Effect of Long-term Care Utilization on Health Care Utilization of the Medicaid Elderly (국민기초생활보장수급자의 장기요양 서비스 이용 여부가 의료 이용에 미치는 영향)

  • This study examined the effect of long-term care utilization on the health care utilization of Medicaid elderly. The subjects were 5,834 long-term care insurance with the level 1 Medicaid elders, who received either service or non-service. This study examined the impact of long-term care service on the probability of health care utilization and the costs of health care utilization. The total medical cost and inpatient day between 2009-2007 were significant factors affecting long-term care utilization (${\beta}=.29$, p< .001, ${\beta}=.33$, p< .001 ) and this variable explained 22.6% of the total medical cost and 22.4% of the inpatient day. The results showed that non-service in long-term care was associated with an increase in health care utilization. The current long-term care insurance system should place higher priority and more resource allocation on long-term care utilization to increase the efficiency of the insurance system.

  • Due to the rapid population's aging, the life span's expansion and social & cultural characteristics, the standards of age classes in senescence are changed. Thus, this study aimed to identify the influential factors on health care utilization in the elderly by age class, and targeted 2,937 adults in their ages over 55 years old from the Korea Health Panel's 2009 Yearly Integrated Data. This study investigated the target health care utilization in the elderly in relation with the gender, the education level, the health-care insurance type, the income, the current job state, the chronic disease. In order to analyze the influential factors on health care utilization in the elderly, the multiple linear regression analysis was conducted to the data. As the results from this study, Concerning the influential factors on the health care utilization, the income, the subjective health state, the chronic disease and the regularly meals for the young-old influenced. Concerning the influential factors on the health care utilization, the subjective health state for the old. Concerning the influential factors on the health care utilization, the subjective health state, the income for the older-old influenced. Concerning the influential factors on the health care utilization, the education level, the spouse, the economic activity and the drinking oldest-old influenced. Therefore, it will need to provide systemic health-care & medical services, to develop health-care & medical programs and the health-care & medical policies and to execute them according to the age classes in senescence.

Outpatient Health Care Satisfaction and Influential Factors by Medical Service Experience (의료서비스경험에 따른 외래 의료서비스 만족도와 영향 요인)

  • Objectives: This study investigated outpatient health care satisfaction and influential factors using data from the "2018 Health Care Experience Survey". Methods: Data on 6,705 participants in the Health Care Experience Survey were statistically examined using a t-test, ANOVA, and multiple regression analysis. Results: First, the level of satisfaction among outpatients was generally positive, and satisfaction among older people was high. Second, the significant factors of outpatient overall satisfaction were gender, consultation time, waiting time, reason for choosing institution, satisfaction with doctors, nurses, institutions, and the therapeutic result. Third, influential factors of intention to recommend were education, institution type, reason for choosing institution, satisfaction with doctors, nurses, institutions, and the therapeutic result. Conclusions: In order to improve the overall satisfaction of outpatients, the treatment method should be improved. For example, the doctor should devote more time for and engage in sufficient conversation with the patient, the nurse should be polite, and patients should be given easy-to-understand explanations.

A Study of Job Educational Satisfaction for Activating of Tailor Made Visiting Home Health Care (맞춤형 방문건강관리사업 활성화를 위한 직무교육 만족도)

  • Purpose: The aim of this study was to establish basic data for developing education program of tailor made visiting home health care. Method: For this study, the subject was recruited in one directorial area among 14 nation-wide areas. The data collection was done using self-report questionnaire developed by Korea Health Industry Development Institute. Ninety one questionnaires were analyzed and response rare was 88.3%. Result: The major results were as follows. Aspect of education operations, the satisfaction of education space was revealed high. However, the satisfaction of education time, hour, and schedule were low. The overall satisfaction of education contents were revealed high. The most useful curriculums were found Chronic disease management, Infant & toddler health management, and Case management of tailor made visiting home health care. However the most unuseful curriculums were Nutritional management according to subject's health problem, Health promotion for poor family, and Understanding of visiting service in community health center. Conclusion: With these results, it was identified that the most needed contents of tailor made visiting home health care education program. So these results will be used to develop the more effective education program to activate tailor made visiting home health care service in community health center.

A Study on the Intention to Use Community Care Service by Pre-Caregivers Based on Dementia Under the Long-Term Care Insurance System for the Aged -Special dementia rating (5 rates) in the focus (예비치매 부양자의 재가서비스 이용에 관한 연구 -치매특별등급(5등급) 중심으로)

  • Purpose. The purpose of this study was to provide basic data regarding the intention to use community care service based on special grading for dementia in the South Korean Society. Methods. A survey was conducted using a questionnaire adapted on the basis of literature review regarding the first to fourth grade under the long-term care insurance system for the aged from May 1 to 30, 2015 and a total of 272 questionnaires were statistically processed using SPSS 21.0. Frequency analysis, descriptive statistics, and cross-tabulation analysis were performed. The non-parametric type of test, ${\chi}^2$ test, was used for statistical testing. Results. This study obtained the following results: First, caregivers had greater intention to ask recipients to use daytime and nighttime care service. Second, there were statistically significant differences in caregivers' education, occupation, monthly average income, co-residence status, intimacy, and care-giving burden (emotional, financial, and social) and recipients' activities of daily living, instrumental activities of daily living, mental health status, dementia status, and intention to use community care service based on special grading for dementia. Conclusions. There are very few statistical data and academic materials regarding special grading for dementia, which has recently been established. The results of this study are therefore expected to help make a systematic analysis of the intention to use community care service by caregivers based on special grading for dementia.

Factors Affecting the Use of Postpartum Care Services (출산 여성의 산후관리서비스 이용에 영향을 미치는 요인)

  • Objectives : This study aims to analyze differences in the use of postpartum care services and identify the factors affecting their use. Methods : Data were collected from the 2008-2014 Korean Health Panel. Chi-square tests were conducted to analyze differences in the characteristics of women after birth by the use of postpartum care services. Logistic regression analysis was used to identify factors affecting the use of each type of postpartum care service. Results : Participants with higher education and household income levels were more likely to use a postpartum care facility. Individuals with a lower number of household members, who gave birth in 2011-2013, and who used hospitals specializing in obstetrics and gynecology had a greater likelihood of using a postpartum care facility. The probability of using a postpartum caretaker was higher when participants did not perform any economic activities. Conclusions : It is meaningful to confirm that use patterns and determinants of postpartum care facilities and postpartum caretakers are completely different and that the socioeconomic status of women affects the utilization of postpartum care facilities.

A Case Analysis of Health and Safety Management of Child Care Center (어린이집 질병 및 안전사고 사례분석)

  • The purpose of this study was to investigate the situation of occurrence of disease and accidence of child care center. The subjects of this study were 17 public district child care centers, but only one center kept their health diary. Therefore, it became finally the only subject for this study. The data were collected through the analysis it's health diary and case reports for emergency. The collected data were analyzed by the number of cases, age and sex, types of case, and the emergency case were analyzed by age, background, types of accidents and follow up. The number of cases of disease and accident in 2002 were 572. In sexual difference, boys more have accident than girls. The teachers and the outsiders also frequently use the health care service. Smallpox and epidemic conjunctivitis were spreaded during winter and summer. In causes of accidents, 'accident by other child' were 98%. In emergency cases, 1 pierced wound, 1 dislocation, 2 dental emergencies, 4 eyeball contusion and bleedings and 2 burns were occurred. all the cases of emergency were performed follow up education. On the basis of above data, there will be needed to intensify health and safety subjects in curriculum for the teacher of child care, and health and safety education for child. Each child care centers must have health care manager and the network for emergency. To enhance the quality of child care service, government have to offer financial and systematical support.

The Comprehension of health care service bill of radiological technologist and dental hygienist and the study of necessity of including the service supply resources (방사선사 및 치위생사의 건강관리서비스법안 이해와 서비스 제공요원 포함의 필요성에 관한 연구)

  • This study is to present basic materials to further selection of details of health care service bill and supply resources by analysing the consciousness of radiological technologist and dental hygienist about health care service bill proposed for prevention and early diagnosis of a disease through the improvement of living habit. Subjects were 359 of dental hygienists and radiological technologists investigated from 2010 July to September for three months. As the result, recognition level on health care service bill or its details was less than 30.0% but the opinion that health care service bill is necessary was 78.0%. In its additional item, radiological technologist hope to be maintained currently, and 82.3% of the dental hygienist hope to be added, their prefered additional item was dental disease. A dominant opinion was that dental hygienist and radiological technologist as a service supply personnel were included, and their intention to participate was 8.1 in average. In conclusion, the health care service bill is indispensable, but more systematic study is required based on collecting various opinions about major contents and service provider. Particularly it is inevitable to include the health human resources such as experienced dental hygienist and radiological technologist who had completed the regular course of health education and are having continuous clinical discipline.

Study on Effective Utilization Strategies of the Home Health Care Program in the Community Care Service of Welfare Policy (재가 복지 봉사 사업의 가정 간호 사업연계 필요 조사)

  • This study was designed out to develop a home health care service for nurses working in community care services. This study investigates actual conditions at welfare institutions related to health needs the demands of clients, and the state of home health care services we hope that this study will improve upon the current service system. In Korea home health care services are still developing and only new becoming a part of the health care supply system. The data was collected by recording the client home nursing assessments modified to the situation of UTMB home health agency. In this study 107 clients were selected for home care who needed care for physical and mental deficits. The study lasted from March to November of 1995 at one of the welfare service institutions in Chunchon city. The results show that those who most frequently needed care services were over 50 years old with a health deficit of 80.3%, followed by sex as women who needed care at 59.8%. 50.5% of the clients had very little education. 99.1% of the clients live with their family, and a medical diagnostic analysis reveals that 73.9% of the 5 year period of illnesses were the following : 38.8% - muscular -skeleton system disorder, 24.4% - hypertension and stroke, 25.7% sole disease of arthritis. For behavioral conditions 43.3% of the patients were without care services, 56.6% of the patients were taking treatment that 73.5% of those were taking medication. The most main complaint of patients were 22.4% of pain in the extremities, next were 16.8% of a limitation of body activities, 15.0% was lumbo-sacralgia. According to the investigator who was a senior student nurse, the following suggestions were made: 32.7% for curative medical services, 29.9% for physical exercise, 19.6% for emotional support. Consultation nursing services consisted of 67.2% for physical therapy, 11.2% for the maintaining healing, 9.4% for counseling. The patients at home, required assistance most frequently for muscular-skeleton problems under the category of physical systems (33.3%). But, on the other hand, 49.5% of the patients required care givers at home, 28.2% had a knowledge deficit, 21.0% had malnutrition, 18.4% had bad impaired communication. The character of health problems were devided into chronic disease(67.0%), accidents(I3.1%), and general disease(15.9%). 86% of the disabled client had an impairment of the physical system. Eating (86.9%) , Toileting(77.6%), and personal care showed much the same of ADL condition, the level and range of achievement of mobility, the most frequently self performed was 81.3% only in a room size area, and 40.2% were completely dependent when going out. Although there were a large number of home care services in th community at these welfare institutions, many clients needed a variety of curative services. As policy changes have gathered momentum, responsibility for the development of a more suitable program was demanded by the clients from the community.

Management of Food Service and Health Care in Long-Term Care in Korea - Food Service and Health Care - (노인복지시설의 급식관리 및 영양 건강 증진에 관한 연구 - 급식과 건강관리 -)

  • With increases in senior citizens and changes in family structure, the need for long-term care system for elderly is increasing, however, the capacity and environment of Korean facilities are very limited. Health and nutritional status of long-term care residents are worse than free-living elderly. In this study, general food service management and health care practices in long-term care systems were investigated. Questionnaire were sent to the directors of all 162 long-term care facilities in Korea and 81 returned the complete answers. The results showed 1) There are slow but steady increase in long-term care systems in Korea, however, the capacity is far from adequate. Less than 10,000 elderly were resided in the facilities. Most of the systems were free-nursing homes and supported by the goverment. Staffing structure revealed that most of the facilities had a director, a secretary, nurses, but only 21% of the systerm hired a dietitian. It showed the shortage of nurses, physical therapists, and dietitians. Therefore, food purchasing, menu planning, food delivery, and the other food service management processes are handled by non-professionals, such as director, secretary, or cooks. Modified fool frequency questionnaire were used to get the frequencies of each food items used in menu and a menu-analysis was made on the one-day menu provided by the facilities. The results showed relatively satisfactory in nutrients content and food frequencies., however, this was about what was used in menu, not what was eaten by the residents. Therefore this results did not tell that the food intake status of individuals. In most facilities general health checkup was done on a regular basis, and had residents with various chronic degenerative diseases, such as hypertension, neuralgia, stroke, arthritis, diabetes. But the items checked on health checkup included weight, height, blood and urine tests, X-ray test, which suggested that the checkup lists should be revised to accomodate the health problem of the aged today.
