Analysis of Service Occupation Types and Training Programs of Services for the Aged in Busan (실버서비스를 위한 전문인력개발 프로그램의 분석)

  • Purpose: The purpose of this study was to analyze service occupation types and to develop training programs for the aged. Methods: This study used descriptive study design. The research process consisted of three stages: first, identified the demand of service manpower for the aged; second, investigated present jobs and education programs in Korea and Japan; and last, developed service jobs and education programs for the aged. Results: Potential users considered "health management" to be the most important area. They thought "providing job" as second most important. According to the result of analyzing Korean policies, there were 9 service occupations in 5domains. So, we derived 10 occupations such as 'daily living manager', 'care manager', 'care worker', 'health manager', 'education specialist', 'leisure manager', 'good manager', 'housing manager', 'financial specialist' and 'retirement consultant' in 5domains as healthcare, leisure, goods, housing and finance. Finally, we developed their tailored training programs. Conclusion: According to this study, there should be various occupations qualified by the government, and training programs should be settled. And healthcare providers must included in developing standardized training programs.

  • The subject of this study is to review the practical approaches of Home Care Services. Included is a brief overview of its nature, providers of Home Care Services, recent history of Home Care Services, and the impact of the national movement toward cost containment in health care. The data used in this study are obtained from the Elderly Program of the Medical Services and other data on the Home Care Services in Japan. With the growing elderly population in Japan, it is to be expected that the medical care expenditure for this sector will continue to increase. With the aim of keeping expenditure for medical care within reasonable bounds, it is essential that this increasing expenditure on the elderly be used effectively. With the Health and Medical Services Law for the Aged was enforced, therefore, remuneration for medical treatment of the elderly and what is known as the staff placement standard at hospital for the elderly were rationalized. In addition to rationalization from the point of view of medical care supply, it is necessary to guarantee the appropriate treatment within the community and at home for those elderly who are bedridden but not in need of hospital care. For this it is required that Home Care Services, such as health services like visiting guidance by public health nurse in hospital of Health Center. So that the elderly can feel secure in receiving treatment within the community and at home, allowances for guidance on leaving hospital and for intermittent nursing and guidance thereafter are to be newly introduced. Home care Services in one aspect of comprehensive health care, it is comprised of health services provided to individuals and families in their homes. Its purposes include promoting, maintaining and restoring health, specifically maximazing independent functioning and minimizing the disabling effects of illness, including terminal illness. Services appropriate to the needs of clients and their families are planned, coordinated, and delivered by providers organized for the delivery of home health care through the use of contractual arrangement, employed staff, or a combination of the two.

Geriatric Health Problems and Directions for Nursing Improvements (노인건강문제와 간호의 방향)

  • The elderly population will increase from 5.8% in 1996 to 12.5% in 2020. The related problems of health will also become a very important issue in the future. Therefore it is important to address the problems of geriatric nursing and geriatric health. 87.6% of the aged were ill with chronic degenerative diseases in 1994. In 1995, hospital admission rates (86.8) for those aged 60 or older were higher than that (56.3) of the total age group. Such high medical utilization will increase national health costs. For the development of geriatric nursing, active nursing intervention in various settings combined with education and research should be developed. Considering the health and welfare of the aged and the present status and views in Korea, I suggest the following: First, the health needs of the elderly in the institution, must be met by a plan that fosters geriatric nurses and programmed service development. Second, health service for the residential elderly must be provided in day care centers, short stays, nursing homes and geriatric hospitals. Geriatric nursing services should be provided in home residential areas, public health centers, public health subcenters as well as having, community health practitioners in primary health care posts and home health nurses. Third, geriatric nursing curriculum must be developed adjust to situations and culture of Korea and be included in the nursing curriculum. And gerontological nurse practitioner or geriatric specialist must be fosteraged to provide the professional care for the aged. Geriatric nursing research should be also achived for geriatric nursing improvements.

Related Factors to the Service Level of Aged Care Facilities in Korea (노인요양시설 서비스 제공 수준의 관련 요인 분석)

  • The objective of this study is to examine relevant factors of the service level of aged care facilities. The sample used in this study consisted of 357 aged care facilities in Korea. Data were collected with self-administered questionnaire and 140 returned questionnaire were analyzed by SPSS Version 12.0. The major findings of the study are as follows: First, there was no significant mean difference in the service level by the facility characteristics, except the length of operation. Second, it was found that both administrative characteristics and employer characteristics were positively associated with the level of nursing and supportive services. Third, the study results revealed that the following three variables of employee education and training, community networks, and employer's philosophy and management principles had significant positive effects on the level of nursing services. Meanwhile, the following two variables of employee education and training, and community networks had significant positive effects on the level of supportive services. In conclusion, in order to improve their service level, the managers of aged care facilities in Korea should make efforts to provide more employee education and training, establish networks with the community stakeholders, for example, local clinics and hospitals. It is also recommended for the government to make a policy inducing more qualified private investors to enter the aged care market, as well as to strengthen the qualification of the managers of the public aged care facilities.

Effects of the Health Status and Health Behavior on Health-related Quality of Life of the Elderly Living Alone and Living with Their Families: Using Data from the 2014 Community Health Survey (독거노인과 가족동거노인의 건강상태와 건강행위 경험이 건강 관련 삶의 질에 미치는 영향: 2014 지역사회 건강조사 자료 활용)

  • Purpose: The purpose of this study is to identify factors influencing the quality of life of the elderly living alone and living with their families with regard to their health statuses and health behavior experiences. Methods: We used source data from the 2014 Community Health Survey. The subjects of this study included some elderly people aged 65 and over, and analyzed the data of 13,373 elders living alone and 13,322 elders living with family. Results: Factors influencing the quality of life of the elderly living alone and living with their families include gender, age, education, household income, current occupation, subjective stress level, depression, number of diagnosed diseases, walking exercise, the experience of health screening, and the experience of not having necessary medical services (p<.001). Region was a significant variable influencing the quality of life of the elderly living with their families (p<.001). Conclusion: In order to improve the quality of life of the elderly, it is necessary to provide sound conditions for working, emotional support, walking exercise and promotion of health screening, and to supplement the environment and institution for them to receive necessary medical services.

Evaluation of Service Quality in Aged Care Facilities : A Focus Group Study (노인장기요양시설 서비스의 질 평가 탐색 : 포커스 그룹 연구)

  • Purpose: This study was conducted to explore the meaning and contents of high-quality aged care facilities and provide basic data for evaluation of service quality in such facilities. Methods: The focus group interviews and participants consisted of two user groups, for a total of 16 family caregivers of the elderly living in facilities and four service provider groups, for a total of 26 chief managers and caregivers working in aged care facilities. All interviews were recorded and transcribed as they occurred. Content analysis was used and debriefing notes were referred to in order to analyze the data. Results: Four themes of a high-quality aged care facility emerged from the analysis; 1) a place to rest for comfortable later years; 2) systematic value-based management; 3) providing professional care; 4) comprehensive service provision in response to diverse needs. Conclusion: The findings of the study showed the importance of client centered care and ethical mindset of providers, which had not been included in the existing quality evaluation programs. Based on these results, medical treatments, end-of-life care and more comprehensive and extended services including family care need to be provided in facilities to ensure good quality aged care.

Levels of Health-related Quality of Life (EQ-5D) and Its Related Factors among Vulnerable Elders Receiving Home Visiting Health Care Services in Some Rural Areas (일부 농촌지역 맞춤형 방문건강관리 대상 취약계층 노인들의 건강 관련 삶의 질 (EQ-5D) 수준 및 관련요인)

  • Purpose: The purpose of this study is to identify levels of quality of health-related life (EQ-5D) and its related factors among vulnerable elders receiving home visiting health care services in some rural areas. Methods: The subjects of this study were vulnerable elders aged 65 or higher receiving visiting health care benefits by a public health center in a county. Results: Levels of health-related EQ-5D were high when the subjects were males, their ADL and IADL were high. The EQ-5D of the vulnerable elders and social supports had a positive correlation, while their IADL and depression had a negative correlation with the EQ-5D. We can see that the EQ-5D related factors are variables that have significance influence on gender, subjective health status, BMI, IADL, depression levels, and social support. Conclusion: To improve the EQ-5D of the vulnerable elderly, it needs maintenance of independent IADL levels, social supporting systems using small meeting of the elderly in community. It is necessary to maintain independent IADL levels, enhance social supporting systems including small group gatherings for elders living alone by means of places like community halls, and develop specific intervention programs for the prevention and management of depressive elders.

Relationship between Overall and Oral Health Status and Obesity in Middle-Aged Women in Jeju (제주지역 중년여성의 건강 및 구강건강상태와 비만과의 관련성)

  • Objectives: The purpose of this study was to discuss the overall and oral health conditions that affect obesity in middle-aged women in Jeju and to help them operate the effective obesity program in the future. Methods: Participants of this study were 171 middle-aged women living in Seogwipo City, Jeju Special Self - Governing Province. Data were collected through the convenience sampling method. Body mass index and waist circumference of each participant were measured, and a self-report questionnaire was applied. Results: The overall health status of obesity comprised depression, stress, diabetes, and menopause. The oral health conditions affecting obesity were bad breath and chewing. Hypertension was the main factor affecting body mass index and depression was the main factor influencing waist circumference. Obesity was more influenced by overall health status factors than by oral health status factors. Conclusions: The results of this study are expected to be useful as basic data for efficient health care services in the prevention of obesity.

Understanding Factors Associated with Unmet Need for Outreach Community Health Service among Older Adults in Seoul (노인 방문건강관리 서비스 미충족 영향요인: 서울시 찾아가는 동주민센터 사업을 중심으로)

  • The purpose of this study was to examine various factors influencing the needs of Seoul's newly implemented outreach community services for older adults, and to suggest the direction of the outreach community health services in Seoul. A multi-level regression was conducted using data collected by face-to-face interviews from 1,000 individuals aged 65 and 70 in 17 districts, where participated in the Seoul's outreach community services. The results demonstrated that socioeconomic status (higher income and living alone), health status (having multiple chronic conditions and depression, lower health literacy), limited experience of the outreach community services, and low government trust at the individual level were associated with higher unmet need for the community outreach services. In addition, shorter participation period of the outreach services and financial independency at the district level were associated with higher unmet need for the services. The findings from this study implies the need for improving the quality of services by focusing on vulnerable groups such as individuals with lower income and worse health status. In addition, the outreach community health services may need to target individuals aged 66 to increasing efficiency of the services through utilizing results of life-cycle health checkup by the National Health Insurance Corporation.

The Evaluation of the Community based Opthalmologic Health Services for Elderly in Chuncheon City, Kangwon Province (지역사회 중심의 노인 안보건사업 평가 - 강원도 춘천시를 중심으로 -)

  • This study is designed to evaluation of the Community based Opthalmologic Health Services(CBOHS) for elderly carried out for last three years from 1994. The subjects were total 236 aged over 57 in Chuncheon city. Kangwon province. and the period of data collection was from Feb. 24th. 1996 to Feb. 27th. 1996. For evaluation of CBOHS. the questionnaire was consisted of participation rate. and perception about the service. the level of satisfaction and things to improve. Result were obtained as follows: 1. The number of participants was $95(40.3\%)$ among total subjects. The places provided services were the health service center of Seoul National University in Chuncheon city and community health center. The participants were known from announcements of community staff$(66.3\%)$. 2. Non participants were $59.7\%$ among total subjects. mam causes of missing wasn't known'. 3. The services that participants screening test$(41.2\%)$. provided provided operation$(45.5\%)$. 4. The question about things to improve were continuous follow up and more many glasses provision These fingings may indicate that the CBOHS need more announcement. continuous follow up and provide more many glasses. As proved the satisfaction of CBOHS for elderly. the next study can be performed about the relevant factor of the services.
