Object: To analyze the effect of a media-campaign for "walking exercise participation improvement", which impacted walking-related awareness and behavior change of residents in Seoul. Method: This study used three campaign media including printing information, walking exercise indication board and a public advertisement of cable TV to lead a walking-related awareness change and practice frequency(number of days per week walking) and time(minutes per day walking) of walking exercise. To evaluate the exposure and message-recall levels of a campaign and effects of awareness change and walking practice, this study used a questionnaire survey(N=377). Result: 1) Group of exposure to campaign more participate and had the higher frequency(p=.015) and time(p=.023) in walking exercise and in comparison with group of nonexposure. 2) Group of changed awareness to campaign more participate and had the higher frequency and time in walking exercise and in comparison with group of no changed perception(p <.05). 3) Level of message recall of ${\ulcorner}$printing information${\lrcorner}$ was associated with number of days per week walking, and level of message recall of ${\ulcorner}$public advertisement of cable TV${\lrcorner}$ was associated with minutes per day walking at a statistically significant level(p <.05). Conclusion: These results suggest that media campaign can enhance the success of community-based efforts to promote awareness change and walking practice.
Background: Selective health benefit was introduced for decreasing economic burden of patients. Medical devices with economic uncertainty have been covered as selective health benefit by National Health Insurance since December 2013. We aimed to analyze impact of selective health benefit to medical expenditure and provider behavior focused on electrosurgery (ultrasonic shears, electrothermal bipolar vessel sealers) for gastric cancer patients covered since December 2014. Methods: We used the National Health Insurance claims data of 2,698 patients underwent gastric cancer surgery between August 2014 and March 2015. Medical cost and patient sharing per inpatient day were analyzed to verify that covering electrosurgery increased medical expenditure and changed provider behavior from open surgery to endoscopic or laparoscopic surgery. Additionally, we analyzed the claim rate of medical device or goods relating gastric endoscopic and laparoscopic surgery. Results: Medical cost and patient sharing per inpatient day were increased after covering electosurgery as selective health benefit (39,724/1,421 won). However, there were no medical expenditure increases after adjusting claim of electosurgery and patient sharing was decreased 1,057 won especially. The coverage of selective health benefit did not increase the claim rate of medical device or goods related endoscopic or laparoscopic surgery, either. Conclusion: Covering electosurgery decreased patient economic burden and did not change of provider behavior. Expanding selective health benefit is needed to decrease economic burden of severe patients. Further study should evaluate the long term effect with accumulated data.
This study was conducted to contribute to the expansion of activity areas for school nurses in the field of health promotion services whose importance are increasing daily. To achieve such goals the researcher proposed that health promotion programs for obesity management in overweight schoolchildren conducted by literature review to apply to school settings. In this program, the principal components developed a behavior modification program and an exercise program, which could be educated and managed by a nursing staff during for 9 weeks, three times per week, 60 minutes each. The program consisted of exercise, health education. health counseling, and a direct nursing care. This kind of approach may support the need of behavior modification programs and exercise programs on a regular base via the social organization, which may mean an intentional change of life style. A comprehensive approach to health promotion services proposed by this research would contribute greatly to the effectiveness of school health services and to promote health in overweight schoolchildren.
Objectives: This study identified how personal characteristics, healthy behavior and social capital might influence on physical activity of adults. Methods: This study used data from the health survey of a city of Korea. We surveyed 1,000 adults sampled by stratified sampling methods from 67,889 households. Outcome variable was the stage of physical activity which was broken into 5 categories. Sociodemographic factors, healthy behavior, self-rated health status and social capital were used as control variables. Sociodemographic factors included age, sex, educational status, economic status measured by deprivation score, residential period within survey city. Social capital was measured by Integrated Questionnaire for the Measurement of Social Capital (SC-IQ). This study used chi-square test and ordered logistic regression models to examine the associations between independent variables and physical activity. Variables were added to the regression model in three groups using a hierarchical approach. Results: Physical activity was significantly more likely to become active if they have higher educational status, healthier behavior. Among the six dimensions of SC-IQ, only "groups and networks" that is structural dimensions of social capital and "trust and solidarity" that is cognitive dimensions of social capital were significantly related to physical activity of adults. We found that a person having higher density of membership and having larger size of networks showed the high possibility of active physical activity. A person having high solidarity was significantly associated with physical activity, but general trust was inversely related to physical activity. Output dimensions of social capital did not show significant relationship to physical activity. Conclusion: We found that social capital is useful concept to explain health behaviors like physical activity. However we must consider social, cultural and political context of the study to evaluate the effect of social capital to health status and health determinants and to capture the exact meaning of relationship between them. We suggest further researches to refine the concept of social capital and to explain the relationship of social capital to diverse health determinants.
The purpose of this study was to analyze the converged influencing factors on the stages of exercise behavior changes in shift work nurses. The subjects were 120 shift work nurses working in hospitals who understood the purpose of the study and agreed to participate in the study. Data were analyzed using frequency, percentage, mean, standard deviation, t-test, $X^2$ test, and multiple logistic regression analysis. The exercise motivation factors of shift work nurses had a significant effect on stages of exercise behavior change (${\beta}=2.480$, p=.022), health status perceptions(${\beta}=1.151$, p=.013). Social support factors (${\beta}=1.819$, p=.002) and marital status (${\beta}=-1.820$, p=.004) also had significant effects on the stages of exercise behavior change of shift work nurses. In other words, subjects with change in exercise behavior had 11.9 times higher motivation and 3.1 times higher health status perception than those without change in exercise behavior, social support was 6.1 times higher, and unmarried subjects showed 16 times showed higher than that of married subjects. Therefore, it is important to develop a strategy to practice continuous and regular exercise in consideration of the exercise motivation, social support, and health status perception of the nurses in shift work.
Objectives: Recent studies have shown that tailoring to women's individual beliefs and stage of cancer screening adoption increase the probability that cancer screening will ensue. To identify variables associated with cancer screening behavior, many studies for cancer screening have used the Transtheoretical Model(TTM). This study was carried out to identity the cognitive and behavioral factors associated with breast cancer screening by stages of change among women, forties aged. Methods: Building on the TTM constructs, we collected the data to test the association with cognitive and behavioral factors for breast cancer screening by stage of change among women, forties aged (N=232), using the self-reported questionnaire. The stages of change were grouped according to screening participation and intention for breast cancer as precontemplation, contemplation, preparation, action, and maintenance. We found out the association between breast cancer screening and cognitive and behavioral factors, and testified the difference between stages of change by chi-square test, one-way ANOVA, and multiple comparison analysis(Duncan test). Results: Analyses of 232 women showed that participation on mammography was 68.1% within lifetime and 46.1% within last 2 years, and we found out the association with breast cancer screening participation, intention and cognitive-behavioral factors. The stages of change based on participation and intention were different from the decisional balance, the screening attitude, and the self-efficacy(p<0.01). The decisional balance was differ from stages of change because the difference on opinions about pros(positives) and cons(negative) were likely to significant by stages of change(p<0.05, p<0.01). Conclusion: To increase the screening rate for breast cancer, it should be developed the tailored message and recommend guideline. And the tailored message should be designed to increase the pros of breast cancer screening(mammography) and to decrease the cons, and considered the woman's stage of adoption.
This study focuses on investigating and analyzing overall information use behavior of the health fields researchers. Questionaries were 116 researchers in there nursing university or college and EMT college. Four areas were tested on the basis of the purpose that there would exist unique characteristics in the information seeking behavior of health fields researchers. The findings of the study shows that health fields researchers prefer monographs and journals; they tend to use current materials; the first information source for their research activities is the cited references in the book or journal article; they are likely to delegate literature searching to the librarian, but they tend to conduct direct searching in online setting; in general, they react positively for the rapid change in information environment caused by recent development of information technology. It is emphasized that to design an efficient information system and provide an effective information service in the health science related library or information center, it is essential to consider the major characteristics and attributes of health fields researchers in terms of their information use behavior.
Hyperlipemia is the most leading risk factor of cardiovascular disease which is the main cause of death in Korea. However, there is a tendency to neglect the prevention and treatment since it has no specific symptoms. It has been reported that the level of serum-lipid can be lowered by the improvement of eating habits. Therefore, it is highly likely that the development of programs on the improvement of eating habits through behavioral theory is required to the community nursing practice. The theory of planned behavior, which assumes that human behaviors are determined by one's intention to carry out the behavior, can be characterized by the point that behaviors are not only individual factors but also social behaviors relating to subjective norms. It is widely recognized that this theory has a high predictability on health behavior due to it's simplicity clearness, and measurability as well as high quality of being general. Thus, the theory of planned behavior could be useful in developing a model of a health promotion program to the change of behaviors of the risk group of cardiovascular disease. Consequently, based on the theory of planned behavior, the purpose of this study is to develop an intention promotion program of the diet, and then to testify the effects. The sample of this study consisted of 26 industrial workers who had proved hyperlipemia from a medical examination in 1996 (experimental group 13, control group 13). The intention promotion program, which includes education, monitoring, pressure, counselling on the level of individuals, families and organizations, was conducted for 10 weeks The purpose of this program was to promoting intention of the diet through changes of the prerequisite factors of intention such as behavioral belief, outcome evaluation, normative belief and control belief. When it came to data analysis, the ${\chi}^2$-test and Fisher's Exact test were used to compare the general characteristics between the experimental and the control group, an independent t-test for the other variables. ANOVA was used to the test hypothesis, and the Pearson correlation test for variable's correlation. The results of this study can be summarized as follows ; 1) There was a significant increase in the intention(F=18.51, p=.00) of diet in the experimental group. 2) Diets(F=32.51, p=.001) in the experimental group were better carried out than in the control group. 5) There was a moderate correlation between the intention of diet and performance (r=.587. p=.003). From the results, it can be concluded that the intention promotion program is very effective, leading to the change of health promotion behavior. Above all, it is really valuable that the intention promotion program in this study regards health promotion behavior as a social behavior and that intervention was done on the level of family and organization. Consequently, when performing a health promotion program, social approach elevating the intention should go hand in hand in order to make the program effective.
Risk information may be one of the most important factor for worker's safe behavior because that safe behavior can be oriented by attitude based on risk information. Traditionally KAP(knowledge, attitude and practice) model was useful frame for the change of human behavior. Knowledge is formed by information through experience and education. Worker's health may be prevented by his or her own active safe behavior based on risk information. This paper is to investigate the effect of labor union on the provirion of risk information by labor union. Data for analysis is the third Korean Working Conditions Survey done by Occupational Safety and Health Research Institute in 2011. The sample size is 50,032 economic active person surveyed by household interview survey with structure questionnaire by trained interviewer. The difference of risk information provision among employees was tested by mean difference test. The level of risk information of employees of companies with labor union is higher than that with non labour union. This paper has some implication for the promotion of safe behavior of employees through risk information provision mediated by labor union. Some limitation of this study may be considered because of using the cross sectional survey data.
Purpose: This study was to examine the differences in health belief variables and HPV (Human Papillomavirus) knowledge in Pap smears according to the stages of change using Trans-theoretical Model (TTM) among married nurses. Methods: The research design was a cross sectional survey. Participants were 387 married nurses working in general hospital located metropolitan city. Measurement variables were perceived threatening, perceived severity, benefits of Pap, HPV knowledge and stages of the change in Pap smear. Results: Classified stages were pre-contemplation 3.9%, contemplation 17.8%, relapsing 29.5%, action 31.0%, and maintenance 17.8%. Accuracy rate of HPV knowledge were low ranging from 15% to 45%. Among the variables, perceived threatening (F=3.56, p=.007), perceived severity (F=9.73, p<.001) and HPV knowledge (F=7.52, p<.001) were significantly different by stages of change. Conclusion: Application of TTM to Pap smears was efficient to know the nurse's level of health behaviors, Health belief variables and HPV knowledge were main factors to identify the stages of change. The continuation of Pap smears and the education in relation to HPV knowledge should be encouraged for married nurses.
