Methods: The subjects were 5,149 adults over 19 years old who participated in the 5th Korea National Health and Nutrition Examination Survey 2012. Data were analyzed by chi-square test and logistic regression analysis using SPSS 18.0 statistical package program. The questionnaire consisted of 17 questions of independent variables and 2 kinds of dependent variables including dental caries and periodontal diseases by direct interview. The independent variables included 5 questions of general characteristics, 3 questions of health behavior, 5 questions of oral health behavior, and 4 questions of chronic diseases diagnosed by the doctors. Results: The prevalence rate of dental caries was higher in those who did not regularly work out and did not receive regular checkups including dental floss and dental brush use. The prevalence rate of periodontal diseases was higher in male adults, smokers, and those who were obese and had diabetes mellitus, poor oral health care habit without using dental floss and inter-dental brush. Conclusions: In order to prevent the dental caries in the adults, it is very important to do regular exercise and regular dental checkup. The best ways of dental caries prevention include tooth brushing after meal with use of dental floss and inter-dental brush. This study suggests that dental health promotion can be enhanced by smoking cessation, ideal body weight maintenance, timely tooth brushing after meal, and use of dental floss and inter-dental brush.

  • Background: The present study aimed to digitally evaluate the risk of overwork-related adverse effects (OrAEs) among employees from various occupational categories in Taiwan. Methods: Anonymous data of employees from seven companies/factories providing occupational health services were analyzed. The studied population comprised 5505 employees, and the data analyzed included employment duration, working hours, shift work schedules, and health checkup results. The risk for OrAEs was assessed by an index, Karo index (0-4, the larger the value, the higher the risk for OrAEs) obtained using a risk matrix made up of cardiocerebral and occupational risk factors. Karo index values of 3 and 4 were categorized as at high risk for OrAEs (h-OrAEs). Results: The 5505 employees had an average employment duration of 8.5 years and a mean age of 39.4 years. The prevalence rates for h-OrAEs of the seven companies/factories ranged from 3.9% to 34.2%. There were significant differences in prevalence rates for h-OrAEs between employees of retail stores and high-tech manufacturing factories. Multivariate analysis results indicated that workers of high-tech manufacturing factories had significantly higher risk for h-OrAEs compared with retail store workers. Conclusion: In terms of satisfying health risk management and legal requirements in Taiwan, the newly issued Karo index, which covers a wide range of occupational risk factors, can serve as an assessment and a warning tool for managing the risk of OrAEs in workplaces. To reduce risks for h-OrAEs, active and prudent control of cerebrocardiovascular risks and working hours is recommended.

  • The Act on Special Cases Concerning the Punishment, etc. of Child Abuse Crimes intends to encourage reporting and punishment of child abuse by using the concept of 'crime' in child abuse cases. Article 10 of the Act imposes duty to report child abuse on a number of different professions, including medical professionals. Currently, more than 80% of child abuse cases occur among family members and the detection rate of child abuse is as low as 0.5% in Korea. On the other hand, medical professionals can identify child abuse relatively clearly with specific medical opinions. Therefore, it is necessary that medical professions are informed of this duty and does not bear disincentive from reporting. This paper makes policy suggestions in this regard. First, it is necessary that medical students and medical professionals receive regular education about the obligation to report child abuse. Education should include details of the reporting duty, as well as the fact that there is legal obligation to report even if the child abuse is "suspicious", not certain. Second, it is imperative to establish and implement protective programs for medical professionals who report child abuse. The current law provides a rough framework for protection of people who report child abuse, but it is necessary to produce detailed guidelines that are applicable in the context of medical setting. Education for medical students and medical professionals should include the contents of these guidelines, so that they do not hesitate reporting because they fear the aftermath of reporting. Third, it is highly recommended that physicians use the national Baby/Infant Health Checkup Program as an opportunity to detect child abuse. In Korea, the Baby/Infant Health Checkup Program provides physicians to periodically monitor health condition of all babies and children until the age of 71 months. In order to utilize this program for early detection of child abuse, it is imperative that the bBaby/Infant Health Checkup Program is modified to involve child abuse experts and medical professionals who participate in the program are educated about child abuse.

  • Objectives : The aim of this study is to investigate dental clinic fear and anxiety in high school students. Methods : The subjects were 311 high school students aged 19 years. They completed self-reported questionnaire including general characteristics, dental clinic experiences, dentist or dental hygienist confidence, and dental fear. Data were analyzed using SPSS 18.0. Results : Poor oral health status showed negative attitude towards dental clinic visit. Girl students tended to have more fear than boy students. Muscle tension was the most common experience and was closely related to anesthetic needles and drills. Dental anxiety was caused by lack of confidence to dentists and dental hygienists. Students were more afraid of dentists than dental hygienists. Higher score of dental anxiety seemed to be associated with infrequent visit to dental clinic. Conclusions : Frequent regular dental checkup may reduce dental fear and anxiety. Dental clinic staff should try to minimize dental anxiety in performing treatment.

  • Due to the aging effect, older people have relatively weaker muscular performance, less range of motion in the joint articulation, and the lower sense of equilibrium than younger people. These factors attribute to their slow and clumsy ingress/egress motion. In order to analyze ingress/egress motion strategy of the elderly, healthy thirty 65 or more years old volunteers were recruited. The health condition of the each volunteer was verified by the medical checkup and also their physical capabilities were quantified by six fitness tests. Through the video analysis, older driver's ingress/egress motion strategies were classified and statistically investigated. For a comparison purpose, another thirty young volunteers also participated in the same test protocol and their ingress/egress motion strategies were also included in the statistical analysis.

  • Objectives : The purpose of this study was to examine the factors related to the dental caries and periodontal diseases of Korean elderly people in terms of demographic characteristics, oral health promotion behavior and systemic diseases in an effort to provide information on national policy setting and policy evaluation about the prevention of tooth loss resulting from severe oral diseases and the promotion of elderly people's oral health. Methods : The first-, second- and third-year raw data of the 4th National Health and Nutrition Survey were utilized, and the data of 3,882 elderly people who got a dental checkup were analyzed. The statistical package SPSS WIN 19.0 was employed to make a logistic regression analysis. Results : The senior citizens who did toothbrushing less frequently were more likely to have dental caries. As for periodontal diseases, the men were 1.34-fold more likely to have periodontal diseases than the women, and the respondents whose self-rated health state was worse were 1.40-fold more likely to have periodontal diseases than the others whose self-rated health state was better. The senior citizens who ever received treatment from unqualified people were 1.30-fold more likely to have peridontal diseases, and those who took neither vitamin compound nor minerals were 1.30-fold more likely to have periodontal diseases. Those who suffered from low High-density Lipoprotein cholesterol(HDL) were 1.35-fold more likely to have periodontal diseases than the others who didn't. Conclusions : Those whose self-rated health status is worse should especially be concerned about periodontal diseases. Specifically, it's needed to pay attention to the low-income classes, and the government should take measures to provide quality welfare services for elderly people not to receive treatment from unqualified people. Besides, research efforts should be made to determine the relationship between periodontal diseases and low HDL-cholesterolemia.

  • The purpose of this study was to examine the impact of the oral health status and health care of elderly people on their self-rated health(SRH). The subjects on this study were 479 senior citizens who were at the age of 65 and up and resided in the city of Busan. They got a dental checkup, and a survey was conducted by having an one-on-one interview. After the collected data were analyzed, the following findings were given: The senior citizens were diagnosed with a mean of 1.43 systemic diseases, and hypertension(51.8%) was the most prevalent disease among them, followed by diabetes(25.1%), arthritis(41.8%), oral diseases(75.6%), stroke(9.0%) and heart diseases(15.9%). Their self-rated health was better when they were male, when they were aged between 65 and 69, when there was someone with whom they lived, when they were better educated and when they owned their own houses. But their self-rated health was poorer when they felt more oral symptoms, when they had more missing teeth and when they needed both of maxillary and mandibular dentures. Their self-rated health was more positively affected when they were better educated($\beta$=0.894), when they owned their own houses($\beta$=4.220), when they got a dental checkup on a regular basis($\beta$=2.997) and when the rate of their functional tooth was larger($\beta$=0.081). And that was more negatively influenced when they had a denture($\beta$=-1.110), when they had more oral symptoms($\beta$=-1.590) and when they had more systemic diseases($\beta$=3.363). There is a close relationship between the oral health and self-rated health of elderly people. Therefore how to promote their oral health should carefully be considered.

  • Objectives: To identify factors responsible for potentially clinically unnecessary cervical cancer screenings in women with prior hysterectomy. Methods: A retrospective cross-sectional study was conducted using the Centers for Disease Control and Prevention's Behavioral Risk Factor Surveillance System (BRFSS). This study targeted adult women and examined whether they received a both a Papanicolaou (Pap) test and undergone a hysterectomy in the last three years. We conducted multivariate analyses, including weighted proportions and odds ratios (ORs), based on the modified BRFSS weighting method (raking). The inclusion criteria were adult women (>18 years old) who reported having received a Pap test within the last 3 years. Results: Of all women (n=252 391), 72 366 had received a Pap test, and 32 935 of those women (45%, or 12.5 million, weighted) had a prior hysterectomy. We found that age, race/ethnicity, marital status, family income, health status, time since last routine checkup, and health insurance coverage were all significant predictors. Black, non-Hispanic women were 2.23 times more likely to receive Pap testing after a hysterectomy than white women (OR, 2.23; 95% confidence interval [CI], 1.99 to 2.50). Similarly, the odds for Hispanic women were 2.34 times higher (OR, 2.34; 95% CI, 1.97 to 2.80). The odds were also higher for those who were married (OR, 1.17; 95% CI, 1.08 to 1.27), healthier (OR, 1.24; 95% CI, 1.14 to 1.35), and had health insurance (OR, 1.54; 95% CI, 1.28 to 1.84), after controlling for confounders. Conclusions: We conclude that women may potentially receive Pap tests even if they are not at risk for cervical cancer, and may not be adequately informed about the need for screenings. We recommend strategies to disseminate recommendations and information to patients, their families, and care providers.
