Development of Self-management Instrument for Pre-dialysis Patients with Chronic Kidney Disease (투석전 만성신장병 환자의 자가관리 도구 개발)

  • This study aimed to develop a valid and reliable self-management instrument for pre-dialysis patients with chronic kidney disease (CKD). A total of 143 patients with CKD recruited from a medical center completed the questionnaire. The dimension and items of self-management were composed by literature review. The items of self-management was evaluated using exploratory factor analysis and measures of reliability. Five factors were extracted and labelled adherence to treatment regimen and partnership, diet adherence, problem solving, health behavior, and pursuit of psycho-social health. The five factors accounted for 51.1% of total variance. Each factors showed acceptable internal reliability with Cronbach's alpha from 0.64-0.79. The developed self-management instrument can be useful in self control of their disease for patients with CKD, and both evaluating patients' self-management and developing intervention program for health care professionals.

Teaching Status and Knowledge of Elementary School Teachers of Children with Attention Deficit Hyperactivity Disorder (주의력결핍과잉행동장애 아동에 대한 초등교사의 지도실태와 지식)

  • Purpose: The purpose of this study was to analyze teaching status and knowledge about children with Attention Deficit Hyperactivity Disorder (ADHD) in elementary school teachers. Methods: The participants of this study were 204 elementary school teachers in Busan. From July to September 2010, the researchers collected data by using self-report questionnaires consisted of questions about their teachers' characteristics and teaching status and KADDS (36 items) developed by Sciutto, Terjesen and Frank. Frequency, percentage, arithmetic mean, standard deviation, t-test and ANOVA were used to analyze the data using SPSS WIN 16.0 program. Results: Less than 50% of the participants had previous education on ADHD. According to this experience, there were significant differences in participants' knowledge about children with ADHD (t=4.703, p<.001). For the 36 items, participants had an average of $19.19{\pm}4.58$ points out of a possible 36. Moreover, lack of knowledge about teaching children with ADHD and communication problems with the parents of these children were considered to be the most disturbing factors. Conclusion: These results suggest that networks among teachers, parents and medical care professionals should be established. The results also indicate that educational programs for elementary school teachers should be developed in order to identify and teach children with ADHD appropriately.

Study of the Drugs Prescribed on Alzheimer's Disease: from the Insurance Claims Data of Korea National Health Insurance Service (우리나라 건강보험 청구자료를 이용한 알츠하이머성 치매 치료제의 사용현황 분석)

  • Objective: The aims of this study are to investigate the total volume of prescribed medicines against Alzheimer's disease (AD) and the trends of usage by analyzing the claims-data from the Korea National Health Insurance Service. Method: The demographic and claims-data were included the major AD treating medicines such as donepezil, galantamine, rivastigmine and memantine, and analyzed during the period of 2010~2012. The assessing criteria were gender, age, habitation, types of medical institution, code of ingredients, outcomes of treatment, volume and amount of claims, and the numbers of patients with dementias. After trimming the data, it were analyzed by the market size, demographic traits, characteristics of medical service, characteristics of each anti-AD medicine, etc. Results: Among the chosen 4 medicines, donepezil had the top prescription volumes. Most prevalent prescribing preparations of donepezil were conventional types. However, among the non-conventional types, oro-dispersible formulation is the fast increasing one in both volume and growth rate. This specialized preparations to improve both toleration and adherence, tend to being prescribed generally at the tertiary medical institutions. While the younger patients with mild-to-moderate AD mostly treated by expensive medicines in resident at the tertiary hospitals, the rest older patients with severe AD have been treated non-expensive one at long-term care facilities. Conclusion: AD is a chronic illness therefore, long-term use of therapeutic medications are highly important. If an anti-AD treatment was applied steadily in the earlier stages, it would be achieved not only improving the quality of life of patient but also reducing the expenses in the medical and nursing cares. As the socioeconomical impacts of AD is expanding, healthcare professionals need to aware the importance of pharmacotherapy and to improve sociopolitical fundamentals.

A review of the qualification criteria for the national examinations for emergency medical technicians (응급구조사 국가시험 응시 자격기준에 관한 고찰)

  • Purpose: Emergency medical technicians (EMTs) have so far been trained as professionals under the same conditions, with no change in the 1995 Act. We aimed to find ways for them to secure expertise in accordance with social needs by strengthening the quality of the EMT education. Methods: This is a descriptive study comparing the operation status of the national emergency medical examination conducted by the Korea Health Personnel Licensing Examination Institute, and the national examinations of EMT paramedic and EMT basic. The scope of the national examinations for EMT was compared by subject and area. Results: The national written exam for EMT paramedic consists of five areas. EMT basic does not include basic medicine in three related subjects, 11 areas, and 18 detail areas. Paramedic care does not include advanced pediatric life support. In addition, nine areas and 20 detail areas are not included. Conclusion: The study suggests the need for institutional supplementation so that those who have completed EMT basic and the subjects prescribed by the ordinance of the Ministry of Health and Welfare at universities, etc., in the Higher Education Act can take the EMT paramedic national exam.

  • The occurrence of functional gastrointestinal disorders (FGIDs) is a formidable challenge for infants, parents, and healthcare professionals. Although data from the Middle East are scarce, experts consider FGIDs a prevalent condition in everyday clinical practice. The new Rome IV criteria revisited the definitions from a clinical perspective to provide a practical and consistent diagnostic protocol for FGIDs. However, the treatment practices for functional disorders vary considerably among Middle Eastern countries, often resulting in mismanagement with unnecessary investigations and treatments. In addition, the role of various treatment modalities, including probiotics such as Lactobacillus reuteri DSM 17938, in FGIDs requires further discussion and evaluation. During a consensus meeting, a locally relevant approach for treating common FGIDs such as infant regurgitation, infant colic, and functional constipation was discussed and approved by regional experts. The participants suggested a simplified treatment plan and protocol for general pediatricians and other primary care physicians managing FGIDs. This easy-to-follow standardized protocol will help streamline the initial management of this complex disorder in the Middle East region and even globally.

Adherence to Antidepressants in Korean Elderly Patients with Major Depressive Disorder (주요우울장애 노인 환자의 항우울제 복약이행도 연구)

  • Objective: Adherence is an important component in the treatment of various diseases, and poor adherence to antidepressants in patients with major depressive disorder is common. Non-adherence can be more prevalent in elderly patients with multiple morbidity and polypharmacy, resulting in negative treatment outcomes. The purpose of this study was to analyze adherence to antidepressants in Korean elderly patients with major depressive disorder. Method: A retrospective study was conducted using the Korean National Health Insurance claims database, and the subjects of this study were patients aged 65 or older who received at least one prescription of antidepressant monotherapy for the treatment of major depressive disorder between January 1, 2020 and June 30, 2020. Adherence was measured using the proportion of days covered at 6 months after the initial antidepressant prescription date. Logistic regression analysis was used to identify factors associated with adherence. Results: A total of 416,766 patients were finally included in the study. Over half of patients were non-adherent (52.67%) to antidepressants. According to the multivariate logistic regression analysis, national health insurance or medical aid, taking selective serotonin reuptake inhibitors or selective norepinephrine reuptake inhibitors, and having comorbidities were significantly associated with greater rates of adherence in the study subjects. The highest adherence rate was observed in patients taking vortioxetine. Conclusion: There was a considerable rate of non-adherence in Korean elderly patients with major depressive disorder. Health care professionals should try to improve adherence in elderly patients with major depressive disorder.

Factors analysis of the oral health-related quality of life in the elderly (노인의 구강건강관련 삶의 질과 요인분석)

  • Objectives: The purpose of this study was to compare the differences in oral health-related quality of life among elderly people aged over 65 years, in terms of physical, mental and oral health status and to analyze factors affecting their oral health-related quality of life. Methods: From May 9 to June 23, 2017, we randomly visited aged-care community centers in the metropolitan area, and recruited 222 elderly, aged 65 or older. First, each participant completed a questionnaire consisting of 4 general items: 1 systematic disease, and 3 subjective oral conditions. Afterwards, the researchers interviewed the participants to assess their mental status, using MMSE-DS and recorded the responses. Finally, an oral examination was performed to determine the number of remaining teeth. The average oral health-related quality of life according to each characteristic was analyzed by t-test and ANOVA. Hierarchical multiple regression analysis and Pearson's correlation coefficient analysis were used to analyze the correlations between factors and the factors affecting oral health-related quality of life. Results: The mean oral health-related quality of life was 4.15. Participants with 20 or more remaining teeth demonstrated better oral health-related quality of life than those with 19 or less teeth. Higher oral health-related quality of life was also found among elderly without gingival bleeding, self-reported halitosis and dry mouth. In addition, positive correlation with the number of remaining teeth and negative correlation with gingival bleeding, self-reported halitosis and dry mouth, were noted. Finally, the results of the hierarchical multiple regression analysis indicated that remaining teeth, gingival bleeding, self-reported halitosis and education were influential factors in determining the oral health-related quality of life among the elderly. Conclusions: The results of this study confirmed the necessity of better policy support, and the importance of implementing delivered, elderly-centered oral health education program by professionals to prevent tooth loss and manage periodontal diseases.

The effects of behavior of preventive treatment on job satisfaction in dental hygienist (치과위생사의 예방처치업무행태가 직무만족에 미치는 영향)

  • Objectives : The author has studied the effects of behavior of preventive treatment on job satisfaction in dental hygienist. Methods : The hygienists who had worked from June 27th to August 1st in 2011 were explained about this research objectives and only the hygienists who agreed with this research participation were conducted a self-administered Questionnaire survey and the results were collected immediately. The collected questionnaire was 297, but the final 250 questionnaire were decided as a target except for the trustless ones. This analysis was extracted using SPSS(SPSS 12.0 for windows, SPSS Inc, Chicago, USA). Results : 1. Preventive treatment of dental hygienist was shown as this order - Educational needs 4.25, Importance 4.12, Usefulness 4.09, Perform 3.42. Job satisfaction was reached the moderate level. 2. Analysis of preventive treatment of hygienist was shown as this order - Basic brushing education 4.72 was ranked the highest, Taking patient history and Incremental dental health care 4.58 was held the second place. Dental caries activity tests 3.30 was shown the lowest figures. Response of preventive treatment conducting was like this order - Taking patient history 4.30, Dental prophylaxis 4.28, Basic brushing education 4.11, Incremental dental health care 4.04. Dental caries activity tests 2.05 was shown the lowest figures. 3. General characteristics in behavior of preventive treatment was resulted in this case that the respondents who have worked for less one year in current job than the ones who have worked for more than five years was shown such a low level, this was taken as a meaningful difference.(p=0.008) 4. It was shown that the higher fulfillment of preventive treatment(${\beta}$=0.340, p<0.001) and the greater earnings(${\beta}$=0.194, p=0.001) and usefulness of preventive treatment(${\beta}$=0.130, p=0.042), the higher job satisfaction. Conclusions : Considering these results, environment where the hygienists can concentrate on preventive treatment which is their proper job should be built up as soon as possible. And through this environment, the atmosphere where hygienists, as oral health professionals, can play a role to improve the oral health of the people by boosting job satisfaction should be created. Also It is thought that an institutional, actual improvement-changing the social awareness towards hygienists and dentists, the recognition of scope of the hygienists' work - should be established urgently.

A study of dental assistance in dental care (치과의료 영역에서 진료보조에 대한 고찰)

  • Objectives: This study aims to establish the concept of "dental assistance" within the dental field and to identify the scope of dental assistance, which can be utilized in order to distinguish the scope of work between jobs. Methods: The literature on dental hygienist work and dental assistance was reviewed, and the laws and precedents related to dental assistance were examined. Results: The criteria for judging the legitimacy of dental assistance included the following: whether the performance was conducted under the guidance and supervision of a doctor; whether there was a possibility of an aftereffect or side effects of the procedure; whether education was conducted within the curriculum; and whether it was evaluated in the national licensing examination. In addition, the work of the assistant in dental surgery and treatment is judged comprehensively by considering the type of dental surgery and treatment, invasive degree, necessity of expertise, and work proficiency of the dental hygienist. Therefore, it is suggested that dental hygienists may be of assistance for dental treatment/surgery because they meet the requirements of dental professionals, such as dental hygiene curriculum, national licensing examination, duties in the field of dentistry, and work proficiency. Conclusions: Dental assistance (including assistance in dental surgery) for dental treatment, dental/medical history taking, taking vital signs, and blood glucose monitoring should be permitted within the realm of dental hygienist work. Therefore, the actual expertise of dental hygienists should be reflected legally and the work of dental hygienists should be realized.

Informational Needs of Cancer Patients Receiving Chemotherapy (항암화학요법 환자의 정보요구 분석)

  • Purpose: To examine informational needs of cancer patients receiving chemotherapy. Methods: A cross-sectional survey design was utilized. The sample was 198 cancer patients receiving chemotherapy for a diagnosis of stomach or lung cancer at a university hospital. A modified version of Toronto Informational Needs Questionnaire was applied. Results: Subjects reported highest informational needs in prognosis dimension, followed by the treatment dimension including side effects and alternative therapy. Information about medical tests and follow-up care after the treatment was also in great need. Dimensions of support for patients/family and sexuality were low in need. The degree of informational needs was negatively correlated with patient's age and number of children. Differences were found in informational needs according to the educational level employment status, and gender of the patients, while diagnosis and treatment options did not make differences in informational needs. Conclusion: Health care professionals need to perceive informational needs of cancer patients receiving chemotherapy, and to provide them with information especially in the area of prognosis and treatment. Informational intervention would be effective when it is given with consideration of patient's characteristics.
