간호·간병통합서비스 제공을 위한 간호인력 배치기준 개발 (Development of Staffing Levels for Nursing Personnel to Provide Inpatients with Integrated Nursing Care)

  • Purpose: To develop staffing levels for nursing personnel (registered nurses and nursing assistants) to provide inpatients with integrated nursing care that includes, in addition to professional nursing care, personal care previously provided by patients' families or private caregivers. Methods: A time & motion study was conducted to observe nursing care activities and the time spent by nursing personnel, families, and private caregivers in 10 medical-surgical units. The Korean Patient Classification System-1 (KPCS-1) was used for the nurse manager survey conducted to measure staffing levels and patient needs for nursing care. Results: Current nurse to patient ratios from the time-motion study and the survey study were 1:10 and 1:11, respectively. Time spent in direct patient care by nursing personnel and family/private caregivers was 51 and 130 minutes per day, respectively. Direct nursing care hours correlated with KPCS-1 scores. Nursing personnel to patient ratio required to provide integrated inpatient care ranged from 1:3.9 to 1:6.1 in tertiary hospitals and from 1:4.4 to 1:6.0 in general hospitals. The functional nursing care delivery system had been implemented in 38.5% of the nursing units. Conclusion: Findings indicate that appropriate nurse staffing and efficient nursing care delivery systems are required to provide integrated inpatient nursing care.

가정방문 물리치료서비스의 이용의사에 관한 연구 (House Visits by Physical Therapist and Patient Needs)

  • As in house care is develops and becomes part of the health care delivery system in Korea, it is necessary to quantify the viewpoints of in-patients and outpatients. The purposes of this study are to analyze the utilization of house visits by physical therapist and to investigate the viewpoint of in- and out-patients who had rehabilitation treatment at 3 general hospitals in Wonju, Korea. Two hundred and fifty-eight questionnaires were analyzed and the major findings are as follows: 1. Sixty-five point seven percentage of respondents showed their willingness to utilize house visits by physical therapist if an in house physical therapy program were established. 2. The questionnaire used a Likert-type scale to ask the respondents their viewpoints on in house physical therapy services. Most respondents showed a positive attitude, answering 'I agree'. The findings show that house visits by physical therapists should be introduced as an integral system for the delivery of health care in Korea.

  • Quaternary prevention should be implemented to minimize harm to patients because the ultimate goal of medicine is to prevent disease and promote health. Primary care physicians have a major responsibility in quaternary prevention, and the establishment of clinical epidemiology as a distinct field of study would create a role charged with minimizing patient harm arising from over-medicalization.

  • The Korean government has a new system for charging patient care for patients in hospital, on hold for the present(9 / 1995) but to start implementation in certain areas of patient care next year. From the latter half of next year the Ministry of Health and Welfare would like to start demonstration projects for hospitals who want to start using DRGs for frequently seen medical diagnosis and for patients with a course that is predictable and for whom non-insurance costs are minimal : such as the patient who has a delivery, cesarean deliveries, cataract surgery, tonsillectomy or an appendectomy, and apply the DRG system of payment for hospital care for these patients. The purpose of this study was to establish a recording system to give effective home health care to postpartum women and their newborns. Recently the government announced a DRG system to apply to postpartum women for pilot purposes starting next year. This gives impetus to the need to develop home care records that will allow for systematic recording and provide continuity and consistency in care across all health professionals and with in-depth communication between the professions to assure high quality care. There has been a rise in medical costs and a shortage of patient bed space in hospitals, particularly since the introduction of national medical insurance. The study focused on developing client selection criteria, a primary assessment tool, progress notes and nursing diagnoses applicable to postpartum and newborn clients. Selection criteria for home health care, assessment tool content, nurses progress notes and diagnoses were developed through a review of the literature, advice from professionals who are expert in home health care and actual practice in the use of recording tools through workshops. The recommendations based on the research results are as follows : 1) Replication and application of these tools is needed to test the validity of the tools 2) In order to have systematic nursing records standardization of records has to be done after nurses have had experience using them. 3) Reliability and validity of the tools has to be established through applicability to actual care situation.

Exploring the Feasibility of Differentiating IEEE 802.15.4 Networks to Support Health-Care Systems

  • IEEE 802.15.4 networks are a feasible platform candidate for connecting all health-care-related equipment dispersed across a hospital room to collect critical time-sensitive data about patient health state, such as the heart rate and blood pressure. To meet the quality of service requirements of health-care systems, this paper proposes a multi-priority queue system that differentiates between various types of frames. The effect of the proposed system on the average delay and throughput is explored herein. By employing different contention window parameters, as in IEEE 802.11e, this multi-queue system prioritizes frames on the basis of priority classes. Performance under both saturated and unsaturated traffic conditions was evaluated using a novel analytical model that comprehensively integrates two legacy models for 802.15.4 and 802.11e. To improve the accuracy, our model also accommodates the transmission retries and deferment algorithms that significantly affect the performance of IEEE 802.15.4. The multi-queue scheme is predicted to separate the average delay and throughput of two different classes by up to 48.4% and 46%, respectively, without wasting bandwidth. These outcomes imply that the multi-queue system should be employed in health-care systems for prompt allocation of synchronous channels and faster delivery of urgent information. The simulation results validate these model's predictions with a maximum deviation of 7.6%.

남북한 보건의료제도의 비교 (A Comparative Study on the Health Care System of South and North Korea)

  • This Study has attemped to compare the health care systems of South and North Korea. There has been a wide difference in the health care System between the South and North of Korea. In this paper, I have also shown that each health care system has its own unique response to the social, political, and economic conditions of the country. Therefore the author analyzed and summarized the important difference of health care system between the South and the North of Korea as follows. 1. Compared with the Laissez-faire health care system of South Korea, North Korea has the state socialistic health care system which provide health care services to the people free of charge. And the North Korea is marking positive efforts toward the scientification and systemization of Oriental Medicine which is called Dongui-Hak in the North-on the basis of Ju-Che idea. 2. North Korea's health care system appears to be strongly geared toward extensive and preventive treatment and launched the massive sanitary propagation campaign. which have resulted in a great success. North Korea has a system of universal comprehensive care for its population. The government has a central role in planning and regulating health care. 3. The government also employs physicians, nurses, and other professionals to provide health care to patients at public expense. In North Korea, health professionals are government employees. They work for a salary and the system is funded through general taxation. 4. In the North Korea, health services area system of the cities and countre's unit is strictly conducted along with the doctor's area responsibility system. And so without referal card, patients can not use the upper-grade medical facilities. The health care delivery system of North Korea is made up of the fourth level procedue unlike South Korea. 5. General office of Oriental Medicine, Academy of Oriental Medical Science and Guidance Bureau of Oriental Medicine are established in the organization of the Department of Health in the North Korea. And nowadays much emphasis are equally placed on the Oriental Medicine as well as Western Medicine. Both South and North Korea have faced with a critical moment of developing a mutually agreeable and acceptable system of health care for the unified nation.

치과병의원에 근무하는 치과위생사의 사회적 가치관과 보건의료체계에 대한 가치관의 관련성 (The relationship between social values and health system values in dental healthcare hygienist)

  • Objectives : This study was to examine the social value of dental hygienists, their values about the health system and the relationship of all the related variables. Methods : The subjects in this study were 205 dental hygienists who worked in dental clinics and hospitals on Seoul. A survey was conducted from August 12 to October 15, 2010. The questionnaire consisted of nine items about general characteristics, two items about social values and 11 about values of the health system. The items related to social values and values of the health system were prepared by translating the items used in David et al's study, and the Cronbach alpha coefficient of those items respectively 0.80 and 0.76. Results : The dental hygienists got 3.94 in social values, which was above the average. In terms of values about the health care system, their values of the treatment delivery system(3.92) rated highest, followed by values of patient rights(3.79) and values of institutional restrictions(3.25). Their socal values had a closest positive correlation to their values of the treatment delivery system, and their values of patient rights had a strong positive correlation to those of the treatment delivery system and was positively correlated to those of institutional regulations as well. And there was a positive correlation between their values of the treatment delivery system and institutional regulations(r=.276). Conclusions : The above-mentioned findings illustrated that the social values of the dental hygienists had a positive correlation to their values of the health system. Therefore the kinds of educational programs that help dental hygienists to build their social values and values of the health system should be developed to improve their job efficiency as oral health experts.

  • Purpose: The purpose of this study is to explore factors which influence adoption, implementation and continued use of tele-healthcare systems. Methods: Qualitative research was conducted by in-depth interviews with 17 professionals from various fields of organizations involved in developing and implementing tele-healthcare systems. Data were analysed thematically, using a conceptual model of diffusion of innovations. Results: The system users were reacted positively to the 3 attributes out of 9 which decided the adoption of innovation. In addition, it is required to redesign the tele-health care system simpler and easier so that the system users can access to the system much more easily regardless of space and time limitations. From the design stage on an individual level, it is necessary to conduct detailed needs analysis and listen to users who are at the center of innovation diffusion. On an organizational level, it is necessary to actively prepare for possible problems during system implementation, educate the users and build communication channels continuously. Conclusion: This study has identified the factors affecting the innovation of tele-health care systems and contributed to the understanding of the operation of tele-health care systems by the diffusion of innovation theory in community health posts.

  • The delivery of high-quality antenatal care is a perennial global concern for improving maternal and neonatal outcomes. Antenatal care is currently provided mainly on a one-to-one basis, but growing evidence has emerged to support the effectiveness of group antenatal care. Providing care in a small group gives expectant mothers the opportunity to have discussions with their peers about certain issues and concerns that are unique to them and to form a support system that will improve the quality and utilization of antenatal care services. The aim of this article is to promote group antenatal care as a means to increase utilization of healthcare.
