfolio review session, case study, report, self-directed learning and so on. Flexible teaching place should ile offered. 3. Flexible teaching place should be offered. The RN-BSN curriculum should accommodate each RN student's geographical needs and school/work schedule. Therefore, the university should search a variety of teaching places and the RN students can obtain their degrees comfortably throughout the teaching place such as lecture room inside the health care agency and establishment of the branch school in each student's residence area. 4. The RN-BSN program should offer a long distance education to place-bound RN student in many parts of Korea. That is, from the main office of university, the RN-BSN courses are delivered to many areas by Internet, EdNet (satellite telecommunication) and other non-traditional methods. 5. For allowing RN student to take nursing courses, program length should be various, depending upon the student's study/work schedule. That is, the various term systems such as semester, three terms, quarter systems and the student's status like full time or part time should be considered. Therefore, the student can take advantage of the many other educational and professional opportunities, making them available during the school year.

치과의원에서 임플란트 치료 결정에 영향을 미치는 요인에 관한 연구 (A Study on The Factors Influencing the Decision to Get Implant Treatment at Dental Clinic)

  • 본 연구는 의료이용모델인 Andersen 모형을 기본모형으로 하였다. 서울, 경기, 인천 지역에 소재하는 8개의 치과 (병)의원에서 321명의 환자에게 설문조사하여 임플란트 결정여부에 영향을 미치는 요인을 알아보았다. 분석은 빈도표를 이용한 기술분석, t 검정과 로지스틱 회귀분석을 이용한 관계분석으로 진행하였다. 1. 치과를 선택하는 이유로는 25.2%가 의사의 치료 또는 기술의 전문성이었다. 다음으로 의료기관의 명성 19.4%, 편리한 지리적 위치 15.3%, 친절한 서비스 11.2% 등으로 나타났다. 2. 임플란트 치료결정의 이유로는 의사가 믿을 수 있어서가 54.4%로 가장 많았고, 주위의 권유 16.6%, 직원들이 친절해서 7.7%, 유명해서가 5.9%등으로 나타났다. 3. 치과의사 또는 치과위생사의 세일즈 스킬과 임플란트 결정여부의 관계를 알아보기 위해 t 검정을 시행하였다. 임플란트를 결정한 경우, '치과의사는 치료 과정에 대해 자세히 설명함', '치과의사는 질문에 성실히 답해줌', '치과위생사는 나의 상태에 대해 자세히 설명함'과 '치과위생사는 치료과정에 대해 자세히 설명함'에 그렇지 않은 경우에 비해서 상대적으로 동의수준이 높았다. 4. 임플란트 결정여부에 영향을 미치는 요인을 분석하였다. 계층이 상대적으로 높을수록, 병원의 시설이 좋다고 판단할수록 또는 주변에 임플란트 경험자가 있는 경우 임플란트로 결정할 가능성이 높았다. 본 연구에서는 임플란트 결정여부에 영향을 미치는 요인을 밝혔으며, 이는 임상현장에서 고객 마케팅 전략수립의 기초 자료로 활용될 수 있으리라고 본다.

수도권 소재 병원의 내 . 외부고객의 의료이용에 대한 인지도와 만족도 차이 분석 (A Comparison of the Recognition and Satisfaction for Health Care Service between Internal Customer and External Customer)

  • To compare the differences in the recognition on hospital utilization and satisfaction, 368 hospital employees and 485 patients were selected in four hospitals in Seoul and Kyung-gi do. The survey was done using a constructed form of checklist from Apr. 30 to May 10, 1999. And the results are as follows: l. Gaining knowledge of hospitals available among hospital employees differed from that of patients. When finding out about sources of information concerning hospitals, direct visit to the hospital was recognized to be the main factor for the hospital employees in contrast to the patients' recognition which were mass media, personal involvement of job related workplace and recommendations from other hospitals. There was no difference between university and general hospitals. 2. The factors that concern which hospital to choose there was a difference between hospital employees and patients. Hospital employees recommended their own hospital solely based on the reason that it was their work place. On the other hand, the patients made a choice based on the type of medical staff, transportation available and whether it was a university hospital or not. There was no difference between university and general hospitals. 3. The recognition of employees concerning hospital image of a hospital between hospital employees and patients was different. In university hospitals, the employees recognized the name value of university hospital and cooperation as most important, whereas the patients thought convenience, kindness were the main factors. Patients considered general hospitals to be more convenient. There was some difference between university and general hospitals. For university hospitals employees' recognition was higher and for the general hospital patients' recognition is higher on hospital image. 4. The recognition of employees was different from that of patients' on hospital satisfaction. The patients' satisfaction was higher than that of employees'. There was no difference between university and general hospitals. Based on the above findings, the employees' recognition on hospital utilization and satisfaction was different from that of the patients, but there was no difference between university and general hospitals. In both groups choice of hospital was associated with satisfaction. Results showing difference between employees' and patients' recognition can be applied to implement customer-oriented attitude and be used as a baseline data for internal-external marketing planning of hospital management. The study may be limited in that the results cannot be generalized due to its small sample size and not being able to reflect demographic variables and life style. Further studies to investigate the difference of hospital utilization and hospital satisfaction will be necessary to define demographic characteristics and recognition of employees which influences patients' hospital satisfaction.

크레메진의 투석도입 지연효과에 따른 진행성 신부전증환자의 비용감소분 추계 (Estimating the Cost Savings Due to the Effect of Kremezin in Delaying the Initiation of Dialysis Treatments among Patients with Chronic Renal Failure)

  • Objectives : We wanted to evaluate the economic value of a pharmaceutical product, Kremezin, for treating patients with chronic renal failure (CRF) by estimating the amount of cost savings due to its effect for delaying the initiation of dialysis treatments. Methods : We defined a conventional treatment for CRF accompanied by Kremezin therapy as 'the treatment group' and only conventional treatment as 'the alternative group.' The types of costs included were direct medical and nonmedical costs and costs of productivity loss. The information on the effect of Kremezin was obtained from the results of earlier clinical studies. Cost information was derived from the administrative data for 20 hemodialysis and 20 peritoneal dialysis patients from one tertiary care hospital, and also from the administrative data of 10 hemodialysis patients from one free-standing dialysis center. Per-capita cost savings resulting from Kremezin therapy were separately estimated for the cases with delay for the onset of hemodialysis and the cases with immediate performance of peritoneal dialysis. By computing the weighted average for the cases of hemodialysis and peritoneal dialysis, the expected per-capita cost savings of a patient with CRF was obtained. Using a discount rate of 5%, future cost savings were converted to the present value. Results : The present value of cumulative cost savings per patient with CRF from the societal perspective would be $18,555,000{\sim}29,410,000$ Won or $72,104,000{\sim}112,523,000$ Won if Kremezin delays the initiation of dialysis by 1 or 4 years. Conclusions : The estimated amount of cost savings resulting from treating CRF patients with Kremezin confirms that its effect for delaying the onset of dialysis treatments has a considerable economic value.
