Association between Dietary Behavior and Esophageal Squamous Cell Carcinoma in Yanting

  • Background: Yanting is one of high risk areas for esophageal cancer and the screening program was therefore initiated there. This study was aimed to investigate the dietary behaviors on the risk of esophageal squamous cell carcinoma (ESCC), among the individuals with normal and abnormal esophagus mucosa. Materials and Methods: A frequency matched case-controls study was proposed to estimate the different distribution of dietary behavior between individuals of control, esophagitis and cancer groups. Cancer cases were selected from hospitals. Esophagitis cases and controls were selected from screening population for ESCC. Health workers collected data for 1 year prior to interview, in terms of length of finishing a meal, temperature of eaten food and interval between water boiling and drinking. Chi-square, Kruskal-Wallis tests and unconditional logistic regression model were used to estimate differences and associations between groups. Results: Compared with controls, length of finishing a meal ${\geq}15mins$ was related to a reduced OR for cancer (OR=0.46, 95%CI, 0.22-0.97) and even compared with cases of esophagitis, the OR of cancer was reduced to 0.30 (95%CI, 0.13-0.72). The OR for often eating food at a high temperature was 2.48 (95%CI 1.06, 5.82) for ESCC as compared with controls. Interval between water boiling and drinking of ${\geq}10mins$ was associated with lower risk of cancer: the OR was 0.18 compared with controls and 0.49 with esophagitis cases (p<0.05). Conclusions: Length of eating food ${\geq}15mins$ and interval between water boiling and drinking ${\geq}10mins$ are potentially related to reduced risk of esophageal SCC, compared with individuals with normal and abnormal esophageal mucosa. Recommendations to Yanting residents to change their dietary behaviors should be made in order to reduce cancer risk.

Attitude of the Korean dentists towards radiation safety and selection criteria

  • Purpose: X-ray exposure should be clinically justified and each exposure should be expected to give patients benefits. Since dental radiographic examination is one of the most frequent radiological procedures, radiation hazard becomes an important public health concern. The purpose of this study was to investigate the attitude of Korean dentists about radiation safety and use of criteria for selecting the frequency and type of radiographic examinations. Materials and Methods: The study included 267 Korean dentists. Five questions related to radiation safety were asked of each of them. These questions were about factors associated with radiation protection of patients and operators including the use of radiographic selection criteria for intraoral radiographic procedures. Results: The frequency of prescription of routine radiographic examination (an example is a panoramic radiograph for screening process for occult disease) was 34.1%, while that of selective radiography was 64.0%. Dentists' discussion of radiation risk and benefit with patients was infrequent. More than half of the operators held the image receptor by themselves during intraoral radiographic examinations. Lead apron/thyroid collars for patient protection were used by fewer than 22% of dental offices. Rectangular collimation was utilized by fewer than 15% of dental offices. Conclusion: The majority of Korean dentists in the study did not practice radiation protection procedures which would be required to minimize exposure to unnecessary radiation for patients and dental professionals. Mandatory continuing professional education in radiation safety and development of Korean radiographic selection criteria is recommended.

Screening and Characterization of Microorganisms with Fibrinolytic Activity from Fermented Foods

  • Fibrinolytic microorganisms were screened from 42 samples of Korean fermented food (7 kinds of Chungook-jang, 14 kinds of commercial Doen-Jang, 5 kinds of home-made Doen-jang, and 16 kinds of Jeot-gal), 15 samples of Japanese fermented food (5 kinds of home-made soybean paste, and 10 kinds of Natto), and 19 samples of Indonesian fermented food (Tempe) as well as starters of Meju (500 microflora from Korea, and 22 from China). Initially, 11 isolates with strong fibrinolytic activity were selected for further characterization. The fibrinolytic activity of the 11 isolates ranged from 89 to 199% of standard plasmin. Four strains, M5l from Korean fermented food (Meju), I 1-1, I 1-4, and I 5-1 from Indonesian fermented food (Tempe), were chosen based on the degree of activity and reproducibility, and identified as Staphylococcus sciuri, Citrobacter or Enterobacter, Enterococcus faecalis, and Bacillus subtilis, respectively. The first two isolates are pathogenic stains while the latter two are considered as GRAS (Generally Recognized As Safe). Fibrinolytic activity of E. faecalis, characterized and designated as BRCA-5, reached a maximum, when the producer was cultivated in Ml7 broth supplemented with 1.0% glucose for 5 h at 37$^{\circ}C$ with shaking at 180 rpm. Compared to commercial fibrinolytic enzymes, the cell-free culture supernatant of 5. faecaiis BRCA-5 showed stronger activity than plasmin and streptokinase, but similar degree of specific activity as nattokinase and urokinase, aud it also demonstrated anticoagulant and antiplatelet activity ex vivo. These features of E. faecalis make it an attractive agent as a biomaterial for health-promoting foods.

  • To evaluate aortic wall stiffness without influence of different background blood pressure, a new technique was developed and verified. At eight swine descending aortae, volume-pressure measurement was performed using custom-made system. Based on averaged pressure-volume curve, aortic distensibility index was formulated to evaluate aortic wall stiffness regardless of variable blood pressure and aortic size. The variability of aortic distensibility index by pressure change was compared with other parameters for wall stiffness evaluation. Subsequently, the aortic distensibility index was calculated at 100 contrast-enhanced EBCT data sets of normal volunteers in regular health screening program. The measured aortic distensibility index was compared with age, coronary calcium score, and aortic calcium score. Between 50 and 360 mmHg of blood pressure, the coefficient of variance of aortic distensibility index was 22.00% as comparing with 88.99% of classical compliance. Based on age, aortic distensibility index showed correlation coefficient of 0.55, whereas classical compliance showed 0.26. The correlation coefficient with modified aortic calcification was 0.43. Linear regression study revealed statistical significance of correlation coefficients. The aortic distensibility index, the method to evaluate aortic wall stiffness free from variable blood pressure and aortic size, was developed and verified with significant practical feasibility.

Epidemioclinical Feature of Early-Onset Colorectal Cancer at-Risk for Lynch Syndrome in Central Iran

  • Background: Colorectal cancer (CRC) is becoming one of the most complicated challenges of human health, particularly in developing countries like Iran. In this paper, we try to characterize CRC cases diagnosed < age 50 at-risk for Lynch syndrome within central Iran. Materials and Methods: We designed a descriptive retrospective study to screen all registered CRC patients within 2000-2013 in Poursina Hakim Research Center (PHRC), a referral gastroenterology clinic in central Iran, based on being early-onset (age at diagnosis ${\leq}50years$) and Amsterdam II criteria. We calculated frequencies and percentages by SPSS 19 software to describe clinical and family history characteristics of patients with early-onset CRC. Results: Overall 1,659 CRC patients were included in our study of which 413 (24.9%) were ${\leq}50years$ at diagnosis. Of 219/413 successful calls 67 persons (30.6%) were reported deceased. Family history was positive for 72/219 probands (32.9%) and 53 families (24.2%) were identified as familial colorectal cancer (FCC), with a history of at-least three affected members with any type of cancer in the family, of which 85% fulfilled the Amsterdam II Criteria as hereditary non-polyposis colorectal cancer (HNPCC) families (45/219 or 20.5%). Finally, 14 families were excluded due to proband tumor tissues being unavailable or unwillingness for incorporation. The most common HNPCC-associated extracolonic-cancer among both males and females of the families was stomach, at respectively 31.8 and 32.7 percent. The most common tumor locations among the 31 probands were rectum (32.3%), sigmoid (29.0%), and ascending colon (12.9%). Conclusions: Given the high prevalence of FCC (~1/4 of early-onset Iranian CRC patients), it is necessary to establish a comprehensive cancer genetic counseling and systematic screening program for early detection and to improve cancer prognosis among high risk families.

Prevalence of Types of Cancers in the Elderly Covered by Insurance of the Islamic Republic of Iran Broadcasting Company in 2015 - Comparison with Younger Groups

  • Presently, the world population of the elderly is growing. By improving health hygiene and welfare indicators, mortality and birth rates decrease and life expectancy increases, making the present century the century of elderly. Aging is one of the main risk factors for development of cancer, which itself is the second cause of death in old people. This study was conducted to assess the prevalence of cancer in the elderly covered by the Islamic Republic of Iran Broadcasting (IRIB) insurance program and to obtain suitable programs for cancer screening and early detection, increase patient survival, improve elderly care and to reclaim the cost of treatment in comparison to the national and international statistics. This is a cross-sectional study conducted on all elderly patients diagnosed with malignancy based on their pathology reports. In this study, of the total 75,500 patients covered by IRIB insurance, 17.2% belonged to the elderly group, males accounting for 53.3%. The most common cancers in old men were prostatic cancer (61.3%), colon cancer (10.3%) cancer of the hematologic system, bladder cancer (9.6%), lung cancer (9.1%), thyroid cancer (3.9%) and brain tumors (1.3%). In the elderly women, the most common cancers were breast cancer (80.1%), colon cancer (5.1%), thyroid cancers (4.4%), bladder and hematologic system malignancies (3.6), lung cancer (2.9%) and brain tumors (0.7%). In addition, the prevalence of cancer was almost the same as national and international statistics. With the exception of non-melanoma skin cancer no difference was shown in prevalence of cancer between IRIB elderly patients and the other groups of cancer patients in Iran.

탄력밴드저항운동이 노인의 균형 능력 향상에 미치는 영향 (Effects of Elastic Band Resistance Exercise on Improving the Balance Ability in the Elderly)

  • Purpose: This study examines the effects of elastic band of resistance exercise for balance control of the elderly. Methods: Thirty of eighty participants in experiment subjects who demonstrated balance-impairment through the use of primary screening tests including the one leg standing test (OLST), functional reach test (FRT) and timed up and go (TUG) were selected as subjects. Fifteen subjects that underwent muscle-strengthening exercise using an elastic band were selected as the exercise group and fifteen subjects were selected as a control group. Subjects undertook a home-based exercise program three times per week for 9 weeks. Muscle strength, functional assessment and a balance test were quantitatively measured before and after the exercise regimen. Results: After muscle strengthening exercises, changes in maximal voluntary isometric contraction (MVIC) showed a significant increase in all of the lower extremity muscles of the exercise group subjects. There were statistically significant differences between the exercise and control groups for changes in the OLST, FRT and TUG, which are functional assessments of balance ability, and changes of the unit path length and circumference area, measurement items of quantitative analysis. In addition, from examining correlations between MVIC, balance ability, it was found that an increase of muscle strength in the hip joint group of muscles among the lower extremity muscles had greater improvement in correlation with balance ability in this elderly population. Conclusion: Resistance exercise using elastic bands had significant effects on muscle strengthening in elderly subjects, with a resultant increase of lower extremity muscle strength with significant improvement of balance ability.

대장내시경 수검자에서 관찰된 대장용종의 유병률 및 위험인자 (Prevalence and Risk Factors of Colonic Polyps in Colonoscopy Examinees)

  • 본 연구는 검진센터에서 대장내시경을 실시한 무증상 성인들을 중심으로 대장암의 전구병변인 대장용종의 유병률 및 위험인자를 알아보고 무증상의 건강한 성인들에게 대장암 관리를 위한 기초자료를 제공하고자 시행하였다. 2010년 1월부터 12월까지 일개 대학병원 검진센터에서 대장내시경검사를 받은 995명을 대상으로 자기기입식 설문지, 의료기록지, 임상검사 결과지들을 이용하여 후향적으로 시행하였으며, 자료의 분석은 SPSS18.0을 이용하였다. 무증상 성인들의 대장용종 유병률은 남성에서 높았고, 연령대가 높을수록 증가하였다. 남성, 고령, 운동을 하지 않은 경우 독립적인 위험인자로서 대장용종과 밀접한 관련이 있는 것으로 나타났다. 따라서 40대 이상 성인에서는 대장내시경 검사를 통한 대장용종의 조기 발견이 중요하며, 대장암 전구병변인 대장용종의 일차 예방을 위한 지속적인 생활습관 교정 및 운동프로그램 개발이 필요하다.

지역사회 거주 경도인지장애 노인들을 대상으로 시행한 한의치료의 보고: 인지기능을 중심으로 (Results of Korean Medicine Treatment in Community Dwelling Elderly with Mild Cognitive Impairment: Focusing on the Change in Cognitive Ability)

  • Objectives: The purpose of this study was to identify the effectiveness of Korean medicine treatment in mild cognitive impairment (MCI) among a group of community dwelling elderly. Methods: Two-hundred and twenty-nine elderly living in a community and diagnosed with MCI were recruited. Participants were evaluated with various instruments such as the Korean version of Mini-Mental State Examination for Dementia Screening (MMSE-DS) and the Korean version of Montreal Cognitive Assessment (MoCA-K). Korean medicine treatment consisted of herbal medicine, acupuncture, and pharmacoacupuncture. The change in cognitive ability was assessed by using the MMSE-DS and the MoCA-K. Data were analyzed by SPSS/WIN 22.0 using the paired t-test, and the ANOVA. Results: The MMSE-DS and the MoCA-K score generally increased after six months of Korean medicine treatment and the differences in both instruments were statistically significant. Additionally, some consecutive participants maintained long-term cognitive improvement. When analyzed specifically by herbal medicine group based on syndrome differentiation and pharmacoacupuncture group, most showed improvement in the MMSE-DS and the MoCA-K but not all data were statistically significant. The satisfaction score was mostly high and most participants were willing to re-participate in the program. Conclusions: Korean medicine treatment may contribute to the improvement and prevention of cognitive decline in the elderly. However, further systematic research based on large scale sample data and standardized protocols is needed to uplift the welfare and mental health of the elderly.

직업병 유소견자 및 요관찰자 추이 및 사후관리와의 관련성: 납 및 카드뮴 취급근로자를 중심으로 (Relationship of Follow-up Management, Trend of Possible Occupational Disease and Probable Occupational Disease: Focusing on Lead or Cadmium Workers)

  • Objectives: This study identifies the trend of possible occupational disease and probable occupational disease in lead or cadmium workers. It is also required to check the usefulness of follow-up management in lead or cadmium workers by reviewing the relevance between the results of follow-up management and the trend of possible occupational disease and probable occupational disease. Methods: From 2009 to 2018, the results of the Ministry of Employment and Labor's special health-screening program for hazard agents for workers were used. The correlation between the ratio of possible occupational disease and probable occupational disease of lead or cadmium workers and the ratio of those who observed were and underwent observed follow-up management was analyzed to confirm the usefulness of follow-up management. Results: Over the past decade, the average annual proportion of possible occupational disease and probable occupational disease among lead workers has been on the decline. Among cadmium workers, it has generally shown a trend of increasing and decreasing. After the implementation of follow-up management, possible occupational disease in lead workers showed significant relevance to work prohibitions and restrictions, and probable occupational disease in lead workers showed significant relevance to the work prohibitions and restrictions, on-duty treatment, and boundary lines. However, there was no significant correlation between persons involved in cadmium workers. Conclusion: In this study, more active managements such as work ban and restrictions, on-duty treatment among follow-up management of possible occupational disease and probable occupational disease of lead worker and observers are related to a decrease in the ratio of those who have been diagnosed with possible occupational disease and probable occupational disease.
