시공간 관점에서 본 원격진료 이용자 행태 변화에 관한 연구 (A Geographical Study on the Behavior Changes of Telemedicine Participants in Terms of Time and Space)

  • 본 연구에서는 원격진료 이용자 행태를 시공간 재구조 관점에서 살펴보았고, 원격진료의 양면적 성격, 즉 확장성과 제약성에 관해 고찰하였다. 연구 결과에 의하면, 원격진료의 도입은 직접적인 온라인 서비스의 수혜자인 환자의 행태 변화에 영향을 끼치나, 이는 원격진료 이전의 일반적 의료 행태와 확연한 차이를 보인 것은 아니다. 이와 더해, 의원급 병원의 의료진 및 원격진료 최종 서비스 제공자인 토가네현립병원의 의료진의 행태에는 거의 영향을 주지 않는 것으로 나타났다. 원격진료의 도입으로 인해 의료기관에로의 접근성 및 효용성의 향상에 도움이 된 것은 사실이나, 여전히 원격진료는 최소한 한번 이상의 직접 진료를 요구하고 있고, 인간의 생명과 직접적으로 연계됨에도 불구하고 불완전한 기술적 특성을 가지고 있으며, 원격진료 이용에 있어서의 의사-환자, 의사 사이의 합의 문제로 인해 시공간적 제약성을 내포하게 된다. 결과적으로, 당초 원격진료가 처음 고안되었을 당시 많은 이용자들에게 주었던 기대감(확장성)과 달리, 오늘날의 원격진료는 시공간을 초월한 개념이라기보다는 기술, 사회, 문화, 경제 등의 다양한 요인으로 인해 제약성을 내포한 형태로 발전하고 있다고 할 수 있다.

누적외상성질환 위험 요인의 정량적 평가 및 관리를 위한 점검표 개발 -자동차 조립 작업을 중심으로- (The Development of a Checklist for Quantitative Assessment of Risk Factors and Management of Cumulative Trauma Disorders : Application to Automobile Assembly Lines)

  • Objectives : This study was designed to develop and standardize a checklist for ergonomic risk factors, and to provide ergonomic guidelines for managing cumulative trauma disorders (CTDs) in automobile assembly lines. Methods : The Checklist for Ergonomic Risk Factors (CERF-1) was developed based on the results of previous studies, and then modified after performing pilot study. Information on the symptoms possibly related with CTDs was obtained using a self-reported Questionnaire from 465 automobile assembly workers. Their job conditions were examined to assess risk factors through both direct observation and video analysis. Results : Rate of detecting risky job through CERF-1 was 85.6%, and was similar to that (88.8%) by Occupational Safety and Health Adminstration(OSHA) checklist but higher than that (63.7%) by American National Standards Institute(ANSI) Z-365. Relationship of the exposure scores derived from CERF-1 with levels of symptom was greater (r=0.49) than OSHA (r=0.28) and ANSI Z-365 (r=0.22). Considering the relationship, jobs scoring higher than 16 could be classified as the Risk Job. and lower than 16 as the Low Risk Job. Sensitivity and specificity of the Risk Job were 92.5 % and 31.5 %, respectively. Odds ratio (OR) after age adjustment was 5.69 (95 % confidence interval 3.15-10.29) for the Risk Job, and these ORs were significantly different from those of the Low Risk Job. The exposure scores were Quite valid, in that the scores at the main survey were significantly correlated with those at the follow-up survey, as suggested by test-retest(r=0.88) and inter-rater reliability(r=0.80). Conclusions : The CERF-1, developed in this study, will be an efficient tool for evaluation of risk jobs for CTDs in automobile assembly lines, and can be used easily by health care providers.

일상화된 회음절개술에 대한 여성의 지식, 교육요구, 불편감 및 통증정도에 관한 일 연구 (A Study on Women's Level of Educational Need & Knowledge about Routine Episiotomy and the Degree of Discomforts and Pain after Episiotomy)

  • The purpose of this study was to find out women's need and level of knowledge about episiotomy, pain and discomfort related to episiotomy on a cross-sectional survey design. The subjects were 102 postpartal women agreed on oral consent. 34 postpartal women admitted at obstetric ward of H university hospital, 34 postpartal women admitted at 2 Sanhujoriwons, and 34 women within one year afterbirth. They were selected in Seoul, Korea. Data were collected from July, 1 to September 30, 2000, by a structured questionnaire. The instrument used for this study was a questionaire consisted of 5 items of general characteristics, 12 items of obstetric characteristics, 10 items of level of knowledge (Chronbach $\alpha$ .8176), 8 items of need of education(Chronbach $\alpha$ .8836), 3 items of pain (Chronbach $\alpha$ .9252), and 3 items of discomfort (Chronbach $\alpha$ .8092). The data were analyzed by the SPSS/PC+ program using t-test, ANOVA and Scheffe test as a post hoc and Pearson Correlation Coefficient. The results of the study were as follows; 1. 63.2% of respondents had right answer on 6-8 items among 10 items. Only 4.4% of women got right answer on 10 items all. 2. The need of education was high(4.45%) on all items and the range of score was $4.25{\sim}4.64$. 3. The strength of pain was the highest within one week afterbirth(5.93/10) and became lower in 8-14 days afterbirth(2.55). And after 15days of postpartum, the pain level became to the lowest level(1.08). However, pain was delayed until more than one month afterbirth. 4. The level of discomfort was the highest one week afterbirth(6.88/10) and became lower in 8-14 days afterbirth(4.20). And after 15days of postpartum, the discomfort level became to the lowest level(2.47). Universally, the degree of discomfort was higher than pain. 5. There was a strong positive correlation between discomforts and pain ($r=.752^{**}$) and weak positive correlation between discomforts and the level of educational need($r=.308^*$). In conclusion, women have a right to choose whether she will have episiotomy or not according to her decision making based on the comprehensive knowledge of episiotomy before they get episiotomy with consent process and explanation in detail. Women health care providers like nurses have a responsibility to do conscious raising and empowerment for women so that they could lead themselves to choose given medical treatments for women's health and wellbeing and the quality of life in her life cycle.

산전 초음파검사에 대한 산모의 인식도 (Perception of Prenatal Ultrasound by Pregnant Women)

  • The purpose of this study was to assess pregnant women's knowledge of, attitude to, and satisfaction with prenatal ultrasound and to determine the factors that influence their perception. A self-administered questionnaire survey was completed by two hundred eighty five pregnant women who visited the obstetric departments of three hospitals located in Seoul from October 17, 2000 to October 28, 2000. The major results are as follows. 1. Overall, the respondents did not fully understand the purpose of prenatal ultrasound. 92.2% of respondents stated that the main purpose of prenatal ultrasound was to check the fetus' age, growth and development but only 44.5% of respondents were aware of the fact that chromosomal abnormalities cannot be diagnosed only by prenatal ultrasound. The majority of respondents were aware of the diagnostic limitations of ultrasound. 2. While the majority of respondents were aware of the importance of pre-examination information, only 31.8% of respondents received such information from their health care providers. 3. Regarding the examination quality, 80.3% of respondents were satisfied with the competency of the examination. But more than 50% of respondents stated the cost and waiting time were not acceptable. 4. The knowledge of, attitude to, and satisfaction with prenatal ultrasound showed statistically significant differences according to the characteristics of each hospital. 5. From multiple regression analysis, the major components for knowledge of prenatal ultrasound were characteristics of hospitals, religion, income and gestational age. The major components for attitude toward prenatal ultrasound were characteristics of hospitals, religion and gestational age. The major components for satisfaction with prenatal ultrasound were characteristics of hospitals, income and drinking during the pregnancy. In conclusion, the respondents' perception of prenatal ultrasound is considerably low. More effective educational material or programs with prenatal ultrasound information should be provided to pregnant women prior to prenatal ultrasound examination. New strategies such as process reengineering are recommendable to increase the satisfaction with prenatal ultrasound.

환자만족도에 영향을 미치는 의료서비스 품질 요인: A 중소병원 입원.외래환자 간 차이 (Medical Service Factors Associated with Patient Satisfaction: The Difference between Inpatient and Outpatient of A Medium-sized Hospital)

  • 이 연구는 A 중소병원의 입원환자와 외래환자를 대상으로 환자만족도에 영향을 미치는 의료서비스 품질요인이 입원 외래 환자 간 어떠한 차이를 보이는지 분석한다. 설문은 2011년 8월 20일부터 총 4주간 진행되었으며, 조사대상으로는 입원환자 96명, 외래환자 315명 총 411명이었다. 수집된 자료를 활용하여 다중회귀분석을 실시한 결과 입원환자의 경우 병원의 인지도가 환자만족도에 유의한 영향을 미쳤다. 반면, 외래환자는 의료진의 우수성, 병원 편의시설과 환경의 우수성, 친절한 직원, 병원의 인지도, 편리한 진료 및 행정절차 등 다양한 요인이 유의한 관계를 보였다. 이 연구는 중소병원이 의료 서비스를 개선하고 경쟁력을 갖추기 위해서 입원 및 외래 환자를 구분하여 각 환자군 별로 만족도에 영향을 미치는 요인을 차별화 하여 서비스 개선에 반영하는 것이 효과적일 수 있음을 제시해 주고 있다.

한의 외래환자분류체계 개선 및 평가 (Revision and Evaluation of Korean Outpatient Groups-Korean Medicine)

  • Objectives: This study aimed at revising the Korean Out-patient Groups for Korean Medicine (KOPG-OM, version 1.0) based on clinical similarity and resource use, by using the accumulated claims data, and evaluating the validity of the revised classification system. Methods: A clinical specialist panel involving 19 specialists from 8 Korean medicine (KM) specialty areas reviewed the classification tree, diagnosis groups and procedure groups in terms of clinical similarity. Several models of outpatient grouping were formulated, with the validity of each tested based on the $R^2$ coefficient of determination for the treatment costs of all visits. To add age splits, the variances of treatment costs by age groups were also analyzed. These statistical analyses were performed using KM claims data of National Health Insurance from 2010 to 2012. Results: The classification tree designed via panel discussions was used to allocate outpatient cases to 26 diagnosis groups, with cases involving procedures such as acupuncture, moxibustion and cupping, then allocated to 9 procedure groups in each diagnosis group. The cases without procedures were categorized into the visit index - medication group. This process resulted in 298 outpatient groups. The $R^2$ values for treatment costs of all visits ranged from 0.38 to 0.69 depending on the providers' types. Conclusions: The revised model of KOPG-KM has a higher validity for outpatient classification than the current system and can provide better management of the costs of outpatient care in KM.

의학전문대학원 학생들의 HIV/AIDS 관련 지식 및 태도 (Knowledge and Attitude toward HIV/AIDS among Professional Graduate Medical School Students)

  • 본 연구는 의학전문대학원 학생의 HIV/AIDS에 대한 지식과 태도를 조사하기 위해 시행된 횡단적 서술적 조사연구이다. 연구대상은 J도의 일 대학 의학전문대학원 3학년 학생 97명으로 2014년 8월 4일부터 12월 8일까지 자료수집이 실시되었으며, 수집된 자료는 SPSS WIN 19.0 프로그램을 이용하여 분석하였다. 연구결과 의학전문대학원 학생들의 HIV/AIDS 관련 지식과 태도는 각각 15점 만점에 평균 8.91, 7.00으로 나타났으며, 지식과 태도는 대상자들의 특성에 따라 통계적으로 유의한 차이가 없었다. 대상자들의 HIV/AIDS 관련 지식과 태도 간에는 통계적으로 유의한 양적 상관관계(r=.34, p<.01)를 보여 지식이 높을수록 태도 또한 긍정적인 경향을 보임이 확인되었다. 본 연구결과는 예비의료인 및 의료인의 HIV/AIDS 관련 지식을 증진시키고 부정적인 태도를 감소시키는 것을 목적으로 하는 효과적인 교육제공을 위한 구체적인 실행전략 개발에 유용하게 이용될 수 있다.

Knowledge and Opinions Regarding Oral Cancer among Yemeni Dental Students

  • Background: Oral cancer presents with high mortality rates, and the likelihood of survival is remarkably superior when detected early. Health care providers, particularly dentists, play a critical role in early detection of oral cancers and should be knowledgeable and skillful in oral cancer diagnosis. Purpose: The aim of the present study was to assess the current knowledge of future Yemeni dentists and their opinions on oral cancer. Materials and Methods: A pretested self-administered questionnaire was distributed to fourth and fifth year dental students. Questions relating to knowledge of oral cancer, risk factors, and opinions on oral cancer prevention and practices were posed. Results: The response rate was 80%. The vast majority of students identified smoking and smokeless tobacco as the major risk factors for oral cancer. Most of the students (92.6%) knew that squamous cell carcinoma is the most common form of oral cancer, and 85.3% were aware that tongue and floor of the mouth are the most likely sites. While the majority showed willingness to advise their patients on risk factors, only 40% felt adequately trained to provide such advice. More than 85% of students admitted that they need further information regarding oral cancer. As expected, students of the final year appeared slightly more knowledgeable regarding risk factors and clinical features of the disease. Conclusions: The findings of the present study suggest that here is a need to reinforce the undergraduate dental curriculum with regards to oral cancer education, particularly in its prevention and early detection.

Prevalence of Cryptosporidium-Associated Diarrhea in a High Altitude-Community of Saudi Arabia Detected by Conventional and Molecular Methods

  • Cryptosporidium diarrhea represents a relevant clinical problem in developing countries. In Al-Taif, a city of Saudi Arabia that lies at an altitude of an around 2 km above the sea level, Cryptosporidium infection seems to be undiagnosed in nearly all clinical laboratories. Furthermore, nothing was published regarding Cryptosporidium-associated diarrhea in this area. The objectives of this research were to (1) determine the Cryptosporidium prevalence among patients with diarrhea and (2) to estimate the performances of 3 different diagnostic methods. Total 180 diarrheal fecal samples, 1 sample per patient, were collected between January and August 2013. Samples were screened for Cryptosporidium with modified Zeihl Neelsen (ZN) microscopy, $RIDA^{(R)}$ Quick lateral flow (LF) immunotest, and a previously published PCR. The Cryptosporidium prevalence rate was 9.4% (17/180), 10% (18/180), and 11.6% (21/180) by microscopy, LF, and PCR test, respectively. Infection was significantly (P=0.004) predominant among children <5 years (22%) followed by children 5-9 years (11.1%). Although infection was higher in males than in females (16.2% males and 8.5% females), the difference was not statistically significant (P=0.11). Compared to PCR, the sensitivity of microscopy and the LF test were 80.9%, 85.7%, respectively. To conclude, high Cryptosporidium-associated diarrhea was found in this area especially in children ${\leq}9$ years. The PCR test showed the best performance followed by the LF test and ZN staining microscopy. The primary health care providers in Al-Taif need to be aware of and do testing for this protozoon, particularly for children seen with diarrhea.

Effects of Long- and Short-term Consumption of Energy Drinks on Anxiety-like, Depression-like, and Cognitive Behavior in Adolescent Rats

  • Purpose: The purpose of this study was to understand the impact of long- and short-term energy drinks on anxiety-like, depressionlike, and cognitive behavior in adolescent rats. Methods: Adolescent rats (age six weeks) were randomly classified into a control group (CON), a long-term administration group (LT), and a short-term administration group (ST). The LT group was orally administered 1.5 mL/100 g (body weight) of energy drink twice daily for 14 days, the ST group was orally administered for one day, and the control group applied the same amount of normal saline. Later, an open-field test, a forced swim test, novel object recognition test, and an 8-arm radial maze test was conducted to assess the rats' anxiety, depression, and cognitive function. Results: There were different effects in the long- and short-term groups of energy drink administration. In the LT group, anxiety- and depressive-like behavior increased because of increased movement in the side corner and decrease of immobility time. Also, the time to explore novel objects decreased, and the number of correct responses was reduced, indicating a learning and memory function disorder. However, the ST group was not different from the control group. Conclusion: These results indicate that long-term consumption of energy drinks can increase anxiety-like, depression-like behavior, and this can lead to decrease in learning and memory functions. Thus, nurse and health care providers should understand the impact of energy drink consumption in adolescence to provide appropriate practices and education.
