Seven diagnosis-related groups (DRGs) prospective payment system is going to expand to all hospitals including university hospitals this year. However there are few studies on the change of practice pattern under prospective payment system in the university hospital setting. So This study was intended to predict the practice pattern change after the introduction of 7 DRGs prospective payment system in a university hospital setting. To predict the change of practice pattern, this study used simulation technique. Five hundred and nineteen patients classified as 5 DRGs in a university hospital were selected for simulation. The change of practice pattern were predicted based on clinicians' opinion. We also predicted payment change by service items. Major findings of this study are as follows. First, the total medical payment was reduced by 14.4%. The drug payment change (8.8%) took most of total payment reduction. The followings are the change of treatment material cost (3.2%), the change of laboratory tests cost (1.8%), the change of room charge (0.5%), and other payment change (0.1%), respectively. Second, most of the reduction in total medical payment resulted from the decreased amount of medical services themselves. The transfer of medical services to outpatient setting took up only 4.9% of the total payment reduction. The change of unit price or composition took up 5.5% of the total payment reduction. In this study we found that it is possible to reduce the inpatient services through practice pattern change in university hospital setting. However, it needs to be careful to adjust DRG payment after the reduction of provided services, because most of reduction was not due to service transfer but to service volume reduction. It is desirable to utilize the saving from practice pattern change as incentive to improve quality of care.
Purpose: This is a phenomenological study to describe the experiences of nursing students on home visiting nursing service as a community nursing practice. Method: Individual interviews were conducted on subjective experiences of 17 nursing students. Data were analyzed through Colaizzi's method in which meaningful statements were extracted and these were clustered into 6 themes. Result: The nursing students started practice with anxiety and expectation at the same time. They were frightened at the clients' inferior environment and their level of loneliness. They also felt pity and experienced complicated feelings for the clients. However, the home visiting practice was a chance for them to discard prejudice on the clients. Positive experiences on visiting nursing practice reported by the nursing students included lively interactions between nurses and the clients, and variable provision of primary nursing care. However, facts such as much limited visiting time, non-professional and limited scope of practice were reported as negative experiences. They felt both worthiness of the home visit service and restricted self-capability at the same time through the practice. They also felt sorry for the clients because the home visit services were carried out during limited time period. Regardless of this, the home visit experience provided them an opportunity of self-growth. This self-growth includes increased awareness of issues for elderly, building of self-identity as a nursing student, self-reflection, and realization of the value of family. Conclusion: This study may provide data for better understanding of nursing students experiences of home visiting nursing services. However, more study on the barriers of their community health practice is needed in the future. Moreover, it is needed to establish desirable practice environment through the collaborative relationships between the university and staffs in the public health center.

Background: The objective was to assess the knowledge level, attitude, and practice of health care workers towards standard precautions, and to identify the related factors. Furthermore, it was attempted to identify the proportion of having the experience of needle stick injuries (NSIs) and associated factors among participants. Methods: A cross-sectional study was conducted in a teaching hospital among 233 health workers using a self-administrated questionnaire. The questionnaire included eight knowledge items, seven practice items, and five attitude items. Based on the mean score of each category, responses were grouped into "satisfactory" and "unsatisfactory". Univariate, bivariate, and multivariable logistic regression analyses were done. Results: The mean age of the participants 32.95 (SD ± 9.70) and 62.2% of them were women. 57.5% of the staff had a satisfactory level of correct knowledge (>5 correct answers), 37.3% had a satisfactory positive attitude (>3 correct answers), and 30.9% had a satisfactory practice (>3 correct answers) towards standard precautions. The occupation was one of the predictors as doctors were less likely to have satisfactory knowledge and practice compared to nurses (OR = 0.269, 95% CI: 0.10-0.70 and OR = 0.248, 95% CI: 0.08-0.77, respectively). Out of 174 participants, 31.6% of them reported experiencing NSIs and support staff were 71% less likely to experience NSIs compared to nurses & paramedics. Conclusion: The findings revealed a substandard adherence of standard precautions among participants, which highlighted the necessity of the provision of a periodic, tailored training program based on the occupation and risk exposure.
Objectives: Many studies have shown that social distancing, as a non-pharmaceutical intervention (NPI) that is one of the various measures against coronavirus disease 2019 (COVID-19), is an effective preventive measure to suppress the spread of infectious diseases. This study explored the relationships between traditional health-related behaviors in Korea and social distancing practices during the COVID-19 pandemic. Methods: Data were obtained from the 2020 Community Health Survey conducted by the Korea Disease Control and Prevention Agency (n=98 149). The dependent variable was the degree of social distancing practice to cope with the COVID-19 epidemic. Independent variables included health-risk behaviors and health-promoting behaviors. The moderators were vaccination and unmet medical needs. Predictors affecting the practice of social distancing were identified through hierarchical multiple logistic regression analysis. Results: Smokers (adjusted odds ratio [aOR], 0.924) and frequent drinkers (aOR, 0.933) were more likely not to practice social distancing. A greater degree of physical activity was associated with a higher likelihood of practicing social distancing (aOR, 1.029). People who were vaccinated against influenza were more likely to practice social distancing than those who were not (aOR, 1.150). However, people with unmet medical needs were less likely to practice social distancing than those who did not experience unmet medical needs (aOR, 0.757). Conclusions: Social distancing practices were related to traditional health behaviors such as smoking, drinking, and physical activity. Their patterns showed a clustering effect of health inequality. Therefore, when establishing a strategy to strengthen social distancing, a strategy to protect the vulnerable should be considered concomitantly.
The present study is a descriptive survey on factors affecting middle school students' oral health belief and oral health practice. In order to find methods for forming right beliefs and inducing desirable behavioral patterns, we conducted a questionnaire survey with 470 students at two middle schools in Cheongju and drew conclusions as follows. Of the 470 questionnaires, 457 were recovered. With 10 inappropriately answered ones excluded, 446 questionnaires were analyzed. 1. As to oral health belief according to general characteristics, susceptibility was 2.63 points (p=0.035) in male students, 2.68 (p=0.016) in 3rd year students, and 2.92 (p=0.002) in only child students. Seriousness was 2.57 (p=0.017) in male students, 2.67 (p=0.001) in 3rd?year students, and 2.92 (p=0.001) in only child students. barrier was 2.62 (p=0.009) in 3rd year students, and 2.95 (p=0.002) in only child students. Benefit was 3.40 (p=0.011) in female students, and 3.43 (p=0.003) in 1st year students. salience was 3.21 (p=0.006) in female students, and 3.24 (p=0.009) in 1st year students. 2. As to oral health belief according to oral?health?related factors, susceptibility and seriousness were 2.69 (p=0.003) and 2.72 (p=0.000), respectively, in the lower?middle class group, 2.83 (p=0.001) and 2.80 (p=0.003), respectively, in the drink taking group, and 2.80 (p=0.000) and 2.75 (p=0.000), respectively, in the low conversation group. barrier was 2.63 (p=0.018) in the lower?middle class group, 2.67 (p=0.021) in the low conversation group, and 2.77 (p=0.000) in the group fearing the visit to the dental clinic. Benefit was 3.36 (p=0.000) in the high conversation group, and 3.37 (p=0.0l5) in the group visiting the dental clinic for prevention. salience was 3.26 for beverage and 3.20 (p=0.003) for fruit and vegetable. 3. As to oral health practice according to general characteristics, the score of oral health practice was 3.21 (p=0.000) in female students, 3.30 (p=0.000) in those aged 12 or below, and 3.27 (p=0.000) in 1st?year students. 4. As to oral health practice according to factors related to oral health, the point was 3.17 (p=0.002) in the middle upper class group, 3.24 (p=0.001) in the group eating mainly fruit or vegetable, and 3.18 (p=0.030) in those with experience in education. 5. Oral health practice was in a negative correlation with susceptibility (r=-0.143), and in a positive correlation with benefit (r=0.229) and salience (r=0.286).
This study is for the purpose of studying the validity of the construction of items and grading of the evaluation table at clinical practice used as the method of evaluation when clinical practice of Dept. of Dental Technology. 205 dental technicians in 4 cities were questioned to analyze the relation between items of the evaluation table at clinical practice and its grading. The collected materials were inspected by SAS V8 for Windows. The results are as follows: 1. Making up all the items and grading seem to be appropriate because they had the marks more than midium(more than 3.28). 2. Making up detailed items and grading seem to be trusted because they had the marks more than 3.30. 3.The significance level of the relation between items of the evaluation at clinical practice and its grading by t-inspection is p<0.0001 and so it appears to be meaningful. 4. It proved to be meaningful in result of regression analysis of the influence on the construction of the evaluation at clinical practice and its grading. With this result, further study is needed for the better methods of the evaluation in its adjustment and graduated grading among the items.
Purpose: The purpose of this study was to provide baseline data on paid hospital-caregivers' work performance, degree of importance of caregiving, and their knowledge as well as affecting factors on the level of practice when there is no guardian. Methods: The subjects of this study were 155 total caregivers working at 9 hospitals. Data were collected by self-reported questionnaires in December, 2010. The collected data were analyzed using SPSS/WIN 17.0. Results: The caregivers' level of practice showed 4.0/5.0 points, degree of importance, 4.4/5.0 points, and knowledge, 4.3/5.0 points. The level of practice was significantly different depending on the hospital types. Significant correlations were found between level of practice, degree of importance and knowledge. The degree of importance and types of hospital attributed to 17.1% of variance in the level of practice. Conclusion: A systematic hospital caregivers' program focusing on strategy to enhance caregivers' level of practice, degree of importance and knowledge can be effective for paid hospital-caregivers when there is no guardian.
Purpose: While clinical practice is crucial for nursing students to acquire the skills needed to provide professional, high-quality nursing care, further studies on improving undergraduate nursing programs are needed to provide a supportive clinical learning environment for student nurses. This study aimed to understand nursing students' clinical experiences in newborn nurseries and neonatal intensive care units and to provide basic data for the establishment of strategies to promote effective clinical education. Methods: Interviews were held with 15 nursing students at J University who had clinical practice experience in the newborn nursery and neonatal intensive care unit. The collected data were analyzed using the phenomenological analysis method developed by Colaizzi (1978). Results: The nursing students' experiences were grouped into four categories: "expectations for and anxiety about clinical practice", "acquisition of a wide range of knowledge regarding neonatal nursing", "challenges faced in clinical practice", and "experiencing interpersonal changes". Conclusion: The current neonatal practice nursing education system provides students with positive learning experiences. However, the lack of practice opportunities, insufficient instruction, and the theory-practice gap were identified as major issues hindering students' learning needs. These study results are expected to provide basic data for curriculum development to improve undergraduate nursing education.
Background: The purpose of this study is to investigate the factors affecting the healthy living practice rate such as non-smoking, moderate drinking, walking, and low-salt diet by elementary municipality (so called, 'si-gun-hu'). Methods: The 2016 Korean Community Health Survey was used for the analysis. The theoretical model is founded upon the Anderson model, and both the multiple linear regression analysis and the beta regression analysis was performed for estimation. Results: As a result of the beta regression analysis, healthy living practice rate was found to be significantly higher in the areas with a less number of cigarette retailers, participating in healthy city projects, a low proportion of people who perceive their body type as obesity, a higher proportion of women, and a lower proportion of spouses. Conclusion: In order to improve healthy living practices, the regulations on health risk businesses, the spread of Healthy City project, and policy efforts awaring obesity are recommended.
Purpose : This study aimed to understand and describe the experiences of collaborative practice among health professionals in Korea. Methods : Data were collected through individual in-depth interviews conducted from January to March 2021 with 10 health professionals in Korea. All interviews were audiotaped and transcribed verbatim. The transcribed data then underwent qualitative content analysis. Results : Through content analysis, 10 categories and 3 themes were formed. Three themes, "working based on knowledge," "having difficulty in building collaborative relationships," and "playing complementary roles" emerged. Conclusion : The limitation is that the study participants are 10 health professionals in Korea working at healthcare institutions. However, this study is of great significance in that it provided basic data for preparing measures for collaborative practice among health professionals in Korea.
